Differentiation of focal liver lesions using three-dimensional ultrasonography: Retrospective and prospective studies

AIM: To differentiate focal liver lesions based on enhancement patterns using three-dimensional ultrasonography (3D US) with perflubutane-based contrast ***: Two hundred and eighty two patients with focal liver lesions,including 168 hepatocellular carcinomas (HCCs),63 metastases,40 hemangiomas and 11 focal nodular hyperplasias (FNHs),were examined by 3D US with perflubutane-based contrast *** ultrasound images and sonographic angiograms were *** 282 lesions,enhancement patterns of 163 lesions between January 2007 and October 2007 were analyzed *** from November 2007 to May 2008,compared with contrast-enhanced (CE) 2D US,CE 3D US was performed on 119 lesions for prospective differential ***,specificity,area under receiver operating characteristic curve (Az) and inter-reader agreement were ***: With the tridimensional view,dominant enhancement patterns were revealed as diffuse enhancement or peripheral ring-like enhancement,followed with washout change for HCCs or metastases,respectively,and peripheral nodular enhancement or diffuse enhancement with spoke-wheel arteries,followed by persistent enhancement for hemangiomas or FNHs,*** CE 3D US,the prospective differentiation of lesions showed sensitivity 92% (mean for two readers),specificity 91% and Az value 0.95 for HCCs,84%,97%,and 0.95 for metastases,91%,98%,and 0.98 for hemangiomas and 80%,99%,and 0.99 for FNHs,respectively,while good to excellent inter-reader agreement was *** significant difference exists between prospective diagnosis accuracy at CE 3D US and that at CE 2D ***: CE 3D US provides a spatial perspective for liver tumor enhancement,and could help in differentiating focal liver lesions.

Levovist ultrasonography imaging in intracystic hemorrhage of simple liver cyst

The differential diagnosis between intracystic hemorrhage and cystadenocarcinoma of the liver is often difficult even with the use of various imaging modalities. A 73-year-old woman was admitted to our hospital with the complaint of right upper quadrant pain. Ultrasonography (US) demonstrated a heterogeneous echogenic cyst measuring 11 cm × 8 cm in size in S2 of the liver, indicated intracystic hemorrhage of simple liver cyst or cystadenocarcinoma, but the differential diagnosis was considerably difficult. Levovist (Schering, Berlin, Germany) US revealed no enhancement of the intracystic structures, suggesting a clot in the case of intracystic hemorrhage. An operation was performed and the resected lesion showed a solitary benign liver cyst, measuring 5.5 cm × 4.7 cm × 8.5 cm containing a large blood clot. The patient had an uneventful recovery after the surgery. Levovist US may play an important role in discrimination between intracystic hemorrhage of simple hepatic cysts and cystadenocarcinoma of the liver.

Fundamental study on mixing layer and horizontal circulation in open-channel flows with rectangular embayment zone

A part of mean kinetic energy in a main-channel is used for production of a large-scale horizontal circulation in the side cavity. However, the details of the mechanism such as energy transport are poorly understood. Therefore, we conducted PIV mea- surements in a laboratory flume and compared space distributions of mean velocity components and Reynolds stress by varying a cavity geometry. In particular, a practical calculation method of Reynolds stress was also developed and its accuracy was examined by comparison with the measured data. Furthermore, contributions of components in an energy transport equation were revealed quantitatively.

Numerical prediction of colloidal phase separation by direct computation of Navier–Stokes equation

Numerical prediction of out-of-equilibrium processes in soft and bio matter containing liquids is highly ***,it is quite challenging primarily because the motions of the components at different hierarchical levels(e.g.,large colloids and small solvent molecules)are spatio-temporally coupled in a complicated manner via momentum *** we critically examine the predictability of numerical simulations for colloidal phase separation as a prototype example of self-organization of soft materials containing a *** use coarse-grained hydrodynamic simulations to tackle this problem,and succeed in almost perfectly reproducing the structural and topological evolution experimentally observed by three-dimensional confocal microscopy without any adjustable ***,comparison with non-hydrodynamic simulations shows the fundamental importance of many-body hydrodynamic interactions in colloidal phase *** predictive power of our computational approach may significantly contribute to not only the basic understanding of the dynamical behavior and self-organization of soft,bio and active matter but also the computer-aided design of colloidal materials.

Muscularis mucosae in desmoplastic stroma formation of early invasive rectal adenocarcinoma

The origin of myofibroblasts or myofibroblastic cells in the desmoplastic stroma associated with carcinoma invasion has been controversial. In the early invasive area of a rectal adenocarcinoma reported here, an obvious transition between the muscularis mucosa and the bundles of eosinophilic stromal cells observed in the carcinomatous stroma was demonstrated both in morphology and in their cytoskeletal phenotype, which conceivably suggests that the smooth muscle cells of the muscularis mucosa could convert to the eosinophilic stromal cells, namely myofibroblasts. Moreover, type I procollagen was demonstrated in both protein and mRNA levels in the areas of eosinophilic stromal cells with a lesser degree of differentiated smooth muscle phenotype that showed a transition from the muscularis mucosa, implying that the myofibroblastic cells converted from smooth muscle cells of the muscularis mucosa could be responsible for type I collagen production. These findings suggest that the muscularis mucosae may not be a passive barrier through which colorectal carcinomas infiltrate into the submucosa, but may play an active role in the formation and remodeling of tumor stroma.

Importance of hepatitis C virus-associated insulin resistance:Therapeutic strategies for insulin sensitization

Application of Brillouin Optical Correlation Domain Analysis for Crack Identification in Concrete Structure

This paper investigates the application of distributed optical fiber strain sensors to civil engineering structures, because no other tool can satisfactorily detect the location of the unpredictable phenomenon. In fact, the locations of cracks in the concrete structure are unknown a priori; therefore, a fully distributed sensor is necessary to detect them. The Brillouin optical correlation domain analysis （BOCDA）, which offers high spatial resolution by using stimulated Brillouin scattering along the whole length of the optical fiber, is used in a wide range of civil engineering applications, and the same has undergone significant development over the last decade. In this paper, it is demonstrated how a BOCDA-based strain sensor can be employed to monitor cracks in concrete. Crack monitoring on the surface of the concrete member provides useful information for evaluating stiffness and durability of the structure, particularly for early detection of tiny cracks, which is essential for preventing crack growth and dispersion. The crack-induced strain distribution was analytically investigated, and it was proved that BOCDA can identify even a small crack before its visual recognition by a beam test. Moreover, periodical crack monitoring was successfully executed on a pedestrian deck for five years.

Krein space representation and Lorentz groups of analytic Hilbert modules

This paper aims to introduce some new ideas into the study of submodules in Hilbert spaces of analytic functions. The effort is laid out in the Hardy space over the bidisk H^2(D^2). A closed subspace M in H^2(D^2) is called a submodule if ziM ? M(i = 1, 2). An associated integral operator(defect operator) CM captures much information about M. Using a Kre??n space indefinite metric on the range of CM, this paper gives a representation of M. Then it studies the group(called Lorentz group) of isometric self-maps of M with respect to the indefinite metric, and in finite rank case shows that the Lorentz group is a complete invariant for congruence relation. Furthermore, the Lorentz group contains an interesting abelian subgroup(called little Lorentz group) which turns out to be a finer invariant for M.

Motor vehicle accidents: How should cirrhotic patients be managed?

Motor vehicle accidents (MVAs) are serious social issues worldwide and driver illness is an important cause of MVAs. Minimal hepatic encephalopathy (MHE) is a com- plex cognitive dysfunction with attention deficit, which frequently occurs in cirrhotic patients independent of severity of liver disease. Although MHE is known as a risk factor for MVAs, the impact of diagnosis and treatment of MHE on MVA-related societal costs is largely unknown. Recently, Bajaj et al demonstrated valuable findings that the diagnosis of MHE by rapid screening using the inhibitory control test (ICT), and subsequent treatment with lactulose could substantially reduce the societal costs by preventing MVAs. Besides the ICT and lactulose, there are various diagnostic tools and therapeutic strategies for MHE. In this commentary, we discussed a current issue of diagnostic tools for MHE, including neuropsychological tests. We also discussed the advantages of the other therapeutic strategies for MHE, such as intake of a regular breakfast and coffee, and supplementation with zinc and branched chain amino acids, on the MVA-related societal costs.
