objectives To analyze the clinical profile,adequacy of treatment with rivaroxaban and outcomes in octogenarians with atrial fibrillation(AF),taking rivaroxaban in clinical *** observational and non-interventional study that included AF adults recruited from 79 Spanish centers,anticoagulated with rivaroxaban ≥ 6 months before being *** were analyzed according to age(≥ 80 vs.rs) at *** out of 1433 patients,453(31.6%) were octogenarians at *** to younger patients,octogenarians had more comorbidities,higher CHA2DS2-VASc(4.5 ± 1.3 vs.3.0 ± 1.4;P ores(2.0 ± 1.0 vs.1.4 ± 1.0;P overall,the dose of rivaroxaban was adequately prescribed in 83.4% of patients,but more frequently in the younger population(71.1% vs.89.1%;P = 0.039).After a mean follow-up of 2.2 ± 0.6 years,annual rates of stroke + systemic embolism + transient ischemic attack,MACE,cardiovascular death and major bleeding were 1.03%,1.24%,1.03% and 1.75%,respectively,in octogenarian *** for progressive heart failure death and major bleeding,rates of outcomes in octogenarians were similar compared to younger *** octogenarians,the concomitant use of antiplatelet agents and non-severe dementia were independently associated with the development of ischemic stroke,whereas previous coronary revascularization and heart failure with MACE,and higher HAS-BLED score with major *** In clinical practice,around one third of patients taking rivaroxaban are *** patients have many comorbidities and a high thromboembolic *** that,rates of adverse events remain *** is adequately prescribed in the majority of octogenarians.

Intimate partner violence(IPV)is simultaneously assumed as a ser.ous crime and a major public health issue,having recurrences as one of its main characteristics and,consequently,re-entries of some alleged offenders in the criminal justice system(CJS).The main goal of this study is to assess if in cases of female victims of IPV,violence decreases after the first entry of the alleged offender in the CJS.A retrospective study was per.ormed based on the analysis of police reports of alleged cases of IPV during a 4-year *** final sample(n=1488)was divided into two groups according to the number.of entries in the CJS(single or multiple)followed by a comparative *** suggest that violence decreases after the first entry of alleged offenders in the ***-entries were found in only 15.5%of the cases but they were accountable for 3.3 times more crimes on ***,victims of recidivism presented more injuries and required more medical ***,a small group of alleged offenders seems to be more violent and accountable for most of the IPV crimes registered in the CJS suggesting that regardless of legal sanctions aiming to deter violence,these measures may not be enough for a certain group of *** study sustains the need for a predictive model to quantify the risk of repeated IPV cases within the Portuguese population.

objective To evaluate immediate transcatheter aortic valve implantation （TAVI） results and medium-term follow-up in very elderly pa- tients with severe and symptomatic aortic stenosis （AS）. Methods This multicenter, observational and prospective study was carried out in three hospitals. We included consecutive very elderly （〉 85 years） patients with severe AS treated by TAVI. The primary endpoint was to evaluate death rotes from any cause at two years. results The study included 160 consecutive patients with a mean age of 87 ± 2.1 years （range from 85 to 94 years） and a mean logistic EuroSCorE of 18.8% ± 11.2% with 57 （35.6%） patients scoring 〉 20%. Procedural success rate was 97.5%, with 25 （15.6%） patients experiencing acute complications with major bleeding （the most frequent）. Global mortality rate during hospitalization was 8.8% （n = 14） and 30-day mortality rate was 10% （n = 16）. Median follow up per.od was 252.24± 232.17 days. During the follow-up per.od, 28 （17.5%） patients died （17 of them due to cardiac causes）. The estimated two year.overall and cardiac survival rates using the Kaplan-Meier method were 71% and 86.4%, respectively. Cox proportional hazard regression showed that the variable EuroSCorE ≥ 20 was the unique vari- able associated with overall mortality. Conclusions TAVI is safe and effective in a selected population of very elderly patients. our findings support the adoption of this new procedure in this complex group of patients.

Background::The burden of human immunodeficiency virus(HIV)infection in people who use drugs(PWUD)is *** aimed to screen HIV infection among PWUD and describe their retention in HIV ***,we also screen for hepatitis C virus(HCV)infection among HIV-seropositive PWUD and describe their linkage to ***::We conducted a prospective study in 529 PWUD who visited the"Ca?ada real Galiana"(Madrid,Spain).The study per.od was from June 1,2017,to May 31,*** diagnosis was per.ormed with a rapid antibody screening test at the point-of-care(PoC)and HCV diagnosis with immunoassay and PCr tests on dried blood spot(DBS)in a central *** PWUD were referred to the *** used the Chi-square or Fisher’s exact tests,as appropriate,to compare rates between ***::Thirty-five(6.6%)participants were positive HIV antibodies,but 34 reported previous HIV diagnoses,and 27(76%)had pr.or antiretroviral *** patients with a positive HIV antibody test,we also found a higher prevalence of homeless(Pon drug use(PWID)(Pmore decades of drug use(P=0.002).All participants received HIV test results at the *** the 35 HIV positives,28(80%)were retained in HIV medical care at the end of the HIV screening study(2018),and only 22(62.9%)at the end of ***,12/35(34.3%)were positive for the HCV rNA *** the latter,10/12(83.3%)were contacted to deliver the HCV results test(delivery time of 19 days),5/12(41.7%)had an appointment and were attended at the hospital and started HCV therapy,and only 4/12(33.3%)cleared ***::We found almost no new HIV-infected PWUD,but their cascade of HIV care was low and remains a challenge in this population at *** high frequency of active hepatitis C in HIV-infected PWUD reflects the need for HCV screening and reinforcing the link to care.

We describe the development of quinolylnitrones(QNs)as multifunctional ligands inhibiting cholinesterases(Ches:acetylcholinesterase and butyrylcholinesterase—h BChE)and monoamine oxidases(hMAo-A/B)for the therapy of neurodegenerative *** identified QN 19,a simple,low molecular weight nitrone,that is readily synthesized from commercially available *** 19 has no typical pharmacophoric element to suggest ChE or MAo inhibition,yet unexpectedly showed potent inhibition of h BChE(IC50=1.06±0.31 nmol/L)and h MAo-B(IC_(50)=4.46±0.18μmol/L).The crystal structures of 19 with hBChE and hMAo-B provided the structural basis for potent binding,which was further studied by enzyme *** 19 acted as a free radical scavenger and biometal chelator,crossed the blood—brain barrier,was not cytotoxic,and showed neuroprotective properties in a 6-hydroxydopamine cell model of Parkinson's *** addition,in vivo studies showed the anti-amnesic effect of 19 in the scopolamine-induced mouse model of AD without adverse effects on motoric function and ***,chronic treatment of double transgenic APPswe-PS1δE9 mice with 19 reduced amyloid plaque load in the hippocampus and cortex of female mice,underscoring the disease-modifying effect of QN 19.
