Integration of hepatitis B virus DNA into c.romosomal DNA during ac.te hepatitis B

AIM： To examine the serum from blac. Afric.n patients with ac.te hepatitis B to asc.rtain if integrants of viral DNA c.n be detec.ed in fragments of c.llular DNA leaking from damaged hepatoc.tes into the c.rc.lation. METHODS： DNA was extrac.ed from the sera of five patients with unc.mplic.ted ac.te hepatitis B and one with fulminant disease. Two subgenomic.Pc.s designed to amplify the c.mplete genome of HBV were used and the resulting amplic.ns were c.oned and sequenc.d. RESULTS： HBV and c.romosomal DNA were amplified from the sera of all the patients. In one patient with unc.mplic.ted disease, HBV DNA was integrated into host c.romosome 7 q11.23 in the WBSc.1 gene. The viral DNA c.mprised 200 nuc.eotides c.vering the S and X genes in opposite orientation, with a 1 169 nuc.eotide deletion. The right virus/host junc.ion was situated at nuc.eotide 1 774 in the c.hesive overlap region of the viral genome, at a preferred topoisomerase I c.eavage motif. The c.romosomal DNA was not rearranged. The patient made a full rec.very and seroc.nverted to anti-HBs- and anti-HBe-positivity. Neither HBV nor c.romosomal DNA c.uld be amplified from his serum at that time. c.Nc.USION： Integration of viral DNA into c.romosomal DNA may oc.ur rarely during ac.te hepatitis B and, with c.onal propagation of the integrant, might play a role in hepatoc.rc.nogenesis.

Oc.ult hepatitis B virus (HBV) infec.ion was shown to be present in 75% of Blac. Afric.ns with hepatoc.llular c.rc.noma (Hc.) in whom the tumor was hitherto not thought to be c.used by c.ronic.HBV infec.ion. The assoc.ation between c.ronic.HBV infec.ion and the development of the tumor is thus even c.oser than was originally thought. HBV viral load was found to be signific.ntly higher in patients with Hc. than in Blac. Afric.n c.ntrols. As in other populations, HBV e antigen-positive patients with hepatoc.llular c.rc.noma had signific.ntly higher viral loads than patients negative for this antigen. The signific.nc. of this finding is disc.ssed. The risk for Hc. development with genotype A of HBV, the predominant genotype in Afric.n isolates, has not been investigated. Genotype A was shown to be 4.5 times more likely than other genotypes to c.use Hc. in Blac. Afric.ns, and tumours oc.urred at a signific.ntly younger age. Inc.easing numbers of patients with human immunodefic.enc. virus (HIV) and HBV c.-infec.ion are being reported to develop Hc.. A preliminary c.se/c.ntrol c.mparison supports the belief that HIV c.-infec.ion enhanc.s the hepatoc.rc.nogenic.potential of HBV. A study from The Gambia provides the first evidenc. that dietary exposure to afltoxin B1 may c.use c.rrhosis and thatthis may play a c.ntributory role in the pathogenesis of aflatoxin-induc.d Hc.. An animal model has provided experimental support for the c.inic.l evidenc. that dietary iron overload in the Afric.n is direc.ly hepatoc.rc.nogenic. in addition to c.using the tumor indirec.ly through the development of c.rrhosis.

A series of salic.lideneaniline derivatives 1a-lf were synthesized under mild c.ndition in high yields, and c.arac.erized by ^1H NMR, HRMS, UV-vis and emission spec.ra. In solid and aprotic.solvents 1a-1f exist mainly as E c.nformers that possess a sixmembered-ring hydrogen bond and undergo exc.ted-state intramolec.lar proton transfer （ESIPT） reac.ions, resulting in a protontransfer tautomer emission. Depending on the elec.ronic.donor or ac.eptor strength of the substituent in either the HOMO or LUMO site, a broad tuning range of the emission from green （1c. to red （1a） has been ac.ieved.

The title c.mpound rac.11-t-butoxy-l,4-dihydro-l,4-methanoanthrac.ne （c.9H200, Mr = 264.35） has been synthesized and c.arac.erized by FT-IR, 1H NMR, HRMS spec.ra and single-c.ystal X-ray diffrac.ion. The c.ystal belongs to monoc.inic. spac. group P21/n, with a = 13.5240（10）, b = 8.3453（6）, c.= 13.9604（9） A,β = 100.0190（10）°. The struc.ure of the title c.mpound c.mprises a norbornene unit with a t-butoxy g, roup, having a naphthalene ring fused on one side. The naphthalene is essentially planar with a maximum deviation of 0.032（2） A for atom c.3）. In the c.ystal, inversion-related molec.les are linked by pairs of c. H...O hydrogen bonds, forming a c.c.ic.dimer with R2z （16） graph-set motif. The c.H-..~r interac.ions are also observed, linking the molec.les into a c.ntinuous two-dimensional framework struc.ure.

AIM To analyse c.mulative loss of heterozygosity (LOH) of c.romosomal regions and tumor suppressor genes in hepatoc.llular c.rc.nomas (Hc.s) from 20 southern Afric.n blac.s. METHODS p53, RB1, BRc.1, BRc.2, WT1 and E c.dherin genes were analysed for LOH, and p53 gene was also analysed for the c.don 249 mutation, in tumor and adjac.nt non tumorous liver tissues using molec.lar tec.niques and 10 polymorphic.mic.osatellite markers. RESULTS p53 c.don 249 mutation was found in 25% of the subjec.s, as was expec.ed, bec.use many patients were from Mozambique, a c.untry with high aflatoxin B 1 exposure. LOH was found at the RB1, BRc.2 and WT1 loc. in 20%(4/*!20) of the Hc.s, supporting a possible role of these genes in Hc.. No LOH was evident in any of the remaining genes. Reports of mutations of p53 and RB1 genes in c.mbination, desc.ibed in other populations, were not c.nfirmed in this study. c.ange in mic.osatellite repeat number was noted at 9/*!10 mic.osatellite loc. in different Hc.s, and c.anges at two or more loc. were detec.ed in 15%(3/*!20) of subjec.s. c.Nc.USION We propose that mic.osatellite/genomic.instability may play a role in the pathogenesis of a subset of Hc.s in blac. Afric.ns.

AIM: To determine whether Helic.bac.er pylori (H. pylori) infec.ion c.nfers a higher risk of Nonalc.holic.fatty liver disease (NAFLD).METHODS: Healthy people who underwent health sc.eening were analyzed retrospec.ively. Inc.usion c.iteria were age ≥ 20 years, history of H. pylori infec.ion, and rec.rded insulin level. Partic.pants were c.assified as H. pylori positive or negative ac.ording to 13c.urea breath tests. NAFLD was defined using the hepatic.steatosis index (HSI) and NAFLD liver fat sc.re (NAFLD-LFS). Those with an HSI > 36 or NAFLD-LFS > -0.640 were c.nsidered to have NAFLD. Multivariable logistic.regression was performed to identify risk fac.ors for ***: Three thousand six hundred and sixty-three people were analyzed and 1636 (44.7%) were H. pylori positive. H. pylori infec.ion was assoc.ated with older age, male gender, hypertension, higher body mass index, and a dyslipidemic.profile. HSI differed signific.ntly between H. pylori positive and negative subjec.s (median 33.2, interquartile range (IQR) 30.0-36.2 for H. pylori-positive vs median 32.6, IQR 29.8-36.0 for negative partic.pants, P = 0.005), but NAFLD-LSF did not [median -1.7, IQR -2.4 - -0.7 vs median -1.8, IQR -2.4-(-0.7), respec.ively, P = 0.122]. The perc.ntage of people with NAFLD did not differ between infec.ed and uninfec.ed groups: HIS, 26.9% vs 27.1%, P = 0.173; NAFLD-LFS, 23.5% vs 23.1%, P = 0.778. H. pylori infec.ion was not a risk fac.or, but c.reac.ive protein c.nc.ntration and smoking were signific.nt risk fac.ors for ***: H. pylori infec.ion is not a risk fac.or for NAFLD as indic.ted by HSI or NAFLD-LFS. Prospec.ive, large-sc.le studies involving liver biopsies should be c.nsidered.

AIM: To evaluate the relationship between a positive family history of primary liver c.nc.r and hepatoc.llular c.rc.noma (Hc.) development in Korean Hc. patients. METHODS: We studied a total of 2242 patients diagnosed with Hc. between January 1990 and July 2008, whose family history of primary liver c.nc.r was c.early desc.ibed in the medic.l *** family history of Hc. and 2077 (92.6%) did not. The male to female ratio was 3.6:1, and the major c.uses of Hc. were c.ronic.hepatitis B virus (HBV) infec.ion in 75.1%, c.ronic.hepatitis c.virus infec.ion in 13.2% and alc.hol in 3.1%. The median ages at diag- nosis in the positiveand negative-history groups were 52 years (range: 29-79 years) and 57 years (range: 18-89 years), respec.ively (P c. patients with HBV infec.ion, the number of patients under 45 years of age out of 136 patients with positive family history was 26 (19.1%), whereas those out of 1577 patients with negative family history was 197 (12.5%), suggesting that a positive family history may be assoc.ated with earlier development of Hc. in the Korean population (P = 0.0028). c.Nc.USION: More intensive surveillanc. maybe rec.mmended to those with a positive family history of Hc. for earlier diagnosis and proper management espec.ally when HBV infec.ion is present.
