Treatment efficacy of radiof requency ablation of 338 patients with hepatic malignant tumor and the relevant complications

AIM： To investigate the treatment efficacy of radiofrequency ablation （RFA） of hepatic malignant tumor and the relevant complications. METHODS： A total of 338 patients with 763 hepatic tumors underwent ultrasound-guided RFA （565 procedures）. There were 204 cases of hepatic cellular carcinoma （HCC） with 430 tumors, the mean largest diameter was 4.0 cm. Of them, 48 patients （23.5%） were in stages Ⅰ-Ⅱ （UICC Systems） and 156 （76.5%） in stages Ⅲ-Ⅳ There were 134 cases of metastatic liver carcinoma （MLC）, with 333 metastases in the liver, the mean diameter was 4.1 cm, the liver metastases of 96 patients （71.6%） came from gastrointestinal tract. Ninety-three percent of the 338 patients were treated using the relatively standard protocol. Crucial attention must be paid to monitor the abnormal changes in ultrasound images as well as the vital signs of the patients to find the possible hemorrhage and peripheral structures injury in time. The tumors were considered as ablated completely, if no viability was found on enhanced CT within 24 h or at 1 mo after RFA. These patients were followed up for 3-57 too. RESULTS： The ablation success rate was 93.3% （401/430 tumors） for HCC and was 96.7% （322/333 tumors） for MLC. The local recurrence rate for HCC and MLC was 7.9% （34/430 tumors） and 10.5% （351333 tumors）, respectively. A total of 137 patients （40.5%） underwent 2-11 times of repeated ablations because of tumor recurrence or metastasis. The 1st, 2nd, and 3rd year survival rate was 84.6%, 66.6%, and 63.1%, respectively; the survival rate from 48 patients of I-II stage HCC was 93.7%, 80.4%, and 80.4%, respectively. The major complication rate in this study was 2.5% （14 of 565 procedures）, which consisted of 5 hemorrhages, i colon perforation, 5 injuries of adjacent structures, 2 bile leakages, and 1 skin burn. Intraperitoneal hemorrhage during and after percutaneous radiofrequency ablation of hepatic tumors: reasons and management

Background Introperitoneal hemorrhage is one of the most common complications of radiofrequency （RF） ablation of hepatic tumors. This study was designed to investigate the reason and management of intraperitoneal hemorrhage occurred during or after percutaneous RF ablation of hepatic tumors.Methods Three hundred and flfty-six patients with hepatic tumors have been treated at 592 procedures of ultrasound guided RF ablation. Intraperitoneal hemorrhage occurred in 5 patients （0. 8% ）. The reasons and management of intraperitoneal hemorrhage in these 5 cases were retrospectively analyzed.Results Two patients with liver metastasis and one hepatocellular carcinoma （HCC） patient suffered from hemorrhage during the RF treatment. Two patients with recurrent HCC after surgery developed hemorrhage 20 minutes or 4 hours after RF treatment. One case of hemorrhage was due to the inappropriate electrode positioning induced liver laceration while treating a 1 cm liver metastasis near the liver capsule. One was due to the injury of a small vessel by the RF needle in another liver metastasis patient. Three cases were due to tumor rupture with two cases induced by cough or position change after treating large protruding HCC lesions. Four （80%） of the 5 cases of hemorrhage were rapidly identified by ultrasound. The causes and sites of bleeding during the RF treatment in three cases were confirmed through ultrasound, which were successfully treated using RF coagulation to achieve hemostasis of the bleeding site. Two patients with post-ablation hemorrhage recovered in one hour and 24 hours, respectively after given blood transfusion and other conservative measures. No surgical intervention was required. Two patients died of wide spread metastasis 23-36 months afterwards and the other three patients have lived for 18-25 months to date.Conclusions It is important to perform close monitoring during and after RF ablation in order to identify intraperitoneal hemorrhage in time. We investigate high time resolution data obtained by the Gravitational wave high-energy Electromagnetic Counterpart All-sky Monitor(GECAM)during the flare event on 2022 April 21 at 01:52 *** subpeaks with durations of 4-6 s have been detected in the hard X-ray precursor phase,and the key feature is that they appear in pairs and seem like double-peak *** subpeaks are rarely observed in hard X-ray band and confirmed by the microwave obtained by Nobeyama Radio Polarimeters(NoRP)and Radio Solar Telescope Network(RSTN).While an exponential function can describe the continuum component of the time profile from the precursor to part of the impulsive *** periods of quasi-periodic pulsations(QPPs)are detected to be about 7.3 and 12.8 s for the precursor and impulsive phase,respectively,with at least 95%confidence *** paired QPPs are assumed to be double-peak QPPs and then the scenario of current loop coalescence model is found to be in good agreement with our *** precursor phase can be interpreted as the oscillating coalescence of two islands,while the impulsive phase can be interpreted as more islands to coalesce one by one to form larger islands.

172例肝癌射频消融治疗预后因素分析

目的探讨原发性肝癌射频消融(RFA)治疗的生存期及影响因素,进行单因素及多因素分析,以指导RFA消融治疗。方法1999至2004年172例原发性肝癌行RFA治疗,其中150例(87.2%)不能或不宜手术治疗。分析24项与生存期可能相关的因素,探讨与生存期的相关性。随访时间4～51个月,随访率94.8%(163例)。单因素分析采用KaplanMeier模型及Logrank检验,多因素分析采用COX比例风险模型。结果原发性肝癌RFA治疗后半年、1年、2年、3年的生存率分别为:91.5%、77.4%、55.7%、51.2%。分析可能影响生存期的24项单因素中8项有统计学意义,分别为:肝功能ChildPugh分级、肿瘤国际抗癌联盟(UICC)分期、初次RFA前甲胎蛋白(AFP)值、门静脉高压、肿瘤复发性、病理分级、治疗目的方案、消融安全范围。根据UICC分期标准,首发癌116例半年、1年、2年、3年的生存率分别为:Ⅰ期和Ⅱ期(33例)达96.7%、92.3%、81.6%、65.3%;Ⅲ期和Ⅳ期(83例)为91.2%、76.3%、56.6%、51.4%;Ⅰ和Ⅱ期与Ⅲ和Ⅳ期之间差异有统计学意义。复发癌56例的生存率分别为88.5%、70.2%、41.4%、24.5%,与首发癌Ⅲ和Ⅳ期差异无统计学意义。与生存率相关的多因素有3项,分别为:ChildPugh分级、门脉高压、治疗目的方案。结论影响原发性肝癌RFA治疗预后的因素可分为两个方面:患者因素为肝功能ChildPugh分级、门脉高压程度;治疗方面因素为治疗目的方案;此结果有助于指导RFA治疗。

There is considerable potential for integrating transarterial chemoembolization(TACE),programmed death-(ligand)1(PD-[L]1)inhibitors,and molecular targeted treatments(MTT)in hepatocellular carcinoma(HCC).It is necessary to investigate the therapeutic efficacy and safety of TACE combined with PD-(L)1 inhibitors and MTT in real-world *** this nationwide,retrospective,cohort study,826 HCC patients receiving either TACE plus PD-(L)1 blockades and MTT(combination group,n=376)or TACE monotherapy(monotherapy group,n=450)were included from January 2018 to May *** primary endpoint was progression-free survival(PFS)according to modified *** secondary outcomes included overall survival(OS),objective response rate(ORR),and *** performed propensity score matching approaches to reduce bias between two *** matching,228 pairs were included with a predominantly advanced disease *** PFS in combination group was 9.5 months(95%confidence interval[CI],8.4-11.0)versus 8.0 months(95%CI,6.6-9.5)(adjusted hazard ratio[HR],0.70,P=0.002).OS and ORR were also significantly higher in combination group(median OS,19.2[16.1-27.3]vs.15.7 months[13.0-20.2];adjusted HR,0.63,P=0.001;ORR,60.1%vs.32.0%;Pbination and monotherapy groups,*** results suggest that TACE plus PD-(L)1 blockades and MTT could significantly improve PFS,OS,and ORR versus TACE monotherapy for Chinese patients with predominantly advanced HCC in real-world practice,with an acceptable safety profile.

超声造影对肝癌射频消融筛选适应证的应用价值

目的通过射频消融前超声造影观察肿瘤大小、数目及分布,探讨其对筛选适应证的应用价值.方法北京大学临床肿瘤学院超声科就诊的164例确诊肝细胞癌(hepatocellular carcinoma,HCC)并符合经皮射频消融(radiofrequency Ablation,RFA)入选标准患者,随机分为两组进行对照研究.81例射频消融前采用SonoVue行超声造影(contrast-enhanced ultrasound,CEUS)检查(CEUS组),83例射频消融前未行超声造影检查(对照组).男121例、女43例;年龄38～72岁,平均52.4岁.造影前两组病例的临床资料差异无统计学意义.肿瘤平均直径超声造影组3.6 cm,对照组3.5 cm.治疗后采用常规超声,增强CT和/或超声造影等影像检查进行规律性随访.结果超声造影组81例造影后9例(11.1%)因发现肿瘤数目 >5个 (5例)、范围测量>8 cm(2例)、侵及2支大血管或肠管(2例)被确定为非射频消融适应证,余72例101灶行射频消融治疗.其中超声造影新发现≤1.7 cm 8例12灶中,5例为肝硬化及肝细胞癌治疗后随访病例,3例7个灶为肝细胞癌卫星灶.另有16灶为常规超声不能定性或误诊良性,经超声造影确认并指导射频消融治疗.两组随访6～36个月,超声造影组与对照组消融成功率分别为95.0%和89.6%(P>0.05).对照组新生率高于超声造影组(22.9% vs 9.7%,P<0.05).结论射频消融前超声造影有助于筛选适应证,显著减少新生转移病例.造影所获信息为早期检出微小癌灶提供了手段和依据,从而有效的提高射频消融对肝癌的治疗水平.

It's a challenge for detecting the therapeutic targets of a polypharmacological drug from variations in the responsed networks in thedfferentiated populations with complex diseases,as stable coronary heart ***,in an adaptive,31-center,randomized,double-blind trial invoving 920 patients with moderate symptomatic stable angina treated by 14-day Danhong injection(DHI),a kind of polyphamacological drug with high quality control,or placebo(0.9%saline),with 76-day following-up,we firstly confrmed that DHl couldincrease the proportion of patients with clinically significant changes on angina-frequency assessed by Seattle Angina Questionnaire(ASAQ-AF220)(12.78%at Day 30,95%confidence interval[C]5.86-19.71%,P=0.0003,13.82%at Day 6C0,95%CI 6.82-20.82%,P=0.0001and 8.95%at Day 90,95%CI 2.06-15.85%,P=0.01).We also found that there were no significant differences in new-onset major vascularevents(P=0.8502)and serious adverse events(P=0.9105)between DHl and *** performing the RNA sequencing in 62 selectedpatients,we developed a systemic modular approach tp identfy differentilly expressed modules(DEMs)of DHI with the Z_(summay)valueless than 0 compared with the control group,calculated by weighted gene co-expression network analysis(WGCNA),and sketched out thebasic framework on a modular map with 25 functional modules targeted by ***,the effective therapeutic module(ETM),defined as the highest corelation value with the phenotype alteration(SAQ-AF,the change in SAQ-AF at Day 30 from baseline)calculatedby WGCNA,was identifed in the population with the best effect(ASAQ-AF240),which is related to anticoagulation and regulation ofcholesterol *** assessed the modular flexbility of this ETM using the global topological D value based on Euclidean distance,which is corelated with phenotype alteration(r^(2):0.8204,P=0.019)by linear *** study identified the ant-angina therapeuticmodule in the effective population treated by the multi-target

Cholangiocarcinoma refers to malignant tumors that develop in epithelial lining of biliary system, and it is divided into two categories according to tumor location, intrahepatic cholangiocarcinoma （ICC） and extrahepatic cholangiocarcinoma （ECC）. ICC occurs from the epithelial cells of the intrahepatic bile duct, its branches and interlobular biliary tree; and ECC is divided into hilar cholangiocarcinoma and distal cholangiocarcinoma by the circumscription at the confluence of cystic duct and the common hepatic duct.

Granulomatous lobular mastitis(GLM) is a rare and chronic benign inflammatory disease of the breast. Difficulties exist in the management of GLM for many front-line surgeons and medical specialists who care for patients with inflammatory disorders of the breast. This consensus is summarized to establish evidence-based recommendations for the management of GLM. Literature was reviewed using PubMed from January 1, 1971 to July 31, 2020. Sixty-six international experienced multidisciplinary experts from 11 countries or regions were invited to review the *** of evidence were determined using the American College of Physicians grading system, and recommendations were discussed until consensus. Experts discussed and concluded 30 recommendations on historical definitions,etiology and predisposing factors, diagnosis criteria, treatment, clinical stages, relapse and recurrence of GLM. GLM was recommended as a widely accepted definition. In addition, this consensus introduced a new clinical stages and management algorithm for GLM to provide individual treatment strategies. In conclusion, diagnosis of GLM depends on a combination of history, clinical manifestations, imaging examinations, laboratory examinations and *** approach to treatment of GLM should be applied according to the different clinical stage of GLM. This evidencebased consensus would be valuable to assist front-line surgeons and medical specialists in the optimal management of GLM.
