Comparison of percutaneous transhepatic portal vein embolization and unilateral portal vein ligation

AIM： To compare the effect of percutaneous transhepatic portal vein embolization （PTPE） and unilateral portal vein ligation （PVL） on hepatic hemodynamics and right hepatic lobe （RHL） atrophy.METHODS： Between March 2005 and March 2009, 13 cases were selected for PTPE （n = 9） and PVL （n = 4） in the RHL. The PTPE group included hilar bile duct carcinoma （n = 2）, intrahepatic cholangiocarcinoma （n = 2）, hepatocellular carcinoma （n = 2） and liver metastasis （n = 3）. The PVL group included hepatocellular carcinoma （n = 2） and liver metastasis （n = 2）. In addition, observation of postoperative hepatic hemodynamics obtained from computed tomography and Doppler ultrasonography was compared between the two groups.RESULTS： Mean ages in the two groups were 58.9 ± 2.9 years （PVL group） vs 69.7 ±3.2 years （PTPE group）, which was a significant difference （P = 0.0002）. Among the indicators of liver function, including serum albumin, serum bilirubin, aspartate aminotransferase, alanine aminotransferase, platelets and indocyanine green retention rate at 15 min, no significant differ- ences were observed between the two groups. Preop-erative RHL volumes in the PTPE and PVL groups were estimated to be 804.9 ±181.1 mL and 813.3 4±129.7 mL, respectively, with volume rates of 68.9% ± 2.8% and 69.2% ±4.2%, respectively. There were no significant differences in RHL volumes （P = 0.83） and RHL volume rates （P = 0.94）, respectively. At 1 mo after PTPE or PVL, postoperative RHL volumes in the PTPE and PVL groups were estimated to be 638.4±153.6 mL and 749.8 ± 121.9 mL, respectively, with no significant difference （P = 0.14）. Postoperative RHL volume rates in the PTPE and PVL groups were estimated to be 54.6% ± 4.2% and 63.7% ± 3.9%, respectively, which was a significant difference （P = 0.0056）. At 1 mo after the operation, the liver volume atrophy rate was 14.3% ± 2.3% in the PTPE group and 5.4%± 1.6% in the PVL group, which was a significant difference （P = 0.0061）.CONCLUSION： PTPE is a

Augmentation of tumor necrosis factor family-induced apoptosis by E3330 in human hepatocellular carcinoma cell lines via inhibition of NFκB

AIM- To investigate the reduction of cell viability in human hepatocellular carcinoma （HCC） cell lines induced by inhibition of nuclear factor kB （NFkB）. METIIOI）S： HLE, SKHep1, and HepG2 were incubated and E3330 was used to compare the stimulation of some chemotherapeutic drugs with that of TNF family, Fas ligand, TNF（x and TNF-related apoptosis-inducing ligand （TRAIL） at the point of the reduction of cell viability by inhibiting NFkB. RESULTS： E3330 decreased NFKB levels in HLE cells stimulated by TNF and TRAIL. The cytotoxicity of the combination of TRAIL, TNFa, Fas ligand, and E3330 increased synergistically in a dose-dependent manner compared to either E3330 alone in all HCC cell lines by MTT assay. However, the combination of some chemotherapeutic drugs and E3330 did not decrease the cell viability. CONCLUSION： Inhibition of NFd3 sensitizes human HCC cell lines to TNF-mediated apoptosis including TRAIL, and TRAIL-based tumor therapy might be a powerful potential therapeutic tool in the treatment of human HCC.

Hepatectomy is an effective surgical treatment for multiple bilobar liver metastases from colon cancer;however,one of the primary obstacles to completing surgical resection for these cases is an insufficient volume of the future remnant liver,which may cause postoperative liver *** induce atrophy of the unilateral lobe and hypertrophy of the future remnant liver,procedures to occlude the portal vein have been conventionally used prior to major *** report a case of a 50-year-old woman in whom two-stage hepatectomy was performed in combination with intraoperative ligation of the portal vein and the bile duct of the right hepatic *** procedure was designed to promote the atrophic effect on the right hepatic lobe more effectively than the conventional technique,and to the best of our knowledge,it was used for the first time in the present *** successful induction of liver volume shift as well as the following procedure,the patient died of subsequent liver failure after developing recurrent *** discuss the first case in which simultaneous ligation of the portal vein and the biliary system was successfully applied as part of the first step of two-stage hepatectomy.

Effect of Partially Substituting Ca with Mischmetal on the Microstructure and Mechanical Properties of Extruded Mg-Al-Ca-Mn-Based Alloys

Mg-2Al-1.2Ca-0.2Mn(at%)-based alloys with Ce-rich mischmetal(MM) substitution of 0–0.6 at% for Ca were hot extruded at 400 °C. The effect of MM substitution on the microstructure and mechanical properties of the extruded alloys was investigated. The as-cast Mg-2Al-1.2Ca-0.2Mn alloy is mainly composed of a-Mg, Mg_2Ca and(Mg,Al)_2Ca phases and Al_8Mn_5 precipitates, whereas the substitution of MM brings about the formation of Al_(11)MM_3, Al_2MM phases and Al_(10)MM_2Mn_7 particles with the absence of (Mg,Al)_2Ca phase. The volume fraction of MM-containing phases increases with increasing MM contents. All of the extruded alloys exhibit bimodal microstructure comprising fine dynamically recrystallized grains with almost random orientation and coarse deformed grains with strong basal texture. Dense nanosized planar Al_2Ca and spherical Al–Mn phases precipitate inside the deformed grains. High tensile yield strengths of~ 350 MPa and moderate elongations to failure of >12% are obtained in all extruded alloys; the MM substitution induces negligible difference in the tensile properties at ambient temperature, while the highest MM substitution improves the strength at 180 °C due to the better thermal stability of the fragmented MM-containing phases.

Nitrogen resorption and fractionation during leaf senescence in typical tree species in Japan

Inverse correlation between reactive oxygen species in unwashed semen and sperm motion parameters as measured by a computer-assisted semen analyzer

This study investigated the correlation between sperm motion parameters obtained by a computer-assisted semen analyzer and levels of reactive oxygen species in unwashed semen. In total, 847 patients, except for azoospermic patients were investigated. At the time of each patient＇s first consultation, semen parameters were measured using SMASTM or CellSoft 3000TM, and production of reactive oxygen species was measured using a computer-driven LKB Wallac Luminometer 1251 Analyzer. The patients were divided into two groups： reactive oxygen species - positive and negative. The semen parameters within each group were measured using one of the two computer-assisted semen analyzer systems and then compared. Correlations between reactive oxygen species levels and sperm motion parameters in semen from the reactive oxygen species - positive group were also investigated. Reactive oxygen species were detected in semen samples of 282 cases （33.3%）. Sperm concentration （P 〈 0.01; P 〈 0.01）, motility （P 〈 0.01; P 〈 0.05）, and progressive motility （P 〈 0.01; P 〈 0.01） were markedly lower in the reactive oxygen species - positive group than in the reactive oxygen species - negative group. Among the sperm motion parameters in the reactive oxygen species - positive group, sperm concentration （P 〈 0.01; P 〈 0.01）, motility （P 〈 0.05; P 〈 0.01）, mALH （P 〈 0.05; P 〈 0.01）, and progressive motility （P 〈 0.05; P 〈 0.01） also showed inverse correlations with the logarithmic transformed reactive oxygen species levels. Therefore, this study demonstrated that excessive reactive oxygen species in semen damage sperm concentration, motility, and other sperm motion parameters.

Objective: To evaluate the expression of DNA plasmid-harboring modified antibody gene that produces neutralizing human monoclonal antibodies against four serotypes of dengue virus(DENV) without enhancing activity in BALB/c mice. Methods: We constructed pFUSE-based vectors(pFUSE1 G7 C2hVH and pFUSE1 G7 C2hVL) containing genes encoding the variable domains of the heavy or light chain of the anti-dengue virus antibody 1 G7 C2, a human IgG1 that has been characterized for its neutralizing activity to DENV-1-4. Leucine(L) at positions 234 and 235 on the Fc CH2 domain in pFUSE1 G7 C2hVH was mutated to alanine(A)(LALA mutation) by site direct mutagenesis, and the new plasmid was termed pFUSE1 G7 C2hVHLALA. An equal amount of pFUSE1 G7 C2hVL and 1 G7 C2hG1-LALA plasmids were co-transfected into Chinese hamster ovary cells(CHO-K1) and a single dose of 100 μg 1 G7 C2hG1-LALA plasmid was intramuscularly injected, followed by electroporation in BALB/c mice. The secreted 1 G7 C2hG1-LALA antibodies in cell culture supernatant and mouse serum were examined for their biological functions, neutralization and enhancing activity. Results: The co-transfection of heavy-and light-chain 1 G7 C2hG1-LALA plasmids in CHO-K1 cells produced approximately 3 900 ng/mL human IgG and neutralized 90%-100% all four DENV, with no enhancing activity. Furthermore, the modified human IgG was produced more than 1 000 ng/mL in mouse serum on day 7 post plasmid injection and showed cross-neutralization to four DENV serotypes. Subsequently, antibody production and neutralization decreased rapidly. Nevertheless, the secreted neutralizing 1 G7 C2hG1-LALA in mouse serum demonstrated complete absence of enhancing activities to all DENV serotypes. Conclusions: These findings reveal that a new modified 1 G7 C2h G1-LALA expressing plasmid based on gene transfer is a possible therapeutic antibody candidate against DENV infection.

Investigation of Specifics of Self-Management towards Dealing with Cancer Pain among Adult Outpatients

Purpose: This study aimed to identify specifics of self-management towards dealing with cancer pains among adult outpatients. Methods: Semi-structured interviews were conducted with 16 adult outpatients (9 males and 7 females) who suffered from cancer pain and the data obtained were analyzed using the qualitative synthesis method of KJ Ho. Results: An analysis of the results revealed these key findings for the following symbolic wording of the semi-structured interviews: Cooperative relationship with medical professionals;Problems originating from cancer pain;Self-regulation of analgesic medications based on the interactions with medical professionals;Self-regulation of the analgesic medications based on the physical perceptions of the effects;Self-regulation to maintain a balanced and calm mind, Fulfilling life by the pain alleviation and liberation from the restrictions imposed by the cancer;and Efforts to maintain their lives by themselves. Conclusion: Based on the cooperative relationship established with medical professionals, the patient carried out the self-regulation of cancer pain using analgesics, and self-regulation to maintain a balanced and calm mind. Nurses must therefore sufficiently educate patients regarding how to communicate successfully with medical professionals in order to improve cancer patients’ pain management regarding both physical and mental aspects.

Involvement of A5/A7 noradrenergic neurons and B2 serotonergic neurons in nociceptive processing:a fiber photometry study

In the central nervous system,the A6 noradrenaline(NA)and the B3 serotonin(5-HT)cell groups are well-recognized players in the descending antinociceptive system,while other NA/5-HT cell groups are not well characterized.A5/A7 NA and B25-HT cells project to the spinal horn and form descending *** recorded G-Ca MP6 green fluorescence signal intensities in the A5/A7 NA and the B25-HT cell groups of awake mice in response to acute tail pinch stimuli,acute heat stimuli,and in the context of a non-noxious control test,using fiber photometry with a calcium imaging *** first introduced G-Ca MP6 in the A5/A7 NA or B25-HT neuronal soma,using transgenic mice carrying the tetracycline-controlled transactivator transgene under the control of either a dopamineβ-hydroxylase or a tryptophan hydroxylase-2 promoters and by the site-specific injection of adeno-associated virus(AAV-Tet O(3 G)-G-Ca MP6).After confirming the specific expression patterns of G-Ca MP6,we recorded G-Ca MP6 green fluorescence signals in these sites in awake mice in response to acute nociceptive stimuli.G-Ca MP6 fluorescence intensity in the A5,A7,and B2 cell groups was rapidly increased in response to acute nociceptive stimuli and soon after,it returned to baseline fluorescence *** was not observed in the non-noxious control *** results indicate that acute nociceptive stimuli rapidly increase the activities of A5/A7 NA or B25-HT neurons but the non-noxious stimuli do *** present study suggests that A5/A7 NA or B25-HT neurons play important roles in nociceptive processing in the central nervous *** suggest that A5/A7/B2 neurons may be new therapeutic *** performed procedures were approved by the Institutional Animal Use Committee of Kagoshima University(MD17105)on February 22,2018.
