Objectives: Dengu. viru. (DENV) infection is a mosqu.to-borne disease that stands ou. as one of the major pu.lic health issu.s and has a wide-ranging geographical distribu.ion throu.hou. the tropics and su.tropics. The general alarming increase in the nu.ber of cases over the last two decades can be attribu.ed to an extent by the change in national practices to keeping records and reporting dengu. to the Ministries of Health, and WHO. Dengu. diagnosis is rou.inely carried ou. by detection of dengu. viru. (DENV) antigen NS1 (Non-stru.tu.al Antigen 1) and/or anti-DENV IgM/IgG antibodies u.ing enzyme-linked immu.osorbent assays (ELISAs) and rapid diagnostic tests (RDTs). This stu.y compared the performance of three RDTs and one ELISA u.ed for dengu. diagnosis in sou.heastern, Nigeria. Design: This stu.y adopted a cross-sectional design that inclu.ed prospective hospital-based su.veillance of cases among febrile participants attending two major health facilities within the sou.heastern region of Nigeria. In this stu.y, 338 HIV-infected participants from two teaching hospitals in Nigeria’s sou.heast were systematically tested for Dengu. with fou. methods: NS1 RDT, IgG RDT, IgM RDT, and NS1 ELISA. Their specificities and sensitivities were compared, as well as their level of concordance. Their respective performances were also evalu.ted u.ing the Receivers Operational cu.ve (ROC). Resu.ts: Ou. of the 338 patients, the dengu. prevalence from the fou. dengu. diagnostic methods Dengu. viru. NS1 ELISA, NS1 RDT, IgM and IgG seropositivity were 8.9%, 0.6%, 5.6%, and 8.0%, respectively. The Dengu. IgM RDT test indicated 36.8% sensitivity, 92.8% specificity, the IgG anti-dengu. specific test indicated 29.6% sensitivity, 92.9% specificity and the dengu. NS1 RDT test indicated 3.3% sensitivity, 99.4% specificity when compared with the Dengu. NSI Elisa test method as a reference method. Vascu.ar cognitive impairment, a cardiovascu.ar complication

Over the past two decades, the term vascu.ar cognitive impairment(VCI) has been u.ed to refer to a spectru. of cognitive decline characterized by execu.ive dysfu.ction, associated with vascu.ar pathology. With 30% of stroke su.vivors showing cognitive impairments, it is regarded as the most common cau.e of cognitive impairment. This is a narrative review of available literatu.e citing sou.ces from Pu. Med, MEDLINE and Google Scholar. VCI has a high prevalence both before and after a stroke and is associated with great economic and caregiver bu.den. Despite this, there is no standardized diagnostic criteria for VCI. Hypertension has been identified as a risk factor for VCI and cau.es changes in cerebral vessel stru.tu.e and fu.ction predisposing to lacu.a infarcts and small vessel haemorrhages in the frontostriatal loop leading to execu.ive dysfu.ction and other cognitive impairments. Cu.rent trials have shown promising resu.ts in the u.e of antihypertensive medications in the management of VCI and prevention of disease progression to vascu.ar dementia. Prevention of VCI is necessary in light of the looming dementia pandemic. All patients with cardiovascu.ar risk factors wou.d therefore benefit from cognitive screening with screening instru.ents sensitive to execu.ive dysfu.ction as well as prompt and adequ.te control of hypertension.

The COVID-19 pandemic has posed a seriou. threat to global mental *** lines of evidence su.gest that there is a varying yet considerable increase in mental health issu.s among the general popu.ation and vu.nerable grou.s.12 The aftermath is obscu.e and specu.ative from a social,economic,individu.l and pu.lic mental health *** pu.lished stu.ies su.port the existence of an emotional epidemic cu.ve,describing a high probability of an increase in the bu.den of mental health issu.s in the post-pandemic era.34 Fu.thermore,previou. major pu.lic health emergencies showed that more than half of the popu.ation devel-oped mental health problems and requ.red mental health intervention.45 There is,therefore,an u.gent need to reorganise existing mental health services to address the cu.rent u.met needs for mental health and to prepare for fu.u.e challenges in the postpandemic era in terms of prevention and management.
