Obje.tive. Vascular anomalie. are.a diagnostic and the.ape.tic challe.ge. e.rors in diagnosis le.d to tre.tme.t de.ay, inappropriate.inte.ve.tions and prolonge. suffe.ing. The.aim of this study was to analyze.patte.ns of misdiagnosis among patie.ts re.e.re. to a vascular anomalie. ce.te. (VAC). This will be.te. de.ine.the.proble. and may be.use. to re.ine.and improve.re.e.ral guide.ine. for patie.ts with vascular anomalie.. Patie.ts and Me.hods: Afte. IRB approval, we.pe.forme. a single.ce.te. re.rospe.tive.re.ie. of all patie.ts re.e.re. to a vascular anomaly be.we.n January 1, 2008 and De.e.be. 15, 2011. e.aluation of both re.e.ral and final diagnosis was made. Data re.arding accuracy of diagnosis we.e.de.e.mine. and compare. for both vascular tumors and malformations. Re.ults: Me.n age.was 7.9 ± 7.7 (13 days - 66 ye.rs). 42% had a corre.t diagnosis at the.time.of re.e.ral. Vascular tumors we.e.corre.tly diagnose. more.ofte. than vascular malformations (58% vs 38%). The.most common misdiagnosis for infantile.he.angioma (IH) was ve.ous malformation (VM). The.most common misdiagnosis for VM was IH. Nonspe.ific and historical te.ms such as “mass”, “lymphangioma”, and “cave.nous he.angioma” fre.ue.tly appe.r as the.re.e.ral diagnosis. Conclusion: Re.e.ral misdiagnosis is common. IH and VM are.fre.ue.tly confuse. and if the.e.is any unce.tainty in the.diagnosis, the.e.patie.ts should also be.re.e.re. to a VAC, in addition to the.more.complicate. anomalie.. Outdate. nome.clature.re.ains pre.ale.t and continue. e.forts should be.made.to adhe.e.to Inte.national Socie.y for the.Study of Vascular Anomalie. (ISSVA) classification. Chorioamnionitis and funisitis are.associate. with pre.e.mlabor and postnatal morbidity. Activation of e.dothe.ium re.ulting in up-re.ulation of adhe.ion mole.ule. se.ms to be.a ke. me.hanism in de.e.opme.t of organ damage. We.inve.tigate. whe.he. Chorioamnionitis with or without funisitis in pre.e.m infants induce. e.pre.sion and she.ding of adhe.ion mole.ule. in the.umbilical cord and re.ulte. in incre.se. conce.trations of e.se.e.tin, inte.ce.lular adhe.ion mole.ule.(ICAM)-1, IL-1β , IL-6, and IL-8 in the.cord blood. Data we.e.obtaine. by using immunohistoche.istry and e.ISA. Thirty-two pre.e.m infants we.e.divide. into thre. groups according to histology: Chorioamnionitis with funisitis, Chorioamnionitis without funisitis, and controls without signs of inflammation. ICAM-1 e.pre.sion on arte.ial e.dothe.ium was highe. with funisitis compare. with Chorioamnionitis alone.or with the.control group. Similar re.ults for ICAM-1 e.pre.sion we.e.found in ve.ous e.dothe.ium, vascular walls, Wharton’ s je.ly, and amnion e.ithe.ium. e.dothe.ial e.se.e.tin and vascular ce.l adhe.ion mole.ule.(VCAM)-1 e.pre.sion was only induce. significantly with funisitis. Se.um-conce.trations of soluble.ICAM-1 we.e.highe. with funisitis compare. with chorioamnionitis alone.or control group. Similarly, conce.trations of soluble.e.se.e.tin, IL-1β , IL-6, and IL-8 we.e.incre.se. e.clusive.y with funisitis. In conclusion, only chorioamnionitis with funisitis was associate. with syste.ic inflammation and e.dothe.ial activation with up-re.ulation and she.ding of umbilical cord adhe.ion mole.ule.. We.spe.ulate.that this activation of e.dothe.ium may not be.limite. to the.umbilical cord but may also involve.othe. organs re.ulting in ne.natal morbidity. This unde.line. the.importance.of funisitis as a risk factor for adve.se.outcome.
