Introduction: Although laparoscopic Nissen fundoplication is the most common procedure for the repair for hiatal hernia (HH) repair, HH recurrence due to breakdown of the hiatoplasty has been reported as a common mechanism of failure after primary repair. Left transthoracic anatomic and physiologic repair (AFR) of HH is associated with lower incidence of leak and reoperation but greater morbidity. Adopting the transthoracic approach to a robotic laparoscopic platform may represent the ideal approach to the repair of HH. This study reviews the results of this technique. Methods: A retrospective review was performed on patients who had robotic AFR (RAFR) of large HH. All patients received the previously validated Gastroesophageal Reflux Disease-Health-Related Quality of Life (GERD-HRQL) questionnaire preoperatively and postoperatively. Objectively, symptoms were graded using the Visick Scale. Recurrence was defined as greater than 2 cm or 10% of the stomach above the diaphragm detected by either CT, esophagogram or endoscopy. The preoperative data was compared to the results at 2 years. Results: 396 patients underwent RAPR. The Median GERD-HRQL score was 42 (range 38 - 45) preoperatively and 6 (range 0 - 14) at two years (p < 0.05). Preoperatively 87% of patients were graded as Visick IV. At two years, 95% were graded as Visick I. HH recurrence occurred in 4/396 patients (1%). Conclusion: RAFR of HH is associated with excellent symptom relief and low recurrence rate. RAFR should be considered when deciding on what operation to perform in patients with large paraesophageal hiatal hernias.

The range of relative sea level rise in the northwestern South China Sea since the Last Glacial Maximum was over 100 m. As a result, lowland regions including the Northeast Vietnam coast, Beibu Gulf, and South China coast experienced an evolution from land to sea. Based on the principle of reconstructing paleogeography and using recent digital elevation model, relative sea level curves, and sediment accumulation data, this paper presents a series of paleogeographic scenarios back to 20 cal. ka BP for the northwestern South China Sea. The scenarios demonstrate the entire process of coastline changes for the area of interest. During the late glacial period from 20 to 15 cal. ka BP, coastline slowly retreated, causing a land loss of only 1×104 km2, and thus the land-sea distribution remained nearly unchanged. Later in 15-10 cal. ka BP coastline rapidly retreated and area of land loss was up to 24×104 km2, causing lowlands around Northeast Vietnam and South China soon to be underwater. Coastline retreat continued quite rapidly during the early Holocene. From 10 to 6 cal. ka BP land area had decreased by 9×104 km2, and during that process the Qiongzhou Strait completely opened up. Since the mid Holocene, main controls on coastline change are from vertical crustal movements and sedimentation. Transgression was surpassed by regression, resulting in a land accretion of about 10×104 km2.

Residence time distributions （RTDs） in horizontal fluidised beds have a huge effect on solid product properties and are infuenced by the internal design of the apparatus, e.g. the separation into different compartments by weirs. Weirs can be passed in or against the overall solid transport direction, with the back-flow resulting in axial dispersion, which is a measure of the spread of the RTD. Therefore, the ratio of exchange rates at weirs under different fluidisation conditions provides information on axial dispersion. In this work, a methodology based on particle tracking velocimetry is presented to obtain information on the exchange rates of particles at weirs in horizontal fluidised beds. The internal recirculation is studied for over-flow weirs with respect to different fluidisation conditions, providing a first step towards determining the effects of weirs and fluidisation conditions on axial dispersion and RTDs in horizontal fluidised beds.

The contribution of leakage in a baghouse filter （defined as a short circuit between the upstream and downstream sides of the filter） to the emission of fine particles is quantified in comparison to other dust emission sources, and the influence of key operating variables on overall system response is analyzed. The study was conducted on a well-maintained pilot-scale filter unit （9 bags of 500 g/m^2 calendered polyester needle felt; total surface area 4.2 m^2） operated in Ap-controlled mode over a range of pulsing intensities, with two types of test dust （one free-flowing and the other cohesive） at inlet concentrations of 10 and 30 g/m^3. Leaks included single holes between 0.5 and 4 mm diameter, intentionally placed in either the plenum plate or one of the filter bags, as well as seamlines from bag confectioning. Emissions were sep- arated by source into a transient contribution due to dust penetration through the filter bags after each cleaning pulse, and a continuous contribution from leaks. This separation was based on a novel method of data processing that relies on time-resolved concentration measurements with a specially calibrated optical particle counter. Tiny leaks on the order of 1 mm generated the same emission level as all the bags combined, and dominated continuous emissions. The equivalent leak cross section （leakage = media emission） was about 1 ppm of the total installed filter surface, independent of upstream dust concentra- tion. Leakage through open seamlines amounted to 75% of media emissions in case of free-flowing test dust. Leakage was restricted to aerodynamic diameters less than ~5 μm （roughly the PM2.s mass frac- tion）. For comparison, time-averaged mass penetration through conventional needle-felt media ranged from about 10^-5 to 10^-6, depending on cohesiveness of the particle material and pulse cleaning intensity, giving emission levels between about 0.02 and 0.2 mg/m^3 at the reference concentration of 10 R/m^2.

目的了解长期照护机构护士发生职业倦怠的心理机制,为制订改善护士心理健康的干预策略提供基础。方法采用主题分析法进行2项质性研究。第1项研究于2019年5—7月在德国8家养老院以工作坊形式对110名护士进行了8次小组访谈。作为补充,第2项研究于2019年12月对德国养老院的14名管理人员进行半结构访谈。结果第1项研究确定的3个主要主题包括:挑战的原因、员工的变革机会和组织的变革机会。第2项研究确定的3个主要主题包括:工作动机、替班原因和员工的自我关怀。研究还发现,在应对挑战的过程中,自我损耗——当被要求替班或加班时说不的能力减弱——是护士职业倦怠的重要前因。此外,人力资源不足时员工高水平的利他动机和团队或组织认同感与自我损耗有关;自我价值感低也是导致自我损耗的风险因素。结论关于工作需求与职业倦怠的经典理论模型中常将动机和认同作为资源,但本研究发现与经典理论模型中的一些核心原则不一致。应制订整体性护理干预计划,包括个人辅导、团队干预和组织发展程序。员工自身在长期照护工作中对自我损耗问题要敏感,并应得到支持;政策制定者应该建立合理的资源结构,即不再鼓励这种员工自我损耗行为的结构。

During the regeneration of the tail in the arboreal New Zealand gecko （Hoplodactylus maculatus） a new set of tail scales, modified into pads bearing setae 5-20 μm long, is also regenerated. Stages of the formation of these specialized scales from epidermal pegs that invaginate the dermis of the regenerating tail are described on the basis of light and electron microscopic images. Within the pegs a differentiating clear layer interfaces with the spinulae and setae of the Oberh^utchen according to a process similar to that described for the digital pads. A layer of clear cytoplasm surrounds the growing tiny setae and eventually comities around them and their spatular ends, later leaving the new setae free- standing on the epidermal surface. The fresh adhesive pads help the gecko to maintain the prehensile function of its regenerated tail as together with the axial skeleton （made of a cylinder of elastic cartilage） the pads allow the regenerated tail to curl around twigs and small branches just like the original tail. The regeneration of caudal adhesive pads represents an ideal system to study the cellular processes that determine setal formation under normal or experimental manipulation as the progressive phases of the formation of the setae can be sequentially analyzed.

Purpose: To evaluate the 3-year efficiency and safety of XEN® 45 gel stent implantation in a heterogenous group of open angle glaucoma patients. Methods: In this prospective, non-randomized observational study we identified patients who had undergone either stand-alone XEN® implantation (XEN solo) or XEN® implantation in combination with phacoemulsification (XEN combi). All patients who had undergone an implantation during the period 01.04.17-31.10.19 at the Department of Ophthalmology, Drammen Hospital, Norway, were asked to participate. Success was defined as IOP between 5 - 18 mmHg and 20% pressure reduction without medications. Qualified success required the same pressure interval and reduction but allowed medications. The procedure was deemed as failure if pressure requirements were not met, vision was reduced to light perception or worse, or if there was a converion to secondary glaucoma surgery. Results: Out of 115 patients and 133 eyes identified, 87 patients and 99 eyes consented to participate. All patients were Caucasians with a mean age of 73.6 years. The study had a mean (range) follow-up of 38.9 (28 - 54) months. The mean medicated baseline (SD) was reduced from 22.6 (7.9) mmHg on 3.2 (1.1) medications to 14.2 (5.6) mmHg on 1.4 (1.6) medications. Success and qualified success were achieved in 22.2% and 21.2%, respectively. Needling was performed in 34 eyes. Conclusion: XEN® 45 gel stent implantation is a safe procedure, offering a significantly lower IOP and number of medications in a subset of patients with open angle glaucoma.

WOMBAT is a software package for quantitative genetic analyses of continuous traits, fitting a linear, mixed model; estimates of covariance components and the resulting genetic parameters are obtained by restricted maximum likelihood. A wide range of models, comprising numerous traits, multiple fixed and random effects, selected genetic covariance structures, random regression models and reduced rank estimation are accommodated. WOMBAT employs up-to-date numerical and computational methods. Together with the use of efficient compilers, this generates fast executable programs, suitable for large scale analyses. Use of WOMBAT is illustrated for a bivariate analysis. The package consists of the executable program, available for LINUX and WINDOWS environments, manual and a set of worked example, and can be downloaded free of charge from http://agbu. ***/~kmeyer/wombat.
