Watch and wait policy in advanced neuroendocrine tumors: What does it mean?

Neuroendocrine neoplasms(NENs) are a group of rare and heterogeneous malignancies, which can develop in various organs. The clinical course of NENs is quite heterogeneous, with different spontaneous growth rates after diagnosis, and different degrees of sensitivity to the same therapy even when they have similar characteristics. Watch and wait(W and W), is a term coined to indicate observation being conducted to assess the evolution of the tumor without administering any anti-tumor therapy. It has been applied to NENs since in extremely rare cases they tend to remain stable for a long time. Although W and W has been reported in several guidelines and recommendations it has never been validated, nor has it been specifically investigated. Furthermore it is not standardized. Therefore its application in clinical practice can differ in terms of tumor status assessment, type and timing of imaging or other exams utilized. In conclusion, while undertaking W and W to delay the first-line therapy by some weeks may be justified in good performance asymptomatic patients with low-grade NENs in order to usefully characterize the disease and patient and thereby choose the best therapy and therapeutic strategy, it seems to be far more difficult to justify W and W with the intent of avoiding an anti-tumor treatment. It should be considered that not only do NENs tend to grow even when they have very favorable biological characteristics but also that the alternative to W and W is most commonly a low toxic and effective treatment with somatostatin analogs.

Pancreatic neuroendocrine tumors(panNETs)represent a rare subgroup of neuroendocrine neoplasms(NENs)which showed an impressive increase of incidence over the last decade(1).As over the same period particularly low stage and low grade NETs had the highest increase it is plausible that small(e.g.,dentally detected may have had a major role thanks to the progress of imaging.

We present u＇g＇R optical images taken with the MMT/Megacam and the Subaru/Suprime telescopes of the Extended Groth Strip survey. The total survey covers an area of about -1 degree2, including four sub-fields and is optimized for the study of galaxies at z - 3. Our methods for photometric calibration in AB magnitudes, the limiting magnitude and the galaxy number count are described. A sample of 1642 photometrically selected candidate Lyman-Break Galaxies （LBGs） to an apparent RAB magnitude limit of 25.0 is presented. The average sky surface density of our LBG sample is -1.0 arcmin-2, slightly higher than the previous finding.

POEMS syndrome is a rare paraneoplastic disorder due to an underlying clone of aberrant plasma *** name POEMS is an acronym for some of the major disease manifestations,namely polyneuropathy,organomegaly,endocrinopathy,presence of monoclonal component,and skin *** clinical presentation can be various and could lead to delayed diagnosis and *** is known about the pathogenic mechanism,although the neoangiogenesis due to overproduction of vascular endothelial growth factor(VEGF)by plasma cells seems to play a key *** latest evidence suggests that the blood concentration of this cytokine correlates with the activity of the syndrome:VEGF could then be used as a therapeutic target and a marker to monitor *** case reports have shown the efficacy of this approach,but extended studies are required to better define the use of anti VEGF in patients affected by POEMS syndrome.

PEG-PLGA electrospun nanofibrous membranes loaded with Au@Fe2O3 nanoparticles for drug delivery applications

A PEGylated-PLGA random nanofibrous membrane loaded with gold and iron oxide nanoparticles and with silibinin was prepared by electrospinning deposition. The nanofibrous membrane can be remotely controlled and activated by a laser light or magnetic field to release biological agents on demand. The nanosystems were characterized using scanning electron microscopy, Fourier transform infrared spectroscopy, nuclear magnetic resonance spectroscopy, and thermogravimetric analyses. The drug loading efficiency and drug content percentages were determined by UV-vis optical absorption spectroscopy. The nanofibrous membrane irradiated by a relatively low-intensity laser or stimulated by a magnetic field showed sustained silibinin release for at least 60 h, without the burst effect. The proposed low-cost electrospinning procedure is capable of assembling, via a one-step procedure, a stimuli-responsive drug-loaded nanosystem with metallic nanoparticles to be externally activated for controlled drug delivery.

Hepatocellular carcinoma treated with transarterial chemoembolization: Dynamic perfusion-CT in the assessment of residual tumor

AIM: To asses the value of computed tomography (CT)-perfusion in the detection of residual hepatocellular carcinoma (HCC) vascularization after transarterial chemoembolization (TACE). METHODS: Thirty-two consecutive patients were pro-spectively included in this study. All patients had liver cirrhosis and a conf irmed HCC lesion which was treated with TACE. One month after treatment, perfusion measurements of treated lesions were carried out. The CTperfusion (CT-p) protocol was performed with 16 slice multidetector computed tomography which included the following parameters: 8 dynamic slices/scan per 40 scans after iv injection of 50 mL of iodinated contrast (350 mg/mL) at a flow rate of 6 mL/s. Treated lesions were evaluated using dedicated perfusion software, which generated a quantitative colour map of perfusion. The following parameters were considered: hepatic perfusion (HP), arterial perfusion (AP), blood volume (BV), hepatic perfusion index (HPI), and time to peak (TTP). Perfusion parameters were described with quartile values of their distribution and statistically analyzed. RESULTS: Perfusion parameters of the treated lesions could be quantitatively assessed using CT-p analysis. The presence of residual tumor tissue was observed in 13 of the 32 patients. The values of the perfusion parameters measured within the relapse tissue were: HP (mL/100 g per minute): median = 44.4 (1stqt = 31.3, 3rdqt = 55.8); BV (mL/100 g): median = 18.7 (1stqt = 11.5, 3rdqt = 22.5); AP (mL/min): median = 39.0 (1stqt = 36.5, 3rdqt = 61.3); HPI (%): median = 34.0 (1stqt = 30.4, 3rdqt = 38.9); TTP (s): median = 17.3 (1stqt = 15.8, 3rdqt = 26.5). With the use of the univariate paired Wilcoxon signed rank test, HP, AP and HPI were shown to be significantly higher (Pdency to be greater and lower, respectively, in the relapse site than in the primary lesion. Wearable devices and IoT applications for symptom detection, infection tracking, and diffusion containment of the COVID-19 pandemic: a survey

Until a safe and effective vaccine to fight the SARS-CoV-2 virus is developed and available for the global population, preventive measures, such as wearable tracking and monitoring systems supported by Internet of Things(IoT) infrastructures, are valuable tools for containing the pandemic. In this review paper we analyze innovative wearable systems for limiting the virus spread, early detection of the first symptoms of the coronavirus disease COVID-19 infection, and remote monitoring of the health conditions of infected patients during the quarantine. The attention is focused on systems allowing quick user screening through ready-to-use hardware and software components. Such sensor-based systems monitor the principal vital signs, detect symptoms related to COVID-19 early, and alert patients and medical staff. Novel wearable devices for complying with social distancing rules and limiting interpersonal contagion(such as smart masks) are investigated and analyzed. In addition, an overview of implantable devices for monitoring the effects of COVID-19 on the cardiovascular system is presented. Then we report an overview of tracing strategies and technologies for containing the COVID-19 pandemic based on IoT technologies, wearable devices, and cloud computing. In detail, we demonstrate the potential of radio frequency based signal technology, including Bluetooth Low Energy(BLE), Wi-Fi, and radio frequency identification(RFID), often combined with Apps and cloud technology. Finally, critical analysis and comparisons of the different discussed solutions are presented, highlighting their potential and providing new insights for developing innovative tools for facing future pandemics.

AFGL 5157 NH_3 : a new stellar cluster in the forming process

We present the analysis of Spitzer/IRAC and near infrared imaging obser- vation of AFGL 5157, an active star forming region. In the IRAC images, this region shows strong emissions of polycyclic aromatic hydrocarbons in channel 4 and emis- sions of H2 in channel 2. Many of the H2 features are aligned to form jet-like struc- tures. Three bipolar jets in the NHa core region and a couple of jets northwest of the core have been identified. We identify the possible driving agents of the bipolar jets and show them to be very young. An embedded cluster has been detected in the NH3 core; many members in the cluster have spectral energy distributions that increase from JHK bands toward longer wavelengths, indicative of their early evolutionary stages. Millimeter and submillimeter continuum emissions in the NH3 core and the northwest subregion are found to coincide spatially with these presumable Class0/I sources. The existence of H2 bipolar jets and very young stellar objects suggests that star formation is continuing at the present epoch in these subregions. Combining in- formation from previous studies, we propose a sequential star formation scenario in the whole AFGL 5157 region.
