Analysis of prognostic factors and outcomes of gastric cancer in younger patients:A case control study using propensity score methods

AIM:To understand the clinicopathological and prognostic features of gastric cancer in younger and older ***:Between January 2002 and December2008,1667 patients underwent curative gastric *** comparative purposes,the patients were divided into two groups:younger patients who were less than 40 years old(112 patients),and older patients who were 40 years old and older(1555 patients).In both groups,propensity scoring methods were used to select patients with similar disease statuses.A total of224 matched cases,with 112 patients in each group,were included in the final ***:Compared to the older group,the younger group with gastric cancer had a significantly higher percentage of females(P=0.007),poorly differentiated or signet ring cell carcinoma(P<0.001),advanced T stage gastric cancer(P=0.045),and advanced tumornode-metastasis stage cancer(P=0.036).The older group with gastric cancer had more comorbidities(P<0.001).With the exception of the number of lymph node dissection(P<0.001)and retrieved lymph node(P=0.010),there were no statistically significant differences between the postoperative outcomes of the two *** the follow-up period,there were19 recurrences in the younger group and 11 recurrences in the older *** overall five-year survival rates in the younger and older groups were 84.3%and89.6%,respectively(P=0.172).There were no significant differences(P=0.238)in the overall survival of patients with advanced T stage gastric cancer in the two groups,with five-year survival rates of 70.8%in the younger group and 79.5%in the older *** regard to the age-adjusted survival rate,there was significant difference between the two groups(P=0.225).CONCLUSION:In spite of aggressive cancer patterns in the younger group with gastric cancer,the younger group did not have a worse prognosis than the older group in our study.

A cellular automaton-finite element(CA-FE) model was used to predict the solidification grain structure of permanent mold cast A7086 alloy. In this model, a Gaussian distribution of nucleation sites was adopted, and the Kurz-Giovanola-Trivedi model was extended to a multicomponent Al–Zn–Mg–Cu alloy system to determine the growth kinetics of the dendrite tip. For describing the Gaussian distribution of nucleation sites on the mold surface, an empirical relationship between the initial cooling rate of the melt and the nucleation density was proposed. Under various casting conditions, the calculated grain structures agreed well with the experimental results. Subsequently, the model was applied to the direct-chill(DC) cast billets, and the simulated grain structures reproduced well the experimental results. This confirmed that the current CA-FE model can be used practically and effectively in DC casting processes.

BACKGROUND Mannitol is a hyperosmolar agent and the combination of mannitol and furosemide is a widely used treatment for intracranial pressure *** the hypertonic properties of mannitol to move water out of intracellular spaces,we hypothesized that mannitol combined with furosemide could relieve focal tissue swelling in refractory *** SUMMARY A 90-year-old female had been diagnosed with intracranial hemorrhage and received a combination of mannitol and furosemide for intracranial pressure *** of the intracranial hemorrhage,she had refractory lymphedema of the left lower extremity since ***,after receiving the mannitol and furosemide,the patient’s lower extremity lymphedema improved *** the mannitol and furosemide were discontinued,the lymphedema worsened in spite of complete decongestive therapy(CDT)and intermittent pneumatic compression treatment(IPC).To identify the presumed effect of mannitol and furosemide on the lymphedema,these agents were resumed,and the lymphedema improved *** The present case raises the possibility that a combination of mannitol and furosemide might be considered another effective therapeutic option for refractory lymphedema when CDT and IPC are ineffective.

Mg alloys exhibit a number of good properties such as low density, good castability and high specific strength. However, molten Mg and Mg alloys are ignited without the melt protective gases during melting and casting process due to their high reactivity. The purpose of this study is to investigate effects of Ca and CaO on pure Mg through microstructure observation, ignition test and phase analysis. With increasing Ca and CaO contents, the ignition resistance of Ca or CaO added pure Mg is increased and the grains are refined. As results of XRD and EDS, CaO is reduced to Ca in CaO added pure Mg. Mg2Ca phase is formed even in 0.1 wt.%CaO added pure Mg by reduction mechanism, while Mg2Ca phase is formed over 1.35 wt.% Ca added pure Mg.

Analysis of expanded criteria to select candidates for active surveillance of low-risk prostate cancer

We aimed to analyze the value of each criterion for clinically insignificant prostate cancer （PCa） in the selection of men for active surveillance （AS） of low-risk PCa. We identified 532 men who were treated with radical prostatectomy from 2006 to 2013 who met 4 or all 5 of the criteria for clinically insignificant PCa （clinical stage 〈 T1, prostate specific antigen [PSA] density 0.15, biopsy Gleason score 〈 6, number of positive biopsy cores 〈 2, and no core with 〉 50% involvement） and analyzed their pathologic and biochemical outcomes. Patients who met all 5 criteria for clinically insignificant PCa were designated as group A （n -- 172）, and those who met 4 of 5 criteria were designated as group B （n = 360）. The association of each criterion with adverse pathologic features was assessed via logistic regression analyses. Comparison of group A and B and also logistic regression analyses showed that PSA density 0.15 ng ml-1 and high （〉7） biopsy Gleason score were associated with adverse pathologic features. Higher （〉 Tlc） clinical stage was not associated with any adverse pathologic features. Although 〈 3 positive cores were not associated with any adverse pathology, 〉4 positive cores were associated with higher risk of extracapsular extension. Among potential candidates for AS, PSA density ~ 0.15 ng ml-~ and biopsy Gleason score ~ 6 pose significantly higher risks of harboring more aggressive disease. The eligibility criteria for AS may be expanded to include men with clinical stage T2 tumor and 3 positive cores.
