Objective‘Sludge’refers to administrative burdens or frictions that preclude people from getting what they want or need(eg,duplicative forms,complicated instructions,long waiting times).This mixed methods study evaluated patients’perceptions of sludge in the colorectal cancer(cRc)screening process and some impacts of this *** We employed an exploratory sequential mixed methods study design that comprised patient interviews and a patient *** interviews informed final survey revisions and captured contextual data about patients’experiences with *** transcripts were inductively and deductively analysed to identify overarching *** survey quantified sludge,delayed or forgone screenings,screening experience(Net Promoter Score)and health system distrust(Health System Distrust Scale).We usedχ2 or t-tests for univariable comparisons and logistic or linear regressions to evaluate the association between cumulative sludge score and delayed or forgone screenings,screening experience and health system *** were integrated for *** Southeastern United *** Patients who were 45–75 years of age,at average risk for cRc and had either completed or been referred for cRc screening(colonoscopy or stool-based test)within the previous 12 *** 22 interview participants and 255 survey participants completed the study.38(15%)survey participants rated their screening experience as poor(Net Promoter Score=0–7 out of 10).The mean(SD)Health System Distrust Scale score was 22.4(6.3)out of 45 possible points(higher score=greater distrust).Perceptions of sludge in the cRc screening process varied,with long waiting times and burdensome communication being the most common sources(58%and 35%of participants,respectively).Sludge was positively associated with delayed or forgone screenings(OR=1.42,95%cI 1.28,1.57,pcreening experience(OR=1.15,95%cI 1.04,1.28,p=0.009)and health system distrust(β=0.47,p

[背景]评估由于环境空气中细颗粒物（PM2.5）长期暴露引起的疾病负担,需要了解相对风险度（RR）函数的图形和幅度。然而,在世界上很多观察到高PM2.5环境浓度的地方,都缺乏足够的直接证据确定死亡RR函数的图形。[目的]建立可用于全球PM2.5暴露范围内成人死因的RR函数：缺血性心脏病（IHD）、脑血管疾病（脑卒中）、慢性阻塞性肺病（cOPD）和肺癌（Lc）。建立急性下呼吸道感染（ALRI）发病的RR函数,用于估算〈5岁的儿童中死亡率和健康生活损失年数。[方法]通过整合现有的来自于环境空气污染（AAP）、二手烟草烟雾、家用固体烹饪用燃料和主动吸烟（AS）等研究的RR信息,拟合综合暴露-反应（IER）模型。采用颗粒物质的吸入剂量,通过转化AS暴露来估计全年PM2.5的暴露当量。在估算全球环境PM2.5浓度的基础上,得出各国的人群归因分数（PAFs）。[结果]与以往负担评估中使用的其他7种模型相比,IER模型是一种优越的RR预测方法。在不同国家中,归因于AAP暴露的PAF百分数各不相同,分别为：IHD,2~41;脑卒中,1~43;cOPD,〈1~21;Lc,〈1~25;ALRI,〈1~38。[结论]我们建立了一种以细颗粒物为基础的RR模型,它通过整合来自不同类型的燃烧所产生颗粒物排放的RR信息,涵盖全球范围的暴露。当获得新的RR信息时,可以对这个模型进行更新。

Double-pulse LIBS is a promising technique for deep-sea *** measurements in shallow water with up to 400 mJ each pulse were done to select laser parameters which promote optimized spectral line emission from plasma even at elevated pressures,where line broadening until loss of most of the spectral information can *** emission spectroscopy,using a czerny-turner spectrometer,has been applied to investigate the dependence of the emitted radiation on laser parameters and hydrostatic *** has been found,that higher laser pulse energies,especially with short pulse delay as required in high water pressure,can also have an adverse effect on the measured spectrum.

We present a comprehensive catalog with an updated database of the fossil record of Diplopoda in the *** data was collected from descriptions and reports published from 1854 to the *** also include new records from Mexican amber(Upper Oligocene-Lower Miocene),counting 83 unknown fossil inclusions,with the first records of the orders Polyxenida,Platydesmida,and Julida,as well as the families Sphaeriodesmidae and Trichopolydesmidae within *** to our results,Diplopoda counts 413 records featuring 299 distinct taxa ranging from the Middle Silurian to the Upper Pleistocene,representing three subclasses,two subterclasses,six superorders,25 orders,15 superfamilies,55 families,93 genera,and 161 fossil *** date,no fossils of the order Siphonocryptida have been *** fossil record extends over three geological eras:the Paleozoic,with 161 taxa,including 97 species;the Mesozoic,with 53 taxa and 22 species;and the cenozoic,with 85 taxa and 42 *** referred material shows different forms of fossil *** comprises 123 impressions,99 compressions,26 ichnites,predominantly from Paleozoic and Mesozoic sediments,and 165 amber inclusions from late Mesozoic and cenozoic ***,this catalog allows us to estimate the size and taxonomic composition of Diplopoda in the fossil record worldwide.

Sedimentary records of contaminant inputs in Frobisher Bay,Nunavut

contaminants,such as polychlorinated biphenyls(PcBs),polycyclic aromatic hydrocarbons(PAHs),heavy metals,and per and polyfluoroalkyl substances(PFASs),primarily reach the Arctic through long-range atmospheric and oceanic ***,local sources within the Arctic also contribute to the levels observed in the environment,including legacy sources and new sources that arise from activities associated with increasing commercial and industrial *** city of Iqaluit in Frobisher Bay,Nunavut(canada),has seen rapid population growth and associated development during recent decades yet remains a site of interest for ocean protection,where Inuit continue to harvest country *** the present study,seven dated marine sediment cores collected in Koojesse Inlet near Iqaluit,and from sites in inner and outer Frobisher Bay,respectively,were analyzed for total mercury(THg),major and trace elements,PAHs,PcBs,and *** sedimentary record in Koojesse Inlet shows a period of Aroclor 1260-like PcB input concurrent with military site presence in the 1950e60s,followed by decades of input of pyrogenic PAHs,averaging about ten times background ***-surface sediments in Koojesse Inlet also show evidence of transient local-source inputs of THg and PFASs,and recycling or continued slow release of PcBs from legacy land-based *** in PFAS congener composition clearly distinguish the local sources from long-range *** Koojesse Inlet but still in inner Frobisher Bay,9.2 km from Iqaluit,sediments showed evidence of both local source(PcB)and longrange *** outer Frobisher Bay,an up-core increase in THg and PFASs in sediments may be explained by ongoing inputs of these contaminants from long-range *** context for ocean protection and country food harvesting in this region of the Arctic clearly involves both local sources and long-range transport,with past human activities leaving a long legacy insofar as levels of persistent o

The carbon isotope discrimination （δ^13c） of leaves has been shown to be correlated with the transpiration efficiency of leaves in a wide range of species. This has led to δ^13c being used in breeding programs to select for improved transpiration efficiency. The correlation between δ^13c and transpiration efficiency was determined under well-watered conditions during the vegetative phase in six genotypes of lentil （Lens culinaris Medikus）, six genotypes of chickpea （cicerarietinum L.） and 10 cultivars of narrow-leafed lupin （Lupinus angustifolius L.）. Biomass （dry matter） accumulation and water use （transpiration） varied among the genotypes in all three species and transpiration efficiency was 40% to 75% higher in the most efficient compared with the least efficient genotypes. However, δ^13c and transpiration efficiency were not significantly correlated in any of the species. This suggests that the δ^13c technique cannot be used in selection for transpiration efficiency in the three grain legumes （pulses） studied.

Metabolic syndrome and the urinary microbiome of patients undergoing percutaneous nephrolithotomy

Objective:To identify possible stone-promoting microbes,we compared the profiles of microbes grown from stones of patients with and without metabolic syndrome(MetS).The association between MetS and urinary stone disease is well established,but the exact pathophysiologic relationship remains *** evidence suggests urinary tract dysbiosis may lead to increased nephrolithiasis ***:At the time of percutaneous nephrolithotomy,bladder urine and stone fragments were collected from patients with and without *** sample types were subjected to expanded quantitative urine culture(EQUc)and 16 S ribosomal RNA gene ***:Fifty-seven patients included 12 controls(21.1%)and 45 MetS patients(78.9%).Both cohorts were similar with respect to demographics and non-MetS *** controls had uric acid stone *** EQUc,bacteria were detected more frequently in MetS stones(42.2%)compared to controls(8.3%)(p=0.041).Bacteria also were more abundant in stones of MetS patients compared to *** validate our EQUc results,we performed 16 S ribosomal RNA gene *** 12/16(75.0%)sequence-positive stones,EQUc reliably isolated at least one species of the sequenced *** were detected in both“infectious”and“non-infectious”stone ***:Bacteria are more common and more abundant in MetS stones than control *** findings support a role for bacteria in urinary stone disease for patients with MetS regardless of stone composition.

Thermal conductivity suppression in uranium-doped thorium dioxide due to phonon-spin interactions

In this work,impact of low level of uranium(U)atom substitution on thermal conductivity of thorium dioxide(ThO_(2))is ***_(2)is an electronic insulator with a wide optical band-gap and no unpaired electrons whose thermal transport is governed by phonons.U-substitution introduces unpaired f-electrons resulting in paramagnetic behavior of UeThO_(2)at room temperature,which significantly suppresses its thermal conductivity.A single crystal of UeThO_(2)with graded composition of U is grown using a hydrothermal synthesis method,and thermal conductivity measurements are performed in re-gions with uniform composition of U at levels of 0%,6%,9%and 16%.Measured thermal conductivity profiles over 77e300 K temperature range are analyzed using an analytical expression for phonon-mediated thermal transport based on Klemens-callaway *** dependent thermal con-ductivity is found to deviate significantly from the Rayleigh scattering trend expected for a simple substitutional point defect with a small perturbation to mass and interatomic *** the resonant scattering term,observed large suppression of thermal conductivity at low temperatures can be closely ***,the extracted phonon-spin coupling constants imply a nonlinear relation of phonon-spin interaction intensity with respect to U doping *** study reveals how phonon-spin scattering contributed by unpaired f-electrons in U atoms influences thermal transport in the UeThO_(2)system.
