A man with past lithium use f.r more than 15 years, but of. lithium f.r two years and not carrying the diagnosis of.diabetes mellitus or nephrogenic diabetes insipidus(NDI), presented with coma and hyperglycemic hyperosmolar state(HHS). f.llowing correction of.HHS, he developed persistent hypernatremia accompanied by large volumes of.urine with low osmolality and no response to desmopressin injections. Urine osmolality remained f.er injection of.vasopressin. Improvement in serum sodium concentration f.llowed the intake of.large volumes of.water plus administration of.amiloride and hydrochlorothiazide. Severe hyperglycemia may trigger symptomatic lithium-induced NDI years af.er cessation of.lithium therapy. Patients with newonset diabetes mellitus who had been on prolonged lithium therapy in the past require monitoring of.their serum sodium concentration af.er hyperglycemic episodes regardless of.whether they do or do not carry the diagnosis of.NDI.

AIM To identif. patients with end-stage renal disease treated by peritoneal dialysis(PD) who had zero body f.t(Bf. as determined by analysis of.body composition using anthropometric f.rmulas estimating body water(V) and to compare nutritional parameters between these patients and PD patients whose Bf.was above *** Body weight(W) consists of.f.t-f.ee mass(f.M) *** f.rmulas f.r calculating V allow the calculation of.f.M as V/0.73,where 0.73 is the water f.action of.f.M at normal *** f.om loss of.Bf.has adverse survival outcomes in *** wasting was def.ned as zero Bf.when V/0.73 is equal to or exceeds *** study,which analyzed 439 PD patients at their f.rst clearance study,used the Watson f.rmulas estimating V to identif. patients with V_(Watson)/0.73 ≥ W and compared their nutritional indices with those of.PD patients with V_(Watson)/0.73 f.ed at the f.rst clearance study two male patients with V_(Watson)/0.73 ≥ W among 439 patients on *** to 260 other male patients on PD,the two subjects with advanced wasting had exceptionally low body mass index and serum albumin *** f.rst of.the two subjects also had very low values f.r serum creatinine concentration and total(in urine and spent peritoneal dialysate) creatinine excretion rate while the second subject had an elevated serum creatinine concentration and high creatinine excretion rate due,most probably,to non-compliance with the PD *** Advanced wasting(zero Bf. in PD patients,identif.ed by the anthropometric f.rmulas that estimate V,while rare,is associated with indices of.poor somatic and visceral nutrition.

AIMTo test whether muscle mass evaluated by creatinine excretion （EXCr） is maintained in patients with end-stage kidney disease （ESKD） treated by peritoneal dialysis （PD）, we evaluated repeated measurements of.EXCr in a PD *** hundred and sixty-six PD patients （94 male, 72 f.male） receiving the same PD dose f.r the duration of.the study （up to approximately 2.5 years） had repeated determinations of.total （in urine plus spent dialysate） 24-h EXCr （EXCr T） to assess the adequacy of.PD by creatinine clearance. All 166 patients had two EXCr T determinations, 84 of.the 166 patients had three EXCr T determinations and 44 of.the 166 patients had f.ur EXCr T measurements. EXCr T values were compared using the paired t test in the patients who had two studies and by repeated measures ANOVA in those who were studied three or f.ur *** patients who were studied twice, with the f.rst and second EXCr T measurements perf.rmed at 9.2 ± 15.2 mo and 17.4 ± 15.8 mo af.er onset of.PD, respectively, EXCr T did not dif.er between the f.rst and second study. In patients studied three times and whose f.al assessment occurred 24.7 ± 16.3 mo af.er initiating PD, EXCr T did not dif.er between the f.rst and second study, but was signif.cantly lower in the third study compared to the f.st study. In patients who were studied f.ur times and whose f.urth measurement was taken 31.9 ± 16.8 mo af.er onset of.PD, EXCr T did not dif.er between any of.the studies. The average EXCr T value did not change signif.antly, with the exception of.the third study in the patients studied thrice. However, repeated determinations of.EXCr T in individuals showed substantial variability, with approximately 50% of.the repeated determinations being higher or lower than the f.rst determination by 15% or *** average value of.EXCr T remains relatively constantf.r up to 2.5 years of.f.llow-up in PD patients who adhereto the same PD schedule. Nephropathy secondary to BK virus, a member of.the Papoviridae f.mily of.viruses, has been recognized f.r some time as an important cause of.allograf. dysf.nction in renal transplant recipients. In recent times, BK nephropathy(BKN) of.the native kidneys has being increasingly recognized as a cause of.chronic kidney disease in patients with solid organ transplants, bone marrow transplants and in patients with other clinical entities associated with immunosuppression. In such patients renal dysf.nction is of.en attributed to other f.ctors including nephrotoxicity of.medications used to prevent rejection of.the transplanted organs. Renal biopsy is required f.r the diagnosis of.BKN. Quantitation of.the BK viral load in blood and urine are surrogate diagnostic methods. The treatment of.BKN is based on reduction of.the immunosuppressive medications. Several compounds have shown antiviral activity, but have not consistently shown to have benef.cial ef.ects in BKN. In addition to BKN, BK viral inf.ction can cause severe urinary bladder cystitis, ureteritis and urinary tract obstruction as well as manif.stations in other organ systems including the central nervous system, the respiratory system, the gastrointestinal system and the hematopoietic system. BK viral inf.ction has also been implicated in tumorigenesis. The spectrum of.clinical manif.stations f.om BK inf.ction and inf.ction f.om other members of.the Papoviridae f.mily is widening. Prevention and treatment of.BK inf.ction and inf.ctions f.om other Papovaviruses are subjects of.intense research.

目的:调查通过献血途径感染人类免疫缺陷病毒(HIV)的203例患者的情绪状况及生活质量。方法:采用贝克抑郁问卷、复合性国际诊断用检查提纲和Sf.36对203例通过献血途径感染HIV的患者(HIV阳性组)及年龄、性别和受教育年限相匹配的198例HIV阴性献血者进行调查。结果:①HIV阳性组献血次数多于HIV阴性组(52.2±95.4/13.2±23.9,P<0.001),已婚者少于HIV阴性组(88.6%/ 95.5%,P=0.020),丧偶者多于HIV阴性组(10.9%/4.5%,P=0.020),失业率高于HIV阴性组(21.7%/4.0%,P<0.001),前一年全年工作时间少于HIV阴性组(5.0±3.5/7.5±3.0,P<0.001),家庭月收入低于HIV阴性组(476.3±412.7/709.9±513.7,P<0.001)。②HIV阳性组被诊断为获得性免疫缺陷综合征(AIDS)者占56.6%。HIV阳性组抑郁症的终生患病率为13.8%,HIV阴性组的终生患病率为5.1%。HIV阳性组的抑郁总分(11.3±11.3/6.6±9.6)、躯体(3.3±2.9/2.0±2.6)和非躯体因子分(8.0±8.8/4.6±7.4)均高于HIV阴性组(P<0.001)。HIV阳性组达轻度抑郁以上者占40.4%。HIV阳性组已制定自杀计划者高于HIV阴性组(8.4%/3.0%,P<0.05)。HIV阳性组总体生理健康(48.3±9.7/ 55.5±7.0)和心理健康(48.8±8.9/55.1±7.8)得分均低于HIV阴性组(P<0.001)。③HIV阳性组的贝克抑郁问卷总分与T细胞亚群CD4细胞计数无显著相关,与总体生理健康及心理健康呈显著负相关(r=- 0.46～0.68,P<0.001)。结论:HIV阳性患者生存状况较差。对HIV阳性感染者而言,抑郁给患者生活质量造成全面的影响,抑郁和自杀风险仍然是需要干预和处理的问题。

目的:调查通过献血途径感染人类免疫缺陷病毒患者的应对方式及生活质量。方法:本调查于2006-01/07完成。共选取201例通过献血途径感染人类免疫缺陷病毒者入病例组,其中113例诊断为获得性免疫缺陷综合征。另取200例年龄、性别和受教育年限匹配的人类免疫缺陷病毒阴性献血者作为对照组。调查对象均签署知情同意书。采用Carver应付策略问卷简版和Sf.36生存质量问卷进行调查:①Carver应付策略问卷简版:包括14个分量表,每个分量表包括两个条目,采用4级评分,1代表从来没这么做,4代表常常这样做,每个分量表粗分总分为8。②Sf.36生存质量问卷:包括8个维度,各维度采用转换分数记分,最高分100分,最低分0分。生理健康总分和心理健康总分按标准分计算。分析Carver应对策略问卷的验证性因素及信度,比较两组调查对象的应对策略及生活质量,对人类免疫缺陷病毒感染者的应对方式与生活质量进行相关性分析。结果:发出问卷430份,选出内容详实问卷401份,有效率为93.3%。①Carver应付策略问卷中文版各因子的Cronbach’sα系数在0.55～0.84之间,14个因子模型验证性因素分析各拟合指标分别为χ2/df.0.49,If.=1.05,AGf.=0.93,Cf.=1.00,NNf.=1.07,MI=0.44～0.96,RMSEA=0.00,RMR=0.04。符合Carver量表的理论构想,具有很好的结构效度。②人类免疫缺陷病毒感染者前5位的应对方式是接受、积极应对、计划、使用工具支持和使用情感支持。③与对照组相比,人类免疫缺陷病毒感染者更多的采用否认、行为解脱和自责的应对方式(P<0.05～0.01);且在生理健康和心理健康各个方面均较对照组差(P<0.01)。④人类免疫缺陷病毒感染者的自责和行为解脱与总的生理健康和心理健康成负相关(r=-0.22～-0.44,P<0.01)。积极应对、计划和正性重构等应对方式也和反映个体生理健康的活力呈正相关(r=0.26,0.21和0.27,P<0.05)。总体健康知觉与计划、使用情感支持、使用工具支持、自我分散、宣泄和自责等应对方式均呈负相关(r=-0.14～-0.28,P<0.05～0.01)。结论:①Carver应对策略问卷中文版具有较好的信效度。②国内通过献血途径感染人类免疫缺陷病毒者主要的应对方式较积极,但相对正常对照而言,仍表现出更多的消极应对方式,且生活质量全面下降。③人类免疫缺陷病毒感染者的自责和行为解脱对生活质量存在负性影响;个体的躯体功能状况越好,越能采用问题集中应对的方式;自我健康知觉越差的被试,越有应对的动机。

覆盖作物有助于防治土壤侵蚀 ,阻止土壤养分淋溶 ,固定大气中的氮 ,改善土壤状况和保护幼苗。但是它也会耗用土壤水 ,因此而影响下茬作物的土壤水状况。这种土壤水的耗用 ,在降雨充足的地方 ,也许不会引起下茬作物水短缺 ,但在半干旱地区会导致减产。当覆盖作物是用来增加入渗和减少蒸发、或是排除湿地中过多水分以使下茬作物及时成苗时 ,它的影响是积极的。而当覆盖作物限制了下茬作物土壤水分或恶化了湿地状况时 ,它的影响是消极的。覆盖作物更适合于降雨较可靠的湿润、半湿润地区而非降雨有限的半干旱地区。没有利用覆盖作物的地方 ,利用保土耕作有助于保持土壤水分和弥补许多失去的覆盖作物的作用。
