Despite significant advances in abdominal wall reconstruction,parastomal hernias remain a complex problem,with a high risk of recurrence following repair.While a number of surgical hernia repair techniques have been proposed,there is no consensus on optimal management.Several clinical variables must be considered when developing a comprehensive repair plan that minimizes the likelihood of hernia recurrence and surgical site occurrences.In this review,we describe the incidence of parastomal hernias and discuss pertinent risk factors,medical history findings,physical examination findings,supplementary diagnostic modalities,parastomal hernia classification systems,surgical indications,and repair techniques.Special consideration is given to the discussion of mesh reinforcement,including available biomaterials,anatomic plane selection,and the extent of mesh reinforcement.Although open repairs are the primary focus of this article,minimally invasive laparoscopic and robotic approaches are also briefly described.It is our hope that the provided surgical outcome data will help guide surgical management and optimize outcomes for affected patients.

The COMBO stent(OrbusNeich Medical BV,the Netherlands)is a stainless-steel platform with the biodegradable abluminal coating containing antiproliferative sirolimus(5 mg/mm)and the luminal stent surface covered with anti-CD-34 antibody.The CD-34 antibodies essentially capture endothelial progenitor cells resulting in rapid re-endothelialization of the treated segment.

BACKGROUND A fracture of the acetabulum is an uncommon,but serious injury.Established outcome tools do not reflect the patient’s perspective after fracture of the hip joint.Originally designed for post-arthroplasty patients,the Forgotten Joint Score(FJS)is a patient-reported outcome measurement(PROM)tool evaluating the diseasespecific health-related quality of life(HR-QoL).AIM To validate the FJS in patients after acetabular fracture.METHODS In a prospective mono-centric cohort study,we evaluated 100 patients at mean 5.2±3.6 years after a fracture of the acetabulum.The validation study followed the complete COSMIN checklist protocol.For calculation of convergent validity,we used the Tegner-Activity Scale,the Western Ontario and McMaster Universities Osteoarthritis Index,the EuroQol-5D,and a subjective rating of change as an anchor variable.RESULTS We confirmed good internal consistency with a Cronbach‘s alpha of 0.95.With an intraclass correlation coefficient of 0.99(95%CI:0.97,0.99),test-retest reliability of the FJS was excellent.Correlation coefficients between the questionnaires were moderate to high ranging from|0.56|to|0.83|(absolute value).No relevant floor or ceiling effects occurred.Standard error of measurement was 3.2 and smallest detectable change(SDC)was 8.8.Thus,changes greater than 8.8 points between two assessments denote a real change in FJS.CONCLUSION The FJS is a valid and reliable tool for evaluation of patient-reported outcome in posttraumatic condition after acetabular fracture.The SDC indicating a real clinical improvement was 8.8 points in the FJS.We could confirm responsiveness of the FJS and found no relevant floor-or ceiling effects.

Machine learning(ML)applications in weather and climate are gaining momentum as big data and the immense increase in High-performance computing(HPC)power are paving the way.Ensuring FAIR data and reproducible ML practices are significant challenges for Earth system researchers.Even though the FAIR principle is well known to many scientists,research communities are slow to adopt them.Canonical Workflow Framework for Research(CWFR)provides a platform to ensure the FAIRness and reproducibility of these practices without overwhelming researchers.This conceptual paper envisions a holistic CWFR approach towards ML applications in weather and climate,focusing on HPC and big data.Specifically,we discuss Fair Digital Object(FDO)and Research Object(RO)in the DeepRain project to achieve granular reproducibility.DeepRain is a project that aims to improve precipitation forecast in Germany by using ML.Our concept envisages the raster datacube to provide data harmonization and fast and scalable data access.We suggest the Juypter notebook as a single reproducible experiment.In addition,we envision JuypterHub as a scalable and distributed central platform that connects all these elements and the HPC resources to the researchers via an easy-to-use graphical interface.

AIM To stably correct tyrosinaemia in proliferating livers of fumarylacetoacetate-hydrolase knockout(Fah-/-) mice by homologous-recombination-mediated targeted addition of the Fah gene.METHODS C57 BL/6 Fah?exon5 mice served as an animal model for human tyrosinaemia type 1 in our study. The vector was created by amplifying human Fah c DNA including the TTR promoter from a lentivirus plasmid as described. The Fah expression cassette was flanked by homologous arms(620 bp and 749 bp long) of the Rosa26 gene locus. Mice were injected with 2.1 × 108 VP of this vector(r AAV8-ROSA26.HAL-TTR.FahROSA26.HAR) via the tail vein. Mice in the control group were injected with 2.1 × 108 VP of a similar vector but missing the homologous arms(r AAV8-TTR.Fah). Primary hepatocytes from Fah-/-recipient mice, treated with our vectors, were isolated and 1 × 106 hepatocytes were transplanted into secondary Fah-/-recipient mice by injection into the spleen. Upon either vector application or hepatocyte transplantation NTBC treatment was stopped in recipient mice. RESULTS Here, we report successful HR-mediated genome editing by integration of a Fah gene expression cassette into the "safe harbour locus" Rosa26 by recombinant AAV8. Both groups of mice showed long-term survival, weight gain and FAH positive clusters as determined by immunohistochemistry analysis of liver sections in the absence of NTBC treatment. In the group of C57 BL/6 Fah?exon5 mice, which have been transplanted with hepatocytes from a mouse injected with r AAV8-ROSA26.HAL-TTR.Fah-ROSA26.HAR 156 d before, 6 out of 6 mice showed long-term survival, weight gain and FAH positive clusters without need for NTBC treatment. In contrast only 1 out 5 mice, who received hepatocytes from r AAV8-TTR.Fah treated mice, survived and showed few and smaller FAH positive clusters. These results demonstrate that homologous recombinationmediated Fah gene transfer corrects the phenotype in a mouse model of human tyrosinaemia type 1(Fah-/-mice) and is long lasting in a proliferating state of the

OBJECTIVE Wide spread abuse of synthetic cathinones found in bath salts preparations has resulted in regulation of some cathinones internationally.Chemists skirt these laws by altering the chemical structures of first-generation cathinones(ie,MDPV,methylone,and mephedrone),resulting in second-generation cathinones(eg,α-PVP,α-PPP,MDPPP,and MDPBP).Although MDPV is a more effective reinforcer than cocaine,little is known about the reinforcing effectiveness of secondgeneration cathinones.To test the hypothesis that synthetic cathinones with higher selectivity for DAT relative to SERT are more effective reinforcers.METHODS Monoamine transporter inhibition was determined using synaptosomes prepared from rat brains.The relative reinforcing effectiveness of intravenously self-administered MDPV,MDPBP,MDPPP,α-PVP,α-PPP,and cocaine were directly compared through evaluations of (1)dose-response curves under a progressive ratio(PR)schedule of reinforcement and (2)demand curves obtained for each drug in male Sprague-Dawley rats.RESULTS Rank order selectivity for DAT/SERT wasα-PVP>MDPV>α-PPP≈MDPBP>MDPPP>cocaine.Comparisons of the maximum number of infusions obtained under a PR schedule of reinforcement(α-PVP>MDPV>α-PPP>MDPBP≈MDPPP>cocaine)and the essential value obtained for each drug in demand analyses(α-PVP>MDPV>α-PPP≈MDPBP≈MDPPP>cocaine)suggest relative reinforcing effectiveness is related to DAT/SERT selectivity.CONCLUSION These data provide evidence that DAT/SERT selectivity accounts for select synthetic cathinones functioning as more effective reinforcers than cocaine and may predict the abuse-related effects of novel synthetic cathinones in humans.

Cancer: Education and Primary Prevention Starts in Childhood and Adolescence

At least one third of all newly diagnosed cancers could be prevented if lifestyle factors were changed. The University Cancer Center Dresden initiated two programs aiming at cancer awareness and intentions to engage in health-promoting behavior among children and adolescents. Study 1 examined sun protection knowledge of 80 preschool children in a non-randomized design and Study 2 inspected 235 7th grade students’ knowledge of cancer and its behavioral risk-factors as well as intentions on health-promoting behavior using a randomized pre-post design on group-level. Study 1 showed significant improvement of sun protection knowledge in preschool children (p 0.05). Study 2 was effective in increasing knowledge about risk factors for cancer (p 0.001) and in increasing intentions to engage in health-promoting behavior (p 0.001). Communicating health-related behavior in preschools as well as school-based programs targeting multiple cancer-related risk factors are promising tools for primary prevention of cancer.
