Low skeletal muscle mass and high visceral adiposity are associated with recurrence of acute cholecystitis after conservative management:A propensity score-matched cohort study

Background:Recurrent acute cholecystitis(RAC)can occur after non-surgical treatment for acute cholecystitis(AC),and can be more severe in comparison to the first episode of AC.Low skeletal muscle mass or adiposity have various effects in several diseases.We aimed to clarify the relationship between RAC and body parameters.Methods:Patients with AC who were treated at our hospital between January 2011 and March 2022 were enrolled.The psoas muscle mass and adipose tissue area at the third lumbar level were measured using computed tomography at the first episode of AC.The areas were divided by height to obtain the psoas muscle mass index(PMI)and subcutaneous/visceral adipose tissue index(SATI/VATI).According to median VATI,SATI and PMI values by sex,patients were divided into the high and low PMI groups.We performed propensity score matching to eliminate the baseline differences between the high PMI and low PMI groups and analyzed the cumulative incidence and predictors of RAC.Results:The entire cohort was divided into the high PMI(n=81)and low PMI(n=80)groups.In the propensity score-matched cohort there were 57 patients in each group.In Kaplan-Meier analysis,the low PMI group and the high VATI group had a significantly higher cumulative incidence of RAC than their counterparts(log-rank P=0.001 and 0.015,respectively).In a multivariate Cox regression analysis,the hazard ratios of low PMI and low VATI for RAC were 5.250(95%confidence interval 1.083-25.450,P=0.039)and 0.158(95%confidence interval:0.026-0.937,P=0.042),respectively.Conclusions:Low skeletal muscle mass and high visceral adiposity were independent risk factors for RAC.

Hepatitis A complicated with acute renal failure and high hepatocyte growth factor: A case report

A 58-year-old man was admitted to our hospital. Laboratory data showed severe liver injury and that the patient was positive for immunoglobulin M anti-hepatitis A virus(HAV) antibodies. He was also complicated with severe renal dysfunction and had an extremely high level of serum hepatocyte growth factor(HGF). Therefore, he was diagnosed with severe acute liver failure with acute renal failure(ARF) caused by HAV infection. Prognosis was expected to be poor because of complications by ARF and high serum HGF. However, liver and renal functions both improved rapidly without intensive treatment, and he was subsequently discharged from our hospital on the 21 st hospital day. Although complication with ARF and high levels of serum HGF are both important factors predicting poor prognosis in acute liver failure patients, the present case achieved a favorable outcome. Endogenous HGF might play an important role as a regenerative effector in injured livers and kidneys.

A reactor system combining reductive dechlorination with co-metabolic oxidation for complete degradation of tetrachloro-entylene

A laboratory sequential anaerobic aerobic bioreactor system, which consisted of an anaerobic fixed film reactor and two aerobic chemostats, was set up to degrade tetrachloroethylene (PCE) without accumulating highly toxic degradation intermediates. A soil enrichment culture, which could reductively dechlorinate 900 μM (ca. 150 mg/L) of PCE stoichiometrically into cis 1,2 dichloroethylene ( cis DCE), was attached to ceramic media in the anaerobic fixed film reactor. A phenol degrading strain, Alcaligenes sp. R5, which can efficiently degrade cis DCE by co metabolic oxidation, was used as inoculum for the aerobic chemostats consisted of a transformation reactor and a growth reactor. The anaerobic fixed film bioreactor showed more than 99 % of PCE transformation into cis DCE in the range of influent PCE concentration from 5 μM to 35 μM at hydraulic retention time of 48h. On the other hand, efficient degradation of the resultant cis DCE by strain R5 in the following aerobic system could not be achieved due to oxygen limitation. However, 54% of the maximum cis DCE degradation was obtained when 10 μmol of hydrogen peroxide (H 2O 2) was supplemented to the transformation reactor as an additional oxygen source. Further studies are needed to achieve more efficient co metabolic degradation of cis DCE in the aerobic reactor.

Abiotic degradation of four phthalic acid esters in aqueous phase under natural sunlight irradiation

Abiotic degradability of four phthalic acid esters （PAEs） in the aquatic phase was evaluated over a wide pH range 5-9. The PAE solutions in glass test tubes were placed either in the dark and under the natural sunlight irradiation for evaluating the degradation rate via hydrolysis or photolysis plus hydrolysis, respectively, at ambient temperature for 140 d from autumn to winter in Osaka, Japan. The efficiency of abiotic degradation of the PAEs with relatively short alkyl chains, such as butylbenzyl phthalate （BBP） and di-nbutyl phthalate （DBP）, at neutral pH was significantly lower than that in the acidic or alkaline condition. Photolysis was considered to contribute mainly to the total abiotic degradation at all pH. Neither hydrolysis nor photolysis of di-ethylhexyl phthalate （DEHP） proceeded significantly at any pH, especially hydrolysis at neutral pH was negligible. On the other hand, the degradation rate of di- isononyl phthalate （DINP） catalyzed mainly by photolysis was much higher than those of the other PAEs, and was almost completely removed during the experimental period at pH 5 and 9. As a whole, according to the half-life （t1/2） obtained in the experiments, the abiotic degradability of the PAEs was in the sequence： DINP （32-140 d）〉 DBP （50-360 d）, BBP （58-480 d）〉 DEHP （390-1600 d） under sunlight irradiation （via photolysis plus hydrolysis）. Although the abiotic degradation rates for BBP, DBP, and DEHP are much lower than the biodegradation rates reported, the photolysis rate for DINP is comparable to its biodegradation rate in the acidic or alkaline condition.

Optimal turning method of composting regarding hygienic safety

The new turning method was proposed and verified its effectiveness to pathogens by laboratory scale experiments. Considering the results obtained from the previous studies, it could be said that turning of a composting pile was essential in terms of hygienic aspects but the number of turning should be minimized. Effectiveness of inactivation was estimated for each composting run. From this estimation, turning by layers, which is different from conventional turning that mixes compost pile entirely, was proposed and investigated its performance by experiments. Composting operations with static pile method, complete mix(conventional) turning method, and proposed turning(layer turning) method were done and their effectiveness on inactivation of indicator microorganism was evaluated and compared. As results, the conventional turning method was not a proper method in terms of pathogen inactivation, whereas, the proposed turning method showed an excellent performance and should be employed in a composting operation.

INFLUENCE OF THREAD PITCH ON LOAD DISTRIBUTION ON THREADED END OF PRESSURE CYLINDER

One of the design methods for closing the end of a pressure cylinder is to screw down a screw plug on the threaded end of the cylinder. In this case, there is the problem of stress concentration in the threaded end of the pressure cylinder. To solve the problem, it is necessary to know accurately the load distribution on the threaded end of the cylinder. To find the load distribution on the threaded end of the pressure cylinder engaged with the screw plug, the following experiments are carried out. Applying the tensile load between the screw plug and the pressure cylinder and regarding the situation above as equivalent to the situation in which the internal pressure is applied, the load distribution is measured with the strain gauge. The influence of thread pitch on the load distribution on the threaded end of the cylinder is presented and an optimum pitch for design is discussed.

Chemical Composition of the Essential Oil of Mastic Gum and their Antibacterial Activity Against Drug-Resistant Helicobacter pylori

Mastic gum is derived from the tree named Pistacia lentiscus that is grown only in Island Hios of Greek.Since Mastic was first reported to kill Helicobacter pylori(H.pylori)in 1998,there has been no further study to elucidate which component of mastic specifically shows the antimicrobial activity against H.pylori.In this study,we examined which component of mastic gum was responsible for anti-H.pylori activity.We prepared the essential oil of mastic gum and identified 20 constituents by GC–MS analysis.Ten standard components were assayed for anti-H.pylori activity,and it clarified that a-terpineol and(E)-methyl isoeugenol showed the anti-H.pylori activity against four different H.pylori strains that were established from patients with gastritis,gastric ulcer and gastric cancer.These components could be useful to overcome the drug-resistance H.pylori growth in stomach.

Driving force and structural strength evaluation of a flexible mechanical system with a hydrostatic skeleton

The purpose of this study was to build a flexible mechanical system with a hydrostatic skeleton.The main components of this system are two type flexible bags.One is a structural bag with constant inner pressure.The other is an actuator bag with controlled inner pressure.To design the system,it was necessary to estimate both structural deformation and driving force.Numerical analysis of flexible bags,however,is difficult because of large nonlinear deformation.This study analyzed structural strength and driving force of flexible bags with the nonlinear finite element analysis (FEA) software ABAQUS.The stress concentration dependency on the bag shape is described and the driving force is calculated to include the large deformation.From the analytical results,this study derives an empirical equation of driving force.The validity of the equation was confirmed by condition-changed analyses and experimental results.

Biodegradability of four phthalic acid esters under anaerobic condition assessed using natural sediment

Biodegradability of di-n-butyl phthalate （DBP）, butylbenzyl phthalate （BBP）, di-ethylhexyl phthalate （DEHP）, and di-isononyl phthalate （DINP） under an anaerobic condition was evaluated using three natural sediment microcosms obtained from ponds in Osaka, which had not been significantly polluted by the chemicals. The degradabilities of the four phthalic acid esters（PAEs） were analyzed by a first-order kinetic model with a lag phase and ranked as DBP〉BBP〉〉DEHP〉DINP. The PAEs with shorter alkyl-chains, DBP and BBP, were degraded with quite short lag phases near to zero and short half-lives of a few days. The PAEs with longer alkyl-chains, DEHP and DINP, were degraded with lag phases of 5-30 d and the quite long half-lives of a couple of hundred days. Although no data was available on the anaerobic biodegradability of D1NP before this study, it was clarified that DINP can be degraded with slow degradation rates. The fact that all the three intact sediments were capable of biodegradation of the PAEs suggests that potential of anaerobic biodegradation of PAEs is widespread in the aquatic environment.

Accelerated degradation of a variety of aromatic compounds by Spirodela polyrrhiza-bacterial associations and contribution of root exudates released from S. polyrrhiza

Removal experiments of phenol, aniline, 2,4-dichlorophenol, nonylphenol and bisphenol A （BPA） using Spirodela polyrrhiza- bacterial associations revealed that all compounds but BPA underwent accelerated removal. The mechanisms differed depending on the substrates. It was found that S. polyrrhiza has a great ability to release phenolic compound-rich root exudates, and the exudates seem to stimulate bacterial degradation of a variety of aromatic compounds.
