Elderly patients with non-cardiac admissions and elevated highsensitivity troponin:the prognostic value of renal function

BACKGROUND High-sensitivity cardiac troponin(hs-cTn)levels are frequently elevated in elderly patients presenting to the emergency department for non-cardiac ***,most studies on the role of elevated hs-cTn in elderly populations have investigated the prognostic value of hs-cTn in patients with a specific diagnosis or have assessed the relationship between hs-cTn and *** To investigate the in-hospital prognosis of consecutive elderly patients admitted to the internal Medicine Department with acute non-cardiac events and increased hs-cTni *** in this retrospective study,we selected patients who were aged≥65 years and admitted to the internal Medicine Department of our hospital between January 2019 and December 2019 for non-cardiac *** patients were those who had hs-cTni concentrations≥100 ng/*** investigated the independent predictors of in-hospital mortality by multivariable logistic regression *** One hundred and forty-six patients(59%female)were selected with an age range from 65 to 100(mean±SD:85.4±7.61)*** median hs-cTni value was 284.2 ng/*** 72(49%)patients the diagnosis of hospitalization was an infectious *** overall in-hospital mortality was 32%(47 patients).individuals who died did not have higher hs-cTni levels compared with those who were discharged alive(median:314.8 vs 282.5 ng/L;P=0.565).There was no difference in mortality in patients with infectious vs non-infectious disease(29%vs 35%).Multivariable analysis showed that age(OR 1.062 per 1 year increase,95%Ci:1.000-1.127;P=0.048)and creatinine levels(OR 2.065 per 1 mg/dL increase,95%Ci:1.383-3.085;Pindependent predictors of *** was 49%in patients with eGFRin/1.73 *** Myocardial injury is a malignant condition in elderly patients admitted to the hospital for non-cardiac *** presence of severe renal impairment is a marker of extremely high in-hospital mortality.

BACKGROUND Kidney transplantation leads to continuous improvement in the survival rates of kidney transplant recipients(KTRs)and has been established as the treatment of choice for patients with end-stage kidney ***-related quality of life(HRQoL)has become an important outcome *** is highly important to develop reliable methods to evaluate HRQoL with disease-specific *** To translate the disease-specific instrument Kidney Transplant Questionnaire 25(KTQ-25)to the Greek language and perform a cross-cultural *** The translation and adaptation of the original English version of the KTQ-25 to the Greek language were performed based on the international Quality of Life RESULTS Eighty-four KTRs(59 males;mean age 53.5±10.7 years;mean estimated glomerular filtration rate 47.7±15.1 mL/min/1.73 m2;mean transplant vintage 100.5±83.2 months)completed the Greek version of the KTQ-25 and the 36-item Short-Form Health Survey,and the results were used to evaluate the reliability of the Greek *** Cronbach alpha coefficients for all the KTQ-25 dimensions were satisfactory(physical symptoms=0.639,fatigue=0.856,uncertainty/fear=0.661,appearance=0.593,emotions=0.718,total score=0.708).The statistically significant correlation coefficients among the KTQ-25 dimensions ranged from 0.226 to *** correlation coeffi-cients of the KTQ-25 dimensions with the SF-36 physical component summary(PCS)ranged from 0.196 to 0.550;the correlation coefficients of the KTQ-25 with the SF-36 mental component summary(MCS)ranged from 0.260 to 0.655;and the correlation coefficients of the KTQ-25 with the total scores with the SF-36 PCS and MCS were 0.455 and 0.613,*** According to the findings,the Greek version of the KTQ-25 is valid and reliable for administration among kidney transplant patients in Greece.

Using fracture-based continuum modeling of coupled geomechanical-hydrological processes for numerical simulation of hydraulic fracturing

Taxonomy,distribution,and contemporary exposure of terrestrial mammals to floods and human pressure across different areas for biodiversity conservation in China

A significant research focus is placed on identifying animal species and areas at future risk to human-induced alterations of the environment and long-term changes in climatic ***,the extent to which exposure to extreme climatic events and intense human pressure can increase the risk of harmful impacts on species remains poorly *** on terrestrial mammals in China,one of the world’s megadiverse countries,we investigated patterns of contemporary exposure to floods and human pressures and determined their taxonomic representation and distribution across three major area-based conservation schemes,namely,national nature reserves(NNRs),priority areas for biodiversity conservation(PABCs),and key biodiversity areas(KBAs).Among the 440 species assessed with moderate or high exposure to floods,327(∼75%)also qualified as moderate or high in exposure to intense human *** species mainly belong to the orders Chiroptera,Eulipotyphla,and ***,there were 305,311,and 311 species with moderate or high exposure to flood and intense human pressure represented across NNRs,PABCs,and KBAs,*** findings support the prioritization of KBAs for expansion of site-based protection efforts such as NNRs in China,considering threats to species from exposure to adverse effects from both extreme climate and human pressure.

BACKGROUND Transplant recipients commonly harbor multidrug-resistant organisms(MDROs),as a result of frequent hospital admissions and increased exposure to antimi-crobials and invasive *** To investigate the impact of patient demographic and clinical characteristics on MDRO acquisition,as well as the impact of MDRO acquisition on intensive care unit(iCU)and hospital length of stay,and on iCU mortality and 1-year mortality post heart *** This retrospective cohort study analyzed 98 consecutive heart transplant patients over a ten-year period(2013-2022)in a single transplantation *** was collected regarding MDROs commonly encountered in critical *** Among the 98 transplanted patients(70%male),about a third(32%)acquired or already harbored MDROs upon transplantation(MDRO group),while two thirds did not(MDRO-free group).The prevalent MDROs were Acinetobacter baumannii(14%),Pseudomonas aeruginosa(12%)and Klebsiella pneumoniae(11%).Compared to MDRO-free patients,the MDRO group was characterized by higher body mass index(P=0.002),higher rates of renal failure(P=0.017),primary graft dysfunction(10%vs 4.5%,P=0.001),surgical re-exploration(34%vs 14%,P=0.017),mechanical circulatory support(47%vs 26%P=0.037)and renal replacement therapy(28%vs 9%,P=0.014),as well as longer extracorporeal circulation time(median 210 vs 161 min,P=0.003).The median length of stay was longer in the MDRO group,namely iCU stay was 16 vs 9 d in the MDRO-free group(P=0.001),and hospital stay was 38 vs 28 d(P=0.006),while 1-year mortality was higher(28%vs 7.6%,log-rank-χ2:7.34).CONCLUSiON Following heart transplantation,a predominance of Gram-negative MDROs was *** acquisition was associated with higher complication rates,prolonged iCU and total hospital stay,and higher post-transplantation mortality.
