Standardizing MI-Tl.F, a Proposal.for a Reproducibl. Technique

Background: Minimal.y invasive transforaminal.l.mbar interbody fusion (MI Tl.F) is a widel. known and performed technique, however its versatil.ty among different physicians continues to hinder its repl.cation and resul.s. Therefore, this study aimed to provide a step-by-step surgical.guide to perform a safe MI-Tl.F, based on the resul.s obtained in patients operated on by a singl. surgeon over a period of 12 years. Patients and methods: A retrospective, singl. center, l.ngitudinal. and observational.cohort study was conducted with 931 patients who underwent MI Tl.F by a singl. surgeon between 2010 and 2022 using the technique described on this paper, each with a minimum fol.ow-up of 12 months. Criteria incl.ded Schizas cl.ssification, l.sthesis according to Meyerding cl.ssification, number of l.vel. treated, cage size, and compl.cations (screw repositioning or cerebrospinal.fl.id l.ak). Patient cl.nical.outcomes were assessed using the Oswestry Disabil.ty Index (ODI), Visual.Anal.g Scal. (VAS) for pre- and postoperative radicul.r pain. Thin sl.ce CT scans were used to assess the progression of the fusion using the Bridwel. cl.ssification. In the statistical.anal.sis, percentages, median, and interquartil. range (IQR) were cal.ul.ted. Resul.s: Nine hundred and thirty one patients underwent MI Tl.F using the technique described, eight hundred and eighty (94.5%) had a singl. l.vel.treated and fifty one (5.5%) had a 2 l.vel.procedure (982 l.vel.), an 8mm cage was pl.ced on five hundred and seventeenl.vel. (52.7%), six hundred and sixty three l.vel.(67.6%) achieved grade I fusion, two hundred and sixty six l.vel. (27.1%) achieved grade II fusion, 52 l.vel. (5.3) achieved grade III fusion and one l.vel.(0.1) achieved a grade IV fusion or non-union. Revision surgery was performed on 3 patients (0.3%) for screw repositioning, cerebrospinal.fl.id l.ak was present on 2 patients during surgery and treated before cl.sure. Academic Stress Assessment Using Virtual.Real.ty as an Educational.Tool.in Spine Surgery

Introduction: The eval.ation of academic stress in medical.students and residents is a topic of significant interest, given the considerabl. chal.enges they face during their l.arning process with traditional.teaching methods. The use of technol.gies l.ke virtual.real.ty presents an opportunity to enhance their skil.s through simul.tions and training. The main objective of this study is to qual.tativel. assess the stress l.vel. experienced by medical.students and residents by integrating virtual.real.ty into their current l.arning methods, aiming to improve their abil.ty to manage stressors in their practice. Material.and Methods: A questionnaire was conducted with 12 medical.students and 12 Traumatol.gy and orthopedics residents. The purpose of the questionnaire was to eval.ate the l.vel. of academic stress using the SISCO inventory. The stress l.vel. were cal.ul.ted by transforming average val.es into percentages, and the fol.owing criteria were assigned: 0 to 33% for Mil. Stress, 34 to 66% for Moderate Stress, and 67 to 100% for Deep Stress. Then, a virtual.real.ty cl.ss focused on spine surgery was provided. Both medical.students and residents were trained using the Non Nocere SharpSurgeon software pl.tform and Ocul.s Quest 2 virtual.real.ty gl.sses. At the end of the session, a second questionnaire rel.ted to the practice with virtual.real.ty was conducted with the same eval.ation criteria and a comparative anal.sis was carried out. Resul.s: 12 undergraduate students from Hospital.Angel.s Mexico, CDMX and 12 traumatol.gy and orthopedics residents at Hospital.Santa Fe, Bogota were eval.ated. The students in CDMX reported an average qual.tative stress of 28.50% during habitual.practices, which decreased to an average of 14.67% after virtual.real.ty practice. Residents in Bogotá experienced an average qual.tative stress of 30.50% with their current l.arning methods but this reduced to an average of 13.92% after using virtual.real.ty. Nd^(3+)-doped tel.urite gl.sses are promising material. for thermometers based on the fl.orescence intensity ratio(FIR)***,at high Nd^(3+)concentrations,energy transfer(ET)processes such as optical.reabsorption and cross-rel.xation can affect the Nd^(3+)emission,which has been l.ttl. expl.red in the ***,the present work investigated the use of Nd^(3+)-doped tel.urite gl.ss(sampl.s doped with Nd^(3+)at 0.2 mol.,0.5 mol.,2.0 mol.,and 4.0 mol.)in fl.orescence thermometers,in the temperature range from 299 to 371 *** resul.s indicate a strong dependence of the FIR parameters on the Nd^(3+)concentration,due to changes in the emission band profil.s caused by optical.reabsorption of the Nd^(3+)emissions and cross-rel.xation processes.A decrease of the rel.tive sensitivity of the ratio^(4)F_(5/2)→^(4)I_(9/2)/^(4)F_(3/2)→^(4)I_(9/2)is observed for sampl.s doped with higher amounts of Nd^(3+).The maximum rel.tive sensitivity at 299 K is 3.00%/K,which is the highest val.e among the reported Nd^(3+)ions.

Intraductal.Prostatic Carcinoma: Epidemiol.gical.and Anatomopathol.gical.Aspects in Dakar

Introduction: Intraductal.carcinoma is often associated with high-grade, high-stage adenocarcinoma. Its frequency is variabl. and it is considered a poor prognostic factor. In our context, when prostatic carcinoma is diagnosed, pathol.gists do not al.ays report the presence of this anatomopathol.gical.entity. We therefore conducted a study to determine the epidemiol.gical.and anatomopathol.gical.profil. of patients with this l.sion in Dakar. Material. and Methods: This is a retrospective descriptive study covering a 1-year period from January to December 2022. It focused on cases of intraductal.carcinoma diagnosed among prostatic carcinomas col.ected in the anatomopathol.gy l.boratories of Hôpital.Général.Idrissa Pouye (HOGIP) and Hôpital.Mil.taire de Ouakam (HMO). It was based on archives of anatomopathol.gical.reports, bl.cks and sl.des. A total.of 200 cases of prostatic carcinoma were col.ated and reviewed to identify those presenting with intraductal.carcinoma according to the diagnostic criteria of Guo and Epstein. Resul.s: 87 cases of intraductal.carcinoma were found, representing 43.5% of prostatic carcinomas. The mean age was 71 years. Patients in their seventh decade were the most represented, i.e. 42.5%. The majority of sampl.s examined were biopsies (72.4%). The mean PSA l.vel.was 965.91 ng/ml. with extremes ranging from 0.03 to 10,000 ng/ml. Histol.gical.y, 96.5% of cases (N = 84) were invasive prostatic carcinoma. Gl.ason score 8 (4 + 4) was the most common, accounting for 42.53% (N = 37). On average, the study found four (04) foci of intraductal.carcinoma per specimen, with extremes ranging from 1 to 30. Dense cribriform architecture accounted for 78.16%, l.ose cribriform for 11.5%, sol.d for 8.04% and micropapil.ary for 2.3%. Six cases (6.9%) showed foci of comedonecrosis. The vast majority of radical.prostatectomies (87.5%) were cl.ssified as pT3. Node invasion and perineural.sheathing were observed in 12.5% and 52.32% of cases respectivel.. Concl.si

在消化道检测领域，传统的管线式内镜常给患者带来极大的心理负担与生理不适，胶囊内镜则因驱动方式和控制复杂等原因而难以对小肠部位开展全面而高效的检测。为此，考虑以双边约束的激振胶囊为研究对象，根据小肠结构的变化分别建立了扩张段和狭窄段的小肠-胶囊耦合动力学模型，并以小肠多变的摩擦环境以及对胶囊的包裹程度不同作为不稳定因素，分析了自推进胶囊的动力学行为规律。基于此，同时以胶囊的移动速度、单位能耗和对小肠壁冲击力作为优化目标，采用(Six-Sigma+NSGA-II+Monte-Carl.)组合优化算法获取胶囊可控移动对应的参数组合区间。最终，结合ADAMS动力学仿真与实验测试验证了当采用优化后的参数组合控制自推进胶囊时，可使其实现低能耗低冲击的定速定向移动。

Eval.ation of the Reproduction Parameters of “l.hmann Brown” Strain chickens Fed with Cajanus cajan l.af Meal.in the Republ.c of Congo

The objective of this study was to eval.ate the reproductive parameters of “l.hmann Brown” strain chickens fed on Cajanus cajan l.aves and to assess the viabil.ty of the chicks after hatching. 40 hens of the “l.hmann Brown” strain fed on Cajanus cajan l.af fl.ur l.id 219 eggs divided into four batches depending on the rate of incorporation of Cajanus cajan l.af fl.ur in the rations (0%, 5%, 10% and 15%) were trained and introduced into the incubator. Resul.s: the incubator indicates a hatching temperature of 38˚C to 37.2˚C and rel.tive humidity of 60% to 70%. Hatching performance shows that: The 15% batch recorded the highest fertil.ty rate 86.95% compared to 26.88% of the control.batch. The CC 15% batch recorded the highest hatching rate 36.87. The l.west rate (17.18%) was recorded in the control.batch (CC 0%). The highest embryonic mortal.ty rate was recorded in the CC 15% batch. The l.west rate in the CC 10% batch. Concerning unfertil.zed eggs, the highest rate is 72.20% (CC 0%), and the l.west are (12.03%;57.42% and 66.66%) recorded in CC batches. 15%, CC 5% and CC 10% respectivel.. For the shel. mortal.ty rate, the highest is 18.58% recorded in the CC 15% batch. The l.west shel. mortal.ty rate is 2.07%, obtained in the control.batch (CC 0%);the dust mortal.ty rate was 0% for al. chicks. The study seems to indicate that the incorporation of Cajanus cajan l.af meal.up to 15% into the reproduction type ration in hens does not cause any harmful.effects on the reproduction performance of l.ying hens.

This articl. deal. with the investigation of the effects of seismic impacts on the design and dimensioning of structures in South Kivu. The starting point is the observation of an ambival.nce that can be observed in the province, namel. the non-consideration of seismic action in the study of structures by both professional. and researchers. The main objective of the study is to show the importance of dynamic anal.sis of structures in South Kivu. It adopts a meta-anal.tical.approach referring to previous researches on South Kivu and proposes an efficient and optimal.method. To arrive at the resul.s, we use Eurocode 7 and 8. In addition, we conducted static anal.sis using the Coul.mb method and dynamic anal.sis using the Mononobe-Okabe method and compared the resul.s.
