Use of shear wave elastography for the diagnosis and follow-up of biliary atresia: A meta-analysis

BACKGROUND Timely differentiation of biliary atresia(BA)from other infantile cholestatic diseases can impact patient ***,non-invasive staging of fibrosis after Kasai hepatoportoenterostomy has not been widely *** wave elastography is an ultrasound modality that detects changes in tissue *** authors propose that the utility of elastography in BA can be elucidated through meta-analysis of existing *** To assess the utility of elastography in:(1)BA diagnosis,and(2)post-Kasai fibrosis *** A literature search identified articles that evaluated elastography for BA diagnosis and for post-Kasai *** studies met criteria for meta-analysis:Eleven for diagnosis and nine for follow-up *** diagnostic odds ratio(DOR),sensitivity,and specificity of elastography were calculated through a random-effects model using meta-Disc *** Mean liver stiffness in BA infants at diagnosis was significantly higher than in non-BA,with overall DOR 24.61,sensitivity 83%,and specificity 79%.Post-Kasai,mean liver stiffness was significantly higher in BA patients with varices than in patients without,with DOR 16.36,sensitivity 85%,and specificity 76%.Elastography differentiated stage F4 fibrosis from F0-F3 with DOR of 70.03,sensitivity 96%,and specificity 89%.Elastography also differentiated F3-F4 fibrosis from F0-F2 with DOR of 24.68,sensitivity 85%,and specificity 81%.CONCLUsION Elastography has potential as a non-invasive modality for BA diagnosis and surveillance *** paper’s limitations include inter-study method heterogeneity and small sample ***,standardized,multi-center studies are recommended.

The role of quick sepsis-related Organ Failure Assessment score as simple scoring system to predict Fournier gangrene mortality and the correlation with Fournier’s Gangrene severity Index: Analysis of 69 patients

Objective:Fournier’s gangrene is a rare but life-threatening infection disease with high mortality *** quick sepsis-related Organ Failure Assessment(qsOFA)is a new and simpler scoring system that may identify patients with suspected infection who are at greater risk for a poor *** purpose of this study was to find out role of qsOFA in determining prognosis of Fournier’s gangrene ***:This study is a case control with retrospective review of Fournier’s gangrene patients treated at Hasan sadikin Hospital from January 2013 to December 2017 who met inclusion *** were divided into two groups according to qsOFA score as high qsOFA(2-3)and low qsOFA(0-1).Results:From 69 patients,the mortality rate was 24.6%.The sensitivity of qsOFA score to predict mortality was 88.2%;the specificity was 94.2%;positive predictive value was 83.3%;negative predictive value was 96.1%;positive likelihood ratio was 15.2;negative likelihood ratio was 0.12;and the area under the receiver operating characteristic curve of qsOFA was 94.2%.There was significant association between qsOFA scale and mortality with p-value of *** qsOFA score has strong positive correlation with Fournier’s Gangrene severity Index(psion:qsOFA scoring system has a high prognostic value and can be used to determine prognosis of Fournier’s gangrene patients.

On-resin peptide modification renders an easy-to-operate method that combines solid-phase peptide synthesis efficiency and avoids tedious purification procedures. Herein, we report the transition-metal-free and redox-neutral approach for solid-phase met diversification with substrate diversity, which could be applied to synthesize cyclic peptides of different sizes.

Mobile health apps (MHAs) and medical apps (MAs) are becoming increasinglypopular as digital interventions in a wide range of health-related applications inalmost all sectors of healthcare. The surge in demand for digital medical solutionshas been accelerated by the need for new diagnostic and therapeutic methods inthe current coronavirus disease 2019 pandemic. This also applies to clinicalpractice in gastroenterology, which has, in many respects, undergone a recentdigital transformation with numerous consequences that will impact patients andhealth care professionals in the near future. MHAs and MAs are considered tohave great potential, especially for chronic diseases, as they can support the selfmanagementof patients in many ways. Despite the great potential associated withthe application of MHAs and MAs in gastroenterology and health care in general,there are numerous challenges to be met in the future, including both the ethicaland legal aspects of applying this technology. The aim of this article is to providean overview of the current status of MHA and MA use in the field ofgastroenterology, describe the future perspectives in this field and point out someof the challenges that need to be addressed.

Long-Term Trends of Carbon Monoxide Total Columnar Amount in Urban Areas and Background Regions:Ground-and satellite-based spectroscopic Measurements

A comparative study was carried out to explore carbon monoxide total columnar amount（CO TC） in background and polluted atmosphere, including the stations of Zss（Zvenigorod）, ZOTTO（Central siberia）, Peterhof, Beijing, and Moscow,during 1998–2014, on the basis of ground-and satellite-based spectroscopic measurements. Interannual variations of CO TC in different regions of Eurasia were obtained from ground-based spectroscopic observations, combined with satellite data from the sensors MOPITT（2001–14）, AIRs（2003–14）, and IAsI met Op-A（2010–13）. A decreasing trend in CO TC（1998–2014） was found at the urban site of Beijing, where CO TC decreased by 1.14% ± 0.87% yr^（-1）. Meanwhile, at the Moscow site, CO TC decreased remarkably by 3.73% ± 0.39% yr^（-1）. In the background regions（Zss, ZOTTO, Peterhof）, the reduction was 0.9%–1.7% yr^（-1） during the same period. Based on the AIRsv6 satellite data for the period 2003–14, a slight decrease（0.4%–0.6% yr^（-1）） of CO TC was detected over the midlatitudes of Eurasia, while a reduction of 0.9%–1.2% yr^（-1） was found in southeast Asia. The degree of correlation between the CO TC derived from satellite products（MOPITTv6 Joint, AIRsv6 and IAsI met Op-A） and ground-based measurements was calculated, revealing significant correlation in unpolluted *** in polluted areas, IAsI met Op-A and AIRsv6 data underestimated CO TC by a factor of 1.5–2.8. On average, the correlation coefficient between ground-and satellite-based data increased significantly for cases with PBL heights greater than 500 m.

Macitentan (MAC) is a pulmonary arterial hypertension (PAH) drug marketed as a tablet and often has stability issues in the final dosage form. Quantitative determination of MAC and its associated impurities in tablet dosage form has not been previously reported. This study quantified impurities present in Macitentan tablets using a binary solvent-based gradient elution method using reversed phase-high performance liquid *** developed method w as validated per International Conference on Harmonization (ICH) guidelines and the drug product w as subjected to forced degradation studies to evaluate stability. The developed method efficiently separated the drug and impurities (48 min) w ithout interference from solvents,excipients,or other impurities. The developed method met all guidelines in all characteristics w ith recoveries ranging from 85%-115%,linearity w ith r 2≥0. 996 6,and substantial robustness. The stability-indicating nature of the method w as evaluated using stressed conditions (hydrolysis:1 N HCl at 80℃/15 min; 1 N NaOH at 25℃/45 min; humidity stress (90%relative humidity) at 25℃for 24 h,oxidation:at 6%(v/v) H2O2,80℃/15 min,thermolysis:at105℃/16 h and photolysis:UV light at 200 Wh/m2; Fluorescent light at 1. 2 million luxh). Forced degradation experiments show ed that the developed method w as effective for impurity profiling. All stressed samples w ere assayed and mass balance w as> 96%. Forced degradation results indicated that MAC tablets w ere sensitive to hydrolysis (acid and alkali) and thermal conditions. The developed method is suitable for both assay and impurity determination,w hich is applicable to the pharmaceutical industry.

Next-generation lithography for 22 and 16 nm technology nodes and beyond

In this paper, next-generation lithography (NGL) for the 22 and 16 nm technology nodes and beyond is reviewed. A broad range of topics, including history, technologies, critical challenges, and the most plausible candidates are discussed. The 22 and 16 nm technology nodes rely on NGL. NGLs have been extensively studied. Because of technological issues, the semiconductor industry has stopped pursuing several NGLs, such as X-ray proximity lithography, ion projection lithography, and scattering with angular limitation projection electron lithography. Currently, the primary candidate technologies are extreme ultraviolet lithography (EUVL), maskless lithography (ML2), and nanoimprint lithography (NIL), with EUVL being the leading candidate. since EUVL was first proposed in 1988, many studies have been conducted. Currently, there is no "show stopper" for EUVL. Moreover, challenges are present in almost all aspects of EUVL technology. Almost all primary semiconductor manufacturing companies plan to implement commercial pre-production step-and-scan exposure tools in 2011. However, EUVL power at an intermediate focusing level has not yet met the volume manufacturing requirements. EUVL resists have been significantly improved recently, but there is still a critical need to meet requirements on resolution, line width roughness, and sensitivity. Creating a defect-free EUVL mask is another obstacle to the application of EUVL. ML2 and NIL, like EUVL, have also undergone significant progress recently, but throughput, defect control, and cost remain the critical impediments for practical application.

Accuracy of biometric formulae for intraocular lens power calculation in a teaching hospital

AIM: To evaluate the accuracy of three commonly used biometric formulae across different axial lengths(ALs) at one United states Veterans Affairs teaching ***: A retrospective chart review was conducted from November 2013 to May 2018. One eye of each patient who underwent cataract surgery with a monofocal intraocular lens(IOL) was included. The range of postoperative follow-up period was from 3 wk to 4 mo. The Holladay 2, Barrett Universal II, and Hill-Radial Basis Function(Hill-RBF) formulae were used to predict the postoperative refraction for all cataract surgeries. For each formula, we calculated the prediction errors [including mean absolute prediction error(MAE)] and the percentage of eyes within ±0.25 diopter(D) and ±0.5 D of predicted refraction. We performed subgroup analyses for short(ALs(AL>25.0 mm).REsULTs: A total of 1131 patients were screened, and 909 met the inclusion criteria. Resident ophthalmologists were the primary surgeons in 710(78.1%) cases. We found no statistically significant difference in predictive accuracy among the three formulae over the entire AL range or in the short, medium, and long eye subgroups. Across the entire AL range, the Hill-RBF formula resulted in the lowest MAE(0.384 D) and the highest percentage of eyes with postoperative refraction within ±0.25 D(42.7%) and ±0.5 D(75.5%) of predicted. All three formulae had the highest MAEs(>0.5 D) and lowest percentage within ±0.5 D of predicted refraction(short ***: In cataract surgery patients at our teaching hospital, three commonly used biometric formulae demonstrate similar refractive accuracy across all ALs. short eyes pose the greatest challenge to predicting postoperative refractive error.
