Prospective application of a bleeding and ischemic risks-adjusted antithrombotic protocol in elderly patients revascularized with everolimus-eluting stents:EPIC05-Sierra75 study

oBJECTIVES Elderly patients show a higher incidence of ischemic and bleeding events after percutaneous transluminal coron-ary intervention(PCI).We sought to investigate outcomes in elderly patients treated with antithrombotic strategy guided by bleeding and ischemic risks after revascularization with last generation everolimus-eluting stent(EES).METHoDS Prospective multicenter registry including patients over 75 years revascularized with EES and antithrombotic ther-apy guided by clinical presentation,PCI complexity and PRECISE DAPT ***-primary safety endpoints were:(1)composite of cardiac death,myocardial infarction and stent thrombosis and;(2)bleeding(BARC 2-5).Primary efficacy endpoint was target lesion revascularization.A matched group of patients revascularized with current drug-eluting stents and no such tailored antith-rombotic therapy was used as *** Finally,1064 patients were included in SIERRA-75 cohort,80.8±4.2 years,36.6%women,71%acute coronary syn-dromes(ACS)and 53.6%complex ***-primary safety endpoint of major adverse cardiovascular events was met in 6.2%,co-primary safety endpoint of bleeding in 7.8%and primary efficacy endpoint of TKLR in 1.5%.The multivariable adjusted model showed no significant association of the prescribed short/long dual antiplatelet therapy(DAPT)durations with any endpoint suggesting a well tailored *** stent thrombosis reported in the subgroup with 1-3 months DAPT *** compared to control group,bleeding BARC 2-5 was significantly lower in SIERRA-75 group(7.4%vs.10.2%,P=0.04)as well as the net safety-efficacy endpoint(14.3%vs.18.5%,P=0.02).CoNCLUSIoNS In elderly population,the application of this risks-adjusted antithrombotic protocol after revascularization with last generation EES seems to be associated with an improved prognosis in terms of ischemic and bleeding outcomes.

Bilateral Gluteal Reconstruction with Double Flap Secondary to Biopolymers Injection

The injection of adjuvant substances for aesthetic purposes is a public health problem, for 40 years this problem has been described throughout our continent, the vast majority of women patients are affected by the infiltration of silicon, mineral oils, automotive oil, methyl methacrylate, cement and various oily substances. This is a 54-year-old female patient who for about 30 years for cosmetic purposes was injected with mineral oil (quantity unknown) in both glutei with the aim of buttock augmentation. Physical examination was observed an important deformity in each buttock, hyperkeratosis with discoloration in both buttocks, loss of bilateral projection, right buttock with a secondary tumor of 22 cm × 11 cm and left 22 cm × 10 cm, stony appearance, local hyperthermia, painful on manipulation, with diffuse nodular lesions measuring 2 cm × 2 cm, without evidence of loss of continuity that would condition secretion leakage. Surgical planning is based on the very important size of the initial defect that we would have to reconstruct, so that the rotation of two wide-based flaps could guarantee better survival and occlusion of the defect, since the realization of a graft would cause a significant deformity, and the realization of a microsurgical flap in a fibrous tissue would inevitably result in its necrosis. The pathophysiology of the adjuvant disease is extremely complex, therefore it must be addressed by a multidisciplinary group with extensive experience.

Many carnivores require large ranges to meet their ecological and energetic needs;however,anthropogenic changes threaten species and their *** traps have been used to effectively collect data on carnivores in a variety of habitat types;however,a single survey effort is typically limited to species that have similar body size,habitat use and movement patterns,and individual identification of animals is not always pos*** evaluated whether scat detection dogs could effectively survey for 4 wide-ranging felids that vary in these characteristics:jaguars(Panthera onca),pumas(Puma concolor),ocelots(Leopardus pardalis)and oncillas(Leopardus tigrinus).From June to october 2009 and May to August 2011,a detection dog-handler team detected 588 scats,from which 176 unique genotypes were *** assigned sex to 84.7%of the genotyped scats and identified 55 individuals multiple *** effectiveness of these noninvasive techniques(detection dogs and genetic analyses of scat)not only opens the door for additional studies in areas that were previously difficult or impossible with standard survey techniques,but also provides conservationists with a set of tools that overcome some of the limitations associated with the use of camera traps alone.

Electric Vehicles Analysis inside Electric Mobility Looking for Energy Efficient and Sustainable Metropolis

This paper aims to study and evaluate electric mobility over time, focusing on the development of the electric car. Methodologically, in order to accomplish this intent, the characterization of the electric vehicle (EV) is made based on the variables which determine its performance, such as: assessment of speeds, distance traveled, analysis of facts related to the energy source (electro-chemical accumulators) and analysis of the determining system of electric mobility (the electric engine as a function of power (W) and voltage (V)). This way, to demonstrate the effects of time, this process will be analyzed from the beginning of the 20th century (1930s) to the present (the first decade of the 21st century), methodologically structured in 4 cycles that show the performance of the EV. The results show the existence of vulnerabilities and of electric mobility potential, as well as the nuances of the development of the electric vehicle along the years and along the transformations in what is considered state-of-the-art. Thus, in the case of batteries, it is evident that the lithium-ion type used nowadays reveals better results due to its higher specific efficient energy, which maximizes energy autonomy to 200 km. In the beginning, the insertion of the electric vehicle was commercially harmed by the fundamental limitations of batteries as a power source. Conclusively, on certain occasions there have been improvements in the aerodynamics, engines, weight and size of the batteries, demonstrating the maturity of EVs.

Comparative Analysis of car.on Monoxide Modeling from Vehicular Sources in Puebla City,Mexico

The results of dispersion modeling of car.on monoxide are reported in this paper. The results of applying the technique of Rapid Assessment of Sources of Environmental Pollution (RASEP) database and the Air Monitoring State System in the City of Puebla, México, were employed. Concentrations of car.on monoxide emitted by cars inferred by RASAP technique with those reported by the environmental monitoring station “Nymphs”, were compared. The date of 21 June 2005-2010 was selected during the peak hour traffic flow. The dispersion of car.on monoxide was modeled by software DISPER and SCRI software, in order to infer the exposure levels of car.on monoxide in the study area. The estimated and monitored concentrations were evaluated with the Mexican regulations for population’s health protection. Regarding the dispersion model, SCRI was discarded for the target validation because it only allows modeling on specific areas. Moreover, the modeling software allowed DISPER simulations with linear sources of car exhaust, so this advantage is considered appropriate to continue using this program. Both estimates obtained by RASEP, as modeled by DISPER and recorded by the monitoring system exceed the maximum permissible limits of NoM-021-SSA1-1993 getting an impermissible for the area of influence of the assessment monitoring station. The DISPER software has potential use for such evaluations, but more work is required on the system for their inferences can be validated by reproducible physical and chemical measurements.

Cyclin-dependent kinases 4 and 6(CDK4/6)inhibitors demonstrated activity in terms of progression-free survival(PFS)in advanced dedifferentiated liposarcoma(DD-LPS),a sarcoma with CDK4 ***4 overexpression is by far more common than amplification in sarcomas and it might be a rational target for CDK *** investigators of this study found that CDK4 overexpression,while not of CDKN2A,was the most consistent predictive factor for palbociclib efficacy in *** adult-type soft-tissue sarcoma,excluding DD-LPS,or bone sarcoma patients,progressing after at least one systemic line,whose tumors overexpressed CDK4,but not CDKN2A at baseline biopsy,were accrued in this single-arm phase II trial(EudraCT number:2016-004039-19).With the main endpoint of a 6-month PFS rate,40%was considered promising in this *** was administered orally at 125 mg/day for 21 days in 28-day cycles.A total of 214 patients with 236 CDK4/CDKN2A determinations were assessed for prescreening,archival material(141),and screening,baseline biopsy(95).There were 28(29%)with favorable mRNA profiles from 95 screened patients at *** 23 enrolled patients,21 evaluable,the 6-month PFS rate was 29%(95%CI 9–48),and there were 6 patients out of 21 with a PFS longer than 6 *** median PFS and overall survival were 4.2(95%CI 3.6–4.8)and 12(95%CI 8.7–15.4)months,*** research showed a significant correlation between CDK4 mRNA and protein *** was active in a variety of sarcoma subtypes,selected by CDK4/CDKN2A,and deserves further investigation in the sarcoma context.

Association of CYP2C19 genotype with type 2 diabetes

Background: CYP2C19 is a major isoform of cytochrome P450 that metabolizes a number of commonly prescribed drugs such as omeprazole, diazepam, tolbutamide and propranolol. Its expression is regulated by the constitutive androstane receptor (car), involved in glucocorticoids synthesis. Since a number of crossliniks have been described for CYPs and some hormones, an association of CYP2C19 with type 2 diabetes is likely. Methods: Two groups were studied, 352 diagnosed with type 2 diabetes patients and 342 healthy volunteers form Mexico City. Both groups were tested for CYP2C19*2 and *3 alleles. We carried out an allelic discrimination using TaqMan assay for *2, and used FRET sensor and anchor probes for *3. Results: Ninety one percent of the subjects had the wild type allele, 9% have the *2 allele;no subject presented the *3 allele. The CYP2C19*2 allele is associated with type 2 diabetes (p = 0.012). Admixmap program was used to correct the admixture of this population and get the correlation. This was further confirmed in a linear model with a 67% power and by the method of Strom and Wienker for association on subjects within the mean range of Amerindian ancestry only (60%). Conclusion: Type 2 diabetes patients have significatly more *2 allele than healthy volunteers, more evident for the patients with the homocygous genotype.

Angiogenesis at Different Stage of Pregnancy Ingoatplacenta

The increase intransplacental exchange depends primarily on the great development of the placental vascular bed. The aim of this study was to evaluate the pattern of placental vascular development at day 50 and 100 of pregnancyin goats and the relationship with the vascular endothelial growth factor （VEGF） immunolocalization. The placental morphometric variables： capillary area density, capillary perimeter density, capillary number density and average capillary perimeter of the caruncular （car） and cotyledonary （CoT） tissues were measured at day 50 （n = 5） and 100 （n = 5） of gestation. Also, the immunolocalization of VEGF was performed. In the car and CoT tissues a significant increase of capillary area density and capillary perimeter density at day 100 of the gestation was observed. In the CoT tissue, this was due to the increase in the capillary number density; however in the car tissue this was due to the increase in the capillary size. We can conclude that in both tissues there were different strategies to increase the capillary area and, thereby, the nutrients exchange. The immunolocalization of VEGF in the CoT tissue showed the endothelial cells of blood vessels and trophoblastcells surrounding the villi intensely inmunostained, which would indicate the involvement of VEGF in the placental vascular development.

Stem cell therapy in the management of shoulder rotator cuff disorders

Rotator cuff tears are frequent shoulder problems that are usually dealt with surgical repair. Despite improved surgical techniques, the tendon-to-bone healing rate is unsatisfactory due to difficulties in restoring the delicate transitional tissue between bone and tendon. It is essential to understand the molecular mechanisms that determine this failure. The study of the molecular environment during embryogenesis and during normal healing after injury is key in devising strategies to get a successful repair. Mesenchymal stem cells(MSC) can differentiate into different mesodermal tissues and have a strong paracrine, anti-inflammatory, immunoregulatory and angiogenic potential. Stem cell therapy is thus a potentially effective therapy to enhance rotator cuff healing. Promising results have been reported with the use of autologous MSC of different origins in animal studies: they have shown to have better healing properties, increasing the amount of fibrocartilage formation and improving the orientation of fibrocartilage fibers with less immunologic response and reduced lymphocyte infiltration. All these changes lead to an increase in biomechanical strength. However, animal research is still inconclusive and more experimental studies are needed before human application. Future directions include expanded stem cell therapy in combination with growth factors or different scaffolds as well as new stem cell types and gene therapy.
