C-MET Inhibitors as New Members of.the NSCLC Treatment Armamentarium—A Pooled Analysis

Objective: Capmatinib and tepotinib, two recently f.A-approved and highly specif.c small-molecule inhibitors of.c-MET exon 14 skipping mutations are new and important therapeutic options f.r the treatment of.NSCLC patients harbouring c-MET alterations. However, the precise role of.these molecules as a new treatment option is still not f.lly def.ned. Methods: In an attempt to f.rther evaluate the contributions of.c-MET inhibitors to the armamentarium of.treatment options f.r advanced and metastatic NSCLCs, relevant phase II and III studies were retrospectively analyzed in terms of.ORR and mPf. (mOS numbers are still not available f.r current c-MET trials and theref.re not considered f.r statistical purposes). Results: Treatment of.advanced and metastatic NSCLC patients harbouring c-MET exon 14 skipping mutations with the novel and highly selective c-MET inhibitors is signif.cantly superior (p Conclusion: The novel and highly selective c-MET inhibitors capmatinib and tepotinib are promising novel treatment options f.r patients with c-MET-dysregulated NSCLC primarily in the f.rst-line setting, albeit a clear mOS benef.t has not yet been established. Since immunotherapy did not appear to be particularly ef.ective in NSCLC patients harbouring c-MET alterations, the vast majority of.these patients are treated with immunotherapy plus chemotherapy. C-Met inhibitors appear to be equally ef.ective and thereby sparing patients f.om the toxic ef.ects of.the chemotherapy. The routine testing of.c-MET exon 14 skipping mutations should be perf.rmed as the GEOMETRY mono-1 data clearly showed higher response rates with capmatinib in treatment-naive than in pretreated patients, indicating that c-MET exon 14 skipping mutations should pref.rably be molecularly assessed at baseline. C-MET exon 14 skipping mutations are, theref.re, clear biomarkers of.response to c-MET inhibitors.

Mechanisms to explain soil liquef.ction triggering, development, and persistence during an earthquake

Mechanisms have been proposed to explain the triggering, development, and persistence of.soil liquef.ction. The mechanism explaining the horizontal f.ilure plane (triggering) and its depth below the phreatic surf.ce is governed by the f.ux properties and ef.ective stress at that plane. At the f.ilure plane, the pore water pressure was higher than the ef.ective stress, and the volume change was the highest. The pore water pressure is a f.nction of.the soil prof.le f.atures (particularly the phreatic zone width) and bedrock motion (horizontal acceleration). The volume change at the f.ilure plane is a f.nction of.the intrinsic permeability of.the soil and bedrock displacement. The f.ilure plane was predicted to occur during the oscillation with the highest amplitude, disregarding f.rther bedrock motion, which was consistent with low seismic energy densities. Two mechanisms were proposed to explain the persistence of.soil liquef.ction. The f.rst is the existence of.low-permeability layers in the depth range in which the f.ilure planes are predicted to occur. The other allows f.r the persistence and development of.soil liquef.ction; it is consistent with homogeneous soils and requires water inf.ow f.om bedrock water springs. The latter explains many of.the f.atures of.soil liquef.ction observed during earthquakes, namely, surf.cial ef.ects, “instant” liquef.ction, and the occurrence of.short- and long-term changes in the level of.the phreatic surf.ces. This model (hypothesis), the relationship between the f.ux characteristics and loss of.soil shear strength, provides self.consistent constraints on the depth below the phreatic surf.ces where the f.ilure planes are observed (expected to occur). It requires f.rther experimental and observational evidence. Similar reasoning can be used to explain other saturated soil phenomena. © 2023 The Authors

Buechel Pappas Resurf.cing Shoulder Replacement: Evolution and over 40 Years of.Clinical Experience

Background: Early exploration of.the semi constrained “f.oating-Socket” total shoulder replacement (TSR) in 1974 led to a prolif.ration of.various unconstrained designs that allowed resection or retention of.the humeral head, depending upon the pathological process involved. Degenerative glenohumeral arthritis with mild to moderate involvement of.subchondral bone allowed f.r a resurf.cing option, while severe humeral head involvement required a partial or f.ll humeral head replacement attached to an intramedullary stem f.r f.xation. All components evolved f.om cemented to cementless application by 1982. The purpose of.this paper is to describe the progression of.Buechel-Pappas (B-P) shoulder replacement development f.om the early 1970’s in both cemented and cement less applications. Methods: Clinical evaluations of.“f.oating-Socket” TSR, f.llowed by B-P stem-type, resurf.cing types, bipolar-type and revision components, all of.which comprise the B-P Shoulder Replacement System, were perf.rmed over a 49-year period. Results: “f.oating-Socket” implants improved the results of.simple, constrained ball-in-socket designs, but generally f.iled by glenoid component loosening in both chimpanzee and human applications. Unconstrained resurf.cing-type components, both anatomical humeral head and f.ll proximal humeral components, were quite successf.l, with minimal f.ilures observed in long-term studies. Bipolar salvage implants, used f.r severe proximal def.ciencies, revisions and massive rotator cuf. arthropathy, were also very successf.l;providing overhead range of.motion in many patients. Conclusions: Resurf.cing hemiarthroplasty, in patients with intact or repairable rotator cuf. mechanisms, gave the most satisf.ctory results and were the least technically complicated to perf.rm, requiring minimal instrumentation. Resurf.cing of.f.ll proximal humeral def.ciencies, using f.moral resurf.cing components, gave similar clinical results to more complex semi-constrained devices,

A New Method f.r Deriving High-Vertical-Resolution Wind Vector Data f.om the L-Band Radar Sounding System in China

High-vertical-resolution radiosonde wind data are highly valuable f.r describing the dynamics of.the meso- and microscale atmosphere. However, the current algorithm used in China’s L-band radar sounding system f.r calculating high-vertical-resolution wind vectors excessively smooths the data, resulting in signif.cant underestimation of.the calculated kinetic energy of.gravity waves compared to similar products f.om other countries, which greatly limits the ef.ective utilization of.the data. To address this issue, this study proposes a novel method to calculate high-vertical-resolution wind vectors that utilizes the elevation angle, azimuth angle, and slant range f.om L-band radar. In order to obtain wind data with a stable quality, a two-step automatic quality control procedure, including the RMSE-f.(root-mean-square error f. test and elemental consistency test are f.rst applied to the slant range data, to eliminate continuous erroneous data caused by unstable signals or radar malf.nctions. Then, a wind calculation scheme based on a sliding second-order polynomial f.tting is utilized to derive the high-vertical-resolution radiosonde wind vectors. The evaluation results demonstrate that the wind data obtained through the proposed method show a high level of.consistency with the high-resolution wind data observed using the Vaisala Global Positioning System and the data observed by the new Beidou Navigation Sounding System. The calculation of.the kinetic energy of.gravity waves in the recalculated wind data also reaches a level comparable to the Vaisala observations. © Institute of.Atmospheric Physics/Chinese Academy of.Sciences, and Science Press 2024.

Comparisons of.oxbow lake f.sh assemblages in relation to bigheaded carp establishment in the lower White River, Arkansas

Background Since the late 1990s, bigheaded carps(largely silver carp [Hypophthalmichthys molitrix] but also bighead carp [H. nobilis]) have established throughout the lower Mississippi River basin. Using previously studied oxbow lakes in the lower White River basin, Arkansas, we compared current(2017, “post-carp” establishment) f.sh assemblages to historical(2002, “pre-carp” establishment) f.sh assemblages. f.sh assemblages were comprehensively assessed using multiple gears, including boat electrof.shing, mini-f.ke nets, and experimental small-mesh gill *** T-tests suggested that f.sh assemblage indices of.richness, diversity, evenness, and dominance were of.en greater(P f.ected by boat electrof.shing and experimental gill nets. However, all indices were generally similar(P > 0.05) between the pre-carp and post-carp period with f.sh assemblages depicted using mini-f.ke nets. Nonmetric multidimensional scaling analyses indicated that f.sh assemblages dif.ered structurally between pre-carp and post-carp periods. Assemblage dif.erences were linked to both small and large abundance changes f.r more than 20 species. Abundances of.gizzard shad(Dorosoma cepedianum), emerald shiner(Notropis atherinoides), pugnose minnow(Opsopoeodus emiliae), crappies(Pomoxis spp.), bluegill(Lepomis macrochirus), orangespotted sunf.sh(L. humilis), and yellow bass(Morone mississippiensis) declined between the pre-carp and postcarp periods. Two-Stream Approximation to the Radiative Transf.r Equation:A New Improvement and Comparative Accuracy with Existing Methods

Mathematical modeling of.the interaction between solar radiation and the Earth's atmosphere is f.rmalized by the radiative transf.r equation(RTE), whose resolution calls f.r two-stream approximations among other methods. This paper proposes a new two-stream approximation of.the RTE with the development of.the phase f.nction and the intensity into a third-order series of.Legendre polynomials. This new approach, which adds one more term in the expression of.the intensity and the phase f.nction, allows in the conditions of.a plane parallel atmosphere a new mathematical f.rmulation of.γparameters. It is then compared to the Eddington, Hemispheric Constant, Quadrature, Combined Delta f.nction and Modif.ed Eddington, and second-order approximation methods with ref.rence to the Discrete Ordinate(Disort) method(δ –128 streams), considered as the most precise. This work also determines the conversion f.nction of.the proposed New Method using the f.ndamental def.nition of.two-stream approximation(f.TSA) developed in a previous work. Notably,New Method has generally better precision compared to the second-order approximation and Hemispheric Constant methods. Compared to the Quadrature and Eddington methods, New Method shows very good precision f.r wide domains of.the zenith angle μ 0, but tends to deviate f.om the Disort method with the zenith angle, especially f.r high values of.optical thickness. In spite of.this divergence in ref.ectance f.r high values of.optical thickness, very strong correlation with the Disort method(R ≈ 1) was obtained f.r most cases of.optical thickness in this study. An analysis of.the Legendre polynomial series f.r simple f.nctions shows that the high precision is due to the f.ct that the approximated f.nctions ameliorate the accuracy when the order of.approximation increases, although it has been proven that there is a limit order depending on the f.nction f.om which the precision is lost. The Importance of.Posterior Tibial Slope in TKR: Pre and Post-Operative Measurements in United States Osteoarthritic Patients

Background: Sagittal plane placement of.the tibial component in total knee replacement (TKR) has important implications f.r maximizing the range of.f.exion motion, allowing collateral ligaments to f.nction more normally, as well as providing ideal compressive loading on the tibial bone-prosthesis interf.ce. This study attempts to quantif. the normal posterior tibial slope (PTS) angle pre-operatively and post-operatively in osteoarthritic patients af.er using a conventional extramedullary tibial resection guide to assess its ef.ectiveness. Methods: f.rty-nine primary cementless total knee replacements in 34 osteoarthritic patients were measured radiographically pre-operatively and one year post-operatively to determine the PTS and its ef.ect on range of.motion. Lateral X-rays, using the anterior cortical line of.the tibia, were employed f.r all measurements. Results: Pre-operative PTS measured 11.83˚ (range 5˚ - 18˚), while post-operative PTS of.implanted tibial components measured 11.30o (range 4˚ - 18˚). The pre-operative range of.motion of.112˚ (range 30˚ to 135˚) was improved to 119˚ (range 90˚ to 135˚) post-operatively af.er 1 year. Conclusions: Anterior tibial shaf. ref.rencing using a conventional extramedullary tibial resection guide provides an easy and convenient method f.r reproducing the anatomical PTS during TKR. This methodology provided improvement in average range of.motion f.om 112˚ pre-operatively to 119˚ post-operatively at one year.

Existing lithospheric velocity models exhibit similar structures typically associated with the f.rst-order tectonic f.atures, with dissimilarities due to dif.erent data and methods used in model generation. The quantif.cation of.model structural similarity can help in interpreting the geophysical properties of.Earth's interior and establishing unif.ed models crucial in natural hazard assessment and resource exploration. Here we employ the complex wavelet structural similarity index measure (CW-SSIM) active in computer image processing to analyze the structural similarity of.f.ur lithospheric velocity models of.Chinese mainland published in the past decade. We take advantage of.this method in its multiscale def.nition and insensitivity to slight geometrical distortion like translation and scaling, which is particularly crucial in the structural similarity analysis of.velocity models accounting f.r uncertainty and resolution. Our results show that the CW-SSIM values vary in dif.erent model pairs, horizontal locations, and depths. While variations in the inter-model CW-SSIM are partly owing to dif.erent databases in the model generation, the dif.erence of.tomography methods may signif.cantly impact the similar structural f.atures of.models, such as the low similarities between the f.ll-wave based f.EA18 and other three models in northeastern China. We f.nally suggest potential solutions f.r the next generation of.tomographic modeling in dif.erent areas according to corresponding structural similarities of.existing models. © 2023 The Authors
