Lung Cancer in Women

At the same smoking level, women have a higher risk of lung cancer. The aim of the study was to evaluate epidemiological, clinical and histological profile of women＇s lung cancer. A retrospective study including women patients with a confirmed lung cancer followed at the Pneumology Department of University Hospital Center Mohammed VI of Marrakesh in Morocco between January 2010 and December 2014. 14 cases were included in the study. The average age of patients was 54.2 years. Active smoking was noted in 2 patients （14.4%）. The chest X-ray showed an atelectasis in 6 cases （42.8%）, hilar opacity in 6 cases （42.8%） and alveolar opacity in 2 cases （14.4%）, pleural effusion was present in 3 cases （21.4%）. Histological confirmation was obtained in 10 cases （71.4%） by bronchial biopsy, 2 cases with pleural biopsy （14.4%）, one case （7.1%） by lymph node biopsy and one case trans-parietal biopsy （7.1%）. An adenocarcinoma was found in 9 cases （64.3%）, squamous cell carcinoma in 4 cases （28.6%） and small cell carcinoma in one case （7.14%）. The study shows that the lung cancer in women in our context occurs more often in non-smoker patients and it＇s usually an adenocarcinoma type.

The experimental processes are difficult to model by physical laws, because a multitude of factors can intervene simultaneously and are responsible for their instabilities and their random variations. Two types of factors are to be considered;those that are easy to manipulate according to the objectives, and those that can vary randomly (uncontrollable factors). These could eventually divert the system from the desired target. It is, therefore, important to implement a system that is insensitive to fluctuations in factors that are difficult to control. The aim of this study is to optimize the synthesis of an apatitic calcium carbonate phosphate characterized with a Ca/P ratio equal to 1.61 by using the experimental design method based on the Taguchi method. In this process, five factors are considered and must be configured to achieve the previously defined objective. The temperature is a very important factor in the process, but difficult to control experimentally, so considered to be a problem factor (noise factor), forcing us to build a robust system that is insensitive to the last one. Therefore, a much simpler model to study the robustness of a synthetic solution with respect to temperature is developed. We have tried to parameterize all the factors considered in the process within a wide interval of temperature variation (60˚C - 90˚C). Temperature changes are no longer considered as a problem for apatitic calcium carbonate phosphate synthesis. In this finding, the proposed mathematical model is linear and efficient with very satisfactory statistical indicators. In addition, several simple solutions for the synthesis of carbonate phosphate are proposed with a Ca/P ratio equal to 1.61.

Assessment of Fatigue in Undifferentiated Spondyloarthritis (USpA)

Objectives: This study aimed to evaluate the fatigue in patients with undifferentiated spondyloarthritis [USpA], and its relationship with disease-specific variables, spinal mobility measures and healthy related quality of life [HRQOL]. Methods: Eighty patients with USpA and forty healthy subjects were included in this study. The multidimensional assessment of fatigue [MAF] and the generic instrument Short Form 36 [SF 36] were used in patient and control groups to assess fatigue and [HRQOL]. Fatigue was also assessed with the Bath ankylosing spondylitis disease activity index [BASDAI] fatigue item. The evaluation included the activity of the disease [BASDAI], functional status [Bath ankylosing spondylitis functional index], and visual analog scale [VAS] of axial and joint pain. Demographics and disease-related data were obtained. Results: Patients with USpA had higher scores in MAF total and in all MAF subgroup scales than controls. All SF-36 subgroups scores were also found to be significantly lower in patients. Severe fatigue experienced 60% in patients. MAF total scores were found to be significantly correlated with morning stiffness, BASDAI, Bath ankylosing spondylitis functional index (BASFI), BASDAI fatigue, VAS-axial, and SF-36 subgroups scores in patients [p < 0.05]. Conclusions: The patients with USpA defined significantly more fatigue and lower HRQOL when compared with healthy persons. MAF was found to be related with the clinical and functional status and health-related quality of life of patients with USpA.

Extra Renal Rhabdoid Tumor: A Rare Cause of Congenital Soft Tissue Tumor

Rhabdoid tumors (RTs) are a well-defined entity in the kidney or central nervous system of infants or children. However, soft-tissue involvement is uncommon. It’s an exceptional neonatal tumor of soft tissue. The imaging characteristics of this tumor are not specific. Biopsy allows diagnosis;the histomorphological characteristics of rhabdoid tumors, their immunoreactivity to epithelial markers and vimentin, and the INI-1 loss are important tools for diagnosis. RT tumors are aggressive and have a rapidly fatal clinical course in most cases. Despite multidisciplinary therapy, the survival rate is very low. We report a rare case occurring in a male neonate who presents at birth with a voluminous right axillary mass. A CT scan showed a well-limited tumor mass with lobulated contours. An ultrasound-guided biopsy was performed on day 8, showing the morphology and immunoprofile of RT. The mass showed rapid growth. The child was admitted for respiratory distress at 3 weeks. A thoraco-abdominal CT showed an increase in the size of the mass with the appearance of multiple lymph nodes and pleural, hepatic, and renal metastases. The child died two days later.

Osteitis Condensans Ilii:Case Report

Back pain is one of the most frequent complaints during the last period of pregnancy. In a small group of patients the persistent pain during the postpartum period may be secondary to osteitis condensans ilii (OCI), and rarely can occur suddenly in the elderly. OKI is a rare benign pathology, low back or buttock pain should be suspected in patients coming from. Although the exact etiology is unknown, the most accepted hypothesis, get on the sacroiliac joint, depending on the mechanical stress, changes may develop in the iliac bone. In practice, It is important to recognise this disease as it can be potentially confused with similar causes of axial low back pain such as seronegative spondyloarthropathy, metastatic disease, or sacroilitis. Here two woman patients with low back pain and buttock pain secondary to unilateral an uncommon benign condition called osteitis condensans ilii is presented. This report emphasis the salient features and the differential diagnosis in diagnosing and managing this rare pathology.

Rupture of a True Aneurysm of the Posterior Meningeal Artery at Its Anomalous Origin from the Internal Carotid Artery: Case Report

The posterior meningeal artery (PMA) usually originates from the third segment of the vertebral artery. Many variations in its origin and course have been observed;however, as far as we know, the association of true aneurysm of the PMA and its anomalous origin from the internal carotid artery has not been reported previously. We reported the case of a 59-year-old woman who suddenly presented a loss of consciousness without head trauma, computed tomography (CT) revelated intracerebellous hematoma associated with a subarachnoid hemorrhage of the posterior cerebral fossa. Cerebral angiography demonstrated a true aneurysm of the PMA which originated from the internal carotid artery.

Epidemiology and Outcome of Primary Cerebral Lymphoma in Immunocompetent Patients: A Monocentric Study

Primary cerebral lymphoma (PCL) is a form of extranodal non-Hodgkin’s lymphoma with a poor prognosis. Very few cohorts have been reported in the literature. It is a rare form in 1% of extranodal lymphomas, and 3% to 4% of brain tumors. The most common histological type is diffuse large B-cell lymphoma. Survival is improved by combining immunotherapy with chemotherapy. This is a descriptive retrospective study conducted at the Casablanca hematology center over a period of 9 years. The aim of this study was to report our experience by studying the clinical, paraclinical, therapeutic and evolutionary profile of patients with primary cerebral lymphoma. We present a study of 22 patients with PCL. The clinical, radiological and histological findings are shown along with the results of treatment. Patients were aged 25 - 75 years (mean 47 years) with a male predominance (77%). Computed tomography (CT) scans were performed on 16 patients (73%) and Brain magnetic resonance imaging for 16 patients (73%). Typically, lesions were multiple, isodense, and showed uniform enhancement with contrast medium. Immunocytochemical studies demonstrated 21 B-cell and 1 mantle-cell lymphomas. All patients received chemotherapy through high-dose MTX with whole brain radiotherapy (WBRT). After a median follow-up of 19 months, 54% are in complete remission, 32% have died, and 14% are lost to follow-up. Overall survival at 24 months and 36 months were 72% and 52%. The event-free survival at 24 months and 36 months were 60% and 48%. This study was to investigate the clinical features of PCNSL, and evaluate the efficacy of high-dose methotrexate (MTX)-based chemotherapy for immunocompetent Moroccan patients with PCNSL.

Peters-Plus Syndrome: What Outcome in the Absence of Genetic Confirmation? A Case Report

Anterior segment dysgenesis is a group of non-acquired ocular anomalies whose cause is multifactorial;many genes are involved. It is characterized by developmental anomalies of the tissues of the anterior segment, of which Peters-Plus syndrome is included. Our aim is to describe the different ophthalmological and systemic aspects of Peters-Plus syndrome in order to improve the quality of diagnosis of this syndrome even in the absence of genetic confirmation, especially in low-income countries or when genetic studies are not available. In this observation, we report the case of a newborn with Peters-Plus syndrome admitted to the neonatology unit. The diagnosis was made on the basis of clinical-radiological criteria, and treatment consisted of caring for the baby and the parents, given the particular psychological context often associated with the birth of a baby with polymalformative syndrome. From this study, Peters-Plus syndrome should be borne in mind in a fetus with typical ocular anomalies, unusual facial appearance and long tubular bone insufficiency, especially in the presence of a positive family history. In such cases, prenatal diagnosis could be an option for the couples. A genetic study should be undertaken to confirm the clinical diagnosis and provide appropriate genetic counseling and prenatal diagnostic options.

Cutaneous Metastasis of Renal Cell Carcinoma: A Case Report

Renal cell carcinoma (RCC) is known as a highly aggressive malignancy with frequent metastasis that the major sites being lungs, lymphatic ganglions, bones, liver and brain. Cutaneous metastasis is uncommon with only a few reported cases in the literature. We report a case of a women patient aged 49 years old with RCC who underwent nephrectomy and 8 months previously, she consulted the dermatological service due to the development of recent skin lesions that had been diagnosed as cutaneous metastasis of her RCC. The treatment has associated surgical excision and systemic therapy based on sunitinib.
