Safety of the long-term use of proton pump inhibitors

The proton pump inhibitors (PPIs) as a class are remarkably safe and effective for persons with peptic ulcer disorders. Serious adverse events are extremely rare for PPIs, with case reports of interstitial nephritis with omeprazole, hepatitis with omeprazole and lansoprazole, and disputed visual disturbances with pantoprazole and omeprazole. PPI use is associated with the development of fundic gland polyps (FGP); stopping PPIs is associated with regression of FGP. In the absence of Helicobacter pylori infection, the long-term use of PPIs has not been convincingly proven to cause or be associated with the progression of pre-existing chronic gastritis or gastric atrophy or intestinal metaplasia. Mild/modest hypergastrinemia is a physiological response to the reduction in gastric acid secretion due to any cause. The long-term use of PPIs has not been convincingly proven to cause enterochromaff in-like cell hyperplasia or carcinoid tumors. PPIs increase the risk of community acquired pneumonia, but not of hospital acquired (nosocomial) pneumonia. There is no data to support particular care in prescribing PPI therapy due to concerns about risk of hip fracture with the long-termuse of PPIs. Long-term use of PPIs does not lead to vitamin B12 def iciencies, except possibly in the elderly, or in persons with Zollinger-Ellison Syndrome who are on high doses of PPI for prolonged periods of time. There is no convincingly proven data that PPIs increase the risk of Clostridium difficile-associated diarrhea in persons in the community. The discontinuation of PPIs may result in rebound symptoms requiring further and even continuous PPI use for suppression of symptoms. As with all medications, the key is to use PPIs only when clearly indicated, and to reassess continued use so that long-term therapy is used judiciously. Thus, in summary, the PPIs are a safe class of medications to use longterm in persons in whom there is a clear need for the maintenance of extensive acid inhibition.

Technological Impact on the Quality of Palm Oil from Burundi: Elaeis guineensis, Variety of dura and Tenera

The objective of this study is to determine the factors responsible for the acidification of Burundi palm oils. The investigation of the duration of fermentation of palm fruits performed among artisanal producers found that more than 89% of producers from commune of Rumonge ferment for more than 5 days while more than 61% of those from commune of Mutimbuzi ferment for 4 days. The determination of acid value using the method of ISO 660:2009 showed that dura and Tenera varieties fermented for 4 days had respectively 5.9 ± 2.3 and 5.8 ± 1.8 mg of KOH/g of oil for clustered fruits, 7.03 ± 3.4 and 7.02 ± 3.2 mg of KOH/g for destemmed fruits. After 8 days, the acid values of dura and Tenera varieties obtained were respectively 10.9 ± 4.7 and 12.5 ± 5.4 mg KOH/g for clustered fruits, 12.5 ± 5.4 and 12.5 ± 4.1 mg KOH/g for destemmed fruits. No significant difference was recorded between varieties. On the other hand, a highly significant difference at p d between fruits fermented in the open air (e.g. around the foot of the oil palm, FFOA), in a shed when the fruits were not covered (SFNC) and in a shed when the fruits were covered by branches or straw (SFC). At 8 days of fermentation, the acid values of FFOA, SFNC and SFC were 9.1 ± 0.7 destemmed fruit and 9.1 ± 0.7 d fruit, respectively. In light of these results, the acidity of Rumonge palm oil is probably due to the long fermentation time and the technique of covering the fruits during fermentation. These results will allow producers to extract good quality oil and consequently improve the health of consumers.

In the last two decades the study of red blood cell elasticity using optical tweezers has known a rise appearing in the scientific research with regard to the various works carried out. despite the various work done, no study has been done so far to study the influence of friction on the red blood cell indentation response using optical tweezers. In this study, we have developed a new approach to determine the coefficient of friction as well as the frictional forces of the red blood cell. This approach therefore allowed us to simultaneously carry out the indentation and traction test, which allowed us to extract the interfacial properties of the microbead red blood cell couple, among other things, the friction coefficient. This property would be extremely important to investigate the survival and mechanical features of cells, which will be of great physiological and pathological significance. But taking into account the hypothesis of friction as defined by the isotropic Coulomb law. The experiment performed for this purpose is the Brinell Hardness Test (dB).

Fatty Acid Composition of Hazelnut Kernel Oil from Coula edulis Collected in the Republic of Congo

Coula edulis is non-timber forest product (NTFP) used in Africa for its hazelnuts, which contain edible seeds with a demonstrated nutritional potential. However, there have been very few scientific studies of this species in the Republic of Congo. Thus, the aim of the current study was therefore to determine the fatty acid composition of the oil extracted from Coula edulis hazelnut seeds collected at random in the Republic of Congo. The oil was extracted using the Soxhlet method and its fatty acid composition was determined by gas chromatography. The extracted oil from Coula edulis hazelnut kernels is rich in monounsaturated fatty acids (95.28%), particularly oleic acid (94.5%), which classifies it as an oleic oil and gives it interesting nutritional and therapeutic properties. On the other hand, saturated fatty acids (4.15%) and polyunsaturated fatty acids (0.35%) are not well represented. Its low poly-unsaturated fatty acid content makes it more stable when stored at room temperature.

Vitamin d, Parathyroid Hormone, Insulin Sensitivity and Islet β-Cell Secretory Function in diabetic Patients from South Kivu in the democratic Republic of Congo: Cross-Sectional Study

Background: The role of vitamin d and parathyroid hormone in the metabolic profile of type 2 diabetes mellitus in sub-Saharan Africa has not been adequately assessed. The aim of this study was to determine the prevalence of low vitamin d level and secondary hyperparathyroidism and their association with insulin sensitivity and β-cell secretory function among Congolese type 2 diabetics. Methodology: Fasting glycaemia, fasting insulin, 25OH d3 and human parathyroid hormone (hPTH) were measured in one hundred and eighty-four type 2 diabetic patients followed as outpatients in South Kivu. Levels of 25OH d3 65 pg/ml defined low vitamin d and elevated parathyroid hormone levels, respectively. The HOMA model was used to measure insulin sensitivity and β-cell secretory function. Results: Medians (IQR) were 25.3 (20.4 - 32.4) ng/ml for 25OH d3 and 53.7 (38.4 - 115.7) pg/ml for hPTH. 58.7% of diabetics had insulin resistance, 126 (68.5%) had low vitamin d and 80 (43.5%) had hyperparathyroidism. In multivariate analysis, hPTH (partial r = −0.28;p = 0.0002) and 25OH d3 (partial r = 0.16;p = 0.03) showed an independent association with insulin sensitivity after adjustment for body mass index and waist circumference. Finally, hPTH (partial r = 0.27;p = 0.0002) was the sole determinant of β-cell secretory function. Conclusions: This study confirms the high prevalence of low vitamin d level and secondary hyperparathyroidism and their association with insulin resistance and impaired islet β-cell secretory function among Congolese with type 2 diabetes mellitus. Vitamin d and calcium supplementation should be envisaged for cases of deficiency in this region.

Aim: Sub-Saharan Africa is undergoing an epidemiological transition responsible for a change in the metabolic profile in favour of insulin resistance. The aim of this study was to assess the dynamics of the prevalence of insulin resistance and associated risk factors in diabetic patients in the democratic Republic of Congo between 2005 and 2023. Method: We measured fasting blood glucose and insulin levels and looked for metabolic syndrome parameters (2009 criteria) in type 2 diabetes patients in 2005-2008 (n = 176) and in 2018-2023 (n = 303). The HOMA model was used to measure insulin sensitivity and islet β-cell secretory function. Results: Between 2005 and 2013, the trend was towards an increase in the prevalence of insulin resistance (from 13.1% to 50.8%;p Conclusion: This present study shows an increase in insulin resistance in Congolese urban areas and a persistence of atypical diabetes mellitus in Congolese rural areas, confirming the particularity of the pathophysiology of the disease in African areas currently influenced by the epidemiological transition. Further studies using an appropriate methodology are required.

Hydrogeochemical Characterization of Groundwater from Fissured Aquifers in the Angovia Mine Operating Permit Area (Central-West Côte d’Ivoire)

The main objective of this study is to determine the hydrogeochemical specificities of the groundwater of the Angovia mine operating permit, located in the Yaouré mountains in the center-west of Côte d’Ivoire. To do so, descriptive and multivariate statistical analysis methods with the SOM (Self Organizing Maps) algorithm were applied to the physicochemical parameters of 17 boreholes using the calcite (ISC) and dolomite (ISd) saturation indices. The results obtained have shown that the groundwater in the Angovia mine operating permit area has an average temperature of 27.52°C (long rainy season) and 27.87°C (long dry season) and has an average pH of 7.09 ± 0.35 during the main rainy season and 7.32 ± 0.35 during the main dry season. They are mineralized with an average electrical conductivity of 505.98 ± 302.85 μS/cm during the long rainy season and with 450.33 ± 233.74 μS/cm as average during the long dry season. The main phenomena at the origin of groundwater mineralization are water residence time, oxidation-reduction and surface inflow. The study of the relative age of the water shows that the groundwater in the Angovia mine operating permit area is mainly undersaturated with respect to calcite and dolomite. They are therefore very old in the aquifer with a slow circulation speed during the long rainy season and the long dry season.

Assessing soil quality is essential for crop management and soil temporal changes. The present study aims to evaluate soil quality in the Ferralitic soils context countrywide. This assessment was done using multivariate soil quality indice (SQI) models, such as additive quality index (AQI), weighted quality indexes (WQIadd and WQIcom) and Nemoro quality index (NQI), applied to two approaches of indicator selection: total data set (TdS) and minimum data set (MdS). Physical and chemical soil indicators were extracted from the ORSTOM’s reports resulting from a sampling campaign in different provinces of Gabon. The TdS approach shows soil quality status according to eleven soil indicators extracted from the analysis of 1,059 samples from arable soil layer (0 - 30 cm depth). The results indicated that 87% of all provinces presented a very low soil quality (Q5) whatever the model. Among soil indicators, exchangeable K+ and Mg2+, bulk density and C/N ratio were retained in MdS, using principal component analysis (PCA). In the MdS approach, 50 to 63% of provinces had low soil quality grades with AQI, WQIadd and NQI, whereas the total was observed with WQIcom. Only 25% of provinces had medium soil quality grades with AQI and NQI models, while 12.5% (NQI) and 25% (AQI) presented high quality grades. Robust statistical analyses confirmed the accuracy and validation (0.80 r P ≤ 0.016) of AQI, WQIadd and NQI into the TdS and MdS approaches. The same sensitivity index value (1.53) was obtained with AQI and WQIadd. However, WQIadd was chosen as the best SQI model, according to its high linear regression value (R2 = 0.82) between TdS and MdS. This study has important implications in decision-making on monitoring, evaluation and sustainable management of Gabonese soils in a pedoclimatic context unfavorable to plant growth.
