The apple(Malus domestica)is one of the world's most commercially important perennial crops and its improvement has been the focus of human effort for thousands of ***,we genetically characterise over 1000 apple accessions from the United States department of Agriculture(USDA)germplasm collection using over 30,000 singlenucleotide polymorphisms(SNPs).We con fi rm the close genetic relationship between modern apple cultivars and their primary progenitor species,Malus sieversii from Central Asia,and fi nd that cider apples derive more of their ancestry from the European crabapple,Malus sylvestris,than do dessert *** determine that most of the USDA collection is a large complex pedigree:over half of the collection is interconnected by a series of fi rst-degree *** addition,15%of the accessions have a fi rst-degree relationship with one of the top 8 cultivars produced in the *** the exception of‘Honeycrisp',the top 8 cultivars are interconnected to each other via pedigree *** cultivars‘Golden delicious'and‘Red delicious'were found to have over 60 fi rst-degree relatives,consistent with their repeated use by apple *** detected a signature of intense selection for red skin and provide evidence that breeders also selected for increased fi *** results suggest that Americans are eating apples largely from a single family tree and that the apple's future improvement will bene fi t from increased exploitation of its tremendous natural genetic diversity.

AIM:To investigate the serovirological prevalence and clinical features of hepatitis E virus(HEV) infection in end-stage renal failure patients and in the healthy ***:HEV infection is a viral disease that can cause sporadic and epidemic *** studies unexpectedly showed a high prevalence of HEV antibodies in immunosuppressed subjects,including hemodialysis(HD)patients and patients who had undergone kidney transplant.A cohort/case-control study was carried out from January 2012 to August 2013 in two hospitals in southern Italy(Foggia and *** Rotondo,Apulia).The seroprevalence of HEV was determined in 801 subjects;231 HD patients,120 renal transplant recipients,and450 health *** HD patients and the recipients of renal transplants were attending the departments of Nephrology and Dialysis at two hospitals located in Southern Italy,and were included progressively in this *** samples were tested for HEV antibodies(Ig G/Ig M);in the case of positivity they were confirmed by a Western blot assay and were also tested for HEV-RNA,and the HEV genotypes were ***:A total of 30/801(3.7%)patients were positive for anti-HEV Ig(Ig G and/or Ig M)and by Western *** healthy population presented with a prevalence of 2.7%,HD patients had a prevalence of 6.0%,and transplant recipients had a prevalence of 3.3%.The overall combined HEV-positive prevalence in the two groups with chronic renal failure was 5.1%.The rates of exposure to HEV(positivity of HEV-Ig G/M in the early samples)were lower in the healthy controls,but the difference among the three groups was not statistically significant(P>0.05).Positivity for anti-HEV/Ig M was detected in 4/30(13.33%)anti-HEV Ig positive individuals,in 2/14 HD patients,in1/4 transplant individuals,and in 1/12 of the healthy *** relative risk of being HEV-Ig M-positive was significantly higher among transplant recipients compared to the other two groups(OR=65.4,95%CI:7.

PURPOSE: Ileal pouch-anal anastomosis has come to represent the procedure of choice for patients requiring surgery for mucosal ulcerative colitis. In contrast, a proven diagnosis of Crohn’s disease is generally held to preclude ileal pouch-anal anastomosis. However, patients with ileal pouch-anal anastomosis for apparent mucosal ulcerative colitis who are subsequently found to have Crohn’s disease have a variable course. We reviewed our experience in this scenario to determine whether selected patients with Crohn’s disease may be candidates for ileal pouchanal anastomosis. METHODS: A retrospective review of the prospectively maintained ileal pouch-anal anastomosis database was undertaken to identify patients with a diagnosis of Crohn’s disease after ileal pouch-anal anastomosis. Clinical outcome and quality-of-life data were obtained from the database and chart review. End points were the development of recrudescent Crohn’s disease, pouch failure, and quality of life and functional outcome at the time of data collection. Differences between groups were calculated using the chisquared test. Cumulative incidence of recrudescent Crohn’s disease and pouch loss were calculated by the Kaplan-Meier method. Factors predictive of development of recrudescent Crohn’s disease and pouch loss were examined by univariate analysis. RESULTS: Sixty patients (32 females; median age, 33 (range, 15-74) years) who underwent ileal pouch-anal anastomosis for mucosal ulcerative colitis subsequently had that diagnosis revised to Crohn’s disease. Median follow-up of all patients was 46 (range, 4-158) months at time of data collection by which time 21 patients (35 percent) had developed recrudescent Crohn’s disease. No pre-ileal pouch-anal anastomosis factors examined were predictors of the development of recrudescent Crohn’s disease on univariate analysis. INTRODUCTION:This study was designed to evaluate long-term outcomes for patients undergoing Kock continent ileostomy,identify factors associated with adverse outcomes,and compare changes in quality of life after removal of the ***:The records of all patients(n = 330)undergoing continent ileostomy at the Cleveland Clinic Foundation between 1974 and 2001 were ***-related,intraoperative,and postoperative factors were evaluated as predictor variables of long-term pouch *** of life was evaluated using the continent ileostomy surgery follow-up questionnaire and the Cleveland Global Quality of Life scale(n = 216).These were compared between patients with continent ileostomy(n = 181)and patients who underwent removal of the continent ileostomy and conversion to an end stoma(n = 35).RESULTS:The median patient follow-up was 11(range,1-27)*** median revision-free pouch interval was 14(95 percent confidence interval,11-17)*** 10-year and 20-year pouch survival was 87 and 77 percent,*** had an average of 3.7(range,1-28)complications and 2.9(range,1-27)pouch revisions during *** multivariate analysis,Crohn’s disease(hazard ratio = 4.5),female gender(hazard ratio = 2.4),fistula development(hazard ratio = 3),and body mass index(hazard ratio = 2.4 per 5 unit increase)were independent predictors of pouch *** of life measurements for patients with a continent ileostomy were higher on all scales in comparison with patients who had the Kock reservoir and then reverted to a Brooke ***:despite the associated morbidity with continent ileostomy surgery,long-term results and quality of life were *** ileostomy may be offered as an attractive long-term option to select patients whose only alternative is an end ileostomy.

评价９９ｍＴｃ-甲氧基异丁基异腈（ＭＩＢＩ）ＳＰＥＣＴ定量分析在心肌梗塞（ＭＩ）区存活心肌判断中的准确性，及视觉判断９９ｍＴｃ-ＭＩＢＩＳＰＥＣＴ结果在缺血与坏死心肌鉴别中的局限性．方法：３４例ＭＩ患者进行了９９ｍＴｃ-ＭＩＢＩＳＰＥＣＴ心肌灌注显像及１８Ｆ标记的脱氧葡萄糖（ＦＤＧ）ＰＥＴ显像．结果：当局部心肌９９ｍＴｃ-ＭＩＢＩ放射性小于左室心肌峰计数的４０％，存活心肌存在的可能性仅为２４％左右；相反，当局部９９ｍＴｃ-ＭＩＢＩ放射性大于左室心肌峰计数的４０％，则９０％以上的心肌节段均为存活心肌．故视觉判断不可逆性缺损明显低估存活心肌．约５９％的不可逆性缺损节段在ＰＥＴ显像中有明确１８Ｆ-ＦＤＧ摄取，提示为存活心肌．结论：９９ｍＴｃ-ＭＩＢＩＳＰＥＣＴ定量分析能明显提高存活心肌的定位诊断，慎重判断及定义心肌灌注显像中的不可逆性缺损区，重视定量分析的研究及应用对ＭＩ区存活与坏死心肌的鉴别诊断有重要价值．

The world pandemic due to coronavirus disease 2019,known as COVID-19,embodies a high rate of disease transmission that causes a critical hospitalization *** of May 15,2020,the disease has been the cause of more than 4 million infections and more than 280000 deaths all over the *** the beginning,we underestimated the disease;now,we have sufficient information and it is clear that it is not just a respiratory *** fact,if a prompt treatment is not initiated,the disease may evolve towards an abnormal immune response and cytokine storm with an important thrombophilic ***,we think that while waiting for certainties to be established by evidence-based medicine,it is not ethical to not try off-label therapies for some of the well-known drugs,as they could have some efficacy based on their mechanisms of action.

补偿收缩砂浆通常为预拌水泥基材料。在众多粉状组成成分中便含有膨胀剂,以死烧氧化钙(CaO)或硫铝酸钙(CSA)为代表的膨胀剂,在钢筋或粗糙混凝土基层摩擦力的限制作用下,在修补砂浆中产生压应力。由于CaO或CSA膨胀剂只有在有水的情况下才能水化,因此为保证使用效果,砂浆必须仔细养护,如不进行仔细的潮湿养护,修补工作很可能由于没有产生有效的压应力而失败,预压应力由限制膨胀产生。本工作致力于研究含CaO或CSA膨胀剂、减缩剂(SRA)砂浆在有无潮湿养护下的性能。由于有减缩剂SRA与膨胀剂的配合,即使在没有任何潮湿养护措施下,也会产生限制膨胀。但是,当CaO与减缩剂SRA复合使用时,这种效应比CSA膨胀剂更明显。
