Severe Traumatic Brain Injury with Sphenopalatine Artery Ruptured: Case Report and Review of the Literature

Emergency endovascular procedure for external carotid exclusion is required to save patients with life-threatening massive epistaxis from a ruptured sphenopalatine artery secondary to severe traumatic brain injury. We report a case of a 20-year-old pedestrian admitted with a severe traumatic brain injury (TBI) secondary to an automobile (a lorry) road accident at high velocity. He presented to the emergency room in a coma with a Glasgow coma scale of 6/15 and massive epistaxis. He underwent emergency stabilization by sedation and was intubated. Body CT-Scan shows fracture of the palatine and pterygoid plate and multiple fractures of the skull with intracranial right frontal lobe hematoma. CT-Angiography was done immediately and confirmed a rupture of the sphenopalatine artery. The patient died of massive epistaxis while waiting for the endovascular procedure. This is a rare clinical case that needs unusual emergency endovascular management. Arterial embolization in emergency settings should be thought in front of any patient with persistent post-traumatic nose bleeding and be carried out as soon as possible to save the patient’s life.

Railway accident prevention and protection are a key part of a wider picture of transport safety. The rail sector thus needs to improve its knowledge of trespassing and suicide, including at level crossing, in order to work out suitable responses by analyzing measures already taken in various countries, Governments, the rail industry and road organizations have been implementing a variety of countermeasures for many years to improve railway safety. These actions are substantial and have resulted in a continuing decrease in the number and the severity of accidents. This paper presents existing suitable techniques that are used in the preventative measures targeted to reduce railway suicides, trespassing and level crossing user accidents. It describes them in terms of their capability to effectively reduce accidents, their cost-effectiveness and their integration within the railway transport system as a whole.

Pancreaticoduodenectomy in the elderly over 80 years:a case report

BACKGROUND: An increasing number of elderly patients have been considered for major surgical procedures, such as pancreaticoduodenectomy. The decision to recommend this operation for localized pancreatic cancer or other periampullary process in a very elderly patient is complicated by the frailty of the patient and the poor prognosis of the disease. Moreover, increased surgical experience associated with better patient selection may reduce the mortality rate, even in very elderly patients (over 80 years of age), after pancreaticoduodenectomy. METHODS: An 84-year-old woman underwent pancreatico- duodenectomy for ampullary adenocarcinoma. The tumor was classified pT3N0M0. RESULT: A good postoperative outcome was obtained. The patient is still alive, 18 months after operation. CONCLUSIONS: Radical resection of periampullary tumors is safe in selected patients of advanced age, with morbidity and mortality rates approaching those observed in younger patients. Age alone should not be a contraindication for pancreatic resection.

The modem tramway has resurfaced as the cure to today＇s urban transport problems such as pollution, road congestion and uneven access to transit. However, trams at intersections often experience frequent and extended delays due to vehicles crossing the tram tracks. There is an increased potential for conflict between trams and vehicles at these locations and crashes are common. The question of the effects of human factors on trams crossroads safety has been little dealt with in literature. The general aim of this article is to further knowledge about the influence of tramway and surrounding environment on car＇s driver behavior at intersections. Understanding these influences, involve conducting a systematic review of the cognitive tasks related to driving and identifying the hazards that can arise at each task, and what factors can make these more or less likely to arise, considering the environmental design at intersections and behavioral factors. To achieve that, the HAZOP （Hazard and Operability Study） approach is conducted for this study. Concerning data collection, the methodology includes site visits to record user behavior and questionnaires to determine the opinion, concerns and knowledge of car drivers in interaction with the tram environment.

The Mediterranean durum wheat landraces are genetically diverse and important sources for improving resistance to abiotic and biotic stresses and developing adapted and productive durum wheat varieties in the Mediterranean region. To study the diversity two distant countries (MoroccoandSyria) durum landraces were studied. Fifty-one microsatellites were used as molecular markers tool to determine the genetic structure and spatial adaptation of these landraces. We used two spatially-explicit methods (Bayesian and Eigen) to determine the genetic diversity and structure of a population composed of Moroccan (98) and Syrian (90) durum wheat landraces. Non-spatial methods were also applied for comparison. A significant genetic difference was detected between the landraces originated from Morocco and Syria. Six subpopulations were revealed for each country using the Bayesian method and the Eigenanalysis, which generated PC1 and sPC1, showed similar structure. Eigenanalysis exhibited a significant global genetic structure for both countries landraces;and showed that neighboring landraces tend to have close genetic profile. The two first axes of PC1 and sPC1 had discriminated four out of the six subpopulations revealed by the Bayesian methodology. Also, our study detected the close relationship between the durum landraces from the coastal areas of Syria and the Moroccan landraces from the Atlantic coastal regions where the Phoenicians/Carthaginians had settled in Morocco. These results demonstrate the importance of using the spatial models in genetic analysis of durum wheat landraces;and also recommend the use of the easily usable Eigenanalysis to analyze the genetic diversity and structure.

Composition, Stability and Probable Structure of a Colourless Organometallic Complex (Gd(III)-Malic Acid)

The formation of colourless gadolinium complexes (x,y,z), between x gadolinium ions, y ligands and z protons, of some organic acids, has been studied in aqueous solution. In this work we present the results of investigations on the interaction of the gadolinium ion (Gd3+) with malic acid (C4H6O5, a-hydroxyl dicarboxylic acid), in dilute aqueous solution for pH values between 5.5 and 7.5. Colourless gadolinium complexes of malate ions have no absorption band UV-visible, the indirect photometric detection (IPD) technique was used and studies have identified a major tri-nuclear complex of malate ion (﹣OOC-CH2-CHOH-COO﹣). The formation of this new colourless complex is derived from three Gd(III) ions that react with two malate ions and two hydronium ions (H3O+), giving for this colourless complex, a (3,2,2) composition and apparent stability constant depends on the acidity of the medium, with logK'322 = 18.88 ± 0.05 at pH = 6.30. To complement previous results and to propose a probable structure for this new complex detected in solution, studies of IR spectroscopy have been conducted to identify the chelation sites for both ligands. The results were analysed and show that this organometallic gadolinium complex, contains two different sites, respectively, two lateral tetradentate mono-nuclear sites and a single central bidentate mono-nuclear site. From these results, the reaction of formation, the stability constant and the probable structure of this new colourless organometallic gadolinium complex are proposed.

Methyl Cholate and Resorcinarene New Carriers for the Recovery of Cr(Ⅲ) Ions by Supported Liquid Membranes (SLM)s

The technique of supported liquid membranes was used to achieve the facilitated transport of Cr(III) ions, using tow amphiphilic carriers, the methyl cholate and resorcinarene. For prepared SLMs, toluene as organic phase and film of polyvinylidene difluoride, as hydrophobic polymer support with 100 μm in thickness and 0.45 μm as the diameter of the pores. The macroscopic parameters (P and J0) on the transport of these ions were determined for different medium temperatures. For these different environments, the prepared SLMs were highly permeable and a clear evolution of these parameters was observed. The parameter J0 depended on the temperature according to the Arrhenius equation. The activation parameters, Ea, ΔH≠ and ΔS≠, for the transition state on the reaction of complex formation (ST) , were determined. To explain these results for this phenomenon, and achieve a better extraction of the substrate, a model based on the substrate complexation by the carrier and the diffusion of the formed complex (ST) was developed. The experimental results verify this model and determine the microscopic parameters (Kass and D*). These studies show that these parameters Kass and D* are specific to facilitated transport of Cr(III) ions by each of the carriers and they are changing significantly with temperature.

To control the stability of the lysine acetylsalicylate compound (LAS) in aqueous solution, some studies of the hydronium ion-catalyzed, hydroxide ion-catalyzed, and spontaneous reactions of this active ingredient in water solutions have been carried out. The pH-rate profile (log kobs = f(pH)), shows that the hydrolysis reaction of the LAS, is conducted by a catalysis acid-base mechanism, with multiple reaction pathways. The rate constants, kH, kOH and k0 to the reaction pathways catalyzed by H3O+, HO– ions and to the spontaneous reaction, for the hydrolysis reaction of the reagent LAS, were determined. The results show that the studied compound LAS is unstable in basic medium and the hydrolysis reaction catalyzed by HO– ions is *** a known acidity (pH ≈ 10), studies conducted for different temperatures of the medium, clearly indicate, that the experimental rate constant kobs,depends on the temperature according to the Arrhenius equation. The activation parameters: activation energy (Ea), enthalpy (ΔH≠) and entropy (ΔS≠), for the transition state were determined, The very negative value obtained for the activation parameter ΔS*, first indicates that in the transition state there is gain in order, then this late state, resembles the products and that probably for the mechanism of the LAS hydrolysis reaction catalyzed by HO– ions, the rate-determining step is a bimolecular reaction. Finally from all these results, the mechanism for the reaction pathway catalyzed by HO– ions has been elucidated.
