Phenotypic differences between coryneform bacteria isolated from seminal fluid of healthy men and men with chronic prostatitis syndrome

We compared the potential phenotypic properties of coryneform bacteria associated with chronic prostatitis syndrome （CPS）, such as secretory inhibitor of lysozyme （SIL） and secretory inhibitor of platelet microbicidal protein （SIPMP）. A total of 110 clinical isolates of coryneform bacteria isolated from the seminal fluid of healthy men and men with CPS were tested. SIPMP production was tested by inhibiting platelet microbicidal protein （PMP） bioactivity against Bacillus subtilis, and was expressed as percentage of inhibition of PMP bactericidal activity. SIL production was tested by inhibiting lysozyme activity against Micrococcus lysodeikticus and was expressed in microgram per millilitre of inactivated lysozyme. A significantly higher proportion of CPS strains （58.7% vs. 19.2 %） was SIPMP-positive compared with non-CPS strains （P 〈 0.01）. Of the CPS strains tested, 77.8% were SIL-positive compared with 34% of the non- CPS isolates （P 〈 0.05）. These results suggest that the diagnosis of CPS should not rely solely on classical parameters, for example, the identification and counting of microorganisms, but the functional significance of these parameters must be estimated, possibly by the concentration of different bacterial substrains, detection of opportunistic microorganisms with pathogenic properties, such as pronounced resistance to the cationic antimicrobial peptides, and/or the ability to inhibit the antimicrobial host defence factors.

Is diabetes a causal agent for colorectal cancer? Pathophysiological and molecular mechanisms

The possible relationship between diabetes mellitus (dM) and colorectal cancer (CRC), concerning pathophysiological and molecular mechanisms is highlighted in this review. The most recent and complete articles and developments in this particular field were thoroughly reviewed. Common risk factors, such as obesity, sedentary lifestyle, and Western diet between dM and CRC, led to the theory that dM might be a causal agent for CRC development. Various studies have connected type 2 dM and CRC, either proximal or distal, in both sexes. Additionally, chronic insulin treatment has been linked with increased colorectal tumor risk among type 2 diabetic patients. Interestingly, elevated hemoglobin A1c has been proven to be an independent predictor of aggressive clinical behavior in CRC patients. These mechanisms include the insulin-like growth factor-hyperinsulinemia theory and the participation of oncogenic intracellular signaling pathways. Furthermore, it has been proposed that Cox-2 inhibitors might have a role in decreasing the incidence of CRC. Finally, the use of statins to reduce the risk for colon cancer in patients with diabetes has remained controversial. diabetic patients over 50 should receive counseling regarding their elevated risk for CRC, and screening colonoscopy should be recommended before initiating insulin therapy. However, there are no current guidelines, and this strategy is not yet applicable to some countries, as the corresponding risk would not allow screening colonoscopy to be adopted. There is strong evidence to indicate that dM is a causal agent for CRC development. This conclusion provides new impetus for re-evaluating CRC screening worldwide.

Statistical variability and fragility assessment of ballistic perforation of steel plates for 7.62 mm AP ammunition

The paper describes field test results of 7.62×51 mm M61 AP(armour piercing) ammunition fired into mild steel targets at an outdoor *** targets varied from 10 mm to 32 mm in *** tests recorded penetration depth,probability of perforation(i.e.,complete penetration),muzzle and impact velocities,bullet mass,and plate yield strength and *** measured penetration depth exhibited a variability of approximately±12%.The paper then compared ballistic test results with predictive models of steel penetration depth and thickness to prevent *** parameters were derived for muzzle and impact velocity,bullet mass,plate thickness,plate hardness,and model error.A Monte-Carlo probabilistic analysis was then developed to estimate the probability of plate perforation of 7.62 mm M61 AP ammunition for a range of impact velocities,and for mild steels,and High Hardness Armour(HHA) *** perforation fragility analysis considered the random variability of impact velocity,bullet mass,plate thickness,plate hardness,and model *** a probabilistic analysis allows for reliability-based design,where,for example,the plate thickness with 95% reliability(*** 1 in 20 shots will penetrate the wall) can be estimated knowing the probabilistic distribution of ***,it was found that the plate thickness to ensure a low 5% probability of perforation needs to be 11-15% thicker than required to have a 50/50 chance of perforation for mild steel *** would need to be 20-30% thicker if probability of perforation is reduced to zero.

Hepatotoxicity reports in the FdA adverse event reporting system database:A comparison of drugs that cause injury via mitochondrial or other mechanisms

Translating radiology reports into plain language using ChatGPT and GPT-4 with prompt learning:results,limitations,and potential

The large language model called ChatGPT has drawn extensively attention because of its human-like expression and reasoning *** this study,we investigate the feasibility of using ChatGPT in experiments on translating radiology reports into plain language for patients and healthcare providers so that they are educated for improved *** reports from 62 low-dose chest computed tomography lung cancer screening scans and 76 brain magnetic resonance imaging metastases screening scans were collected in the first half of February for this *** to the evaluation by radiologists,ChatGPT can successfully translate radiology reports into plain language with an average score of 4.27 in the five-point system with 0.08 places of information missing and 0.07 places of *** terms of the suggestions provided by ChatGPT,they are generally relevant such as keeping following-up with doctors and closely monitoring any symptoms,and for about 37%of 138 cases in total ChatGPT offers specific suggestions based on findings in the *** also presents some randomness in its responses with occasionally over-simplified or neglected information,which can be mitigated using a more detailed ***,ChatGPT results are compared with a newly released large model GPT-4,showing that GPT-4 can significantly improve the quality of translated *** results show that it is feasible to utilize large language models in clinical education,and further efforts are needed to address limitations and maximize their potential.

Inflammatory bowel disease course in Crohn's disease:Is the natural history changing?

Crohn’s disease (Cd) is a multifactorial potentially debilitating disease. It has a variable disease course, but the majority of patients eventually develop penetrating or stricturing complications leading to repeated surgeries and disability. Studies on the natural history of Cd provide invaluable data on its course and clinical predictors, and may help to identify patient subsets based on clinical phenotype. Most data are available from referral centers, however these outcomes may be different from those in population-based cohorts. New data suggest the possibility of a change in the natural history in Crohn’s disease, with an increasing percentage of patients diagnosed with inflammatory disease behavior. Hospitalization rates remain high, while surgery rates seem to have decreased in the last decade. In addition, mortality rates still exceed that of the general population. The impact of changes in treatment strategy, including increased, earlier use of immunosuppressives, biological therapy, and patient monitoring on the natural history of the disease are still conflictive. In this review article, the authors summarize the available evidence on the natural history, current trends, and predictive factors for evaluating the disease course of Cd.

Asymptotic stabilization of dynamically quantized nonlinear systems in feedforward form

This paper studies the stabilizability of an n-dimensional quantized feedforward nonlinear system. The state of that system is first quantized into a finite number of bits, and then sent through a digital network to the controller. We want to minimize the number of transmitted bits subject to maintaining asymptotic stability. In the prior literature, n bits are used to stabilize the n-dimensional system by assigning one bit to each state variable （dimension）. Under the stronger assumption of global Lipschitz continuity, this paper extends that result by stabilizing the system with a single bit. Its key contribution is a dynamic quantization policy which dynamically assigns the single bit to the most ＂important＂ state variable. Under this policy, the quantization error exponentially converges to 0 and the stability of the system can, therefore, be guaranteed. Because i is the minimum number of quantization bits （per sampling step）, the proposed dynamic quantization policy achieves the minimum stabilizable bit number for that n-dimensional feedforward nonlinear system.
