Acute biliary pancreatitis （ABP） is rare in childhood and endoscopic sphincterotomy should be avoided in the child due to the risk of both early and late complications but, when necessary, the optimal timing between endoscopic procedure and cholecystectomy is still uncertain, A nine years old child with acute biliary pancreatitis underwent successfull laparo-endoscopic ＂Rendez-Vous＂ procedure in which endoscopic drainage of the common bile duct and laparoscopic cholecystectomy were performed simultaneously. This is the first case reported of laparo-endoscopic RendezoVous in a child. The excellent outcome of this patient and the review of the literature concerning other available options for the treatment of such cases suggest that this procedure offers great advantages, especially in children, of reducing the required number of treatments, the risk of ineffectiveness, the number of anaesthesia, the length of hospital stay and the risk of iatrogenic morbidity.

AIM: To investigate our clinical experience with combined laparo-endoscopic Rendezvous (RV) for the treatment of patients affected by gallstones and common bile duct (CBD) stones and especially to study the never evaluated opinion of the endoscopist concerning the difficulty of the intraoperative endoscopic procedure during the RV in comparison with standard endoscopic retrograde cholangio-pancreatography (ERCP). METHODS: Eighty consecutive patients affected by cholecystolithiasis and diagnosed or suspected CBD stones were treated with a standardized "tailored" RV. The relevant technical features, the feasibility, the effectiveness in stone clearance, the safety but also the simple evaluation of difficulty and agreement of the endoscopist were analyzed with a questionnaire. RESULTS: The feasibility was 97.5% and the effectiveness 100% concerning CBD clearance and solution of coexisting problems at the papilla. Minor morbidity was 3.3%, the operating time was prolonged by a mean of 14 min, the mean hospital stay was 3.8 d and only one stone’s recurrence occurred. The endoscopist evaluated the procedure to be simpler than standard ERCP-ES in 81.2% of the ***: Simultaneous RV carries higheffectiveness and safety at least comparable to those reported for other options. The endoscopist is very often satisfied with this approach because of the minimization of some steps of the endoscopic procedure and avoidance of relevant iatrogenic risk factors. If the mandatory collaboration between surgeons and endoscopists is guaranteed, this approach can often be preferable for the patient, the surgeon, the endoscopist and the hospital.

Colonic perforation during endoscopic diagnostic or therapeutic procedures,represents an uncommon occurrence even if,together with haemorrhage,it is still the most common complication of colonoscopy,with an incidence ranging between 0.1% and 2% of all colonoscopic procedures. The ideal treatment in these cases remains elusive as the endoscopist and the surgeon have to make a choice case by case,depending on many factors such as how promptly the rupture is identified,the condition of the patient,the degree of contamination and the evidence of peritoneal irritation. Surgical interventions both laparotomic and laparoscopic,and other medical non-operative solutions are described in the literature. Only three cases have been reported in the literature in which the endoscopic apposition of endoclips was used to repair a colonic perforation during colonoscopy. Ours is the first case that the perforation itself was caused by the improper functioning of a therapeutic device.

The benefits of regular physical activity are well ***,few studies have examined the effectiveness of integrating physical activity(PA)into curricula within a post-secondary *** investigate the incorporation of PA into medical curriculum,we developed a series of optional exercise-based review sessions designed to reinforce musculoskeletal(MSK)anatomy course *** synchronous sessions were co-taught by a group fitness instructor and an anatomy *** fitness instructor would lead students through both strength and yoga style exercises,while the anatomy instructor asked questions about relevant anatomical structures related to course material previously *** the sessions,participants were asked to evaluate the classes on their self-reported exam preparedness in improving MSK anatomy knowledge,PA levels,and mental *** participants completed surveys;a majority agreed that the classes increased understanding of MSK concepts(90.0%)and activity levels(97.7%).Many(70.0%)felt that the classes helped reduce *** majority of respondents(90.0%)agreed that the classes contributed to increased feelings of social ***,medical students saw benefit in PA based interventions to supplement MSK course *** with increasing activity levels and promoting health behaviours,integrating PA into medical curriculum may improve comprehension of learning material,alleviate stress and foster social connectivity among medical students.

The study of the interaction of mud-flows with obstacles is important to define inundation zones in urban areas and to design the possible structural countermeasures. The paper numerically investigates the impact of a mud-flow on rigid obstacles to evaluate the force acting on them using two different depth-integrated theoretical models, Single-Phase Model(SPM) and Two-Phase Model(TPM), to compare their performance and limits. In the first one the water-sediment mixture is represented as a homogeneous continuum described by a shearthinning power-law rheology. Alternatively, the twophase model proposed by di Cristo et al in 2016 is used, which separately accounts for the liquid and solid phases. The considered test cases are represented by a 1D landslide flowing on a steep slope impacting on a rigid wall and a 2D mud dam-break flowing on a horizontal bottom in presence of single and multiple rigid obstacles. In the 1D test case, characterized by a very steep slope, the Two-Phase Model predicts the separation between the two phases with a significant longitudinal variation of the solid concentration. In this case the results indicate appreciable differences between the two models in the estimation of both the wave celerity and the magnitude of the impact, with an overestimation of the peak force when using the Single-Phase Model. In the 2D test-cases, where the liquid and solid phases remain mixed, even if the flow fields predicted by the two models present some differences, the essential features of the interaction with the obstacles, along with the maximum impact force, are comparable.

Improving the Hydromechanical Deep-Drawing Process Using Aluminum Tailored Heat Treated Blanks

The present work demonstrates the effectiveness of combining the hydromechanical deep-drawing process with the Tailored Heat Treated Blank（THTB） technique. In the hydromechanical deep-drawing process, the fluid pressure is used for postponing the fracture occurrence in the blank, while the THTB technique allows to create a material property gradient through a suitable artificial aging treatment carried out prior to the forming process. Since the number of process variables is large, in the present work the authors propose an optimization loop for the determination of the parameters controlling the extension of the blank regions to be subjected to the aging treatment and the temperature levels to be set during the heat treatment. The proposed methodology couples a simple finite element model（Abaqus） with a multiobjective optimization platform（mode FRONTIER）. A preliminary experimental campaign was carried out for determining the effect of the aging treatment on the mechanical（through tensile tests） and deformative（through formability tests）behavior of the AC170 PX aluminum alloy. Optimization results prove the effectiveness of the adopted methodology and put in evidence that the adoption of properly aged blanks in the hydromechanical deep drawing allows to increase the limit drawing ratio and to simplify the process since it is conducted at room temperature.

The ideal endpoint of hepatitis B virus(HBV)antiviral therapy is HBsAg loss,a difficult goal to obtain,especially in HBeAg negative ***,we report the results obtained by the addition of peg-interferonα-2a to a long-lasting nucleos(t)ide analogues therapy in a HBeAg negative,genotype D patient with steadily HBV-DNA negative/HBsAg positive *** 2002,our Caucasian 44-year-old male patient received lamivudine and,4 years later,added adefovir because of a virological *** 2011,considering his young age,liver stiffness(4.3 kPa)and HBsAg levels(3533IU/mL),we added Peg-interferonα-2a for six months(3in combination with nucleos(t)ide analogues followed by 3 mo of Peg-interferonα-2a monotherapy).A decrease of HBsAg levels was observed after 1 mo(1.21log)of Peg-interferon and 3 mo(1.88 log)after the discontinuation of all ***,a complete clearance of HBsAg was obtained with steadily undetectable HBVDNA serum levels(dition of a short course of Peg-interferonα-2a represents an important result with clinical and pharmacoeconomic implications,considering that nucleos(t)ide analogues therapy in HBeAg negative chronic hepatitis B patients is considered a long-lasting/life-long treatment.
