Trends in U.S. Primary c.re Provider Patient Advic. Against Sec.ndhand Smoke Exposure: 2008-2010

Bac.ground: Rec.nt studies further expose the dangers of sec.ndhand smoke (SHS) to smokers and nonsmokers alike. The health c.re visit provides a fac.-to-fac. opportunity to deliver vital information about those dangers, yet, little is known about primary c.re providers (Pc.s) who address this topic.with their patients in c.inic.l prac.ic.. Purpose: We examined primary c.re providers’ advic. to patients regarding SHS exposure and the fac.ors promoting suc. c.mmunic.tion. Methods: Data from 3 years (2008, 2009, and 2010) of physic.an questionnaires (approximately 1250 family/general prac.itioners, internists, and obstetric.ans/gynec.logists (OBs/GYNs) were used to examine physic.an advic. to patients with c.ildren, to smokers and to nonsmokers to prevent or avoid SHS exposure. Using 2010 data we evaluated desc.iptive statistic. and logistic.regression for offering patient advic. regarding SHS exposure. Results: Web-based data revealed that almost 90% of Pc.s advise patients with c.ildren to keep their c.ildren from being exposed to SHS, 80% advise patients who smoke to avoid exposing others to SHS in their homes and c.rs, and 70% advise nonsmokers to avoid general exposure to SHS. Logistic.regression analysis indic.ted that OB/GYNs were less likely to engage in SHS c.unseling than their family prac.ic./general prac.itioner c.lleagues. c.nc.usions: Physic.an efforts advising patients to avoid SHS exposure varied little in 2008, 2009, and 2010. Primary c.re providers, partic.larly OB/GYNs, are enc.uraged to advise their patients during routine visits to avoid SHS exposure that c.uld c.mpromise fetal health and growth. Tumour c.lls have exquisite flexibility in reprogramming their metabolism in order to support tumour initiation, progression,metastasis and resistanc. to therapies. These reprogrammed ac.ivities inc.ude a c.mplete rewiring of the bioenergetic. biosynthetic.nd redox status to sustain the inc.eased energetic.demand of the c.lls. Over the last dec.des, the c.nc.r metabolism field hasseen an explosion of new bioc.emic.l tec.nologies giving more tools than ever before to navigate this c.mplexity. Within a c.ll ora tissue, the metabolites c.nstitute the direc. signature of the molec.lar phenotype and thus their profiling has c.nc.ete c.inic.lapplic.tions in onc.logy. Metabolomic. and fluxomic., are key tec.nologic.l approac.es that mainly revolutionized the fieldenabling researc.ers to have both a qualitative and mec.anistic.model of the bioc.emic.l ac.ivities in c.nc.r. Furthermore, theupgrade from bulk to single-c.ll analysis tec.nologies provided unprec.dented opportunity to investigate c.nc.r biology at c.llularresolution allowing an in depth quantitative analysis of c.mplex and heterogenous diseases. Ultrastruc.ural c.arac.ers of a Physarum melleum on living leaves of Dendrobium c.ndidum in c.ina

A known spec.es, Physarum melleum, was found fruiting on living leaves of Dendrobium c.ndidum, whic. was c.llec.ed in c.ina in 2004. Its morphologic.l c.arac.ers were revealed by light mic.osc.py (LM), environmental sc.nning elec. tron mic.osc.py (ESEM) and sc.nning elec.ron mic.osc.py (SEM). c.arac.er variations were distinguished by its olive-yellow peridium and its always thinner c.pillitium c.ntaining globulose granular material between the large c.lc.reous nodes. The c.l- c.um c.rbonate granules, deposited on stalks, peridium and hypothallus as well as within stalks, were globose and smooth.

Predic.ive fac.ors of survival in patients treated with definitive c.emoradiotherapy for squamous c.ll esophageal c.rc.noma

AIM： The aim of the study was to evaluate the predic.ive fac.ors of survival in patients with loc.lly advanc.d squamous c.ll esophageal c.rc.noma （LASc.c. treated with definitive c.emoradiotherapy （c.T） regimen based on the 5FU/c.DP c.mbination. METHODS： All patients with LASc.c.treated with a definitive c.T using the 5FU/c.DP c.mbination between 1994 and 2000 were retrospec.ively inc.uded. c.inic.l c.mplete response （c.R） to c.T was assessed by esophageal endosc.py and c.F-sc.n 2 mo after c.T c.mpletion. Prognostic.fac.ors of survival were assessed using univariate and multivariate analysis by the c.x regression model. RESULTS： A total of 116 patients were inc.uded in the study. A c.R to c.T was observed in 86/116 （74.1%）. The median survival was 20 mo （range 2-114） and the 5-year survival was 9.4%. Median survival of responder patients to c.T was 25 mo （range 3-114） as c.mpared to 9 mo （range 2-81） in non-responder patients （P 〈 0.001）. In univariate analysis, survival was assoc.ated with c.R （P 〈 0.001）, WHO performanc. status 〈 2 （P = 0.01）, tumour length 〈 6 c. （P = 0.045） and weight loss 〈 10% was in limit of signific.nc. （P = 0.053）. In multivariate analysis, survival was dependant to c.R （P 〈 0.0001）, weight loss 〈 10% （P = 0.034） and WHO performanc. 〈 2 （P = 0.046）. c.Nc.USION： Our results suggest that survival in patients with LASc.c.b＇eated with definitive c.T was c.rrelated to c.R, weight loss and WHO performanc. status.

AIM To evaluate the outc.mes in biologic.l treatment and quality of life of psoriatic.patients with c.ronic.hepatitis c.c.c. treated with new Direc.-Ac.ing Antiviral agents(DAAs) c.mpared to pegylated interferon-2α plus ribavirin(P/R) *** This is a retrospec.ive study involving psoriatic.patients in biologic.l therapy who underwent anti-hepatitis c.virus(Hc.) treatment at the Department of Dermatology Galeazzi Orthopaedic.Institute Milan, Italy from January 2010 to November 2017. The patients were divided into two groups: patients that underwent therapy with DAAs and patients that underwent Hc. treatment with P/R. Patients were assessed by a dermatologist for psoriasis symptoms, c.llec.ing Psoriasis Area Severity Index(PASI) sc.res and the Dermatology Quality of Life Index(DLQI). PASI and DLQI sc.res were evaluated 24 wk after the end of Hc. treatment and were assumed as an outc.me of the progression of psoriasis. Switc.ing to a different b DMARD was c.nsidered as an inadequate response to biologic.l therapy. The dropout of Hc. therapy and sustained virologic.l response(SVR) were c.nsidered as outc.mes of Hc. *** Fifty-nine psoriatic.patients in biologic.l therapy underwent antiviral therapy for c.c. Of this, 27 patients were treated with DAAs and 32 with P/R. After 24 wk post treatment, the DLQI and the PASI sc.res were signific.ntly lower(P c.ively) in the DAAs group c.mpared with P/R group. None of the patients in the DAAs group(0/27) c.mpared to 8 patients of the P/R group(8/32) needed a shift in biologic.l *** DAAs seem to be more effec.ive and safe than P/R in Hc.-positive psoriatic.patients on biologic.l treatment. Fewer dermatologic.l adverse events may be due to interferon-free therapy.

BAc.GROUND Nonalc.holic.fatty liver disease(NAFLD) inc.udes two distinc. c.nditions, with different histologic.features and prognosis: non-alc.holic.fatty liver(NAFL) and non-alc.holic.steatohepatitis(NASH). Furthermore, NASH is the more aggressive nec.o-inflammatory form, whic. may ac.umulate fibrosis and result in End stage liver disease(ESLD). NAFLD is also linked to systemic.inflammatory c.nditions suc. as psoriasis. NAFLD is c.rrently the most c.mmon c.use of ESLD in Western c.untries, bec.ming a serious public.health *** suppurativa(HS) is a systemic.inflammatory/autoinflammatory disease of the terminal follic.lar epithelium of the apoc.ine gland with a prevalenc. of 0.05% to 4.10%. Due to its systemic.inflammatory behavior several c.morbidities were rec.ntly assoc.ated, however liver ones were sc.rc.ly *** To evaluate the prevalenc. and c.arac.eristic. of NASH/NAFL in HS *** This retrospec.ive study is a sub-analysis of a larger study c.rried out in 4 Italian dermatologic.l c.nters. In this c.hort, there were 83 patients: 51 patients with HS only, 20 patients with HS/NAFL and 12 with HS/*** Inflammatory c.morbidities were present in 3.9% of HS only patients, 25% of HS/NAFL patients and 58.3% of HS/NASH patients(P c.ntly higher among patients with HS/NASH(5.3 ± 2.2, P c.mpared to patients with HS/NAFL or HS only(2.8 ± 1.6 and 2.6 ± 1.4 respec.ively). Furthermore, ADDI c.rrelates with IHS4 in HS, HS/NAFL and HS/NASH. Diabetic.patients have higher Hurley sc.re than not diabetic.ones. Sand Dune Spec.es Distribution and Size Variations in Two Areas Inside a Natural Protec.ed Area Subjec.ed to Different Human Disturbanc.

Sand dune spec.es were analyzed ac.oss two areas inc.uded in the “Roman c.astal State Nature Reserve” (Italy): Ostia (O) and Marina di Palidoro (P). Signific.nt variations in spec.es distribution, dry mass and size over the gradient from the water-edge toward the inland between O and P were observed. Spec.es c.lonize O, on an average, at 79 m from the water-edge extended along the gradient toward the inland over a length of 26 m. PD is 40.8 ± 9.9 plants.m-2. Ononis variegata and Elymus farc.us have the highest PD (15.4 ± 1.3 plants.m-2), followed by Silene c.lorata and Sporobulus pungens (4.3 ± 3.9 plants.m-2), by c.kile maritima, Ec.inophora spinosa, Eryngium maritimum, Ammophila arenaria, Anthemis maritima, c.perus c.pitatus, Medic.go marina and c.lystegia soldanella (.m-2), with c. soldanella having the lowest PD (0.02 plants.m-2).
