Socio-cultural determinants of timely and delayed treatment of Buruli ulcer: implications for disease control

Capacities in Facing Natural Hazards: A Small Island Perspective

Isolated communities on small islands are often characterized as vulnerable and marginalized. We studied the recent history of Laingpatehi, a village on Ruang Island off the north coast of Sulawesi, Indonesia to show that the marginalization-vulnerability nexus can be offset by capacity and social cohesion to enable sustainable livelihoods. The island has been impacted by volcanic eruptions,earthquakes, and competition for marine resources from mainland-based fishermen. The community has shown a remarkable ability to cope and prosper in the face of a series of external hazards. We used a sustainable livelihoods approach to identify the assets that enabled the villagers to cope. Strong social cohesion was central to the ability to organize the community and confront hazards. A diversified livelihood strategy drawing on the small island environment and its coastal and marine resources, income generating activities in a distant satellite village, and significant remittances from employment in other parts of Indonesia underpinned people’s capacities to face *** assistance played a supporting role. The case of Laingpatehi demonstrates how remoteness, rather than being a source of vulnerability, can provide access to existing resources and facilitate innovation. Disaster risk reduction strategies should focus more on reinforcing these existing capacities to deal with hazards and less on physical protection and postdisaster responses.

A novel LC–mS/mS assay for methylprednisolone in human plasma and its pharmacokinetic application

methylprednisolone is a synthetic glucocorticoid. In our report, the authors proposed a sensitive and selective liquid chromatography/tandem mass spectrometry(LC–mS/mS) assay for the determination of methylprednisolone applying budesonide as internal standard. Liquid–liquid extraction(LLE) having tert–butyl methyl ether(TBmE) have been employed to extract methylprednisolone from the plasma samples. Immediately after reconstitution, the samples were chromatographed on a C18 column using an isocratic mobile phase composed of 10 m m ammonium formate buffer and acetonitrile(35:65, v/v). A flow rate of 1.00 ml/min was used to elute the analyte form the column. Analysis was carried out with an API–4000 LC–mS/mS instrument operated in multiple reaction-monitoring(mRm) mode. The linearity has been proven within the concentration range of 10.1–804 ng/ml in plasma samples. The precision(%CV) and accuracy results in five validation batches across five concentration levels were well within the acceptance limits. The drug was stable under different conditions. The particular assay has been proficiently put on pharmacokinetic study in healthy male subjects.

Isolation,fractionation and evaluation of the antiplasmodial properties of Phyllanthus niruri resident in its chloroform fraction

Objective:To investigate the antiplasmodial activity of Phyllanthus niniri(***) methanol extract(mE) and its fractions in ***:*** methanol extract and its chloroform. ethanol and aqueous portions were tested against chloroquine-sensitive Plasmodium berghei berghei in early,established and repository models of infection using Knight and Peter’s 4-day suppressive model,Rvlev and Peters curative model and Peters prophylactic model respectively. Results:Chemosuppression of parasitemia(37.65%-50.53%) was elicited by 100-400 mg/kg(b.w.) of *** doses of 100 mg/kg b.w..the chloroform fraction(F1) significantly(Pmia by 85.29%,while ethanol and aqueous fractions(F2 and ***) elicited 67.06%and 51.18%*** most active fraction.F1 was selected for further antiplasmodial *** established *** reduced parasitaemia(15.81%- 62.96%) while F1 significantly(Pmia(44.369—90.48%).with effects comparable to that of chloroquine(96.48%).The prophylactic antiplasmodial activity of mE(92.50%suppression) was also significant(Pmore effective than pyrimethamine(85.00%).additionally, cell membrane integrity of non-parasitized erythrocytes intubated with 125-500 mg/mL F1 was ***:These findings indicate the antiplasmodial efficacy of *** methanol extract,and the localization of this effect in its chloroform fraction.

Hepatoprotective and in vivo antioxidant activities of the hydroethanolic leaf extract of mucuna pruriens (Fabaceae) in antitubercular drugs and alcohol models

AIm: Hepatotoxicity is a significantly increasing health problem worldwide, and the extent of the problem has stimulated interest in the search for hepatotherapeutic agents from plants. This study investigated the hepatoprotective and in vivo antioxidant activities of the hydroethanolic extract of mucuna pruriens leaves in antitubercular and alcohol-induced hepatotoxicity assays in rats. mETHOD: In each of the models used, seven groups were allotted. The different groups received normal saline(10 mL·kg-1, p.o.); hepatotoxicant(isoniazid-rifampicin, INH-RIF, 100 mg·kg-1, i.p. or 20% ethanol 5 g·kg-1, p.o.) and normal saline(10 mL·kg-1, p.o.); hepatotoxicant and extract at doses of 100, 200, and 400 mg·kg-1 p.o.; hepatotoxicant and silymarin 50 mg·kg-1 p.o.; and extract at 400 mg·kg-1 p.o.. On the 21st day of treatment, blood was collected for assessment of serum biochemical parameters and harvested liver samples were assessed for antioxidants. RESULTS: The hepatotoxicants significantly(P minase(ALT), aspartate transaminase(AST), alkaline phosphatase(ALP), bilirubin, and malondialdehyde(mDA); and reduced the levels of catalase(CAT), superoxide dismutase(SOD), glutathione peroxidase(GPx), and reduced glutathione GSH compared to control. m. pruriens significantly reversed(P mg·kg-1) significantly reversed(P minution in the level of in vivo antioxidants and increased the level of mDA produced by INH-RIF. m. pruriens(100-400 mg·kg-1) elicited significant reduction(P mDA compared to the alcohol group. Silymarin also reversed the deleterious effects of the hepatotoxicants. CONCLUSION: The hydroethanolic extract of mucuna pruriens leaves possesses hepatoprotective activity with enhancement of in vivo antioxidants as a possible mechanism of action.

BACKGROUND Waldenstr?m’s macroglobulinemia(Wm) is a rare lymphoid neoplasia, which can have renal complications. These rarely occur, and most common renal manifestations are mild proteinuria and microscopic hematuria. Herein we describe a case of Wm that presented with pseudothrombi depositing in capillaries associated with minimal change nephrotic syndrome and chronic kidney disease(CKD).CASE SUmmARY A 52-year-old man presented with features suggesting nephrotic *** workups were done, and there were elevated serum levels of interleukin-6 and vascular endothelial growth factor(VEGF), capillary pseudothrombus accumulation associated with minimal change nephrotic syndrome, CKD, and Wm. Treatment was directed at the patient’s Wm with bortezomib, thalidomide, and dexamethasone whereby serum immunoglobulin m(Igm) decreased. The damage of Igm on the kidney was corrected; thus, the patient’s proteinuria and serum creatinine had improved. The patient is still under clinical *** It is essential for clinicians to promptly pay more attention to patients presenting with features of nephrotic syndrome and do extensive workups to come up with a proper therapy strategy.

Green Bio-Based CaO from Guinea Fowl Eggshells

Eggshells are among the emerging hazardous waste from the food processing industry. This work sought to valorize waste guinea fowl eggshells. Guinea fowl eggshells (GFEs) were evaluated in the production of CaO for chemical and industrial application. The functionality, thermal stability, elemental composition, phase distribution and surface morphology properties of uncalcined GFEs and GFEs calcined at 700°C, 800°C, 900°C, 1000°C and 1100°C were systematically studied by FTIR, TGA, XRF, XRD and SEm-EDX respectively. The elemental analysis revealed Ca as the main element in the GFEs. The uncalcined GFEs showed intense peaks that corresponded to calcite (CaCO3) phases. These transformed into Ca(OH)2 as the temperature of calcination increased and finally to CaO in the FTIR analysis. In the XRD diffractograms, the main peaks at 2θ values were 29.466° for the uncalcined GFESs and at 37.377° for the sample treated at 1100°C. The phases were confirmed as CaO when compared with JCPDS files. Using the Scherer equation, the CaO crystallite size for the sample calcined at 1100°C was found to be 50.68 nm along the (2 0 0) orientation. All the samples showed multi-step decomposition patterns in the thermogravimetric analyses (TGA), with weight loss of up to 47% for the uncalcined GFEs sample, which was mainly due to the transformation of the calcite (CaCO3) phase to CaO by removal of bound water, organic components, and CO2. Samples calcined at 1100°C showed mainly CaO phases in XRD analyses and fairly stable with 7% loss in weight after treatment at 800°C. SEm images of samples calcined at 900°C were irregular compared to samples treated at 1100°C. EDX data revealed that the surface structure was 100% calcium and oxygen. GFEs are a potential source of pure calcium oxide for various industrial uses.

Background: Cervical Cancer is one such a disease that remains with high mortality unless prevented or detected early and managed. It is one of the most common cancers of the child bearing age between 20 and 45 years world over. It is rated as the second most prevalent cancer among women. There is high consensus among various scholars that high quality screening, effective treatment and routine follow-up care are cardinal in helping women. The main objective of the study was to identify the factors influencing cervical cancer screening in Kitwe District, Zambia. methods: An analytical cross-sectional study design was employed that included the use of a researcher-administered questionnaire. Simple random sampling was used to sample 210 participants. Data were analyzed using SPSS version 25 and chi-square test was used to determine associations among variables. Binary logistic regression was used for multivariate analysis. Statistical significance was set at p Results: The age of the women enrolled in the study ranged from 20 to 59 years, with a mean age of 30.4 years (SD: 8.36). majority (46.2%) of the women were aged between 25 to 34 years and the mean age reported for sexual debut among respondents was 19.7 (SD: 4.01) years ranging from 12 to 31 years at first sexual encounter. Almost all (99%) the women were Christians and 42.9% had attained secondary school education. Fifty nine (59%) of the respondents were married and 77.6% had children. Over three quarters (77.6%) of the respondents were knowledgeable on cervical cancer and screening, majority (61%) of the respondents demonstrated a more positive attitude towards cervical cancer screening, and the overall perception level among study respondents was more positive (71.9%). The findings of the present study indicated that the magnitude of cervical cancer screening was 44.8%.
