Computer-aided sperm analysis： a useful tool to evaluate patient＇s response to varicocelectomy

Preoperative and postoperative sperm parameter values from infertile men with varicocele were analyzed by computer-aided sperm analysis （CAsA） to assess if sperm characteristics improved after varicocelectomy. semen samples of men with proven fertility （n = 38） and men with varicocele-related infertility （n = 61） were also analyzed. Conventional semen analysis was performed according to WHO （2010） criteria and a CAsA system was employed to assess kinetic parameters and sperm concentration. seminal parameters values in the fertile group were very far above from those of the patients, either before or after surgery. No significant improvement in the percentage normal sperm morphology （P = 0. 10）, sperm concentration （P = 0.52）, total sperm count （P = 0.76）, subjective motility （%）（P = 0.97） nor kinematics （P = 0.30） was observed after varicocelectomy when all groups were compared. Neither was significant improvement found in percentage normal sperm morphology （P = 0.91）, sperm concentration （P =0. 10）, total sperm count （P = 0.89） or percentage motility （P = 0.77） after varicocelectomy in paired comparisons of preoperative and postoperative data. Analysis of paired samples revealed that the total sperm count （P = 0.01） and most sperm kinetic parameters： curvilinear velocity （P = 0.002）, straight-line velocity （P = 0.0004）, average path velocity （P = 0.0005）, linearity （P = 0.02）, and wobble （P = 0.006） improved after surgery. CAsA offers the potential for accurate quantitative assessment of each patient＇s response to varicocelectomy.

Measurement of Transcutaneous Bilirubin with Bilicheck as a Jaundice screening Method in Neonates in Pediatric Emergency Departments

Objectives: To study the reliability of a transcutaneous bilirubinometer (Bilicheck) to determine bilirubin levels in neonates consulting for jaundice in a Paediatric Emergency Department (ED), and to evaluate its usefulness as a screening method. Methods: Prospective observational study realized between June of 2005 and December of2005 inneonates consulting at a paediatric emergency department for jaundice, in whom we realized both transcutaneous and total serum bilirubin measurements (TcB and TsB). We collected demographic variables, analytical variables (serum and transcutaneous bilirubin levels), length of stay in the ED, and need for treatment. Results: 66 children were included aged 2 to 31 days (81% of the sample were 2 to 7 days old). There was a close and statistically significant correlation between TcB and TsB (r = 0.81, p s of 0.90, allowing detecting newborns with jaundice susceptible of treatment with TcB levels ≥ 13 mg/dL (sensitivity 92%, specificity 63, 5%, a positive predictive value 39% and a negative predictive value 97%). The number of venous punctures could be reduced in 50%. The medium stay in the ED was of 2 hours when performing serum measurements. Conclusions: A linear correlation exists between TcB-TsB. TcB measurement cannot replace that of TsB, however it could be used as a screening method in an ED to determine which neonates need confirmation by TsB measurement. The use of transcutaneous bilirubinometer would reduce both the number of painful interventions in neonates and the medium length of stay in ED, consequently reducing iatrogenesis.

Assessment of drug-induced hepatotoxicity in clinical practice: A challenge for gastroenterologists

Currently, pharmaceutical preparations are serious contributors to liver disease; hepatotoxicity ranking as the most frequent cause for acute liver failure and post-commercialization regulatory decisions. The diagnosis of hepatotoxicity remains a difficult task because of the lack of reliable markers for use in general clinical practice. To incriminate any given drug in an episode of liver dysfunction is a step-by-step process that requires a high degree of suspicion, compatible chronology, awareness of the drug’s hepatotoxic potential, the exclusion of alternative causes of liver damage and the ability to detect the presence of subtle data that favors a toxic etiology. This process is time-consuming and the final result is frequently inaccurate. Diagnostic algorithms may add consistency to the diagnostic process by translating the suspicion into a quantitative score. such scales are useful since they provide a framework that emphasizes the features that merit attention in cases of suspected hepatic adverse reaction as well. Current efforts in collecting bona fide cases of drug-induced hepatotoxicity will make refinements of existing scales feasible. It is now relatively easy to accommodate relevant data within the scoring system and to delete low-impact items. Efforts should also be directed toward the development of an abridged instrument for use in evaluating suspected drug-induced hepatotoxicity at the very beginning of the diagnosis and treatment process when clinical decisions need to be made. The instrument chosen would enable a confident diagnosis to be made on admission of the patient and treatment to be fine-tuned as further information is collected.

Comparative study of the efficacy and safety of bromfenac, nepafenac and diclofenac sodium for the prevention of cystoid macular edema after phacoemulsification

AIM： To compare the efficacy, tolerability and safety of bromfenac 0.09%, nepafenac 0.1% or diclofenac 0.1% for the prophylaxis of the cystoid macular edema（CME） after phacoemulsification. METHODs： Group sequential observational comparative study. After phacoemulsification, patients received two months for topical treatment of either diclofenac sodium, bromfenac or nepafenac. All patients received concomitant topical tobramycin 0.3% and topical prednisolone 1%. We measured CME using optical coherence tomography（OCT） central foveal thickness, macular thickness and total macular volume. REsULTs： We enrolled 243 patients from January to June 2015, and 35% received diclofenac, 32.9% bromfenac and 32.1% nepafenac. When we compared pre-operative to three weeks to two months, bromfenac was more effective in reducing foveal volume（21.3 and 35.4 mm3, respectively）, compared with the diclofenac（1.3 and 11.5 mm3, respectively）, and the nepafenac group, became more edematous 6.4 and 5.3, respectively. Totally 133 patients completed the post-surgical satisfaction questionnaire. Patients complained of eye stickiness in 13.8% whom we gave nepafenac, versus 10.3% whom we gave diclofenac sodium, and in 0 whom we gave bromfenac. CONCLUsION： Bromfenac is the best tolerated and is more effective than diclofenac and nepafenac in reducing CME after phacoemulsification.

The Conditional Heterocedasticity on the Argentine Inflation. An Analysis for the Period from 1943 to 2013

Numerous economic time series do not have a constant mean and in practical situations, we often see that the variance of observational error is subject to a substantial variability over time. This phenomenon is known as volatility. To take into account the presence of volatility in an economic series, it is necessary to resort to models known as conditional heteroscedastic models. In these models, the variance of a series at a given time point depends on past information and other data available up to that time point, so that a conditional variance must be defined, which is not constant and does not coincides with the overall variance of the observed series. There is a very large variety of nonlinear models in the literature, which are useful for the analysis of any economic time series with volatility, but we will focus in analyzing our series of interest using ARCH type models introduced by Engle （1982） and their extensions. These models are non-linear in terms of variance. Our objective will be the study of the monthly inflation data of Argentina for the period from January 1943 to December 2013. The data is officially published by the National Institute of statistics and Censuses （or INDEC as it is known in Argentina）. Although it is a very long period in which various changes and interventions took place, it can be seen that certain general patterns of behavior have persisted over time, which allows us to admit that the study can be appropriately based on available information.

New Green synthesis and Antineoplastic Activity of Bis (3-Arylimidazolidinyl-1) Methanes

A new green synthesis and anti-tumor activity of the series of bis (3-arylimidazolidinyl-1) methanes 1 - 6 are described. The compounds were synthesized from the corresponding N-arylethylenediamine and trioxane as sources of formaldehyde and the reactions were performed in heterogeneous phase catalyzed by an acidic ion-exchange resin (Amberlyst 15). The compounds were tested with the sulforhodamine B assay according to the protocol of the National Cancer Institute for several cell lines. The results were expressed as percentage inhibition of growth cell in comparison with the full growth of the cells without treatment. Cytotoxicity on normal cells using the Annexing-PI staining and flow cytometry has been evaluated. The parent compound, bis(3-phenylimidazolidinyl-1)methane 1 and the monohalogenated derivatives 4-chlorophenyl 3 and 3-bromophenyl 5 showed antineoplastic activity, 60%, 82% and 89% inhibition growth cell respectively on the human colon cell line (HCT116). The 4-tolyl derivative 6 presented inhibitory activity (73% inhibition of growth cell) on human lung adenocarcinoma cell line (A549) and 62% on human mammary cell line MCF-7.

We present an interplay of high-resolution scanning tunneling microscopy imaging and the corresponding theoretical calculations based on elastic scattering quantum chemistry techniques of the adsorption of a gold-functionalized rosette assembly and its building blocks on a Au(111)surface with the goal of exploring how to fabricate functional 3-D molecular nanostructures on *** supramolecular rosette assembly stabilized by multiple hydrogen bonds has been sublimed onto the Au(111)surface under ultra-high vacuum conditions;the resulting surface nanostructures are distinctly different from those formed by the individual molecular building blocks of the rosette assembly,suggesting that the assembly itself can be transferred intact to the surface by in situ thermal *** unanticipated result will open up new perspectives for growth of complex 3-D supramolecular nanostructures at the vacuum-solid interface.
