Measurement of transcutaneous Bilirubin with Bilicheck as a Jaundice Screening Method in Neonates in Pediatric Emergency Departments

Objectives: to study the reliability of a transcutaneous bilirubinometer (Bilicheck) to determine bilirubin levels in neonates consulting for jaundice in a Paediatric Emergency Department (ED), and to evaluate its usefulness as a screening method. Methods: Prospective observational study realized between June of 2005 and December of2005 inneonates consulting at a paediatric emergency department for jaundice, in whom we realized both transcutaneous and total serum bilirubin measurements (tcB and tSB). We collected demographic variables, analytical variables (serum and transcutaneous bilirubin levels), length of stay in the ED, and need for treatment. Results: 66 children were included aged 2 to 31 days (81% of the sample were 2 to 7 days old). there was a close and statistically significant correlation between tcB and tSB (r = 0.81, p the area under the ROC curve was of 0.90, allowing detecting newborns with jaundice susceptible of treatment with tcB levels ≥ 13 mg/dL (sensitivity 92%, specificity 63, 5%, a positive predictive value 39% and a negative predictive value 97%). the number of venous punctures could be reduced in 50%. the medium stay in the ED was of 2 hours when performing serum measurements. Conclusions: A linear correlation exists between tcB-tSB. tcB measurement cannot replace that of tsB, however it could be used as a screening method in an ED to determine which neonates need confirmation by tsB measurement. the use of transcutaneous bilirubinometer would reduce both the number of painful interventions in neonates and the medium length of stay in ED, consequently reducing iatrogenesis.

Comparative study of the efficacy and safety of bromfenac, nepafenac and diclofenac sodium for the prevention of cystoid macular edema after phacoemulsification

AIM： to compare the efficacy, tolerability and safety of bromfenac 0.09%, nepafenac 0.1% or diclofenac 0.1% for the prophylaxis of the cystoid macular edema（CME） after phacoemulsification. MEtHODS： Group sequential observational comparative study. After phacoemulsification, patients received two months for topical treatment of either diclofenac sodium, bromfenac or nepafenac. All patients received concomitant topical tobramycin 0.3% and topical prednisolone 1%. We measured CME using optical coherence tomography（OCt） central foveal thickness, macular thickness and total macular volume. RESULtS： We enrolled 243 patients from January to June 2015, and 35% received diclofenac, 32.9% bromfenac and 32.1% nepafenac. When we compared pre-operative to three weeks to two months, bromfenac was more effective in reducing foveal volume（21.3 and 35.4 mm3, respectively）, compared with the diclofenac（1.3 and 11.5 mm3, respectively）, and the nepafenac group, became more edematous 6.4 and 5.3, respectively. totally 133 patients completed the post-surgical satisfaction questionnaire. Patients complained of eye stickiness in 13.8% whom we gave nepafenac, versus 10.3% whom we gave diclofenac sodium, and in 0 whom we gave bromfenac. CONCLUSION： Bromfenac is the best tolerated and is more effective than diclofenac and nepafenac in reducing CME after phacoemulsification.

Assessment of drug-induced hepatotoxicity in clinical practice: A challenge for gastroenterologists

Currently, pharmaceutical preparations are serious contributors to liver disease; hepatotoxicity ranking as the most frequent cause for acute liver failure and post-commercialization regulatory decisions. the diagnosis of hepatotoxicity remains a difficult task because of the lack of reliable markers for use in general clinical practice. to incriminate any given drug in an episode of liver dysfunction is a step-by-step process that requires a high degree of suspicion, compatible chronology, awareness of the drug’s hepatotoxic potential, the exclusion of alternative causes of liver damage and the ability to detect the presence of subtle data that favors a toxic etiology. this process is time-consuming and the final result is frequently inaccurate. Diagnostic algorithms may add consistency to the diagnostic process by translating the suspicion into a quantitative score. Such scales are useful since they provide a framework that emphasizes the features that merit attention in cases of suspected hepatic adverse reaction as well. Current efforts in collecting bona fide cases of drug-induced hepatotoxicity will make refinements of existing scales feasible. It is now relatively easy to accommodate relevant data within the scoring system and to delete low-impact items. Efforts should also be directed toward the development of an abridged instrument for use in evaluating suspected drug-induced hepatotoxicity at the very beginning of the diagnosis and treatment process when clinical decisions need to be made. the instrument chosen would enable a confident diagnosis to be made on admission of the patient and treatment to be fine-tuned as further information is collected.

Numerous economic time series do not have a constant mean and in practical situations, we often see that the variance of observational error is subject to a substantial variability over time. this phenomenon is known as volatility. to take into account the presence of volatility in an economic series, it is necessary to resort to models known as conditional heteroscedastic models. In these models, the variance of a series at a given time point depends on past information and other data available up to that time point, so that a conditional variance must be defined, which is not constant and does not coincides with the overall variance of the observed series. there is a very large variety of nonlinear models in the literature, which are useful for the analysis of any economic time series with volatility, but we will focus in analyzing our series of interest using ARCH type models introduced by Engle （1982） and their extensions. these models are non-linear in terms of variance. Our objective will be the study of the monthly inflation data of Argentina for the period from January 1943 to December 2013. the data is officially published by the National Institute of Statistics and Censuses （or INDEC as it is known in Argentina）. Although it is a very long period in which various changes and interventions took place, it can be seen that certain general patterns of behavior have persisted over time, which allows us to admit that the study can be appropriately based on available information.

A Multiparameter Colloidal titrations for the Determination of Cationic Polyelectrolytes

In water treatment processes and conditioning drinking water, PEs are widely used;however, their environmental impact is still doubtful, since residual concentrations increase organic ma.ter content and represents a potential health hazard. this paper demonstrates a multiparametric study of two colloidal titration methods: spectrophotometric and zeta potential end point detection. the first one was optimized to guarantee the accuracy of cationic commercial PE quantification. It includes the indicator dose optimization using analytical criteria for competing equilibria, a calibration curve for two ranges of CPE concentration (1 - 5 ppm and 5 - 100 ppm) and the interference study of flocculant and Sn in the CPE quantification. the second method provides a physicochemical validation of the electric surface phenomena occurring during the colloidal titration and the end point detection. As an additional contribution the zeta potential titration was discussed and proposed as an alternative method for quantifying CPE when the sample is metal free.

Objectives to evaluate whether there is an association between the serum levels of the novel insulin-like adipokine isthmin-1(ISM1)and obesity-related phenotypes in a population of Spanish children and to investigate the plausible molecular alterations behind the alteration of the serum levels of this protein in children with *** the study population is a sub-cohort of the PUBMEP research project,consisting of a cross-sectional population of 119 pubertal children with overweight(17 boys,19 girls),obesity(20 boys,25 girls),and normal weight(17 boys,21 girls).All subjects were classified into experimental groups according to their sex,obesity,and insulin resistance(IR)*** were counted anthropometry,glucose and lipid metabolism,inflammation and cardiovascular biomarkers as well as isthmin-1(ISM1)serum *** population was intended as a discovery population to elucidate the relationship between obesity and ISM1 levels in ***,the study population had blood whole-genome DNA methylation examined,allowing deepening into the obesity–ISM1 molecular *** Higher serum ISM1 levels were observed in boys with obesity than in normal weight(P=0.004)and overweight(P=0.007)***1 serum levels were positively associated with body mass index(BMI)Z-score(P=0.005)and fat mass(P=0.058)and negatively associated with myeloperoxidase(MPO)(P=0.043)in *** we did not find associations between ISM1 serum levels and metabolic outcomes in girls,which may indicate a putative sexual dimorphism,fat mass was positively associated in all children,including boys and girls(P=0.011).DNA methylation levels in two-enhancer-related CpG sites of ISM1(cg03304641 and cg14269097)were associated with serum levels of ISM1 in *** ISM1 is associated with obesity in boys at the pubertal stage,elucidating how this protein might be of special relevance as a new biomarker of obesity in *** studies including a longitudina

巴西、俄罗斯、印度、中国和南非等新兴经济体的快速发展,既改变了国际秩序,也改变了国际发展格局。从历史上看,西方的援助活动一直由经合组织-发展援助委员会（OECD-DAC）主导,并且这种援助带有特定的意图和方向。由于中国等国在南南合作中一直发挥着重要作用,西方援助国和西方援助机构对如何提供发展合作产生了争论。显然,西方的对外援助概念和中国的对外援助概念在不同领域存在着差异。因此,本文希望根据自己国家的发展经验,研究作为21世纪国际发展合作的新塑造者中国的对外援助,尤其是其对拉丁美洲援助的重大贡献。本研究旨在更好地实现和平发展,构建人类命运共同体。

Overcoming the barrier of nanoparticle production by femtosecond laser ablation in liquids using simultaneous spatial and temporal focusing

there exists an increasing demand of industrial-scale production of high-purity ligand-free nanoparticles due to the continuous development of biomedicine, catalysis, and energy applications. In this contribution, a simultaneous spatial and temporal focusing(SStF) setup is first proposed for increasing nanoparticle productivity of the eco-friendly pulsed laser ablation in liquids(PLAL) technique. In spite of the fact that femtosecond pulses have proved to achieve higher ablation rates in air than picosecond pulses, in PLAL this is reversed due to the nonlinear energy losses in the liquid. However, thanks to the incorporation of SStF, the energy delivered to the target is increased up to 70%, which leads to a nanoparticle production increase of a 2.4 factor. this breaks a barrier toward the employment of femtosecond lasers in high-efficiency PLAL.
