Professional Domiciliary Oral Care for Elderly in Nursing Homes—A Randomized Controlled Pilot Trial

Domiciliary dental care (DDC) makes regular dental visits possible for people with different functional limitations who otherwise would not be able to access a dental clinic. DDC also facilitates cooperation with nursing staff. To our knowledge, the effect of DDC on oral health among elderly people in nursing homes has not been studied. As part of a project to establish recommendations for DDC, the aim of this randomized, controlled, single-blind, pilot trial with three parallel arms was to study the effect of professionally conducted oral care intervention, in domiciliary setting, on gingival bleeding on probing, dental plaque and oral mucosa. For three months, the participants were given either A) monthly professional DDC with oral care intervention, B) monthly individual oral care instructions, or C) oral care as usual. A total of 102 (n = 107) individuals completed the study. After three months, gingival bleeding on probing was significantly reduced among more participants in groups A and B compared with group C (p < 0.0004). Dental plaque scores changed significantly over three months for group B compared with group C (p < 0.04). Mucosal-plaque scores were significantly reduced in group A compared with both group B and C (p < 0.0001). All other intergroup differences were statistically non-significant. Compared with care as usual, professional DDC has the most favourable effect on gingival bleeding, whereas individual oral care instructions result in the highest reduction of dental plaque. Both professional oral care interventions and individual oral-hygiene instructions should be included in a domiciliary oralcare programme.

Planetary defense mission concepts for disrupting/pulverizing hazardous asteroids with short warning time

This paper presents an overview of space mission concepts for disrupting or pulverizing hazardous asteroids, especially with warning time shorter than approximately 10 years. An innovative mission concept, referred to as a nuclear hypervelocity asteroid intercept vehicle (HAIV) system, employs both a kinetic-energy impactor and nuclear explosive devices. A new mission concept of exploiting a multiple kinetic-energy impactor vehicle (MKIV) system that doesn’t employ nuclear explosives is proposed in this paper, especially for asteroids smaller than approximately 150 m in diameter. The multiple shock wave interaction effect on disrupting or pulverizing a small asteroid is discussed using hydrodynamic simulation results. A multi-target terminal guidance problem and a planetary defense mission design employing a heavy-lift launch vehicle are also brie y discussed in support of the new non-nuclear MKIV mission concept. The nuclear HAIV and non-nuclear MKIV systems complement to each other to effectively mitigate the various asteroid impact threats with short warning time.

Cardiac evaluation of liver transplant candidates

Physicians previously thought that heart disease was rare in patients with end stage liver disease. However, recent evidence shows that the prevalence of ischemic heart disease and cardiomyopathy is increased in transplant candidates compared to most other surgical candidates. Investigators estimate that up to 26% of all liver transplant candidates have at least one critical coronary artery stenosis and that at least half of these patients will die perioperatively of cardiac complications. Cardiomyopathy also occurs in greater frequency. While all patients with advanced cardiac disease have defects in cardiac performance, a larger than expected number of patients have classical findings of dilated, restrictive and hypertropic cardiomyopathy. This may explain why up to 56% of patients suffer from hypoxemia due to pulmonary edema following transplant surgery. There is considerable controversy on how to screen transplant candidates for the presence of heart disease. Questions focus upon, which patients should be screened and what tests should be used. This review examines screening strategies for transplant candidates and details the prognostic value of common tests used to identify ischemic heart disease. We also review the physiological consequences of cardiomyopathy in transplant candidates and explore the specific syndrome of "cirrhotic cardiomyopathy".

More than Target 6.3： A Systems Approach to Rethinking Sustainable Development Goals in a Resource-Scarce World

The 2030 Agenda for Sustainable Development outlines 17 individual Sustainable Development Goals （SDGs） that guide the needs of practice for many professional disciplines around the world, including engineering, research, policy, and development. The SDGs represent commitments to reduce poverty, hunger, ill health, gender inequality, environmental degradation, and lack of access to clean water and sanitation. If a typical reductionist approach is employed to address and optimize individual goals, it may lead to a failure in technological, policy, or managerial development interventions through unin- tended consequences in other goals. This study uses a systems approach to understand the fundamental dynamics between the SDGs in order to identify potential synergies and antagonisms. A conceptual system model was constructed to illustrate the causal relationships between SDGs, examine system structures using generic system archetypes, and identify leverage points to effectively influence intentional and minimize unintentional changes in the system. The structure of interactions among the SDGs reflects three archetypes of system behavior： Reinforcing Growth, Limits to Growth, and Growth and Underinvestment. The leverage points identified from the conceptual model are gender equality, sustainable management of water and sanitation, alternative resources, sustainable livelihood standards, and global partnerships. Such a conceptual system analysis of SDGs can enhance the likelihood that the development community will broaden its understanding of the potential synergistic benefits of their projects on resource management, environmental sustainability, and climate change. By linking the interactions and feedbacks of those projects with economic gains, women＇s empowerment, and educational equality, stakeholders can recognize holistic improvements that can be made to the quality of life of many of the world＇s poor.

利用高精度Paterson气体介质变形装置对(Mg0.5,Fe0.5)2SiO4和(Mg0.7,Fe0.3)2SiO4橄榄石集合体进行了高温三轴压缩变形试验。首先用人工合成了纯铁橄榄石Fe2SiO4,然后用合成的纯铁橄榄石与天然圣卡罗橄榄石按一定比例混合烧结合成了含铁量分别为50％和30％的富铁橄榄石集合体Fo50和Fo70。三轴压缩蠕变变形试验在温度1223-1473 K和围压 300 MPa的条件下进行,差应力为10-300 MPa,应变率为10-7-10-4s-1,每个试件的最大变形量为20％．利用三维非线性方法对蠕变数据进行拟合,得出的流动律中,Fo50应力指数为4．1,活化能为499kJ／mol;Fo70应力指数为4．3,活化能为510kJ／ mol。将实验结果与Fo90圣卡罗橄榄石集合体的强度进行对比,发现含铁量的增加使橄榄石的强度明显下降,Fo50橄榄石集合体比Fo70的粘性低一个量级,Fo70比Fo90的粘性低一个量级。从而得到了含铁量对橄榄石的粘性(强度)影响的初步试验结果。

Introduction:The Chesapeake Bay was once renowned for expansive meadows of submerged aquatic vegetation(SAV).However,only 10%of the original meadows *** restoration effortswill be complicated by accelerating climate change,including physiological stressors such as a predicted mean temperature increase of 2-6℃and a 50-160%increase in CO_(2)***:As the Chesapeake Bay begins to exhibit characteristics of a subtropical estuary,summer heat waves will become more frequent and *** alone would eventually eliminate eelgrass(Zostera marina)from the *** will favor native heat-tolerant species such as widgeon grass(Ruppia maritima)while facilitating colonization by non-native seagrasses(e.g.,Halodule spp.).Intensifying human activity will also fuel coastal zone acidification and the resulting high CO_(2)/low pH conditions may benefit SAV via a“CO_(2)fertilization effect.”Discussion:Acidification is known to offset the effects of thermal stress and may have similar effects in estuaries,assuming water clarity is sufficient to support CO_(2)-stimulated photosynthesis and plants are not overgrown by ***,coastal zone acidification is variable,driven mostly by local biological processes that may or may not always counterbalance the effects of regional *** precarious equipoise between two forces-thermal stress and acidification-will be critically important because it may ultimately determine the fate of cool-water plants such as Zostera marina in the Chesapeake ***:The combined impacts of warming,coastal zone acidification,water clarity,and overgrowth of competing algae will determine the fate of SAV communities in rapidly changing temperate estuaries.

The "five senses of success" in nursing students: Assessing first-year support engagement

Objectives: This study aimed to identify School and University support services available and accessed by nursing students transitioning into a university environment as many struggle to adjust to competing demands of personal commitments and expectations at ***: A mixed methods design was used,based on activity theory and Lizzio's Five Senses of Success frameworks as exploratory *** study was conducted amongst the first year cohort at one campus of the Bachelor of Nursing (BN) program in Queensland,*** initial baseline assessment of what University and School-based support services were on offer for students,and how the students interacted with the support services was *** was followed by a survey to identify awareness and access to support *** groups were then conducted to clarify the previous results and to determine engagement with these support ***: A randomly selected number of students (n =150) in the first-year 2014 cohort of the BN program (n =300) were included in this *** survey was completed by 54 students and three semistructured focus groups were *** analysis indicated that the support services in place were successful in reaching the majority of students and contributed to their sense of success at university,Specifically students identified that a whole cohort approach to support enhanced their transition to ***: Identifying lesser known services early in the first year will ensure that students are supported and encouraged to use all services,contributing to their sense of success at university.
