In the last few decades structure optimisation has become a main task in a civil engineering project. As a matter of fact, due to the complexity and particularity of every structure, the great amount of variables and design criteria to considerate and many other factors, a general optimisation’s method is not simple to formulate. As a result, this paper focuses on how to provide a successful optimisation method for a particular building type, high-rise reinforced concrete buildings. The optimization method is based on decomposition of the main structure into substructures: floor system, vertical load resisting system, lateral load resisting system and foundation system;then each of the subsystems using the design criteria established at the building codes is improved. Due to the effect of the superstructure optimisation on the foundation system, vertical and lateral load resisting system is the last to be considered after the improvement of floor. Finally, as a case example, using the method explained in the paper, a 30-story-high high-rise residential building complex is analysed and optimised, achieving good results in terms of structural behaviour and diminishing the overall cost of the structure.

Background: In highly myopic eyes, myopic foveoschisis (MF), the earliest stage of myopic traction maculopathy (MTM), is present in up to 34% of patients with pathologic myopia and slowly progresses to form foveoretinal detachment (FRD) or macular hole (MH) with or without macular hole retinal detachment (MHRD) as a part of its natural history. Aim: To describe the microstructural and functional results in three highly myopic eyes that underwent macular surgery for early-stage MTM. The last postoperative structural findings were correlated with the final vision and macular automated microperimetry evaluation. Methods: We retrospectively reviewed three highly myopic eyes that underwent successful fovea-saving internal limiting membrane (FS-ILM) macular surgery for chronic FRD at Oftalmologia Integral ABC, Mexico City, Mexico. We performed postoperative multimodal microstructural and functional evaluations, including SD-OCT, SS-OCT, and microperimetric macular examinations. Results: There was a substantial difference between best-corrected visual acuity (BCVA) preoperatively and postoperatively in all three cases. Postoperative surgery was associated with significant improvement in visual acuity confirmed using a paired-sample permutation test. The mean presurgical BCVA value (LogMAR;mean ± SE) was ~0.83 ± 0.15, and the postsurgical value was ~0.43 ± 0.52 (P = 0.00065). The myopic foveoretinal detachment evaluation was ~7.3 ± 3.5 months, with a mean postoperative follow-up time of ~14 ± 4.08 months. Furthermore, postoperative multimodal imaging tests demonstrated an abnormal microstructural foveal SS-OCT pattern without evidence of macular hole (MH) development at the postoperative follow-up. Macular microperimetry confirmed a subclinical reduced macula threshold sensitivity with an anomalous retinal sensitivity analysis map and a stable central foveal fixation site. To review the classification and general guidelines for treatment of bile duct injury patients and their long term results. In a 20-year period, 510 complex circumferential injuries have been referred to our team for repair at the Instituto Nacional de Ciencias Médicasy Nutrición "Salvador Zubirán" hospital in Mexico City and 198 elsewhere (private practice). The records at the third level Academic University Hospital were analyzed and divided into three periods of time: GⅠ-1990-99 (33 cases), G Ⅱ2000-2004 (139 cases) and GⅢ2004-2008 (140 cases). All patients were treated with a Roux en Y hepatojejunostomy. A decrease in using transanastomotic stents was observed (78% vs 2%, P = 0.0001). Partial segment Ⅳ and Ⅴ resection was more frequently carried out (45% vs 75%, P = 0.2) (to obtain a high bilioenteric anastomosis). Operative mortality (3% vs 0.7%, P = 0.09), postoperative cholangitis (54% vs 13%, P = 0.0001), anastomosis strictures (30% vs 5%, P = 0.0001), short and long term complications and need for reoperation (surgical or radiological) (45% vs 11%, P = 0.0001) were significantly less in the last period. The authors concluded that transition to a high volume center has improved long term results for bile duct injury repair. Even interested and tertiary care centers have a learning curve.

The War of the Triple Alliance was an international military conflict in South America, fought from 1864 to 1870 between Paraguay and the Triple Alliance of Argentina, Brazil, and Uruguay. In 1939, according to the provisions of the Arb6-Cantilo Treaty, it was established that the international boundary between Argentina and Paraguay passed through the deepest channel of the Pilcomayo River. The natural complexity of the river, plus the condition of acting as international border, led to perform this analysis. The main goal was to implement geo-processing techniques to map the evolution in the drainage pattern, linked to natural processes or human activities from 1951 to today. Satellite imagery and historical maps were integrated upon a Geographic Information System （GIS）; to interpret the changes occurred in the last 60 years along the Pilcomayo River channel which is approximately 20,000 km^2 located in Argentina and Paraguay. The collected information allowed the authors to evaluate and demonstrate the degree of correspondence between the river variation and the topographic position of the international boundary between Argentina and Paraguay, and also to diagnose what could occur in the near future.

BACKGROUND Primary hydatid disease in the colon is an extremely rare finding. We performed a systematic review of cases of hydatidosis of the colon published to date. AIM To systematic review the published papers about hydatidosis of the colon. METHODS Following PRISMA guidelines, we performed an unlimited search for articles on CH in the MEDLINE (PubMed), EMBASE, Cochrane, Latindex and Scielo databases, published in English, Spanish, French, German or Italian between January 1, 1960 and December 15, 2018. The search items were:(Colon) AND [(hydatidosis) OR (hydatid disease) OR (echinococcosis)]. RESULTS Nine case reports were found [five men, four women, median age 64.55 years (range: 21-81)]. The most frequent clinical manifestation was abdominal pain (8/9)(88.9%). In almost all cases several image studies were carried out, but abdominal computed tomography was the most used (66.7%). Five resections of the colon (62.5%) and three sutures of the fistula between cyst and colon (37.5%) were performed. Four patients did not present post-operative complications, but one died. Postoperative period was recorded for a mean period of 35 months. No recurrences were recorded. CONCLUSION The most frequent symptom was abdominal pain (evidence level 4). Computed tomography was the most frequently used diagnostic imaging technique (evidence level 4). Colectomy was the most appropriate treatment (evidence level 5). All the diagnostic and therapeutic options for hydatid cysts located in colon have a low level of evidence.

We study the directional excitation of optical surface waves controlled by the magnetic field of *** theoretically predict that a spinning magnetic dipole develops a tunable unidirectional coupling of light to transverse electric(TE)polarized Bloch surface waves(BSWs).Experimentally,we show that the helicity of light projected onto a subwavelength groove milled into the top layer of a 1D photonic crystal(PC)controls the power distribution between two TE-polarized BSWs excited on both sides of the *** a phenomenon is shown to be solely mediated by the helicity of the magnetic optical field,thus revealing a magnetic spin-orbit interaction of ***,this magnetic optical effect is clearly observed via a near-field coupler governed by an electric dipole moment:it is of the same order of magnitude as the electric optical effects involved in the *** opens up new degrees of freedom for the manipulation of light and offers desirable and novel opportunities for the development of integrated optical functionalities.

Three behaviors of epidemiological importance,namely feeding latency,feeding duration and defecation latency,for six populations of Meccus phyllosomus longipennis(Usinger)from areas of central,western and north-central Mexico with high(HP)and low(LP)prevalence of Trypanosoma cruzi(Chagas)human infection were evaluated in this *** median feeding latency(the time taken to begin feeding)was highly variable between ***-instar comparisons showed that at least 65%of the LP populations(N3 to adult)started to feed significantly(P<0.05)later than the HP population,with N1 showing no difference,and N2 from LP populations feeding sooner than those from HP *** six populations had similar median feeding durations within instars.A higher(P<0.05)percentage of the instars from HP populations defecated faster than the respective instars from the three LP *** 25%of the young nymphs(N1 to N3)and females in the HP populations defecated<2 min postfeeding,compared with 4%-6%of the young nymphs and 1.3%-3%of females in the LP ***,17.7%-38.8%of the older nymphs(N4 to N5)in the HP populations and 6.8%-13.4%in the LP populations defecated during or immediately after *** results indicate that the HP populations have a greater potential than the LP populations to transmit *** infections,which may underlie the differences in the prevalence of *** infection in some areas where M *** is currently distributed.
