Additional Dose of Intravenous Tranexamic Acid after Primary Total Knee Arthroplasty Further Reduces Hidden Blood Loss

Background： Total knee arthroplasty （TKA） is the most frequently performed procedure in treating advanced knee osteoarthritis. Excessive perioperative blood loss can sometimes lead to postoperative anemia. Tranexamic acid （TXA） is a potent fibrinolysis inhibitor which has been extensively used at the surgical incision and closure to lower overall blood loss in adult reconstruction surgery. Our previous study suggested that about two-thirds of the total blood loss （TBL） came from hidden blood loss （HBL） on postoperative days 1 and 2. The role of reducing HBL with TXA administration in postoperative TKA patients is unknown. The current study was designed to evaluate the efficiency and safety of supplemental intravenous （IV） TXA in further redticing HBL after primary TKA. Methods： A prospective pilot study was conducted at a single institution on 43 consecutive patients who underwent unilateral TKA from September 2014 to February 2015. All patients were given 1 g of IV TXA 10-15 rain before operation and another 1 g of IV TXA at the time of wound closure on the day of surgery. On postoperative days 1 and 2, the supplemental group （n = 21） was given additional 1 g of TXA intravenously twice a day, whereas the control group （n = 22） received an equal volume of saline. Drain output, hemoglobin （Hb）, and hematocrit （HCT） were recorded preoperatively and 5 consecutive days postoperatively in both groups. HBL was calculated with the Gross formula. Pre- and post-operative lower extremity Doppler venous ultrasound was performed in all patients to detect deep vein thrombosis （DVT）. The indexes were compared using the Mann-Whitney test, whereas the results of Hb and HCT were analyzed by repeated-measures analysis of variance. The difference was considered statistically significant ifP 〈 0.05. Results： The demographics and surgical characteristics of the two groups were comparable. In Vitro Antifungal Activity of Dihydrochelerythrine and Proteomic Analysis in Ustilaginoidea virens

Dihydrochelerythrine(DHCHE)is an isoquinoline compound,which has distinct antifungal activity and can induce apoptosis.The antifungal activity of DHCHE against five rice pathogenic fungi was studied in vitro.At the concentration of 7.5 mg/L,DHCHE exhibited the highest efficacy among tested compounds in inhibiting mycelium growth,with an inhibition rate of 68.8%in Ustilaginoidea virens,which was approximately 2.4 times of that of validamycin(28.7%).After exposure to DHCHE,transmission electron micrographs revealed spores showed incomplete organelles,malformed cell walls and nuclear membranes,as well as irregular lipid spheres.Reactive oxygen species accumulation in treated spores was markedly higher than that in control spores.DHCHE induced cell damage increased in a dose-dependent manner,as indicated by the decrease in mitochondrial membrane potential and initiation of apoptosis.The differences of expression levels of Fip1,ACP1,PMS2 and COX13 that are important for oxidative phosphorylation and mismatch repair pathway were significant,which may be some of the reasons for the induction of apoptosis in DHCHE-treated U.virens.The protein levels of Fip1,ACP1,PMS2 and COX13 agreed with protein fold change ratio from parallel reaction monitoring Gene Ontology terms and Kyoto Encyclopedia of Genes and Genomes pathway of differentially expressed proteins were further analyzed.These findings will help to elucidate the mechanisms associated with antifungal and pro-apoptotic effects of DHCHE on U.virens,thereby aiding the potential development of novel pesticides.

Prevalence and risk factors of diabetes mellitus among elderly patients in the Lugu community

BACKGROUND Age is a significant risk factor of diabetes mellitus(DM).With the develop of population aging,the incidence of DM remains increasing.Understanding the epidemiology of DM among elderly individuals in a certain area contributes to the DM interventions for the local elderly individuals with high risk of DM.AIM To explore the prevalence of DM among elderly individuals in the Lugu community and analyze the related risk factors to provide a valid scientific basis for the health management of elderly individuals.METHODS A total of 4816 elderly people who came to the community for physical examination were retrospectively analyzed.The prevalence of DM among the elderly was calculated.The individuals were divided into a DM group and a non-DM group according to the diagnosis of DM to compare the differences in diastolic blood pressure(DBP)and systolic blood pressure(SBP),fasting blood glucose,body mass index(BMI),waist-to-hip ratio(WHR)and incidence of hypertension(HT),coronary heart disease(CHD),and chronic kidney disease(CKD).RESULTS DM was diagnosed in 32.70%of the 4816 elderly people.The BMI of the DM group(25.16±3.35)was greater than that of the non-DM group(24.61±3.78).The WHR was 0.90±0.04 in the non-DM group and 0.90±0.03 in the DM group,with no significant difference.The left SBP and SBP in the DM group were 137.9 mmHg±11.92 mmHg and 69.95 mmHg±7.75 mmHg,respectively,while they were 126.6 mmHg±12.44 mmHg and 71.15 mmHg±12.55 mmHg,respectively,in the non-DM group.These findings indicate higher SBP and lower DBP in DM patients than in those without DM.In the DM group,1274 patients were diagnosed with HT,accounting for 80.89%.Among the 3241 non-DM patients,1743(53.78%)were hypertensive and 1498(46.22%)were nonhypertensive.The DM group had more cases of HT than did the non-DM group.There were more patients with CHD or CKD in the DM group than in the non-DM group.There were more patients who drank alcohol more frequently(≥3 times)in the DM group than in the non-DM group.CONCLUSION Older adults in the Lugu c

Numerical simulations of propeller cavitation flows based on OpenFOAM

In order to study the cavitation and hydrodynamic characteristics of propeller under uniform and non-uniform flows,numerical investigations are performed using interPhaseChangeDyMFoam in the open source computational fluid dynamics(CFD)software platform OpenFOAM with Schnerr-Sauer cavitation model.The simulation results can be used as a reference to evaluate the working ability of a propeller in case of actual navigation.A new grid encryption method is adopted in the research to better capture the existence of vortex cavitation at the propeller tip.The method of function input is carried out in the study to simulate the condition of non-uniform flow and reduce the calculation amount.Typical unsteady dynamics are predicted by the Reynolds-averaged Navier-Stokes(RANS)method with a modified shear stress transport(SST)k−ωturbulence model.The numerical results of the propeller such as cavitation shape and pressure distribution under uniform and non-uniform flow are analyzed and compared with each other.

Patient-Specific Instruments Based on Knee Joint Computed Tomography and Full-Length Lower Extremity Radiography in Total Knee Replacement

Background： Restoring good alignment after total knee replacement （TKR） is still a challenge globally, and the clinical efficiency of patient-specific instruments （PSIs） remains controversial. In this study, we aimed to explore the value and significance of three-dimensional printing PSls based on knee joint computed tomography （CT） and full-length lower extremity radiography in TKR. Methods： Between June 2013 and October 2014, 31 TKRs were performed using PSIs based on knee joint CT and full-length lower extremity radiography in 31 patients （5 males and 26 females; mean age： 67.6 ± 7.9 years, body mass index [BMI]： 27.4 ±3.5 kg/m2）. Thirty-one matched patients （4 males and 27 females; mean age： 67.4± 7.2 years; mean BM 1：28.1 ± 4.6 kg/m2） who underwent TKR using conventional instruments in the same period served as the control group. The mean follow-up period was 38 months （31-47 months）. Knee Society Score （KSS）, surgical time, and postoperative drainage volume were recorded. Coronal alignment was measured on full-length radiography. Results： Twenty-three （74.2%） and 20 （64.5%） patients showed good postoperative alignment in the PSI and control groups, respectively, without significant difference between the two groups （χ2 = 0.68, P = 0.409）. The mean surgical time was 81.48± 16.40 rain and 72.90 ± 18.10 min for the PSl and control groups, respectively, without significant difference between the two groups （t = 0.41, P = 0.055）. The postoperative drainage volume was 250.9 ± 148.8 ml in the PSI group, which was significantly less than that in the control group （602. 1± 230.6 ml, t = 6.83, P 〈 0.001）. No significant difference in the KSS at the final follow-up was found between the PSI and control groups （91.06 ± 3.26 vs. 90.19±3.84, t= 0.95, P=0.870）. Conclusions： The use of PSls based on knee joint CT and standing full-length lower extremity radiography in TKR resulted in acceptable alignment compared with the use of conventional instruments, although the marginal advantage was not statistically different. Octreotide reverses shock due to vasoactive intestinal peptide-secreting adrenal pheochromocytoma: A case report and review of literature

Vasoactive intestinal peptide-producing tumors(VIPoma) usually originate in the pancreas and are characterized by diarrhea, hypokalemia, and achlorhydria(WDHA syndrome). In adults, nonpancreatic VIPoma is very rare. Herein, we report an unusual case of VIPproducing pheochromocytoma marked by persistent shock, flushing, and watery diarrhea and high sensitivity to octreotide. A 53-year-old woman was hospitalized for sudden-onset hypertension with convulsions, which then rapidly evolved to persistent shock, flushing, and watery diarrhea. Abdominal computed tomography indicated a left adrenal mass, accompanied by bleeding; and marked elevations of both plasma catecholamine and VIP concentrations were documented via laboratory testing. Surprisingly, all clinical symptoms responded swiftly to octreotide treatment. Once surgically treated, hormonal levels normalized in this patient, and the clinical symptoms dissipated. Postoperative pathological and immunohistopathological studies confirmed a VIPsecreting pheochromocytoma with strong, diffuse positivity for somatostatin receptor type 2. During a 6-mo follow-up period, she seemed in good health andwas symptom-free.

One-bit splitting deceptive jamming against SAR

One-bit quantization is a promising technique due to its performance retention and complexity reduction in a deceptive jammer against synthetic aperture radar(SAR).In this paper,the 1-bit quantization technology is utilized to agilely generate split false targets in the SAR imagery once the radar signal is intercepted,which reduces the complexity of the jammer significantly with guaranteed focusing quality.A single-frequency threshold is used to decompose harmonics incurred by the 1-bit quantization,and its parameters are adjusted through different pulse repetition intervals to provide steerable modulations.In this way,the SAR signal is split into coupled false scatterers during the 1-bit interception.By further deploying amplitude,time-delay,and Doppler frequency modulations on the 1-bit intercepted signal,the split false targets are created.The proposed approach is compared with different deceptive jamming methods to show its validity in effectiveness and cost,and numerical experiments are also presented for verification.

Effects of Hydrogen Charging on Mechanical Properties of CLAM Steel at Different Strain Rates

The China low-activation martensitic(CLAM)steel has been proposed as a candidate structural material for nuclear fusion reactors.It is essential to study the influence of hydrogen charging and strain rate on the tensile behavior of CLAM steel considering its service environment.In this study,CLAM steel was investigated using tensile tests operated at room temperature before and after hydrogen charging.The results showed that the elongation loss increased significantly with the increase of the hydrogen charging current density at either a low or a high strain rate,which is related to the hydrogen content in the steel.The hydride was systematically studied,including the morphology and the thermal stability as well as the effects on mechanical properties.The possible mechanism of the formation of hydride during hydrogen charging has been analyzed based on the interaction between alloying elements and hydrogen.

The biochemical property and functional identification of three recombinant glycosyltransferases,includingβ-1,4-rhamnosyltransferase(Cps8R),β-1,4-galactosyltransferase(Cps8) and α-2,3-sialyltransferase(Cps8K)involved in the biosynthesis of the tetrasaccharide repeating unit of serotypeⅧcapsular polysaccharide(CPS)of Group B Streptococci(GBS),were systematically investigated.Subsequently,these recombinant enzymes were employed for one-pot three-enzyme efficient synthesis of the tetrasaccharide repeating unit of GBS serotypeⅧCPS using the chemically synthesized Glca-PP-(CH2)ir-OPh as the starting substrate in a satisfying yieldof 87%.
