Assessing keloid recurrence following surgical excision and radiation

Keloids are a fibroproliferative disorder that can result from a cutaneous injury to the reticular *** rates as high as 100%have been reported following surgical excision ***,a variety of post-surgical techniques have been employed to prevent keloid recurrence,including the use of *** numerous studies have shown post-excisional X-rays,electron beam,lasers and brachytherapy can reduce the rate of keloid recurrence,numerous inconsistencies,including a wide range of definitions for keloid recurrence,make it difficult to compare study *** review aims to examine the various means for defining keloid recurrence in clinical trials involving the use of radiation *** of the Cochrane Library and PubMed were performed to identify the available information for post-surgical keloid recurrence following radiation *** identified study was reviewed for patient followup and criteria used to define keloid *** search results included clinical studies with external beam radiation,brachytherapy and superficial radiation *** studies did not include a definition of keloid recurrence,or defined recurrence only as the return of scar *** studies defined keloid recurrence based on patient self-assessment questionnaires,symptoms and scar elevation and changes in Kyoto Scar Scale,Japan Scar Workshop Scale and Vancouver Scar Scale *** results of this review indicate keloidectomy followed by radiation therapy provide satisfactory recurrence rates;however,clinical studies evaluating these treatments do not describe treatment outcomes or use different definitions of keloid ***,recurrence rates vary widely,making comparisons across studies *** recurrence should be clearly defined using both objective and subjective measures.

Dielectric-loaded rf accelerating structures offer the possibility of an alternative to conventional disk-loaded metal structures,and provide a simple,uniform geometry with no field enhancements on irises,and with demonstrated techniques to suppress higher-order modes that may be advantageous in high-gradient ***,a particularly pernicious form of multipactor has been found to occur in these structures,due to the presence of both normal and tangential rf electric fields at the inner dielectric *** is an electron avalanche phenomenon that occurs in many systems that combine metal or dielectric surfaces,rf electric fields,and vacuum conditions,and it must be controlled or suppressed in practical systems,including both rf sources and accelerating ***-surface multipactor occurs on dielectrics such as rf windows,where the rf electric field is parallel to the material *** is a non-resonant phenomenon that typically saturates at low levels,and can be suppressed by low secondary emission coefficient surfaces as well as other ***-surface multipactor is a resonant process that typically takes place between two metal surfaces in the presence of normal electric fields,when the electron transit time between the surfaces is an odd number of half rf periods and the electron energy gain leads to an impact energy at each surface for which the mean secondary electron yield exceeds ***,multipactor in a cylindrical DLA structure does not fit neatly into either of these *** is in part a resonant process,in which electrons leave the dielectric surface and return to the same surface at higher energy approximately one rf period later,due to the combined effect of the rf electric fields and the space-charge field of the electron cloud generated by the multipactor *** scales nonlinearly upward at higher accelerating gradients,and its presence along the length of an accelerating structure makes efficient

Evidence-based practice(EBP)has been the gold standard in healthcare for nearly three centuries and aims to assist physicians in providing the safest and most effective healthcare for their *** well-established hierarchy of evidence lists systematic reviews and meta-analyses at the top however these methodologies are not always appropriate or possible and in these instances case-control studies,case series and case reports are utilised to support ***-control studies allow simultaneous study of multiple risk factors and can be performed rapidly and relatively cheaply.A recent example was during the Coronavirus pandemic where case-control studies were used to assess the efficacy of personal protective equipment for healthcare *** series and case reports also play a role in EBP and are particularly useful to study rare diseases such as inflammatory bowel disease in transgender and gender non-conforming *** are also vital in generating and disseminating early signals and encouraging further *** these methodologies have weaknesses,particularly with regards to bias and loss of patient confidentiality for rare pathologies,they have an important part to play in EBP and when appropriately utilised can significantly impact upon clinical practice.

Impact of cancer diagnosis on life expectancy by area-level socioeconomic groups in New South Wales, Australia: a population-based study

Objective: Improvement in cancer survival over recent decades has not been accompanied by a narrowing of socioeconomic disparities. This study aimed to quantify the loss of life expectancy(LOLE) resulting from a cancer diagnosis and examine disparities in LOLE based on area-level socioeconomic status(SES).Methods: Data were collected for all people between 50 and 89 years of age who were diagnosed with cancer, registered in the NSW Cancer Registry between 2001 and 2019, and underwent mortality follow-up evaluations until December 2020. Flexible parametric survival models were fitted to estimate the LOLE by gender and area-level SES for 12 common ***: Of 422,680 people with cancer, 24% and 18% lived in the most and least disadvantaged areas, respectively. Patients from the most disadvantaged areas had a significantly greater average LOLE than patients from the least disadvantaged areas for cancers with high survival rates, including prostate [2.9 years(95% CI: 2.5±3.2 years) vs. 1.6 years(95% CI: 1.3±1.9 years)] and breast cancer [1.6 years(95% CI: 1.4±1.8 years) vs. 1.2 years(95% CI: 1.0±1.4 years)]. The highest average LOLE occurred in males residing in the most disadvantaged areas with pancreatic [16.5 years(95% CI: 16.1±16.8 years) vs. 16.2 years(95% CI: 15.7±16.7 years)] and liver cancer [15.5 years(95% CI: 15.0±16.0 years) vs. 14.7 years(95% CI: 14.0±15.5 years)]. Females residing in the least disadvantaged areas with thyroid cancer [0.9 years(95% CI: 0.4±1.4 years) vs. 0.6 years(95% CI: 0.2±1.0 years)] or melanoma [0.9 years(95% CI: 0.8±1.1 years) vs. 0.7 years(95% CI: 0.5±0.8 years)] had the lowest average ***: Patients from the most disadvantaged areas had the highest LOLE with SES-based differences greatest for patients diagnosed with cancer at an early stage or cancers with higher survival rates, suggesting the need to prioritise early detection and reduce treatment-related barriers and survivorship challenges to improve life expec

Background:Determining the incidence and prevalence of injury and illness in short-course triathletes would improve understanding of their etiologies and therefore assist in the development and implementation of prevention *** study synthesizes the existing evidence on the incidence and prevalence of injury and illness and summarizes reported injury or illness etiology and risk factors affecting short-course ***:This review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses *** reporting health problems(injury and illness)in triathletes(all sexes,ages,and experience levels)training and/or competing in short-course distances were *** electronic databases(Cochrane Central Register of Controlled Trials,MEDLINE,Embase,APA PsychINFO,Web of Science Core Collection,and SPORTDiscus)were *** of bias was independently assessed by 2 reviewers using the Newcastle-Ottawa Quality Assessment *** authors independently completed data ***:The search yielded 7998 studies,with 42 studies eligible for ***-three studies investigated injuries,24 studies investigated illnesses,and 5 studies investigated both injuries and *** injury incidence rate ranged 15.7-24.3 per 1000 athlete exposures,and the illness incidence rate ranged 1.8-13.1 per 1000 athlete *** and illness prevalence ranged between 2%-15%and 6%-84%,*** injuries reported occurred during running(45%-92%),and the most frequently reported illnesses affected the gastrointestinal(7%-70%),cardiovascular(14%-59%),and respiratory systems(5%-60%).Conclusion:The most frequently reported health problems in short-course triathletes were:overuse and lower limb injuries associated with running;gastrointestinal illnesses and altered cardiac function,primarily attributable to environmental factors;and respiratory illness mostly caused by infection.

This article attempts to describe the role of tessellated models of space within the discipline of geographic information systems(GIS)—a speciality coming largely out of geography and land surveying,where there was a strong need to represent information about the land’s surface within a computer system rather than on the original paper *** look at some of the basic operations in GIS,including dynamic and kinetic *** examine issues of topology and data structures and produce a tessellation model that may be widely applied both to traditional“object”and“field”data *** on this framework,it can be argued that tessellation models are fundamental to our understanding and processing of geographical space,and provide a coherent framework for understanding the“space”in which we *** first article examines static structures,and a subsequent article looks at“change”—what happens when things move.

Fragmentation model for large L/D (Length over Diameter) explosive fragmentation warheads

New advanced numerical computer model enabling accurate simulation of fragmentation parameters of large Length over Diameter(L/D)explosively driven metal shells has been developed and *** newly developed large L/D multi-region model links three-dimensional axisymmetric high strain high strain-rate hydrocode analyses with the conventional set of Picatinny Arsenal FRAGmentation(PAFRAG)simulation *** standard PAFRAG modeling technique is based on the Mott's theory of break-up of idealized cylindrical"ring-bombs",in which the length of the average fragment is a function of the radius and velocity of the shell at the moment of break-up,and the mechanical properties of the *** the newly developed multi-region model,each of the shell region,the break-up is assumed to occur instantaneously,whereas the entire shell is modeled to fragment at multiple times,according to the number of the regions *** to PAFRAG methodology,the required input for both the natural and the controlled fragmentation models including the geometry and the velocity of the shell at moment of break-up had been provided from the hydrocode analyses and validated with available experimental *** newly developed large L/D multi-region PAFRAG model has been shown to accurately reproduce available experimental fragmentation data.

The Effect of Novel Low Energy Pulsed Light Combined with Galvanic Energy for home-Use hair Removal of Dark Skin

Background and Objectives: Permanent reduction of unwanted hair on skin types V and VI is the most challenging procedure among all hair removal technologies based on selective absorption of light or laser. The objective of this study is to evaluate the safety and efficacy of a low energy pulsed-light device combined with galvanic energy, intended for home-use hair removal on dark skin. Materials and Methods: Fifteen women with skin types V and VI and dark terminal hair in axillaarea self-administrated 6 treatments at 2 week intervals, using a hand-held IPL combined with galvanicenergy device, using hPL (home Pulsed Light) technology. hair count and photographs were performed pre-treatment and 1 and 3 months after the last sixth treatment. Adverse events and subject satisfaction scores were recorded. Results: All patients showed a positive clinical response to treatment, with reduction of unwanted hair. hair counts were significantly reduced by 57.3% 1 month following last treatment and by 44.5% 3 months following last treatment. No adverse events were recorded. Subject satisfaction scores of the device usability and the treatment outcome were high. Conclusions: Low energy pulsed light combined with galvanic energy may be applied safely and effectively for at-home hair removal for people with dark skin types V and VI.
