Macular integrity assessment to determine the association between macular microstructure and functional parameters in diabetic macular edema

AIM： To respectively evaluate macular morphological features and functional parameters by using spectraldomain optical coherence tomography（SD-OCT） and macular integrity assessment（MAIA） in patients with diabetic macular edema（DME）. METHODS： This prospective, non-controlled, open study included 61 eyes of 38 consecutive patients with DME. All patients underwent best-corrected visual acuity（BCVA） measurement, MAIA microperimetry, and SDOCT. DME morphology, including central retinal thickness（CRT） and central retinal volume（CRV）; integrity of the external limiting membrane（ELM） and photoreceptor inner segment/outer segment（IS/OS） junction; and the deposition of hard macular exudates were assessed within a 1000-μm central subfield area. MAIA microperimetry parameters evaluated were average threshold（AT）-retinal sensitivity, macular integrity index（MI）, fixation points within a circle of radius 1°（P1） and 2°（P2）, and bivariate contour ellipse area considering 63% and 95% of the fixation points（A63 and A95, respectively）. RESULTS： MI was significantly higher in eyes with disrupted ELM or IS/OS, compared with eyes with intact ELM and IS/OS. Values of BCVA（log MAR）, total AT, AT within 1000-μm diameter, P2, A63, A95, and CRT were significantly worse in eyes with disrupted IS/OS, compared with eyes with intact IS/OS. The values of BCVA（log MAR）, AT within 1000-μm diameter, and CRT were significantly worse in eyes with disrupted ELM, compared with eyes with intact ELM. These parameters were not significantly different between eyes with or without hard macular exudate deposition. CRV was not significantly different in the presence or absence of the integrity of ELM, IS/OS, or deposition of hard macular exudates. At the center, nasal and temporal sectors of the fovea, significant negative correlations were observed between retinal thickness and AT of the corresponding area. To the Editor： Pulmonary Langerhans cell histiocytosis （PLCH） in adults is a rare disease occurring almost exclusively in smokers. The characteristic high-resolution computed tomography （HRCT） manifestation of PLCH is a combination of cysts （or cavities） and nodules mainly in the upper lung zone. However, not all HRCT patterns of PLCH are typical. Few treatments are effective in current practice regarding PLCH. Targeted therapy with an inhibitor of mutated BRAF （vemurafenib） has been proved effective in Langerhans cell histiocytosis （LCH） harboring BRAF valine at position 600 （V600E） ***2K1 mutations are mutually exclusive with BRAF mutations and might have implications for the use of BRAF targeted therapy. Here, we reported a case of PLCH proven by lung biopsy.

CpG island methylator phenotype in plasma is associated with hepatocellular carcinoma prognosis

AIM： To evaluate the clinical significance of CpG island methylator phenotype （CIMP） in plasma and its association with hepatocellular carcinoma （HCC） progress. METHODS： CIMP status of 108 HCC patients was analyzed using a methylation marker panel in tumor tissues and plasma with methylation-specific polymerase chain reaction. Fifteen samples of non-neoplastic liver tissues and 60 of plasma from healthy persons were examined simultaneously. Examined genes included APC, WIF-1, RUNX-3, DI C-1, SFRP-1, DKK and E-cad.26/108, 24.07% in plasma; WIF-1, 53/108, 49.07% in tissue and 35/108, 32.41% in plasma; RUNX-3, 52/108, 48.14% in tissue and 42/108, 38.89% in plasma; DIC-1, 38/108, 35.18% in tissue and 23/108, 21.30% in plasma; SFRP-1, 40/108, 37.04% in tissue and 31/108, 28.7% in plasma; DKK, 39/108, 36.1% in tis- sue and 25/108, 23.14% in plasma; and E-cad, 37/108, 34.3% in tissue and 18/108, 16.67% in plasma. CIMP＋（≥3 methylated genes） was detected in 68 （60.2%） tumor tissue samples and 62 （57.4%） plasma samples. CIMP was not detected in non-neoplastic liver tissues or plasma of healthy persons. CIMP status in tumor tissues differed significantly in gender, hepatitis B surface antigen, alpha-fetoprotein, and tumor-node- metastasis stage （P 〈 0.05）. Similar results were obtained with plasma samples （P 〈 0.05）. There was no difference in CIMP status in age, presence of hepatitis C virus antibody, cirrhosis, number of nodes, number of tumors, tumor size, or Edmondson-Steiner stage. A one-year follow-up found that the metastatic rate and recurrence rate in the CIMP＋ group were significantly higher than in the CIMP- group as assessed with plasma samples （P 〈 0.05）. CONCLUSION： Plasma DNA can be a reliable sample source for CIMP analysis. CIMP in plasma may serve as a molecular marker of late-stage and poor-prognosis HCC.

Correlation of Epworth Sleepiness Scale with multiple sleep latency test and its diagnostic accuracy in assessing excessive daytime sleepiness in patients with obstructive sleep apnea hypopnea syndrome

Background Excessive daytime sleepiness （EDS） is often associated with obstructive sleep apnea hypopnea syndrome （OSAHS） and contributes to a number of comorbidities in these patients. Therefore, early detection of EDS is critical in disease management. We examined the association between Epworth Sleepiness Scale （ESS） and multiple sleep latency test （MSLT） and diagnostic accuracy of ESS in assessing EDS in OSAHS patients. Methods The ESS, MSLT and overnight polysomnography were administered to 107 Chinese patients to assess EDS and its correlations with polysomnographic parameters. The diagnostic accuracy of ESS in classifying EDS （mean sleep latency （MSL）〈10 minutes） was evaluated by calculating the area under ROC curve. Results As the severity of OSAHS increased, MSL decreased with increase in ESS score. Conversely, patients with worsening EDS （shorter MSL） were characterized by advanced nocturnal hypoxaemia and sleep disruption compared to those with normal MSL, suggesting EDS is associated with more severe OSAHS. There was a negative correlation between ESS score and MSL and both moderately correlated with some polysomnographic nocturnal hypoxaemic parameters. The area under ROC curve of ESS for identifying EDS was 0.80 （95% CI： 0.71 to 0.88） and ESS score 〉12 provided the best predictive value with a sensitivity of 80% and specificity of 69%. Conclusion The ESS score moderately correlates with MSL and our ROC study supports ESS as a screening strategy for assessing EDS in OSAHS.

Background：Myocarditis is an uncommon but serious manifestation of systemic lupus erythematosus （SLE）.This study aimed to investigate clinical characteristics and outcomes of lupus myocarditis （LM） and to determine risk factors of LM in hospitalized Chinese patients with ***：We conducted a retrospective case-control study.A total of 25 patients with LM from 2001 to 2012 were enrolled as the study group,and 1 O0 patients with SLE but without LM were randomly pooled as the control *** analysis was performed using Chi-square tests for categorical variables,and the Student＇s t-test or Mann-Whitney U-test was performed for continuous variables according to the ***：LM presented as the initial manifestation of SLE in 7 patients （28％） and occurred mostly at earlier stages compared to the controls （20.88 ± 35.73 vs.44.08 ± 61.56 months,P =0.008）.Twenty-one patients （84％） experienced episodes of symptomatic heart *** showed that 23 patients （92％） had decreased left ventricular ejection fraction （＜50％） and all patients had wall motion abnormalities.A high SLE Disease Activity Index was the independent risk factor in the development of LM （odds ratio =1.322,P ＜ 0.001）.With aggressive immunosuppressive therapies,most patients achieved satisfactory *** in-hospital mortality was not significantly higher in the LM group than in the controls （4％ vs.2％,P =0.491）.Conclusions：LM could result in cardiac dysfunction and even sudden *** SLE disease activity might potentially predict the occurrence of LM at the early stage of *** echocardiographic findings could confirm the diagnosis of *** aggressive immunosuppressive therapy could improve the cardiac outcome of LM.

LncRNA CDC6通过抑制c-Myc表达调节肺癌细胞增殖、凋亡和上皮间质转化

目的探讨LncRNA CDC6(CDC6)对肺癌细胞增殖、上皮间质转化(epithelial-mesenchymal transition,EMT)和凋亡的影响及其作用机制。方法RT-qPCR检测肺癌组织和细胞中CDC6和c-Myc的mRNA水平,并分析CDC6和c-Myc mRNA水平与患者临床指标的相关性。随后,通过CDC6-siRNA和pcDNA-CDC6转染,或CDC6-siRNA和pcDNA-c-Myc共转染肺癌细胞;MTT法检测细胞增殖能力;流式细胞术检测细胞凋亡;Western Blotting检测增殖、凋亡及EMT相关蛋白的表达。结果LncRNA CDC6在肺癌组织和细胞系中高表达(P<0.05)。CDC6和c-Myc mRNA水平与肺癌的病理分型、TNM分期、分化程度以及淋巴结的转移密切相关。沉默CDC6抑制肺癌细胞增殖,促进细胞凋亡,降低增殖相关蛋白以及EMT相关蛋白表达水平,促进促凋亡蛋白Bax表达,抑制抗凋亡蛋白Bcl-2表达(P<0.05)。过表达CDC6促进肺癌细胞增殖,抑制细胞凋亡,上调增殖相关蛋白以及EMT相关蛋白表达,减少促凋亡蛋白Bax水平,增加抗凋亡蛋白Bcl-2水平(P<0.05)。过表达c-Myc逆转了沉默CDC6对A549细胞增殖、凋亡以及EMT相关蛋白表达的影响。结论LncRNA CDC6通过调控c-Myc促进EMT过程及肺癌细胞的增殖能力,抑制细胞凋亡。

A general method is developed to prepare durable hybrid nanocatalysts by nanostructuring the surface of gold wires via simple alloying and dealloying. The resulting nanoporous gold/Au （NPG/Au） wire catalysts possess nanoporous skins with their thicknesses on robust metal wires specified in a highly controllable manner. As a demonstration, the as-obtained NPG/Au was shown to be a highly active, chemo-selective, and recyclable catalyst for the reduction of nitro com- pounds and azides using organosilanes as reducing agents.
