Catatonia:Our current understanding of its diagnosis, treatment and pathophysiology

Catatonia is a psychomotor syndrome that has been reported to occur in more than 10% of patients with acute psychiatric illnesses. Two subtypes of the syndrome have been identified. Catatonia of the retarded type is characterized by immobility, mutism, staring, rigidity, and a host of other clinical signs. Excited catatonia is a less common presentation in which patients develop prolonged periods of psychomotor agitation. Once thought to be a subtype of schizophrenia, catatonia is now recognized to occur with a broad spectrum of medical and psychiatric illnesses, particularly affective disorders. In many cases, the catatonia must be treated before any underlying conditions can be accurately diagnosed. Most patients with the syndrome respond rapidly to low-dose benzodiazepines, but electroconvulsive therapy is occasionally required. Patients with longstanding catatonia or a diagnosis of schizophrenia may be less likely to respond. The pathobiology of catatonia is poorly understood, although abnormalities in gamma-aminobutyric acid and glutamate signaling have been suggested as causative factors. Because catatonia is common, highly treatable, and associated with significant morbidity and mortality if left untreated, physicians should maintain a high level of suspicion for this complex clinical syndrome. Since 1989, we have systematically assessed patients presenting to our psychiatry service with signs of retarded catatonia. In this paper, we present a review of the current literature on catatonia along with findings from the 220 cases we have assessed and treated.

Micromorphological features of soils formed on calcium carbonate–rich slope deposits in the Polish Carpathians

Seven soil profiles developed on calcium carbonate–rich slope deposits in the Polish Carpathians were studied in order to:i)determine the micromorphological features of heterogeneous soils formed in a carbonate depositional environment,and ii)track primary and secondary calcium carbonate forms and their distribution in such stratified *** cases of soils with different arrangements of calcium carbonate were distinguished,controlled mostly by slope *** instance,the increasing content and random distribution of angular and subangular rock fragments found in the overall soil matrix and the irregular coarse:fine size limit suggested different intensities of accumulation and mixing of soil material transported along the *** processes,together with the calcium carbonate content,mineralogical characteristics and texture influenced the type and arrangement of the bfabric *** calcium carbonate distribution within the soils,besides the obvious inheritance from parent material,was governed by the translocation and mixing of deposits on *** climatic conditions prevailing in the area favour the development of secondary forms of calcium ***,only three of the seven studied profiles contained pedogenic forms of calcium carbonate,yet they were distributed *** occurrence,distribution and preservation of secondary carbonates depended on the content of primary calcium carbonate and soil features such as *** transported material down the slope may indicate a very low content of primary calcium or lack thereof,hence its pedogenic forms could not be created.

Validity and reliability of the Polish version of the Michigan Neuropathy Screening Instrument

BACKGROUND Diabetic sensorimotor polyneuropathy is an important risk factor for foot ulceration and ***,patients with diabetes should be screened for this disorder according to local *** obstacle to the diagnosis of this disease may be the lack of unified diagnostic criteria due to the lack of properly validated scales used for *** To validate both sections(A and B)of the Michigan Neuropathy Screening Instrument(MNSI)in Polish(PL)patients with *** A cross-sectional study using a test(A1,B1)and re-test(A2,B2)formula was performed in 80 patients with *** gold standard used for neuropathy detection was a nerve conduction study(NCS)which was performed in all *** of the MNSI-PL was assessed using the Cronbach’s alpha,Kuder-Richardson formula 20(KR-20),split-half reliability,the Gottman split-half tests,and correlation between first and second half was *** was assessed using an intraclass correlation coefficient(ICC).for external validation,we used simple linear correlation,binomial regression,and agreement between two different tools using a Bland-Altman plot *** The scale was internally consistent(Cronbach’s alpha for the full scale:0.81 for A and 0.87 for B).MNSI-PL scores in test/retest showed high stability(ICC=0.73 for A and ICC=0.97 for B).The statistically important correlations between MNSI-PL and NCS were found for B1,B2,and A1(Pff points of≥3 for section A(sensitivity of 90%-100%;specificity of 33%-40%)and≥2 for section B(sensitivity of 81%-84%;specificity of 60%-70%)were obtained during neuropathy *** The MNSI-PL is a reliable and valid instrument in screening for diabetic neuropathy.

The electric measurements were carried out for La0.8Sr0.2CoO3 nanoceramics by using impedance spectroscopy methods. The resistance of sample was practically independent of frequency in measurement range. Its dependence on reciprocal temperature showed quite complicated mechanism of conduction. The most striking property of investigated sample was its resistance decreasing with increasing applied polarization.

Soil micromorphological and physical properties after application of composts with polyethylene and biocomponent-derived polymers added during composting

Composts are considered to be one of the best soil amendments. However, the effects of composts with added polymeric materials on soil physical,hydraulic, and micromorphological properties have not been widely discussed. Changes in soil physical properties influence the numerous services that soils provide. We studied the impacts of composts with the addition of three different polymers(f1–f3) produced from polyethylene and thermoplastic corn starch on the physical, hydraulic, and micromorphological properties of two soils, a Cambic Phaeozem and a Luvic Phaeozem. Applying composts with polymers had limited or no significant effect on soil bulk density and porosity, but increased the field water capacity by 18%–82% and 3%–6% and the plant-available water content by 15%–23% and 4%–17% for the Cambic Phaeozem and Luvic Phaeozem, respectively. The application of composts with polymers had a greater effect on the Cambic Phaeozem than on the Luvic Phaeozem. It was suggested that the use of modified composts led to changes in soil physical properties and micromorphological features and this effect was dependent on the compost application rate. Composts made with the addition of composite synthetic and natural material-derived polymers during composting were found to be a composite mixture that can be successfully used in agriculture.

Capsule endoscopy in Crohn's disease: Are we seeing any better?

Crohn's disease(CD) is a complex, immune-mediated disorder that often requires a multi-modality approach for optimal diagnosis and management. While traditional methods include ileocolonoscopy and radiologic modalities, increasingly, capsule endoscopy(CE) has been incorporated into the algorithm for both the diagnosis and monitoring of CD. Multiple studies have examined the utility of this emerging technology in the management of CD, and have compared it to other available modalities. CE offers a noninvasive approach to evaluate areas of the small bowel that are difficult to reach with traditional endoscopy. furthermore, CE maybe favored in specific sub segments of patients with inflammatory bowel disease(IBD), such as those with IBD unclassified(IBD-U), pediatric patients and patients with CD who have previously undergone surgery.
