Future Long-Term Care Setting Preferences and Related Factors among Japanese Middle-Aged and Older People Living with HIV

Since the introduction of antiretroviral therapy, the life expectancy of people living with human immunodeficiency virus (PLWH) has extended. This extension has led to an increase in the aging population in Japan and globally. Providing appropriate long-term care (LTC) for PLWH has thus become increasingly critical. Our study aimed to describe LTC setting preferences and related factors among middle-aged and older Japanese people living with HIV. A cross-sectional survey was conducted at two hospitals in Tokyo. One hundred seventy-five outpatients aged 40 years and above participated in this study. Participants completed an anonymous self-administered questionnaire to assess where they wanted to live once they could no longer care for themselves. Approximately 52.0% preferred a designated facility for older adults or LTC, while 30.3% preferred their home or living with family, a partner, or a friend (“familiar housing”). Bivariate analyses revealed that LTC setting preference was significantly associated with marital status, whether or not the participant had at least one child, and household composition. Furthermore, logistic regression analysis revealed that participants living with non-kin were less likely to prefer living in designated housing facilities for older adults or LTC (adjusted odds ratio = 0.17, 95% confidence interval: 0.05 - 0.63). The study findings suggest that family make-up and composition of cohabiters are critical indicators for LTC setting preference in this population. These findings can be the foundation for future care planning and delivery to meet the unique LTC needs and expectations of the aging population with HIV in Japan and similar global settings.

Intranasal administration of stem cell-derived exosomes for central nervous system diseases

Exosomes, lipid bilayer-enclosed small cellular vesicles, are actively secreted by various cells and play crucial roles in intercellular communication. These nanosized vesicles transport internalized proteins, mRNA, miRNA, and other bioactive molecules. Recent findings have provided compelling evidence that exosomes derived from stem cells hold great promise as a therapeutic modality for central nervous system disorders. These exosomes exhibit multifaceted properties including antiapoptotic, anti-inflammatory, neurogenic, and vasculogenic effects. Furthermore, exosomes offer several advantages over stem cell therapy, such as high preservation capacity, low immunogenicity, the ability to traverse the blood-brain barrier, and the potential for drug encapsulation. Consequently, researchers have turned their attention to exosomes as a novel therapeutic avenue. Nonetheless, akin to the limitations of stem cell treatment, the limited accumulation of exosomes in the injured brain poses a challenge to their clinical application. To overcome this hurdle, intranasal administration has emerged as a non-invasive and efficacious route for delivering drugs to the central nervous system. By exploiting the olfactory and trigeminal nerve axons, this approach enables the direct transport of therapeutics to the brain while bypassing the blood-brain barrier. Notably, exosomes, owing to their small size, can readily access the nerve pathways using this method. As a result, intranasal administration has gained increasing recognition as an optimal therapeutic strategy for exosomebased treatments. In this comprehensive review, we aim to provide an overview of both basic and clinical research studies investigating the intranasal administration of exosomes for the treatment of central nervous system diseases. Furthermore, we elucidate the underlying therapeutic mechanisms and offer insights into the prospect of this approach.

To develop porous organic frameworks,precise control of the stacking manner of two-dimensional porous motifs and structural characterization of the resultant framework are *** these points of view,porous molecular crystals formed through reversible intermolecular hydrogen bonds,such as;hydrogen-bonded organic frameworks(HOFs),can provide deep insight because of their high crystallinity,affording single-crystalline X-ray diffraction *** this study,we demonstrate that the stacking manner of hydrogen-bonded hexagonal network(HexNet)sheets can be controlled by synchronizing a homological triangular macrocyclic tecton and a hydrogen-bonded cyclic supramolecular synthon called the phenylene triangle.A structure of the resultant HOF was crystallographically characterized and revealed to have a large channel aperture of 2.4 *** HOF also shows thermal stability up to 290°C,which is higher than that of the conventional HexNet frameworks.

日本具有悠久的学习和使用汉语的历史,并且一直致力于推广汉语教育,这一点与其他国家相比尤其特殊。自中国汉朝开始,日本就着力推进汉语教育,而明治时期出版的教材《亚细亚言语集》,是日本第一部北京官方语言的教材,由日本学者广部精编写,它对日本汉语教育的发展起到了极其重要的作用。中国的改革开放在日本也快速掀起了一股“汉语热”。尽管日本本土出版了众多版本的汉语教材,但是缺乏具有权威性的汉语教材,而中国编辑出版的对日汉语教材在针对性方面又值得商榷。目前,针对对日汉语教材的比较研究寥寥无几,各种版本的日本汉语教材各自的特色与优缺点无法体现。因此有必要对这些教材进行比较研究,为编写具有针对性的对日汉语教材提供依据,以满足如此众多汉语学习者,特别是青少年汉语学习者的需求。本文基于实用性、针对性、趣味性、丰富性等教材编写理论,利用文献资料法、数据统计法与比较归纳法等方法,从语音、词汇、语法、汉字、练习、话题、文化、插图和课文等多个方面入手,对两套具有代表性的对日汉语教材《高校生からの中国語》(从高中学起的汉语)与《跟我学汉语》(日语版)进行全面的量化比较研究,揭示两套教材在选材、编写理念、文化内涵、侧重点等方面的异同,获得关于两套教材内容逐项量化统计与比较分析结果,佐证两套教材存在的优缺点,指出两部教材的互补性,并提出了相应的建议。希望本文的研究结果将有助于教材编写者,结合日本文化背景以及日本人的思维习惯,编写具有针对性的高质量的初级对日汉语教材。

BACKGROUND Duodenal Brunner's gland hyperplasia(BGH)is a therapeutic target when complications such as bleeding or gastrointestinal obstruction occur or when malignancy cannot be ruled ***,we present a case of large BGH treated with endoscopic mucosal resection(EMR).CASE SUMMARY An 83-year-old woman presented at our hospital with *** tests revealed severe anemia,esophagogastroduodenoscopy showed a 6.5 cm lesion protruding from the anterior wall of the duodenal bulb,and biopsy revealed the presence of glandular *** ultrasonography(EUS)demonstr-ated relatively high echogenicity with a cystic *** muscularis propria was slightly elevated at the base of the *** was performed without *** formalin-fixed lesion size was 6 cm×3.5 cm×3 cm,showing nodular proliferation of non-dysplastic Brunner's glands compartmentalized by fibrous septa,confirming the diagnosis of *** of EMR or hot snare polypectomy are rare for duodenal BGH>6 *** this case,the choice of EMR was made by obtaining information on the base of the lesion as well as on the internal characteristics through *** Large duodenal lesions with good endoscopic maneuverability and no evident muscular layer involvement on EUS may be resectable via EMR.

Quest towards ultimate performance in superconducting nanowire single photon detectors

In 2007, superconducting nanowire single photon detectors （SSPD or SNSPD） [ 1 ] made an outstanding impact in the field of quantum information technology by demonstrating quan- tum key distribution （QKD） over a 200-kin optical fiber with a 42-dB optical loss using a practical SNSPD system [2]. This successful demonstration was realized thanks to its extremely low dark count rate （DCR） of a few Hz and short timing jitter of 60 ps, while the system detection efficiency （SDE） showed a poor value of 0.7% at a wavelength of 1550 nm.

Activity coefficients of NiO and CoO in CaO–Al_2O_3–SiO_2 slag and their application to the recycling of Ni–Co–Fe-based end-of-life superalloys via remelting

To design optimal pyrometallurgical processes for nickel and cobalt recycling, and more particularly for the end-of-life process of Ni-Co-Fe-based end-of-life（EoL） superalloys, knowledge of their activity coefficients in slags is essential. In this study, the activity coefficients of NiO and CoO in CaO-Al_2O_3-SiO_2 slag, a candidate slag used for the EoL superalloy remelting process, were measured using gas/slag/metal equilibrium experiments. These activity coefficients were then used to consider the recycling efficiency of nickel and cobalt by remelting EoL superalloys using CaO-Al_2O_3-SiO_2 slag. The activity coefficients of NiO and CoO in CaO-Al_2O_3-SiO_2 slag both show a positive deviation from Raoult＇s law, with values that vary from 1 to 5 depending on the change in basicity. The activity coefficients of NiO and CoO peak in the slag with a composition near B =（%CaO）/（%SiO_2） = 1, where B is the basicity. We observed that controlling the slag composition at approximately B = 1 effectively reduces the cobalt and nickel oxidation losses and promotes the oxidation removal of iron during the remelting process of EoL superalloys.

A facile method for preparation of uniformly decorated-spherical SnO2 by CuO nanoparticles for highly responsive toluene detection at high temperature

We reported a facile preparation of a uniform decoration of spherical n-type SnO2 by p-type CuO nanopa rticles as well as their utilization for enhanced performance on toluene gas *** nanoparticles and spherical SnO2 were synthesized by a facile non-hydrolytic solvothermal reaction,which could easily control their morphology.A uniform CuO nanoparticles decoration onto spherical SnO2 was achieved by a simple sonication and vigorous stirring at room tempe *** revealed orga nic solvents used in the oxide synthesis had a considerable influence on its surface charge that was beneficial for a uniformly electrostatic self-decoration between positively charged p-type CuO nanoparticles and negatively charged n-type spherical ***,CuO was partially reduced to Cu metal during high concentration of toluene exposure destroying p-n contact and developing new metal-semiconductor contact so-called ohmic junction,resulting in extraordinarily responsive and selective to toluene gas at 400℃as compared to a single p-CuO and *** was also found that the amount of particle decoration had an influence on sensor response and *** optimum amount of CuO nanoparticle decoration was0.1 mmol on 0.5 mmol *** re s ponse(S=Ra/Rg)and selectivity of CuO/S nO2 based material toward the exposure of 75 ppm toluene had reached to such high as 540 and 5,*** effect of p-n heterojunction and metal-semiconductor contact on the gas sensing mechanism of p-type CuO/n-type SnO2 was ***,by decorating with CuO nanoparticles,CuO/SnO2 morphology was well-maintained after gas sensing evaluation demonstrated its excellency for high temperature toluene gas sensor application.

AIM： To investigate the role of major non-protein and protein sulfhydryls and disulfides in chemically induced gastric hemorrhagic mucosal lesions （HML） and the mechanism of gastroprotective effect of sucralfate.METHODS： Rats were given 1 mL of 75% ethanol, 25%NaCl, 0.6 mol/L HCI, 0.2 mol/L NaOH or 1% ammonia solutions intragastrically （i.g.） and sacrificed 1, 3, 6 or 12 min later. Total （reduced and oxidized） glutathione （GSH ＋ GSSG）, glutathione disulfide （GSSG）, protein free sulfhydryls （PSH）, protein-glutathione mixed disulfides （PSSG） and protein cystine disulfides （PSSP） were measured in gastric mucosa and liver.RESULTS： Reduced glutathione （GSH） was depleted in the gastric mucosa after ethanol, HCI or NaCl exposure,while oxidized glutathione （GSSG） concentrations increased, except by HCI and NaOH exposure. Decreased levels of PSH after exposure to ethanol were observed,NaCl or NaOH while the total protein disulfides were increased. Ratios of reduced to oxidized glutathione or sulfhydrils to disulfides were decreased by all *** changes in thiol homeostasis were detected in the liver after i.g. abbreviation should be spelled out the first time here administration of ethanol. Sucralfate increased the concentrations of GSH and PSH and prevented the ethanol-induced changes in gastric mucosal thiol concentrations.CONCLUSION： Our modified methods are now suitable for direct measurements of major protein and nonprotein thiols/disulfides in the gastric mucosa or liver.A common element in the pathogenesis of chemically induced HML and in the mechanism of gastroprotective drugs seems to be the decreased ratios of reduced and oxidized glutathione as well as protein sulfhydryls and disulfides.
