This paper presents an extensive survey of the most commonly used tools for diagnosing unbalanced flow in the atmosphere, namely the Lagrangian Rossby number, Psi vector, divergence equation, nonlinear balance equation, generalized omega-equation, and departure from fields obtained by potential vorticity (PV) inversion. The basic thoery, assumptions as well as implementation and limitations for each of the tools are all discussed. These tools are applied to high—resolution mesoscale model data to assess the role of unbalanced dynamics in the generation of a mesoscale gravity wave event over the East Coast of the United States. Comparison of these tools in this case study shows that these various methods agree to a large extent with each other though they differ in details. Medicine as Profession： An Overlooked Approach to Medical Ethics

This article begins with three problems of ＂dual loyalties＂ in medicine, the supposed fact that military physicians are, as medical officers, sometimes required to do what violates ordinary medical ethics--for example, ignore medical need in order to treat their own wounded before civilians or wounded enemy, help make chemical or biological weapons more deadly, or assist at a rough interrogation. These problems are analyzed as special cases of a problem that could arise in any profession, a problem easily resolved using a theory of professional ethics （more or less） absent from medical ethics until now though common outside. Employing a physician--rather than an ordinary officer, some other kind of healer, or scientist--is to enter a sort of ＂Ulysses contract＂ requiring the physician＇s professional standards to preempt obligations otherwise applying to an employee. In this way, the article also illustrates the benefits that might accrue to medical ethics from drawing （more than is now common） on other fields of practical ethics.

The ability to predict the likelihood of impurity incorporation and their electronic energy levels in semiconductors is crucial for controlling its conductivity,and thus the semiconductor’s performance in solar cells,photodiodes,and *** difficulty and expense of experimental and computational determination of impurity levels makes a data-driven machine learning approach *** this work,we show that a density functional theory-generated dataset of impurities in Cd-based chalcogenides CdTe,CdSe,and CdS can lead to accurate and generalizable predictive models of defect properties.

Objectives　To assess the preliminary clinical results of implantation of dual chamber pacemaker defibrillator and to evaluate the safety and effectiveness of placement of endocardial leads in the axillary vein. Methods　Seven patients with ventricular tachycardia and/or ventricular fibrillation (VT/VF), associated with bradyarrhythmia received implantation of a dual chamber pacemaker defibrillator, including 5 patients with coronary artery disease and 2 patients with dilated *** atrial and ventricular leads were introduced via the axillary vein under venographic guidance. Results　Dual chamber pacemaker defibrillators were successfully implanted in the left chest subcutaneous pocket in 5 patients and the left pectoral muscular pocket in 2 patients. All the VT/VF occurring either inducibly during the procedure or spontanuously during follow-up were detected promptly and treated successfully. Both the pacing and sensing functions were satisfactory. The endocardial leads required were successfully introduced via the axillary vein without major ***　Dual chamber pacemaker defibrillators can provide reliable therapy for VT/VF and the dual chamber pacing function. Placement of endocardial leads via the axillary vein under venographic guidance is safe and effective.

CHEK2 (Checkpoint kinase homolog 2) encodes a protein involved in pathways that arrest the cell cycle in response to genomic stress such as DNA damage or replication blocks. Carriers of some of the CHEK2 mutations are at an increased risk of breast cancer. A mutation in the kinase domain of the CHEK2 gene resulting in the R406H substitution has been reported. However, it is currently unknown whether the substitution alters the function of CHEK2 in vitro. We evaluated the effect of the R406H substitution on the normal function of CHEK2 using a yeast complementation assay. The yeast cells lacking Rad53, the yeast homologue of human CHEK2 were transformed with the wild type as well as plasmids carrying mutations resulting in the R406H, 1100delC, and S428F variants. Yeast cells carrying the R406H variant grew at a rate similar to those carrying the wild type CHEK2, whereas the yeast carrying the S428F and 1100delC mutants grew at a slower rate. These results suggest that, unlike the well-known pathogenic alleles such as 1100delC and S428F, the R406H substitution does not abrogate the function of CHEK2. Therefore, this variant is probably not important in development of breast cancer in women.

AIM:To assess the diagnostic yield and clinical value of early repeat colonoscopies for indications other than colorectal cancer(CRC) screening/***:A retrospective review of patients who had more than one colonoscopy performed for the same indication within a three year time frame at our tertiary care referral hospital between January 1,2000 and January 1,2010 was *** criteria included repeat colonoscopies performed for CRC screening/surveillance,poor bowel preparation,suspected complications from the index procedure,and incomplete initial *** outcome was new endoscopic finding that led to an endoscopic therapeutic intervention or any change in clinical *** parameters including age,sex,race,interval between procedures,indication of the procedure,presenting symptoms,severity of symptoms,hemodynamic instability,duration between onset of symptoms and when the procedure was performed,change in endoscopist,withdrawal time,location of colonic lesions and improvement of quality of bowel preparation were analyzed using bivariate analysis and logistic regression analysis to examine correlation with this primary ***:Among 19 772 colonoscopies performed during the above mentioned period,947 colonoscopies(4.79%) were repeat colonoscopies performed within 3 years from the index *** of these repeat colonoscopies,139 patient pairs met the inclusion *** majority of repeat colonoscopies were for lower gastrointestinal bleeding(88.4%),change in bowel habits(6.4%) and abdominal pain(5%).Among 139 eligible patient pairs of colonoscopies,only repeat colonoscopies that were done for lower gastrointestinal bleeding and abdominal pain produced endoscopic findings that led to a change in management [25 out of 123(20.33%) and 2 out of 7(28.57%),respectively].When looking at only recurrent lower gastrointestinal bleeding cases,new endoscopic findings included 8 previously undetected hemorrhoid lesion
