BACKGROUND An isthmocele is a scar defect in the uterine wall due to a cesarean section. Its prevalence is unknown,but it has been associated with secondary infertility.Here,we present a case where the patient suffered from an isthmocele that was associated with persistent hydrometra,which developed during in vitro fertilization.CASE SUMMARY The patient underwent hysteroscopic surgery,which successfully resolved the isthmocele as well as the hydrometra. Afterward,two high-quality,euploid embryos,determined by morphological assessment and pre-implantation genetic diagnostic testing,were transferred. This resulted in uterine pregnancy,as determined byserum β-human chorionic gonadotropin levels on day 14(180 m U/mL) and ultrasound-confirmed presence of a gestational sac with a positive embryocardia at week 6. The pregnancy reached 36 wk without any complications,and the product was born in good health. We report a successful isthmocele treatment in a patient with secondary infertility,in which the isthmocele was the cause of persistent hydrometra.CONCLUSION Hydrometra caused by secondary cesarean is an infertility factor,which can be corrected by hysteroscopy plus ablation of the isthmocele.

BACKGROUND Catatonia is a complex psychomotor syndrome that often goes unrecognized and untreated,even though its classification has evolved in recent years.Prompt and correct identification of catatonia allows for highly effective treatment and prevention of possible complications.The underrecognition of catatonia in older patients is also frequent,and research in this population is scarce.AIM To conduct a systematic review of the literature on catatonia in older people to ascertain its clinical characteristics across settings.METHODS Following the PRISMA guidelines,MEDLINE,EMBASE,and PsycINFO databases were searched from inception to December 2021,with a strategy aimed at identifying all articles published on catatonia in older adults.Titles and abstracts were scanned and selected independently by two authors.Papers investigating issues related to catatonia and/or catatonic symptoms in older people,with English abstracts available,were included.References of selected articles were revised to identify other relevant studies.RESULTS In total,1355 articles were retrieved.After removing duplicates,879 remained.Of the 879 identified abstracts,669 were excluded because they did not meet the inclusion criteria.A total of 210 articles underwent full text review,and 51 were eliminated for various reasons.Fourteen more articles were selected from the references.Overall,173 articles were reviewed:108 case reports,35 case series,11 prospective cohort studies,6 case-control studies,3 retrospective cohort studies and 10 reviews.We found several particular aspects of catatonia in this population.Catatonia in older patients is highly prevalent and tends to have a multifactorial etiology.Older patients,compared to younger patients,have a higher risk of developing catatonia with benzodiazepine(BZD)withdrawal,in bipolar disorder,and in the general hospital.Age,together with other risk factors,was significantly associated with the incidence of deep venous thrombosis,neuroleptic malignant syndrome poor outcome,other complications a

To highlight the importance of small earthquakes in seismic hazard, a study of the 11 May 2016, Mw 4.9, Tesistan, Mexico earthquake is presented. Due to the close proximity of the event to the city, accelerations were considerably higher than those caused by historical severe earthquakes (6.0 < Mw < 8.2). This paper addresses two objectives related to the Tesistan event: the fi rst is to estimate the focal mechanism solution in order to place the event in the context of the tectonic environment of this area. The second is focused on a vulnerability evaluation of buildings that suff ered resonance. Several building′s typologies with variations in construction system and height are assessed in terms of resonance with the structural and soil periods. The results show that around the Zapopan station, strong damage is expected in intermediate to high-rise buildings (12-30 m) with moment resistant frame systems and in reinforced concrete shear walls. Masonry structures around this station may not present resonance. In contrast, in the surroundings of the Guadalajara station, all intermediate height buildings from 9 to 21 m may present resonance.

Over the past four years,signiﬁcant research has advanced our understanding of how external factors inﬂuence tropical cyclone(TC)intensity changes.Research on air-sea interactions shows that increasing the moisture disequilibrium is a very effective way to increase surface heatﬂuxes and that ocean salinity-stratiﬁcation plays a non-negligible part in TC intensity change.Vertical wind shear from the environment induces vortex misalignment,which controls the onset of signiﬁcant TC intensiﬁcation.Blocking due to upper-level outﬂow from TCs can reduce the magnitude of vertical wind shear,making for TC intensiﬁcation.Enhanced TC-trough interactions are vital for rapid intensiﬁcation in some TC cases because of strengthened warm air advection,but upper-level troughs are found to limit TC intensiﬁcation in other cases due to dry midlevel air intrusions and increased shear.Aerosol effects on TCs can be divided into direct effects involving aerosol-radiation interactions and indirect effects involving aerosol-cloud interactions.The radiation absorption by the aerosols can change the temperature proﬁle and affect outer rainbands through changes in stability and microphysics.Sea spray and sea salt aerosols are more important in the inner region,where the aerosols increase precipitation and latent heating,promoting more intensiﬁcation.For landfalling TCs,the intensity decay is initially more sensitive to surface roughness than soil moisture,and the subsequent decay is mainly due to the rapid reduction in surface moistureﬂuxes.These new insights further sharpen our understanding of the mechanisms by which external factors inﬂuence TC intensity changes.

Rather than focusing on a focal spot,aberrated wavefields spread out over a region.As a wave phenomenon,optical aberrations are analyzed in terms of waves propagating in the 3D space.In this work,we report the observation of 2D longitudinal aberrated wavefields.This observation can be visualized by mapping the intensity distributions of surface plasmon polaritons(SPPs)that propagate on a metal/air interface using leakage radiation microscopy.The orientation of the SPP beam is tweaked by tilting and translating the system to mimic aberrated beams,presenting known Seidel terms:defocus,spherical,coma,and tilt aberration.This approach allows the examination of the longitudinal evolution of aberrated beams in a visual and rapid manner,in contrast to more complicated post-processing reconstructions.

The present document describes the experience of using a Geography Information System (GIS) tool to map the spatial distribution of the population of the State of Mexico, Mexico, for 2010. Prior to the development of the experience, dif- ferent projects, tools and products, which offered cartographic representations of the population, were presented, se- lecting one that was developed to function in Idrisi Taiga GIS. Likewise, the GIS tool and the process followed to apply it to a case study in the State of Mexico are described. The result is a raster image that shows the distribution of the population on the analyzed state, offering the possibility to identify the zones where the population concentrates and disperses and to produce an opinion from a geographic point of view on what the origin of these distribution patterns is. Among the conclusions from the analysis of results, distinguishable are the applications that the obtained image can have;for instance, the population can be estimated by basins or risk zones by only adding the values of the cells inside these zones. It is also concluded that the degree of detail expressed by the spatial distribution image of the population will depend on several factors, among them the scale and resolution of the input information and the resolution fixed for the output information.

Keratoconus is a progressive, usually bilateral disease of the cornea that significantly diminishes visual acuity, secondary to a progressive corneal deformity which is characterized by corneal thinning, variable degrees of irregular astigmatism and specific abnormal topographic patterns. Normally it initiates during puberty and is progressive until the third or fourth decade of life, when normally the progression rate is diminished or waned. There are multiple scales to clinically classify keratoconus. One of the most commonly used is Amsler-Krumeich and recently with the development of morphometric and aberrometric techniques, additional scales have been created that allow keratoconus to be classified according to its severity. Despite certain etiology of keratoconus remains unknown, current treatment options are available in patients with ectatic corneas and they vary depending on the severity of the disease and they include spectacles, contact lenses, intrastromal rings, keratoplasty both penetrant or lamellar, cross-linking, refractive lens exchange withintraocular lens implant, phakic intraocular lenses and the combination of these alternatives. Some authors have been using excimer laser in patients with keratoconus but the safety of the procedure is controversial. Currently, the techniques for the management of keratoconus can be classified in 3 types: corneal strengthening techniques, optical optimization techniques and combined techniques.

Serotonin (5HT) in the central nervous system has been associated with pain processing and modulation. The insular cortex (IC) plays an important role in the development and perception of the inflammatory and chronic pain. The role of the serotoninergic system in IC has not been completely studied. We used micro-dialysis in freely moving rats to determine the extracellular release of 5HT and its main metabolite (5HIAA) in the IC during an inflammatory process. Results showed an increase of extracellular levels of 5HT and 5HIAA in the IC during carrageenan-induced inflammation and this augmentation correlates with a decrease of behavioral mechanonociceptive response. Furthermore, the exogenous administration of 5HT and 5HIAA in the IC increases the nociceptive response. Our current data imply that the serotoninergic system in the IC participates in the long-term pain process.

Background: In clinical practice, acute and chronic wounds continue to be a challenge. Unfortunately, wound care is often inadequate or inappropriate. The implementation of wound clinics involving an interdisciplinary team for wound care and treatment yields favorable clinical outcomes as well as a reduction in economic expenses due to a higher rate of healing, a decrease in hospital stays, and a reduction in the number of readmissions. The establishment of wound clinics enhances the utilization of diagnostic, preventive, and therapeutic resources, improving the quality of life of patients and facilitating their reintegration into their normal lives. The objective is solely to describe the importance of wound clinics in the medical profession. Methods: A retrospective literature search was conducted to include articles no more than 10 years old that reported measures or results of the study in order to perform a literature review and describe the importance of wound clinics in medical practice. Results: A total of 7 articles were obtained with the inclusion criteria, the most recent being from the year 2023 and the oldest from 2013. Conclusion: An important area for improvement in the healthcare industry is the management and treatment of complex wounds. The establishment of wound clinics offers comprehensive and specialized care for both acute and chronic complex wounds with the goal of enhancing secondary illness patients’ prevention, treatment, and rehabilitation.
