Efficacy of tolvaptan in patients with refractory ascites in a clinical setting

AIM: To elucidate the efficacies of tolvaptan(TLV) as a treatment for refractory ascites compared with conventional treatment. METHODS: We retrospectively enrolled 120 refractory ascites patients between January 1, 2009 and September 31, 2014. Sixty patients were treated with oral TLV at a starting dose of 3.75 mg/d in addition to sodium restriction(> 7 g/d), albumin infusion(10-20 g/wk), and standard diuretic therapy(20-60 mg/d furosemide and 25-50 mg/d spironolactone) and 60 patients with large volume paracentesis in addition to sodium restriction(less than 7 g/d), albumin infusion(10-20 g/wk), and standard diuretic therapy(20-120 mg/d furosemide and 25-150 mg/d spironolactone). Patient demographics and laboratory data, including liver function, were not matched due to the small number of patients. Continuous variables were analyzed by unpaired t-test or paired t-test. Fisher's exact test was applied in cases comparing two nominal variables. We analyzed factors affecting clinical outcomes using receiver operating characteristic curves and multivariate regression analysis. We also used multivariate Cox's proportional hazard regression analysis to elucidate the risk factors that contributed to the increased incidence of ***: TLV was effective in 38(63.3%) patients. The best cut-off values for urine output and reduced urine osmolality as measures of refractory ascites improvement were > 1800 mL within the first 24 h and > 30%, respectively. Multivariate regression analysis indicated that > 25% reduced urine osmolality [odds ratio(OR) = 20.7; P < 0.01] and positive hepatitis C viral antibodies(OR = 5.93; P = 0.05) were positively correlated with an improvement of refractory ascites, while the total bilirubin level per 1.0 mg/dL(OR = 0.57;P = 0.02) was negatively correlated with improvement. Effect of Initial Geometry on Surface Flatness after Arc Welding Analyzed with MPS Method

The flow behavior in a typical arc welding process is very complex and features rapid melting and solidification. In this work, the flow in the melted region was determined by the effect of surface tension, viscosity, and gravity. Two-dimensional change of the geometry of the melted region was analyzed using the MPS （moving particle semi-implicit） method to melt a small upper plate in a hole at the center of a base plate. The relation between the initial geometry of the upper plate and the surface flatness after welding was calculated. The calculation results showed that the upper plate with a triangular notch at its center minimized the surface unevenness after welding. The depth of notch and thickness of the upper plate were optimized.

PIV Studies on Turbulence Structure in Air/Water Interface with Wind-Induced Water Waves

When wind appears over the free surface, water waves and turbulence are generated by an interfacial shear stress. In particular, turbulent diffusion promotes significantly mass and momentum transport beneath the interface between the water and air significantly in ocean and lakes, and thus it is very important for global environment problems to reveal such turbulence property and coherent structure. Simultaneous measurements of velocities and free-surface elevation allow us to conduct reasonably the phase analysis of the coherent structure in interfacial shear layer. Furthermore, multi-point measurements such as PIV are very powerful to detect the space-time structure of coherent motions. Therefore, in the present study, we developed a specially designed PIV system which can measure the velocity components and surface-elevation fluctuation simultaneously by using two sets of high-speed cameras to reveal the coherent structure in the interfacial shear layer.

Gravity Survey along the Mizuho Traverse Routes, East Antarctica: SEAL Seismic Exploration in 1999-2000

A gravity survey was conducted along the Mizuho traverse routes in austral summer of 1999-2000 by the 41st Japanese Antarctic Research Expedition (JARE-41). The main purpose of the survey was to obtain detail gravity anomaly along the traverse routes, where deep seismic exploration was simultaneously conducted in order to obtain a fine crustal structure of the Mizuho Plateau. By using SCINTREX (CG-3M) gravity meter, the survey was carried out at 160 sites with about 1 km interval in a distance of 190 km from S16 to Z20 on the traverse routes. Free-air and Bouguer anomalies were calculated using precise locations by GPS measurements, by taking into account an effect of the thick icesheet. The furrowed negative Free-air anomalies are identified around H192, where middle points of the whole traverse routes. Two bedrock elevation models derived both by seismic refraction analyses and radio-echo sounding might correspond to the bottom and the top of a mixture layer composed from ice-moraine rocks over the bedrock surface. Thickness of the crust estimated by Bouguer anomalies is 0.5 - 1 km larger toward inland at the terminal point of Z20 along the profile.

蠕变寿命是影响镍基单晶高温合金材料服役寿命和力学性能的关键材料参数。因此,如何准确有效地预测合金的蠕变寿命具有重要现实意义。尽管多年来许多研究学者已经建立起多种蠕变寿命的预测模型,但是由于不同温度应力下的蠕变机制复杂且蠕变过程涉及长时间的显微组织演化,已有模型尚难以实现有效预测。对此,采用物理冶金原理指导下的数据挖掘结合机器学习这一研究策略,通过文献调研建立起了高温低应力下的镍基单晶合金的高质量蠕变数据集,在物理冶金原理指导下对原始数据进行挖掘,提高了原始数据的内在质量,并基于Pearson系数和随机森林平均精确度降低值分别对原始数据特征进行了相关性分析和重要性评估,表明所建立的数据集符合基本的物理冶金学机制,同时阐明了引入的三维物理冶金信息对于蠕变寿命预测的重要意义。随后,基于机器学习方法在数据挖掘后的数据集上对合金的蠕变寿命进行了预测,并根据平方相关系数(R~2)、平均绝对误差(MAE)和过拟合程度评估了不同的机器学习模型。结果表明,支持向量回归(SVR)模型在本研究中具有较好的泛化能力且不容易过拟合,同时结合了物理冶金信息的机器学习模型拥有更好的预测准确性和泛化能力。最终成功地建立起了高温低应力下镍基单晶高温合金成分、工艺以及引入的物理冶金参数和蠕变寿命之间的关系,能够实现对镍基单晶高温合金蠕变寿命的有效预测,并有望应用于基于合金服役条件的成分工艺的反向设计。

Development of inhomogeneous deformation is an interest matter in material engineering. Synchrotron radiation tomography provides 3D distribution map of local strain in polycrystalline aluminum alloy by tracking microstructural features. To perform further deep analysis on development of inhomogeneous deformation, crystallographic grain orientation is necessary. Three-dimensional X-ray diffraction technique was developed. A new crystallographic orientation measurement method was described in 3D space, utilizing grain boundary tracking （GBT） information.

Gekko japonicus group species are common nocturnal geckos in Eastern *** than 25 species of Gekko are distributed in China,Japan,Vietnam,and Laos,and they have similar ecological and morphological *** works reported that the males of these species use acoustic signals in context of *** phylogeographic study revealed that some of them co-occur without causing hybridization,whereas some species hybridize with sympatric *** suggests that reproductive isolation occurs in the former species combinations,whereas that does not work in the later species *** investigated the role of the sexualcalls for species recognition in *** recording by 18 Gekko species revealed that 14 species have species-specific patterns in male sexual calls(named pattern calls),whereas four species did not have any pattern in its calls(named patternless calls).Results of molecular phylogenetic analysis revealed that the species-specific patterns in sexual call has lost probably three *** non-hybridizing species combinations,males of both species had pattern *** which each pattern call species,females of both species discriminated conspecific calls from the other’s *** the other hand,in hybridizing combinations,males of one species have a pattern call,but the other species have a patternless *** pattern call species discriminated conspecific calls from the other’s calls,but the patternless call species did not discriminate *** results suggested that the 14 Gekko species use the species-specific pattern calls for species identification,whereas other four species do not have such acoustic species-identification *** that the distribution area of patternless species is restricted to small islands,the species identification system by calls may be lost by geographic isolation in the islands,and the hybridization is caused by the loss of species recognition system in females of the patternless call

Improvement of prognosis for unresectable biliary tract cancer

AIM:To evaluate the chemotherapeutic outcomes and confirm the recent improvement of prognosis for unresectable biliary tract ***:A total of 186 consecutive patients with unresectable biliary tract cancer,who had been treated with chemotherapy between 2000 and 2009 at five institutions in Japan,were retrospectively *** patients were divided into three groups based on the year beginning chemotherapy:Group A(2000-2003),Group B(2004-2006),and Group C(2007-2009).The data were fixed at the end of December *** survival and time-to-progression were analyzed and compared ***:No patient characteristics were significantly different among the three *** gallbladder was involved in about half of the patients in each group,and metastatic biliary tract cancer was present in three quarters of the *** Group A,5-fluorouracilbased chemotherapies were primarily selected as firstline chemotherapy,and only 24% were treated with second-line *** Group B,gemcitabine or S-1 monotherapy was mainly introduced as firstline chemotherapy,and 51% of the patients who were refractory to first-line chemotherapy were treated with second-line chemotherapy mainly with *** Group C,the combination therapy with gemcitabine and S-1 was mainly chosen as first-line chemotherapy,and 53% of the patients refractory to first-line chemotherapy were treated with second-line chemotherapy mainly with combination *** median timeto-progressions were 4.4 mo,3.5 mo and 5.9 mo in Groups A,B and C,respectively(4.4 mo vs 3.5 mo vs 5.9 mo,P < 0.01).The median overall survivals were 7.1,7.3,and 11.7 mo in Groups A,B and C(7.1 mo vs 7.3 mo vs 11.7 mo,P = 0.03).Induction rates of all three drugs(gemcitabine,platinum analogs,and fluoropyrimidine) in Groups A,B and C were 4%,2% and 27%(4% vs 2% vs 27%,P < 0.01).CONCLUSION:The prognosis of unresectable biliary tract cancer has improved *** three effective drugs(gemcitabine,p

This paper presents an H control scheme for gasoline engines based on a mean-value model. First, a linearized model based on a mean-valve nonlinear model of the engines is proposed that represents dynamical behavior under the multiinput excitation of the fuel injection and the throttle opening around an operating condition. Then, for the feedback control system to regulate of the speed and the air-fuel ratio, the mixed sensitivity function shaping method is applied to the feedback controller design. Furthermore, the parameter identification of the linearized model is shown with experiment results obtained on a full-scale engine transient control testbench. With the identified model, the frequency property and the time response of the control system is demonstrated by numerical simulation.

EFFECTS OF ELECTROACUPUNCTURE ON CARNITINE IN SERUM AND BRAIN

Objective Electroacupuncture （EA） has been shown to have therapeutic effect on chronic fatigue syndrome. But its action has not been made clear. Methods After the rats of EA group were fixed in an animal cage, bilateral Shènshū （肾俞 BL23） were punctured and stimulated with uniform reinforcing and reducing method by twirling the acupuncture needle for one minute. And then the needle handles were connected to an electric stimulator for stimulating the acupoint with dense-sparse waves, frequency of 3 Hz for 30 minutes, and amplitude： positive： 50 V, negative： 25 V. Electrocupuncture was given once a day, continuously for 5 days. The rats of normal control group were not punctured and stimulated. The rats of all groups were killed for collecting blood and brain tissue on the next day after the final treatment. Carnitine in serum and brain tissue was determined. Results Oarnitine contents in serum of EA group and of normal control group were 5.10±0.50 μmol/L and 2.17±0.46 μmol/L, respectively. Oarnitine contents in brain tissue of elctroacupuncture group and of normal control group were 44.66 ± 2.67 μmol/L and 24.05 ± 3.65 μmol/L, respectively. Oarnitine levels in serum and brain of EA group were significantly higher than those of normal control group. （P〈0.001） Conclusion This report presented that carnitine in serum and brain is increased by EA, suggesting that EA may affect energy metabolism and may have effects on fatigue.
