Background Tuberous sclerosis complex(TSC)is an autosomal dominant genetic disorder with a birth incidence of I:6000 in the United States that is characterized by the growth of non-cancerous tumors in multiple organ systems including the brain,kidneys,lungs,and skin.Importantly,TSCis also associated with signicant neurological manifestations including epilepsy TSC-associated neuropsychiatric disorders,intellectual disabilities,and autism spectrum disorder.Mutations in the TSCI or TSC2 genes are well-established causes of TSC,which lead to TSC1/TSC2 deficiency in organs and hyper-activation of the mammalian target of rapamycin signaling pathway.Animal models have been widely used to study the effect of TSCl/2 genes on the development and function of the brain.Despite considerable progress in understanding the molecular mechanisms underlying TSC in animal models,a human-specific model is urgently needed to investigate the effects of TSCl/2 mutations that are unique to human neurodevelopment.Data sources Literature reviews and research articles were published in PubMed-indexed journals.Results Human-induced pluripotent stem cells(iPSCs),which capture risk alleles that are identical to their donors and have the capacity to differentiate into virtually any cell type in the human body,pave the way for the empirical study of previously inaccessible biological systems such as the developing human brain.Conclusions In this review,we present an overview of the recent progress in modeling TSC with human iPSC models,the existing limitations,and potential directions for future research.

Background:There exist few maximal oxygen uptake(VO_(2max))non-exercise-based prediction equations,fewer using machine learning(ML),and none specifically for older adults.Since direct measurement of VO_(2max)is infeasible in large epidemiologic cohort studies,we sought to develop,validate,compare,and assess the transportability of several ML VO_(2max)prediction algorithms.Methods:The Baltimore Longitudinal Study of Aging(BLSA)participants with valid VO2_(max)tests were included(n=1080).Least absolute shrinkage and selection operator,linear-and tree-boosted extreme gradient boosting,random forest,and support vector machine(SVM)algorithms were trained to predict VO_(2max)values.We developed these algorithms for:(a)the overall BLSA,(b)by sex,(c)using all BLSA variables,and(d)variables common in aging cohorts.Finally,we quantified the associations between measured and predicted VO_(2max)and mortality.Results:The age was 69.0±10.4 years(mean±SD)and the measured VO_(2max)was 21.6±5.9 mL/kg/min.Least absolute shrinkage and selection operator,linear-and tree-boosted extreme gradient boosting,random forest,and support vector machine yielded root mean squared errors of 3.4 mL/kg/min,3.6 mL/kg/min,3.4 mL/kg/min,3.6 mL/kg/min,and 3.5 mL/kg/min,respectively.Incremental quartiles of measured VO_(2max)showed an inverse gradient in mortality risk.Predicted VO_(2max)variables yielded similar effect estimates but were not robust to adjustment.Conclusion:Measured VO_(2max)is a strong predictor of mortality.Using ML can improve the accuracy of prediction as compared to simpler approaches but estimates of association with mortality remain sensitive to adjustment.Future studies should seek to reproduce these results so that VO_(2max),an important vital sign,can be more broadly studied as a modifiable target for promoting functional resiliency and healthy aging.

We describe a 63-year-old male who appears to have undergone an early form of the arterial switch operation for D-transposition of the great arteries performed in the mid-1960s.We review the clinical and imaging data that support our conclusion.He had a diagnostic cardiac catheterization which demonstrated severe pulmonary hypertension responsive to epoprostenol and oxygen.Our case may represent one example of the experimental surgical work done prior to Dr.Adibe Jatene’s description of theﬁrst successful arterial switch performed in 1975.

We present detailed characterization of laser-driven fusion and neutron production(-10^(5)/second) using 8 mJ, 40 fs laser pulses on a thin(<1 μm) D_2O liquid sheet employing a measurement suite. At relativistic intensity(~ 5 × 10^(18)W/cm^(2))and high repetition rate(1 kHz), the system produces deuterium±deuterium(D-D) fusion, allowing for consistent neutron generation. Evidence of D-D fusion neutron production is verified by a measurement suite with three independent detection systems: an EJ-309 organic scintillator with pulse-shape discrimination, a ~3He proportional counter and a set of 36 bubble detectors. Time-of-flight analysis of the scintillator data shows the energy of the produced neutrons to be consistent with 2.45 MeV. Particle-in-cell simulations using the WarpX code support significant neutron production from D-D fusion events in the laser±target interaction region. This high-repetition-rate laser-driven neutron source could provide a low-cost, on-demand test bed for radiation hardening and imaging applications.

BACKGROUNDIrritable bowel syndrome(IBS),defined according to the Rome IV diagnostic criteria,is a chronic functional gastrointestinal disorder characterized by recurrent abdominal pain related to altered bowel habits.First-line recommended treatments are limited to combining drugs targeting predominant symptoms,particularly pain(antispasmodics),constipation(laxatives),and diarrhea(loperamide),yielding only a limited therapeutic gain.GASTRAP^(■)DIRECT is a class IIa medical formulation composed of a combination of chitin-glucan and simethicone indicated for the symptomatic treatment of gas-related gastrointestinal disorders by combining different mechanisms of action.AIM To evaluate the efficacy,tolerability,and safety of 4-week GASTRAP^(■)DIRECT treatment in patients with IBS.METHODS In this prospective,multicenter,open-label trial,120 patients with IBS received three sticks of GASTRAP^(■)DIRECT(1.5 g/d of chitin-glucan and 0.75 mg/d of simethicone)per day for 4 weeks.The primary endpoint was the responder rate,defined as the number of patients whose abdominal pain score decreased by≥30%from baseline to week(W)4.The analysis was performed using the per-protocol set.Cardinal symptoms,impact of global symptoms on daily life,change in stool consistency,and improvement in defecatory disorders were evaluated.RESULTS Overall,100 patients were evaluated.At W4,67%(95%CI:57-75)showed improvement in abdominal pain(score:5.8±2.4 vs 2.9±2.0,P<0.0001).Similar improvements were observed for bloating[8.0±1.7 vs 4.7±2.9,P<0.0001;60%(95%CI:50-70)responders],abdominal distension[7.2±2.1 vs 4.4±3.1,P<0.0001;53%(95%CI:43-63)responders],and impact of global symptoms on daily life[7.1±2.0 vs 4.6±2.9,P<0.0001;54%(95%CI:44-64)responders].Stool consistency improved in most patients(90%and 57%for patients with liquid and hard stools,respectively).Overall,42%of patients with defecatory disorders reported very much/considerable improvements by W2.No severe adverse event occurred,and tolerability was rated“good”or“very good”by 93%of patients

Enhanced weathering of alkaline rocks and minerals is a negative emissions technology (NET) that is potentiallyscalable to deliver gigaton-level carbon dioxide removal (CDR) for climate change mitigation. This techniquerelies on the acceleration of naturally occurring weathering reactions with water and carbon dioxide by reducingthese substances into a fine powder with high specific surface area. The ex situ enhanced weathering processchain consists of the acquisition of suitable natural or synthetic materials, grinding to a fine particle size,transportation, and application on suitable sites. Future enhanced weathering systems can be envisioned assupply chain-like networks which have to be optimized to deliver maximum CDR given physical constraints.There is a notable research gap in the literature on decision support for such systems. To address this gap, amixed integer linear programming (MILP) model is developed in this work to optimize enhanced weatheringnetworks for CDR. The model also incorporates the availability of multiple transportation options and constraintson network topology. Two test cases are used to demonstrate the model capability to determine optimal andnear-optimal networks. The top ten solutions in these two scenarios yield total CDR levels of 3.4316–3.4363 Mtand 15.27017–15.27960 Mt.

The internal oxide precipitates were supposed to be spherical in Wagner’s original theory,while the fol-lowing research demonstrated that this assumption is an exception rather than the truth,which caused deviations in the application of this theory.In this study,the internal oxide precipitates have a needle-like and a near-spherical morphology in a Fe-9Cr ferritic and a Fe-17Cr-9Ni austenitic steels after expo-sure to 600℃ deaerated steam for 600 h,respectively.The nano-to-atomic scale characterization shows that the morphology of the internal oxide precipitates is controlled by the structure of the interfaces be-tween the metal matrix and the internal oxide,while the interface structure is mainly affected by the crystallographic structure of the two phases and their orientation relationship.In addition,the internal oxide precipitation-induced volume expansion and the outward Fe diffusion-induced volume shrink oc-cur simultaneously during the oxidation process.The stress status in the internal oxidation zone(IOZ)is the competing result of the two factors,which could dynamically affect the high-temperature oxidation.The results obtained in this study suggest that there is potential to control the distribution,morphology,and interface structure of the internal oxide precipitates by selecting appropriate base metal and internal oxide-forming element,in order to obtain better high-temperature oxidation-resistant materials.
