A reference evapotranspiration map for Bosnia and Herzegovina

There are three major challenges for climatic data availability for Reference Evapotranspiration (ET0) calculations in Bosnia and Herzegovina:limited data availability,discontinuity of data records,and low station *** main objective of this study was to apply reliable methods to calculate and spatially distribute ET0,while considering the impact of the elevation.A 20 m spatial resolution map was created for mean ET0 values at monthly,vegetation period (April-September) and annual temporal resolution at municipality (143 municipalities),regional (4 regions) and national spatial resolutions.108 weather stations for the period 1961-2016 (56 years) were utilized for spatial interpolation of ET0 using kriging with external drift *** required elevation for ET0 estimation at each grid node was ext'acted from the digital elevation model of Bosnia and ***0 was calculated using FAO-56 PenmanMonteith,and in cases where only minimum and maximum temperature data were available the Hargreaves-Samani equation adjusted with locally appropriate empirical radiation coefficient was used.A gradual decrease of mean ET0 values from the southern to central and northern to central part of the country is *** the all seasons (monthly,vegetation and annually),the southern region has greater ET0 than the other three regions (north,west and central-east),which are *** long-term mean annual ET0 for Bosnia and Herzegovina is 716 mm,approximately 78% (559 mm) of which occurs during in the vegetation period.

Characteristic clinical features of Aspergillus appendicitis:Case report and literature review

This work aims to facilitate diagnosing Aspergillus appendicitis, which can be missed clinically due to its rarity, by proposing a clinical pentad for Aspergillus appendicitis based on literature review and one new case. The currently reported case of pathologicallyproven Aspergillus appendicitis was identified by computerized search of pathology database at William Beaumont Hospital, 1999-2014. Prior cases were identified by computerized literature search. Among 10980 pathology reports of pathologically-proven appendicitis, one case of Aspergillus appendicitis was identified(rate = 0.01%). A young boy with profound neutropenia, recent chemotherapy, and acute myelogenous leukemia presented with right lower quadrant pain, pyrexia, and generalized malaise. Abdominal computed tomography scan showed a thickened appendiceal wall and periappendiceal inflammation, suggesting appendicitis. Emergent laparotomy showed an inflamed, thickened appendix, which was resected. The patient did poorly postoperatively with low-grade-fevers while receiving antibacterial therapy, but rapidly improved after initiating amphotericin therapy. Microscopic examination of a silver stain of the appendectomy specimen revealed fungi with characteristic Aspergillus morphology, findings confirmed by immunohistochemistry. Primary Aspergillus appendicitis is exceptionally rare, with only 3 previously reported cases. All three cases presented with(1)-neutropenia,(2)-recent chemotherapy,(3)-acute leukemia, and(4)-suspected appendicitis;(5)-the two prior cases initially treated with antibacterial therapy, fared poorly before instituting antiAspergillus therapy. The current patient satisfied all these five criteria. Based on these four cases, a clinical pentad is proposed for Aspergillus appendicitis: clinically-suspected appendicitis, neutropenia, recent chemotherapy, acute leukemia, and poor clinical response if treated solely by antibacterial/anti-candidial therapy. Portal hypertensive gastropathy:A systematic review of thepathophysiology,clinical presentation,natural history andtherapy

AIM: To describe the pathophysiology, clinical presentation, natural history, and therapy of portal hypertensive gastropathy(PHG) based on a systematic literature ***: Computerized search of the literature was performed via Pub Med using the following medical subject headings or keywords: "portal" and "gastropathy"; or "portal" and "hypertensive"; or "congestive" and "gastropathy"; or "congestive" and "gastroenteropathy". The following criteria were applied for study inclusion: Publication in peer-reviewed journals, and publication since 1980. Articles were independently evaluated by each author and selected for inclusion by consensus after discussion based on the following criteria: Well-designed, prospective trials; recent studies; large study populations; and study emphasis on PHG. RESULTS: PHG is diagnosed by characteristic endoscopic findings of small polygonal areas of variable erythema surrounded by a pale, reticular border in a mosaic pattern in the gastric fundus/body in a patient with cirrhotic or non-cirrhotic portal hypertension. Histologic findings include capillary and venule dilatation, congestion, and tortuosity, without vascular fibrin thrombi or inflammatory cells in gastric submucosa. PHG is differentiated from gastric antral vascular ectasia by a different endoscopic appearance. The etiology of PHG is inadequately understood. Portal hypertension is necessary but insufficient to develop PHG because many patients have portal hypertension without *** increases in frequency with more severe portal hypertension, advanced liver disease, longer liver disease duration, presence of esophageal varices, and endoscopic variceal obliteration. PHG pathogenesis is related to a hyperdynamic circulation, induced by portal hypertension, characterized by increased intrahepatic resistance to flow, increased splanchnic flow, increased total gastric flow, and most likely decreased gastric mucosal flow. New era of electrochemotherapy in treatment of liver tumors in conjunction with immunotherapies

Electrochemotherapy is a local ablative therapy that increases the cytotoxicity of either bleomycin or cisplatin by applying electric pulses(electroporation)to *** has already been widely used throughout Europe for the treatment of various types of human and veterinary cutaneous tumors,with an objective response rate ranging from 70%-90%,depending on the tumor ***,electrochemotherapy was introduced for the treatment of primary liver tumors,such as hepatocellular carcinoma(HCC).The complete response rate was 85%per treated lesion,with a durable ***,electrochemotherapy could become a treatment of choice for HCC,especially after achieving a transition from an open surgery approach to a percutaneous approach that uses dedicated *** elicits a local immune response and can be considered an in situ ***,among others,is a potentially immunogenic tumor;thus,electrochemotherapy could boost adjuvant immunotherapy to achieve a better and longer-lasting antitumor ***,therapeutic strategies that combine electrochemotherapy with immune checkpoint inhibitors or adjuvant treatment with cytokines are indicated for *** therapy using electroporation as a delivery system for plasmid DNA coding for interleukin-12 is a highly promising *** electroporation approach has shown efficacy in preclinical settings and veterinary oncology and is awaiting translation for the treatment of liver tumors,i.e.,HCC.

A further study of the kinetics and optimization of the essential oil hydrodistillation from lavender flowers

Hydrodistillation has commonly been used to recover essential oil from various plant materials,including lavender(Lavandula officinalis) *** main objectives of the present study were to model the kinetics of the lavender essential oil(LEO) hydrodistillation using a phenomenological model,to evaluate the statistical significance of the hydromodule and hydrodistillation time on LEO yield combining a 4~2 full factorial design with the response surface methodology,to model statistically LEO yield by multiple non-linear regression and to determine the optimal process conditions that provided the maximum LEO *** fast-essential oil distillation(washing stage) in the initial period and the slow diffusion stage until the saturation occurring simultaneously were observed,justifying the use of the phenomenological *** increasing the hydromodule,the saturation LEO yield and the washable fraction of the LEO decreased while the washing and diffusion rate constants *** of the LEO oil yield and the hydrodistillation kinetics is important from the techno-economical point of view.

The aim of this literature review was to summarize available findings from publications that reported expenditure on traditional/complementary and alternative medicine(TM/CAM)within a representative general population sample of a nation or a defined geographical area.A total of 24 publications met the inclusion criteria and were included in the *** expenditure on TM/CAM varies worldwide,but direct comparison of the findings of publications included in this review is limited due to the differences in the definitions of TM/CAM,inclusion of various forms of TM/CAM,use of different names and categorization,as well as differences in reported currencies and time periods in which data were *** about the expenditure on TM/CAM in most countries throughout the world are *** national studies should be conducted in order to provide up-to-date assessment of the TM/CAM related expenditure patterns and *** nomenclature,definition of TM/CAM and standardized instruments would provide basis for comparability of data of studies conducted in various regions and time periods.

Cross-Sectional Survey on Complementary and Alternative Medicine Awareness among Health Care Professionals and Students Using CHBQ Questionnaire in a Balkan Country

Objective： To conduct a study on attitudes, knowledge, and use of complementary and altemative medicine （CAM） therapies in Serbia. Available data about CAM therapies in the region are scarce, opinions lacking from health sector. Balkan region countries had a delay in issuing national policies on CAM therapies. Methods： The questionnaire used was based on previously validated CAM Health Belief Questionnaire （CHBQ）, formulated as 5-item Likert type scale, adjusted for local environment. Health care students and professionals were evaluated. The questionnaire comprehended 10 closed questions on attitudes, knowledge and use of CAM therapies. This survey was conducted in eight cities of Serbia, January 2010-July 2011. A total of 797 participants was included. The second group of participants was 145 healthcare professionals （50 academic staff, 64 clinical staff, 19 pharmacists, 6 other clinical branch specialists and 6 nurses）. Data were collected by an interview. Examinees could acquire maximum of 70 points, 35 representing neutral attitude. Restdts： Students of dentistry （54.65 ± 6.07） were better informed on CAM therapies than medicine students （50.26 ± 7.92）. Pharmacy students （51.16 ± 7.10） accepted low-scientific CAM. Pharmacists scored better than university professors （55.12±6.55 vs. 50.29± 9.50）. Primary health care professionals had better awareness than pharmacists in dispensing pharmacies. Both groups of participants preferred use of vitamins over any other CAM therapy. Conclusion： These pioneering efforts in the region exposed weaknesses in CAM attitudes of current and future health care professionals. Nevertheless, awareness on alternative medicine treatment choices is growing among Balkan prescribers. Supportive legal framework would facilitate dissemination of CAM medical practices.

Cystic lymphangioma of the pancreas

Lymphangioma of the pancreas is an extremely rare benign tumour of lymphatic origin, with fewer than 60 published cases. Histologically, it is polycystic, with the cysts separated by thin septa and lined with endothelial cells. Though congenital, it can affect all age groups, and occurs more frequently in females. Patients usually present with epigastric pain and an associated palpable mass. Complete excision is curative, even though, depending on the tumour location, surgery may be simple or involve extensive pancreatic resection and anastomoses. The authors present a 49-year-old woman in whom a polycystic septated mass, 35 mm× 35 mm in size, was discovered by ultrasonography （US） in the body of the pancreas during investigations for epigastric pain and nausea. At surgery, a well circumscribed polycystic tumor was completely excised, with preservation of the pancreatic duct. The postoperative recovery was uneventful. Histology confirmed a microcystic lymphangioma of the pancreas. Immunohistochemistry showed cystic endothelial cells reactivity to factor Ⅷ -RA （＋＋）, CD31 （＋＋＋） and CD34 （-）. Postoperatively, abdominal pain disappeared and the patient remained symptomfree for 12 mo until now. Although extremely rare, lymphangioma of the pancreas should be taken into consideration as a differential diagnosis of a pancreatic cystic lesion, especially in women.

切尔诺贝利核电站事故发生后,先后有30万左右的军人参加事故救援。本文介绍了乌克兰四零八军区医院对切尔诺贝利核电站事故伤员救治工作的任务。介绍了医疗救援三个阶段的时间划分、任务重点、兵力的投入及其结构组成。介绍了医院内部专门科室的建立、对受照剂量为0·25～0·50Gy的594例伤员健康状况的分析,其血液学改变的发生率随受照剂量增加而增多。介绍了参与救治工作医师们研究的主要内容,强调要考虑小剂量辐射对心血管系统和胃肠道的远期效应。根据救治工作的经验,分析了救治过程中存在的问题。
