Multicenter Survey of the Isometric Lower Extremity Strength in Patients with Type 2 Diabetes (MUSCLE-std): Design and Study Protocol

Background: The lower extremity muscle strength has been shown to be weaker in patients with diabetes mellitus, primarily due to the effects of diabetic polyneuropathy, a common diabetic complication. With respect to assessments of the lower extremity muscle strength, reference values for the knee extension force in healthy Japanese subjects have been reported. The clinical significance of applying knee extension force in the assessment of efficacy of exercise therapy and patient education has also been demonstrated, and the use of an individualized approach in physical exercise based on the quantitative assessment of the lower extremity muscle strength is becoming one of the most important strategies for promoting effective rehabilitation. Methods: The study is a multicenter, cross-sectional investigation. We will measure the knee extension force and evaluate the diabetic polyneuropathy in patients with type 2 diabetes using uniform equipment and methodology at all centers. We will compare the knee extension force between the subjects with and without diabetic polyneuropathy and will investigate the associations between the knee extension force and both glycemic control and exercise habits. The trial registration is UMIN CTR (UMIN000002810). Discussion: The lower extremity muscle strength has been insufficiently investigated in Japanese patients with diabetes. This study was designed to establish reference values for the knee extension force in Japanese diabetic patients with and without diabetic polyneuropathy.

Synthesis of MgSiN<sub>2</sub>Powders from the Mg-Si System

Magnesium silicon nitride (MgSiN2) was synthesized without any additives under a nitrogen gas flow (200 mL/min) using a nitriding method. The effects of temperature and holding time on its purity and morphology were investigated. A single-phase MgSiN2 powder was obtained at 1350℃ for 1 h and 1250℃ for 11 h. However, the decomposition of MgSiN2 occurred at 1450℃, suggesting that the optimum temperature for the preparation of MgSiN2 from Mg-Si system was 1350℃. The phase purity, morphology, size of the product and elemental composition of the samples were detected by X-ray diffraction (XRD), scanning electron microscopy (SEM) and energy spectrometer (EDS), respectively. The evaporation of Mg and Si resulted in the formation of many voids in the blocky product. The temperature gradient promotes the growth of MgSiN2 on the surface of massive products along the tip. The concentration gradient of Mg and Si vapors in the void resulted in the columnar growth of MgSiN2.

Multi-Factorial Functional Analysis of the Patients with Reconstruction for Oral Carcinoma

This paper is to evaluate postoperative function which has undergone reconstruction with vascularized or pedicled flaps for the treatment of oral carcinomas. The subjects consisted of 22 cases of 17 male and 5 female patients, and their mean age was 55 years. The sites of reconstruction were the tongue in 12 cases, mandible in 10 cases. The reconstruction was performed using deltopectoral (DP) flaps in 6 cases, pectoralis major musculo cutaneous (PMMC) flaps in 3 cases, DP flap + PMMC flap in one case, forearm (FA) flaps in 8 cases and peroneal (P) flaps with fibula in 4 cases. Postoperative masticatory, speech, swallowing functions and facial appearance were evaluated by Multi-factorial analysis. All functional results seem to be related to the type of resection. By multi-factorial analysis, the type of mandibular resection, flap area, and anterior resection were related to the masticatory or speech function in tongue resection. In mandibular resection, masticatory function was subjected to age;speech function was subjected to type of mandibular resection and plate reconstruction. The facial appearance was subjected to flap area. These results suggested that postoperative function was not related to only one factor, but using the classification of each defect site as one of the factor of multi-factorial analysis was useful.

Narrow portion of the terminal choledochus is a cause of upstream biliary dilatation in patients with anomalous union of the pancreatic and biliary ducts

AIM: To clarify the pathogenesis of biliary dilatation associated with anomalous union of the pancreatic and biliary ducts (AUPBD).METHODS: Direct cholangiopancreatograms of 350 adult patients with or with suspicion of hepatobiliary or pancreatic disorders were reviewed. AUPBD was diagnosed cholangiopancreatographically, when the pancreaticobiliary ductal union was located above the narrow distal segment of the bile duct, which represents the action of the sphincter of Oddi. The narrow portion of the terminal choledochus was defined as symmetrical stricture of the common bile duct just above the pancreaticobiliary ductal ***: AUPBD was found in 36 patients. Among cholangiopancreatographic features, the narrow portion of the terminal choledochus was the most pathognomonic for AUPBD (accuracy, 98%); it was present in 29 (81%)patients with AUPBD, but was not found in any patients without AUPBD. Among patients with AUPBD, biliary dilatation (＞10 mm) was more frequent in those with the narrow portion of the terminal choledochus (23/29)than in those without (2/7; P = 0.018) AUPBD. Among the patients with both AUPBD and the narrow portion of the terminal choledochus, there was a strong negative correlation between the minimum diameter of the narrow portion and the maximum diameter of the choledochus (r = -0.78, P＜0.001), suggesting that the degree of biliary narrowing at the narrow portion correlates with that of upstream biliary ***: The narrow portion of the terminal choledochus, a pathognomonic radiologic feature of AUPBD, may be a cause of biliary dilatation in patients with AUPBD.

Significance of a novel sucrose permeability test using serum in the diagnosis of early gastric cancer

AIM: To investigate the usefulness of sucrose permeability test using serum in the diagnosis of gastric diseases, with special reference to early gastric cancer (EGC).METHODS: A total of 63 subjects, including 11patients with gastric ulcer, 20 patients with gastric cancer (13, early; 7, advanced) and 32 healthy controls,were studied. Blood and urine samples were collected repeatedly for 5 h before and after the sucrose *** levels were measured by a newly developed enzymatic ***: Serum sucrose levels started to increase15 min after loading, and peaked at 60 min in the gastric disease groups. The levels for gastric ulcer, EGC and advanced gastric cancer (AGC) at 60 min were significantly higher than that in the healthy controls(26.9±2.4, 34.4±5.0, and 71.8±15.6 vs 7.9±0.7 mol/L,respectively, P＜0.01). The cut-off level set at 15.4 mol/L(60 min) offered the best distinction between EGC patients and healthy controls; and the sensitivity and specificity were 92.3% and 93.8%, respectively, while those of the urine method were 76.9% and 93.8%,***: The gastric permeability test using serum is reliable for the detection of EGC, and this test can provide results much earlier than the conventional urine method. This test may offer a useful alternative to more invasive tests for EGC.

Catheter tract implantation metastases associated with percutaneous biliary drainage for extrahepatic cholangiocarcinoma

AIM: To estimate the incidence of catheter tract implantation metastasis among patients undergoing percutaneous transhepatic biliary drainage (PTBD) for extrahepatic cholangiocarcinoma, and to provide data regarding the management of this unusual complication of PTBD by reviewing cases reported in the ***: A retrospective analysis of 67 consecutive patients who underwent PTBD before the resection of extrahepatic cholangiocarcinoma was conducted. The median follow-up period after PTBD was 106 mo. The English language literature (PubMed, National Library of Medicine, Bethesda, MD, USA), from January 1966through December 2004, was ***: Catheter tract implantation metastasis developed in three patients. The cumulative incidence of implantation metastasis reached a plateau (6%)at 20 mo after PTBD. All of the three patients with implantation metastasis died of tumor progression at 3, 9, and 20 mo after the detection of this complication. Among the 10 reported patients with catheter tract implantation metastasis from extrahepatic cholangiocarcinoma (including our three patients), two survived for more than 5 years after the excision of isolated catheter tract ***: Catheter tract implantation metastasis is not a rare complication following PTBD for extrahepatic cholangiocarcinoma. Although the prognosis for patients with this complication is generally poor, the excision of the catheter tract may enable survival in selected patients with isolated metastases along the catheter tract.

COVID-19 has highlighted the underlying global burden from non-communicable diseases (NCDs) and the lack of resiliency in health systems to tackle highly preventable risk factors, which have left populations vulnerable to acute health crises. The persistent issue of unhealthy behaviors, particularly those related to a high-sodium diet, suggests inadequate policy attention and insufficient funding for public health and behavioral research. This year, Japan will host the Tokyo Nutrition for Growth (N4G) Summit 2021. The summit is being held in the era of COVID-19, offering a historic opportunity to make a difference on the global health agenda for food and nutrition and to reaffirm the global commitment to prevent future health crises by addressing the growing burden of NCDs and building better prepared and more resilient health systems. This perspective article suggests that we, global health researchers and decision-makers, should rethink the value of using “Umami” substances as candidates for a practical salt-reducing alternative that originated in Japan (while maintaining the affordability, accessibility, availability, and desirability of healthy food). Reduced salt intake will help address the global syndemic of COVID-19 pandemic and high rates of hypertension, heart diseases, diabetes, and other NCDs. By addressing these issues, it will make countries more resilient to future pandemic threats.
