Epidemiology of School-Related Injuries in Belgium. A Better Knowledge for a Better Prevention

The landmark Convention on the Rights of the Child states that children around the World have right to safe environment. In Belgium, as in many countries, children spend, on average, 4.5 weekdays at school, during nearly 8 hours per day. Studies have shown that, the risk of school-related injury exceeds the risk during leisure time. Literature reports that school accidents account for 10 to 30% of all accidents among pupils. Despite that, few papers treat of the school-related injuries. Consequently, based on 1540 accidents forms from an insurance company, injury places, mechanisms involved, body parts injured and nature of injuries were described. Head injuries, upper and lower limbs injuries were investigated in more details. Pearson’s chi-square test was used to assess the relationships between the variables and multivariate logistic regression models were used to study the three specific types of injuries cited above. Gender ratio (M/F) was equal to 1.6 with 25.9% of children under 6 years, 29.4% of 6 - 9 years, 26.9% of 10 - 12 years and 17.8% from 13 years or more. The major places of injuries were the playground (56.9%) and the physical education (19.7%). Falls were observed in 52.1% of cases and contacts were reported in 24.3%. The head injuries account for 40.6%, the upper limbs for 32.0% and the lower limbs injuries for 20.2%. The bruises and the scratches were observed in 23.8% and the wounds in 21.5%. Fractures were reported in 16.1%. In conclusion, having a routine access to the data from the insurance companies could be an important source of information for an injury surveillance system in which the school injuries will be included. Taking into account this data will require an awareness of all the concerned persons about the relevance of such a system and a harmonization of the accidental forms.

Socioeconomic Indices Performance for HIV Diagnostic on Severely Malnourished Children in Burkina Faso

Children living with HIV should be diagnosed as early as possible and linked to antiretroviral treatment. The costs of HIV virological testing, individual and social implications of the disease require critical quality of the medical approach to diagnostic strategy. Socioeconomic data can be used to refine the medical decision. The objective of this analysis was to determine informative capacity of socioeconomic indices in the diagnosis and approach of HIV in severely malnourished children using receiver operating characteristic (ROC) curves. Materials and Methods: This was a test performance analysis conducted from data of severely malnourished children at an inpatient feeding therapeutic center in Ouagadougou. Anthropometric and social indices have been used as tests;their sensitivity, specificity and predictive values have been calculated. ROC curves and areas under curves were obtained. Medical decision aid scheme has been validated from severely malnourished children data. Results: Socioeconomic indices showed high performance to discriminate severely malnourished HIV infected and uninfected children, with high area under ROC curves. The medical decision aid scheme specificity reached 97.6% with predictive values close to 85%. Conclusion: This approach can help for expanding ART for children in low income countries and achieve the maximum ART benefits in child socioeconomic kneed context.

Aims: Establish profiles of young “at risk” of injuries, first, on an overall point of view and, secondly, for some types of injuries (sport, home, road traffic, school and work injuries). Methods: We have taken nearly 50 variables into consideration: 17 variables for construction of the socioeconomic status, 9 variables for the investigation of symptoms, 11 concerning drugs, 5 for healthy habits, 3 for investigating the violence behavior, 4 concerning the school, 3 for subjective health and finally 3 for social network. We have used the principal component analysis, the multiple correspondence analysis and the weighted-frequency score for reducing the number of them. After these reductions, 15 variables were available for analyses. The relationship between injuries and investigated factors was assessed using the Pearson’s chi-square test. We also calculated odds ratio (OR) with their 95% confidence intervals (95%CI) to estimate the strengths of the associations. To further assess these relationships but also for taking into account the potentials confounding effects of some variables, logistic regression model and multinomial logistic regression model were applied. Results: The whole injury prevalence was equal to 45.6% and among the injured, the proportion of the several types was equal to 33.8% for sport injuries, 32.2% for home injuries, 16.6% for traffic injuries, 11.6% for school injuries and 5.7% for work injuries. We can say that, in light of the variables studied, the “at risk” profile for having reported an injury is being a boy, being younger, having drug experiences, with the violent profile, and declaring several symptoms. There are no consistent and marked deviations in this study from the findings obtained in previous studies. Conclusion: Analyzing injuries in general is interesting but for preventing them it is important to know in which activities children and students are engaged when they are injured.

Influence of Proxies on the Reported Frequency of Falls, Fear of Falling and Activity Restriction in Older People

Introduction: In older populations, the help of a third person may be necessary for a certain number of individuals in order to fill in questionnaires. The influence of this assistance on the collected information can raise questions, among others concerning the concordance between the information provided by the persons themselves and by their proxies, or the introduction of a potential bias. Our study’s objective is to examine, among older people, the differences in the reported frequency of falls, fear of falling and activity restriction due to fear of falling, according to the fact that people had filled in a questionnaire with or without the help of a third person. Methods: The data used come from the secondary analysis of a cross-sectional survey using a self-administered questionnaire and conducted in 2006 in a Belgian semi-rural town among non-institutionalised persons aged 65 years and over. Results: Among the 501 questionnaires, 16.4% were filled in with the help of another person. In bivariate analysis, the persons who needed help reported fall history, fear of falling and activity restriction significantly more often. In multivariable analysis, when considering concurrently the covariates (sex, age, living alone and fall history for fear of falling and for activity restriction), the influence of having received help to fill in the survey was only significant for activity restriction. Conclusion: Our study shows that it is important to know whether or not a person has received help to fill in a questionnaire. This can actually influence the reported frequency of certain outcomes, even, for activity restriction, when considering simultaneously the effect of other covariates. Given the current and future ageing of our populations, it seems that more extensive exploration is needed of the influence of proxies on the collected information about falls, fear of falling and activity restriction among older people.

Impact of HIV/AIDS on mortality and nutritional recovery among hospitalized severely malnourished children before starting antiretroviral treatment

In low income countries, severe acute malnutrition remains a major problem for HIV-infected children and an important risk factor for mortality. This study aims to analyze HIV impact on mortality rate and nutritional recovery among severely malnourished HIV/AIDS uninfected and infected children. This was a retrospective cohort study conducted from data of 521 hospitalized severely malnourished children. We used Pearson’s Chi square test to compare proportions;and Student’s independent t-test to compare means;general linear model to analyze repeated measurements. We used mortality relative risk with confidence interval (CI 95%), Kaplan-Meir survival curves and Cox proportional hazard models to analyze the HIV impact on mortality rate. Case fatality rate differed significantly from SAM HIV uninfected (10.7%) and HIV infected children (39.7%), p les infections and metabolic complications have synergism on death occurrence in sever acute malnutrition;when associated to HIV infection, case fatality rate increases many times. Weight gain and anthropometrics index evolution were very slow for SAM HIV infected children, and specific diet may be needed for more nutritional recovery. Effective interventions, updated and adapting to local country context, to improve survival of severely malnourished HIV/AIDS infected children in HIV and SAM prevalent settings are urgently needed in the area of SAM’s

Impact of multivitamin and mineral supplements on mortality and nutritional status of hospitalized severely malnourished HIV/AIDS infected children

Micronutrient deficiencies, in HIV/AIDS infected children, are common and associated with disease progression and increased mortality. There is no evidence to support specific recommendation for in-patient treatment of severely malnourished HIV/AID infected children. We analyzed the impact of daily oral multivitamin and mineral supplementation on nutritional status and on mortality rate, of severely malnourished HIV/AIDS infected infants, hospitalized in a feeding therapeutic center. Children were randomized at admission in one of the two treatment groups: WHO standard group or additional dose group. The trial has been approved by the national health research ethics committee. Case fatality rate was high in each group. There was no difference in mortality rate between WHO standard group (mortality rate = 16.9%) and “additional dose” group (mortality rate = 17.6%). Kaplan Meir survival curves did not differ between the two groups, (p Log Rank = 0.87). During the inpatient follow-up, the median weight gain (g/kg/d) was low in groups and did not differ between WHO standard group (8.88g/kg/d Min = 4.70, Max = 17.23) and “additional dose” group (10.50g/kg/d, Min = 3.29 Max = 17.81) (Wilcoxon test p-value = 0.10). From admission to discharge, anthropometric Z-scores means evolution did not differ between the two groups: WHZ (p = 0.74), WAZ (p = 0.79) and MUAC (p = 0.73). In this randomized trial among severely malnourished HIV/AIDS infected children, we found no evidence that the provision of daily additional micro-nutrient dose during inpatient treatment, compared to WHO standard, improves nutritional status or reduces case fatality rate. Rigorous malnutrition inpatient treatments, including effective HIV/AIDS management, improve nutritional status of severely mal-nourished HIV infected children.

Antibiotics prescribed to febrile under-five children outpatients in urban public health services in Burkina Faso

Appropriate use of antibiotics remains critical for success in achieving MDG4. The aim of this study was to investigate antibiotics prescribing practices to febrile under-five children outpatients in urban public health services in a low income country. Methods: From March to April 2013, a cross-sectional epidemiological study of care facilities visit by under-five age, for febrile illness, was carried out in urban health services in Bobo-Dioulasso, Burkina Faso. Patient demographics, diagnoses and medications were recorded. We calculated for each diagnoses several indicators for antibiotics use. Results: Our study showed an over-prescription of antibiotics at the university teaching hospital (78.08%) and at the first level facilities (57.71%) for under-five outpatients for febrile illness. There was evidence of high antibiotic prescription in children with diarrhea (more than 9 on 10 at university teaching hospital of diarrhea cases and 60% at the first level facilities), in children with Upper respiratory tract infections (respectively 60% and 85.2% of cases at university teaching hospital and at the first level facilities) and in children with malaria (respectively 47.5% and 17.6% of cases at university teaching hospital and at the first level facilities). Overuse, misuse and inappropriately prescribed antibiotic coexisted in our results: at university teaching hospital 90.9% of diarrhea cases, 60% of URTI cases,?47.5% of malaria cases received antibiotic prescription;at first level heath care facilities 85.2% of URTI, 17.6% of malaria cases received an prescribed antibiotic and 11.8% of LRTI did not received a prescribed antibiotic. Developing countries have poor access to newer antibiotics and irrational antibiotics use remains a global problem. Overuse and misuse of antibiotics combat, rigorous infectious diseases diagnosis, antimicrobial resistance consequences education of users and health professional’s prescribers, and improved surveillance of antimicrobial resistance, must be strengthened.

Assessment of Knowledge, Attitude and Practice of the General Population of Bukavu in the Democratic Republic of Congo on Blood Donation and Blood Transfusion

Objective: To assess the knowledge, attitudes and practices regarding blood donation in the general population. Methods: A cross-sectional study was conducted and enrolled 416 people between 18 and 65 years old at random, living in three health zones in the city of Bukavu in eastern DRC. These subjects responded to a questionnaire on knowledge, attitudes and practices regarding blood donation. Results: Our sample consisted of 61.5% men, 70.9% of people from one level of education at the secondary level and 60% unemployed. According to the surveyed subjects blood was considered as a fuel of the body in 44.6% and as a source of life in 44.1%. Sixty-one percent of the population did not know the practice of blood donation;this knowledge differed significantly (p level of education. 67.1% of people knew that the blood had negative effects and 27.4% did not know where the blood bags were stored. Channels of knowledge about blood donation practices were associations of blood donors (30.9%), awareness campaigns (18.2%), school (17.3%) and media (15.5%), churches (10%), and friends (8.1%). Approximately 85% of subjects who knew blood donation agreed with blood donation and blood transfusion but only 54.9% had donated blood in their lifetime, with a proportion of loyalty to blood donation (31.8%). In the population 59.4% were willing to donate blood. Motivation to donate blood most common (66%) was volunteerism while 19.1% were willing to donate blood for a family member and 12.6% willing to donate blood against money. The reasons for refusal to donate blood identified were fear of contracting diseases especially HIV, lack of information, religious beliefs, the sale of the collected blood by medical staff, and fear of test result for HIV. Conclusion: Factors of refusal to donate blood were identified. Efforts and new strategies tailored to these factors must be developed and implemented to increase blood donation.

Hepatitis B, Hepatitis C and HIV among Children 6 to 59 Months in the Community in the Democratic Republic of Congo

Objectives: To determine the prevalence of HBV, HCV and HIV among children 6 to 59 months and determine the risk factors. Materials and Methods: Descriptive and analytical study on children aged 6 to 59 months of community Maniema (DRC) conducted between 24 June and 24 July 2013. We enrolled 781 children aged 6 to 59 months. For association between the presence of viral markers and potential risk factors the chi-square test of Pearson was applied, the odds ratios (OR) and their 95% confidence intervals (95% CIs) were calculated. The Fisher exact test was used when the conditions for application of the chi-square test of Pearson were not met. Results: The median age of the children was 34 months, 51% of female children. Ten percent of children had a history of blood transfusion;6.6% and 64% had fever and anemia. The prevalence of HBsAg was 3.6%, the prevalence of antibodies against hepatitis C was 2.8% and HIV was 3.7%. The risk factors were HBsAg, male gender (5.5% male vs 2.0 % female;OR = 2.8(1.3 - 6.9) p = 0.001) and urban areas (5.0% rural vs 0.5% urban, OR = 0.1 (0.01 - 0.72), p = 0.01). For HCV and HIV, these factors were the history of blood transfusion in the child and the mother. For HCV 7.6% of children with blood transfusion history;OR = 3.2 (1.1 - 8.5);p = 0.05 and 11.1% history’s mother blood transfusion, OR = 5.6 (2.1 - 14.1). For HIV 11.4% of children with blood transfusion history, OR = 3.8, p = 0.005 and 9.8% history’s mother blood transfusion, OR = 2.9, p = 0.04. Conclusion: Vaccination against hepatitis B in children must be widespread, educational messages to the population must target the risk factors for these viruses. A screening of hepatitis in pregnant women must be coupled with rigorous selection policy for blood donors before each donation and qualification of any unit of blood.

Hepatitis B, Hepatitis C and HIV in Pregnant Women in the Community in the Democratic Republic of Congo

Objectives: To determine the prevalence of HBV, HCV and HIV in pregnant women and identify risk factors. Materials and Methods: Descriptive and analytical study on pregnant women in the community Maniema (DRC) between 24 June and 24 July 2013. We enrolled a total of 581 pregnant women. Results: Prevalence among pregnant women was 4.1% (2.5% to 6.3%) for HIV, 5.9% (3.9% to 8.4%) for the HBsAg and 4.1% (2.5% to 6.4%) for hepatitis C. The risk factor was for HIV, blood transfusion history in women. For HBsAg the main risk factor was the tattoo history and no statistically significant factor was found for hepatitis C. Conclusion: To reduce the risk of transmission of hepatitis from mother to child, all pregnant women should be screened prenatally. A strengthening of routine screening of blood products must be done and a health education of the population on the mode of contamination should be conducted.
