Beneficial effect of sulphate-bicarbonate-calcium water on gallstone risk and weight control

AIM:To investigate the effect of drinking sulphate-bicarbonate-calcium thermal water (TW) on risk factors for atherosclerosis and cholesterol gallstone disease.METHODS:Postmenopausal women with functional dy-spepsia and/or constipation underwent a 12 d cycle of thermal (n=20) or tap (n=20) water controlled drinking.Gallbladder fasting volume at ultrasound,blood vitamin E,oxysterols (7-β-hydroxycholesterol and 7-ketocholesterol),bile acid (BA),triglycerides,total/low density lipoprotein and high density lipoprotein cholesterol were measured at baseline and at the end of the study.Food consumption,stool frequency and body weight were recorded daily.RESULTS:Blood lipids,oxysterols and vitamin E were not affected by either thermal or tap water consumption.Fasting gallbladder volume was significantly (P<0.005) smaller at the end of the study than at baseline in the TW (15.7±1.1 mL vs 20.1±1.7 mL) but not in the tap water group (19.0±1.4 mL vs 19.4±1.5 mL).Total serum BA concentration was significantly (P<0.05) higher at the end of the study than at baseline in the TW (5.83±1.24μmol vs 4.25±1.00μmol) but not in the tap water group (3.41±0.46μmol vs 2.91±0.56μmol).The increased BA concentration after TW consumption was mainly accounted for by glycochenodeoxycholic acid.The number of pasta (P<0.001),meat (P<0.001) and vegetable (P<0.005) portions consumed during the study and of bowel movements per day (P<0.05) were significantly higher in the TW than in the tap water group.Body weight did not change at the end of the study as compared to baseline in both groups.CONCLUSION:Sulphate-bicarbonate-calcium water consumption has a positive effect on lithogenic risk and intestinal transit and allows maintenance of a stable body weight despite a high food intake.

The knowledge on post-transcriptional regulation mechanisms implicated in seed development(SD)is still limited,particularly in one of the most consumed grain legumes,Phaseolus vulgaris L.We explore for the first time the miRNA expression dynamics in P.vulgaris developing seeds.Seventy-two known and 39 new miRNAs were found expressed in P.vulgaris developing seeds.Most of the miRNAs identified were more abundant at 10 and 40 days after anthesis,suggesting that late embryogenesis/early filling and desiccation were SD stages in which miRNA action is more pronounced.Degradome analysis and target prediction identified targets for 77 expressed miRNAs.While several known miRNAs were predicted to target HD-ZIP,ARF,SPL,and NF-Y transcription factors families,most of the predicted targets for new miRNAs encode for functional proteins.MiRNAs-targets expression profiles evidenced that these miRNAs could tune distinct seed developmental stages.MiRNAs more accumulated at early SD stages were implicated in regulating the end of embryogenesis,postponing the seed maturation program,storage compound synthesis and allocation.MiRNAs more accumulated at late SD stages could be implicated in seed quiescence,desiccation tolerance,and longevity with still uncovered roles in germination.The miRNAs herein described represent novel P.vulgaris resources with potential application in future biotechnological approaches to modulate the expression of genes implicated in legume seed traits with impact in horticultural production systems.

Genetic alterations in pancreatic tumors can usually be classified in:(1)Mutational activation of oncogenes;(2)Inactivation of tumor suppressor genes;and(3)Inactivation of genome maintenance genes controlling the repair of DNA damage.Endoscopic ultrasound-guided fine-needle aspiration has improved preoperative diagnosis,but the management of patients with a pancreatic lesion is still challenging.Molecular testing could help mainly in solving these“inconclusive”specimens.The introduction of multi-gene analysis approaches,such as next-generation sequencing,has provided a lot of useful information on the molecular characterization of pancreatic tumors.Different types of pancreatic tumors(e.g.,pancreatic ductal adenocarcinomas,intraductal papillary mucinous neoplasms,solid pseudopapillary tumors)are characterized by specific molecular alterations.The aim of this review is to summarize the main molecular alterations found in pancreatic tumors.

The Use of Spatial Sampling Designs in Business Surveys

An innovative use of spatial sampling designs is here presented. Sampling methods which consider spatial locations of statistical units are already used in agricultural and environmental contexts, while they have never been exploited for establishment surveys. However, the rapidly increasing availability of geo- referenced information about business units makes that possible. In business studies, it may indeed be important to take into account the presence of spatial autocorrelation or spatial trends in the variables of interest, in order to have more precise and efficient estimates. The opportunity of using the most innovative spatial sampling designs in business surveys, in order to produce samples that are well spread in space, is here tested by means of Monte Carlo experiments. For all designs, the Horvitz-Thompson estimator of the population total is used both with equal and unequal inclusion probabilities. The efficiency of sampling designs is evaluated in terms of relative RMSE and efficiency gain compared with designs ignoring the spatial information. Furthermore, an evaluation of spatially balancing samples is also conducted.

Accuracy of a predictive model for severe hepatic fibrosis or cirrhosis in chronic hepatitis C

AIM: To assess the accuracy of a model in diagnosing severe fibrosis/cirrhosis in chronic hepatitis C virus (HCV)infection.METHODS: The model, based on the sequential combination of the Bonacini score (BS: ALT/AST ratio,platelet count and INR) and ultrasonography liver surface characteristics, was applied to 176 patients with chronic HCV infection. Assuming a pre-test probability of 35%,the model defined four levels of post-test probability of severe fibrosis/cirrhosis: 90% (almost absolute).The predicted probabilities were compared with the observed patients' distribution according to the histology (METAVIR).RESULTS: Severe fibrosis/cirrhosis was found in 67 patients (38%). The model discriminated patients in three comparable groups: 34% with a very high (>90%)or low (75%) or low (<10%) probability of cirrhosis, leaving only 33% of the patients still requiring liver biopsy.

Changes in nutritional status after liver transplantation

Chronic liver disease has an important effect on nutritional status, and malnourishment is almost universally present in patients with end-stage liver disease who undergo liver transplantation. During recent decades,a trend has been reported that shows an increase in number of patients with end-stage liver disease and obesity in developed countries. The importance of carefully assessing the nutritional status during the workup of patients who are candidates for liver replacement is widely recognised. Cirrhotic patients with depleted lean body mass(sarcopenia) and fat deposits have an increased surgical risk; malnutrition may further impact morbidity, mortality and costs in the post-transplantation setting. After transplantation and liver function is restored, many metabolic alterations are corrected,dietary intake is progressively normalised, and lifestyle changes may improve physical activity. Few studies have examined the modifications in body composition that occur in liver recipients. During the first 12 mo, the fat mass progressively increases in those patients who had previously depleted body mass, and the muscle mass recovery is subtle and non-significant by the end of the first year. In some patients, unregulated weight gain may lead to obesity and may promote metabolicdisorders in the long term. Careful monitoring of nutritional changes will help identify the patients who are at risk for malnutrition or over-weight after liver transplantation. Physical and nutritional interventions must be investigated to evaluate their potential beneficial effect on body composition and muscle function after liver transplantation.

Hydroperoxides and cytokines as biomarkers in detecting atherosclerosis predisposition in cigarette smokers

Objectives: Smoking increases oxidative modification of LDL, associated with lower HDL plasma levels, systemic inflammatory response and endothelial dysfunction. We tested the hypothesis that the risk status for coronary atherosclerosis disease (CAD) of cigarettes smokers might be identified by means of serum oxidative levels and vascular inflammation determination. Design and Methods: Oxidative stress levels, cytokines, and the metabolic status were investigated on 499 subjects admitted to our institute. The association between biomarkers and smoking habits in the presence/absence of disease and with the number of vessel affected, was studied. Results: Oxidative stress and inflammatory levels (p < 0.001) were strongly induced by smoking habits. Serum values of the subjects categorised as CAD, non CAD and healthy subjects differed significantly (p < 0.001) only for the degree of oxidative stress. Glycaemia was able to affect C-reactive protein serum levels with a positive association (p < 0.05). The analysis of the study population indicated that serum oxidative stress levels significantly increased with increasing number of vessels affected (p < 0.01). When statistical analysis was performed separately in both smoking groups, smokers did not show any particular difference for both oxidative stress and inflammation markers between the two groups of cardiovascular patients (CAD and non CAD) and the control group, while for non smokers, the differences were evident. Conclusion: These findings indicate that the considered biomarkers, especially oxidative stress, can be useful to predict the biological damage caused by cigarette smoking, as well as to identify subjects characterised by a higher risk of cardiovascular event, but cannot evaluate the presence of disease in subjects with smoking habit.

Confirmation of Epidural Catheter Location by Epidural Pressure Waveform Recordings by the Compuflo<sup>®</sup>Cath-Checker System

Background: Pulsatile waveforms originating from the spinal cord and transmitted through the dura in synchrony with heart rate have been used to confirm the epidural location of the catheter. Lumbar epidural space identification using the CompuFlo? instrument has been reported and validated. The aim of this preliminary study was to evaluate the new CompuFlo instrument which allows the identification of pulsatile waveform recordings. Methods: We tested 30 epidural catheters previously successfully used for post cesarean analgesia and about to be removed. All patients were given 5 mL 2% lidocaine to test the catheter before its removal. After priming with 5 mL saline, the catheter was connected to CompuFlo? to record the occurrence of pulsatile waveforms and/or their disappearance during its removal. Results: Pulsatile waveforms were observed in all the catheters properly located in the epidural space and disappeared when the catheter was extracted from the epidural space. No waveforms were recorded in 2 cases in which no sensory block occurred after the test dose (catheter dislodgement). The pressure waveform analysis through the epidural catheter had a sensitivity of 100%, a positive predictive value of 100%, a specificity of 100% and a negative predictive value of 100%. Conclusions: In this preliminary trial pulsatile pressure waveform recording with CompuFlo? CathCheck? System through the epidural catheter resulted in high sensitivity and positive predictive value.
