Impact of hepatitis C virus core mutations on the response to interferon-based treatment in chronic hepatitis C

AIM To determine whether hepatitis C virus(HCV) core substitutions play a role in the response to interferon-based treatment in Caucasian patients. METHODS One hundred eight HCV chronically infected patients initiating treatment with pegylated IFN plus ribavirin for 48 wk were tested for baseline substitutions at codons 70 and 91 of the viral core protein(Big Dye Terminator vers.3.1, Applied Biosystems,) and for genetic polymorphisms in host IL28 B gene rs12979860(Custom TaqM an 5' allelic discrimination assay; Applied Biosystems).RESULTS Of the patients, all were infected with HCV genotype 1b, 44.4% had low baseline HCV viral load, and 37.9% had mild/moderate fibrosis. Only 38.9% achieved therapeutic success, defined as sustained virological response(SVR). Eighty-eight percent of the patients presented at least one substitution at core position 70(R70Q/H) or/and position 91(L91M). The favorable IL28 B CC polymorphism was detected in only 17.6% of the patients. In the univariate analysis, young age(P < 0.001), urban residence(P = 0.004), IL28 B CC genotype(P < 0.001), absence of core mutations(P = 0.005), achievement of rapid virologic response(P < 0.001) and early virological response(P < 0.001) were significantly correlated with SVR. A multivariate analysis revealed three independent predictors of therapeutic success: young age(P < 0.001), absence of core substitutions(P = 0.04) and IL28 B CC genotype(P < 0.001); the model correctly classified 75.9% of SVR cases with a positive predictive value of 80.7%. CONCLUSION HCV core mutations can help distinguish between patients who can still benefit from the affordable IFNbased therapy from those who must be treated with DAAs to prevent the evolution towards end-stage liver disease.

High speed machining and high precision machining are two tendencies of themanufacturing technology worldwide. The motorized spindle is the core component of the machine toolsfor achieving the high speed and high precise machining, which affects the general developmentlevel of the machine tools to a great extent. Progress of the key techniques is reviewed in thispaper, in which the high speed and high precision spindle bearings, the dynamical and thermalcharacteristics of spindles, the design technique of the high frequency motors and the drivers, theanti-electromagnetic damage technique of the motors, and the machining and assembling technique areinvolved. Finally, tha development tendencies of the motorized spindles are presented.

BACKGROUND Many scores have been suggested to assess the severity of acute pancreatitis upon *** extrapancreatic necrosis volume is a novel,promising score that appears to be superior to other scores investigated so *** To evaluate the discriminatory power of extrapancreatic necrosis volume to identify severe cases of acute *** A total of 123 patients diagnosed with acute pancreatitis at Institute of Gastroenterology and Hepatology,St Spiridon Hospital between January 1,2017 and December 31,2019 were analyzed *** was classified according to the revised Atlanta classification(rAC)as mild,moderate,or *** was also evaluated by computed tomography and classified according to the computed tomography severity index(CTSI)and the modified CTSI(mCTSI).The results were compared with the extrapancreatic volume necrosis to establish the sensitivity and specificity of each *** The CTSI and mCTSI imaging scores and the extrapancreatic necrosis volume were highly correlated with the severity of pancreatitis estimated by the rAC(r=0.926,P<0.001 and r=0.950,P<0.001;r=0.784,P<0.001,respectively).The correlation of C-reactive protein with severity was positive but not as strong,and was not significant(r=0.133,P=0.154).The best predictor for the assessment of severe pancreatitis was the extrapancreatic necrosis volume[area under the curve(AUC)=0.993;95%confidence interval(CI):0.981-1.005],with a 99.5%sensitivity and 99.0%specificity at a cutoff value of 167 mL,followed by the mCTSI 2007 score(AUC=0.972;95%CI:0.946-0.999),with a 98.0%sensitivity and 96.5%specificity,and the CTSI 1990 score(AUC=0.969;95%CI:0.941-0.998),with a 97.0%sensitivity and 95.0%*** Radiological severity scores correlate strongly and positively with disease *** necrosis volume shows the best diagnostic accuracy for severe cases.

Dynamic Control of a Flexible Shaft Mounted in Adaptive or Active Bearing

The dynamic behavior of rotors is highly influenced by bearing characteristics. In previous works, the authors have shown that it may be beneficial to adapt the bearing behavior to the shaft behavior. Several adaptive and active components will be developed in this paper in order to control the shaft dynamical amplitude. Different models of hydrodynamic bearings behavior are described. The Reynolds equation resolution may be done by numerical or analytical solutions. A physical analysis of the equation of thin films will identify the most sensitive parameters. The shaft flexibility is taking into account by a modal approach. The fluid-structure coupling process is a simulation, step by step, of the rotor behavior. At each step, the nonlinear fluid force is numerically calculated to obtain the unbalanced shaft response. The results, presented in this paper, concern the dynamic response of unbalanced shaft mounted in adaptive or active bearings： bearings with variable clearance, variable viscosity or variable housing speed. It is shown that the fluid bearing parameters must be adapted to the rotor speed （in particular near or far a critical speed）. Then, the paper presents a new kind of active bearing. It works with a mechanical control of the housing position. Several parameters are tested and compared. The robustness of the dynamic control parameters is presented. In conclusion, the bearing adaptation could be very useful to control the shaft dynamic. This limits the effect of the critical speed, in particular by diminishing the shaft amplitude and the dynamic forces transmitted to the housing.

Theoretical Study and Performance Analysis of an Adsorption Chiller

The thermodynamic cycle for an adsorption system is presented inp-T diagram. In order to investigate the performance of the adsorption system, a lumped parameter transient model of the chiller is developed, in order to predict the behaviors of the adsorption chiller system and find the influence of working conditions on its operation. For the working process of the main components of the system, including adsorber, condenser and evaporator, the coupled unsteady equations were set up for each stage. The model was then solved using stable numerical methods from EES （equation engineering solver）, and the performance of the adsorber and condenser/evaporator of the system was analyzed. The condensation, evaporation and adsorber temperature values as well as the adsorption ratio and desorption ratio were obtained as function of operating time. Also, the coefficient of performance was analyzed in function of the heat source temperature and the cooling source temperature.

Paleosols from North Part of Barlad City （Romania）

The loess deposits comprise several paleosol layers reflecting alternation of drier and wetter climate during Quaternary. Such a situation occurs in north of Barlad, on The Sohodau＇s Hill. Morphological study of the quarry paleosols from north of Barlad was accomplished based on field observations and macroscopic physic-chemical results. Three levels of paleosols with variable thickness were determined. These three fossils layers are interbedded by four loess deposits. The physical-chimical data provide important information for the paleosol genesis and depositional/climatic environments. The carbon content and C/N ratio indicate the strength of pedogenesis in the Pleistocene and trends of biomass accumulation.

Radiofrequency Thermo-Ablation of Morton’s Neuroma: A Valid Minimally Invasive Treatment Procedure in Patients Resistant to Conservative Treatment

Background: The authors present the personal results of the review of a group of cases treated for Morton’s neuroma with continuous radiofrequency (CRF) thermo-ablation of the plantar intermetatarsal nerve. Methods: This retrospective review consisted of 29 patients treated between January 2008 and December 2011, with a minimum 1 year follow-up. Three patients underwent bilateral treatment and 10 patients underwent concomitant treatment procedures for associated foot disorders: hallux valgus (7), tailor’s bunion (2) and hammertoe (5). The procedure was performed in a day hospital setting under direct fluoroscopic control. Results: The follow-up protocol used a visual analog scale (VAS) for pain (0 = no pain, 10 = worst pain) and a visual analog scale (VAS) for patient satisfaction (0 = no satisfaction, 10 = complete satisfaction). Success was defined by a decrease of 5 points in pain at 1 year after the treatment procedure. From these 29 cases, with a VAS pain score between severe and worst pain ever, treatment with CRF thermo-ablation at one year follow-up yielded these results: 14 cases with no pain (48%), 7 cases with mild pain (24%), 5 cases with moderate pain (17%), 3 cases still with severe pain (10%). The overall benefit was that 88% of the patients had significant pain improvements. From another point of view, 26 cases (89.6%) had a decrease of the VAS score between 5 and 10 points and in only 3 cases (10.3%) severe pain was reported, even though it was slightly diminished after the procedure. Conclusion: Continuous radiofrequency (CRF) thermo-ablation of Morton’s neuroma (Entrapment) was a safe and minimally invasive surgical procedure which gave these patients great satisfactions and a rapid return to normal activity.
