Laparoscopic partial nephrectomy(LPN)as a minimally invasive nephron-sparing surgery is gradually becoming the preferred surgical treatment for T1a renal carcinoma since it yields faster post-operative recovery and equivalent oncological outcomes to radical nephrectomy.[1]However,it is difficult to perform LPN for complex renal tumors,resulting in a longer warm ischemic time(WIT),which consequently affects the recovery of post-operative renal function.[2]With the development of laparoscopic instruments and techniques and the accumulating experience of surgeons,the WIT during LPN may be shortened using anatomy-based early unclamping(EUC)[1]after the placement of one or two running sutures on the tumor *** this study,we improved the closure techniques for LPN using EUC in patients with complex renal tumors and assessed their effectiveness.

目的探讨外生型肝癌(PHCC)的临床病理特点、外科治疗方式、手术远期预后及其影响因素。方法采用回顾性队列研究的方法,收集东方肝胆外科医院肝外五科于2011年1月至2017年12月期间行根治性肝癌切除术的65例PHCC患者的临床资料及随访数据,按照肿瘤所在位置分为两类:膈面生长型PHCC组(27例)和脏面生长型PHCC组(38例)。对两组患者的基线特征、肿瘤病理特征、围手术期结果和远期预后进行对比研究。结果与膈面生长型PHCC组相比,脏面生长型PHCC组具有更大的肿瘤直径(9.8 vs6.2 cm,P=0.048),更可能合并破裂出血(31.6%vs 11.1%, P=0.046)和伴有微血管侵犯(71.1%vs 44.4%,P=0.031)。两组患者均未出现围手术期死亡,膈面和脏面生长型PHCC组术后并发症发生率分别为25.9%和31.6%,不存在统计学差异(P> 0.05)。在远期预后方面,该65例患者的1、3、5年总生存率(OS)和无复发生存率(RFS)分别为82.6%、51.5%、41.6%和78.3%、60.3%、35.1%。膈面生长型PHCC组的1、3、5年OS和RFS分别为88.1%、64.0%、47.4%和85.0%、55.0%、45.9%,显著高于脏面生长型PHCC组的78.7%、42.7%、37.4%和73.4%、33.8%、30.1%,两组之间的差异具有统计学意义(P=0.031,P=0.042)。Cox多因素回归分析证实,合并肝硬化、Child-Pugh B级、脏面生长型PHCC、合并破裂出血、合并大血管侵犯是影响PHCC患者OS的独立危险因素;此外,脏面生长型PHCC、合并破裂出血、合并微血管侵犯、合并大血管侵犯是影响PHCC患者RFS的独立危险因素。结论 PHCC是一类特殊类型的HCC,肿瘤体积较大,手术切除是主要根治性手段,术后5年生存率可达到40%以上;膈面生长型PHCC的远期预后优于脏面生长型PHCC,肿瘤生长部位是影响PHCC患者OS和RFS的独立危险因素。

Chronic hepatitis C virus(HCV) infection can result in steatosis,a condition displaying aberrant accumulation of neutral lipid vesicles,the component of lipid droplets(LDs),which are essential for HCV ***,the interplays between HCV infection and steatosis remain ***,we show that HCV infected cells having higher levels of CD2 associated protein(CD2 AP),which plays two distinct,yet tightly-linked roles in HCV pathogenesis: elevated CD2 AP binding the domain III of NS5 A via its SH3 *** interaction transports NS5 A to LDs via an actin dependent manner to facilitate viral assembly;up-regulated CD2 AP interacting with E3 ligases cbl/cbl-c to degrade insulin receptor substrate 1(IRS1),which in turn disrupts insulin signaling and increases LDs accumulation thru the IRS1-Akt-AMPK-hormone sensitive lipase(HSL) signaling axis to accommodate viral *** HCV infected mouse model and human subjects,CD2 AP expression is up-regulated and the upregulation correlates well with liver *** importantly,hepatocellular carcinoma showed significantly stronger CD2 AP staining than peritumor ***: CD2 AP was identified as a protein upregulated by HCV infection,which in turn stimulated HCV assembly and steatosis by disrupting insulin signaling,targeting CD2 AP may offer an opportunity for alleviating HCV infection and its associated liver pathology.

In normal physiological anatomy, common bile duct opens in the major papilla and does not communicate with the minor papilla directly. There is little chance to calmulate biliary system via the minor papilla. There was no report of tmns-minor papilla biliary drainage in the literature. We presented a case with malignant obstructive jaundice that underwent trans-minor papilla biliary drainage. A 5F super slim nasobiliary tube was inserted into common bile duct successfully through minor papilla during endoscopic retrograde cholangio-pancreatography （ERCP）. The patient did not complain abdominal pain or any discomfort after procedure. Biliary drainage was effective （500-800 ml per day）. The patient＇s jaundice relieved significantly during follow-up

Objective:Percutaneous radiofrequency ablation(PRFA) is known to be as effective as hepatectomy for small hepatocellular carcinoma(HCC) in the *** wished to ascertain how it is for recurrent small ***:From January 2009 to November 2011,a series of sixty-one patients were included in the study according to the criteria:each patient had one recurrent HCC,less than 5 cm in ***-six of the 61 patients were treated with PRFA and the other 35 were treated with repeat ***:The interval from first surgery to recurrent for repeat hepatectomy and PRFA groups were(1,239.60±1,017.00) d and(903.42±975.11) d respectively(P=0.066).The tumor-free time after repeat hepatectomy and PRFA were(310.23±159.50) d and(278.27±123.29) d respectively(P=0.584).Size of tumor in repeat hepatectomy and PRFA were(7.34±3.16) cm^2 and(5.59±3.40) cm^2(P=0.215),the total expenditure for each patient of the two groups were(26,150.66±7,923.60) yuan and(21,135.00±1,156.76) yuan(RMB),and the time of hospitalization for each of the two groups were(15.29±4.28) d and(7.46±2.20) d(P<0.001).Conclusion:PRFA is proved to be as effective as repeat hepatectomy in the treatment of recurrent small HCC,and superior to repeat hepatectomy as it is less invasive.

Objective: To review our experience in and the re-sults of resecting liver tumors involving the hepatoca-val confluence under intermittent portal triad clam-ping (PTC).Methods: Sixty-eight consecutive patients with livertumors involving the hepatocaval confluence under-went hepatectomies with liver parenchymal transec-tions under intermittent ***: All the tumors were successfully resected un-der PTC, except for one in which the infrahepatic in-ferior vena cava was concomitantly occluded in addi-tion to PTC. There was neither operative death noruncontrollable massive bleeding or air embolism oc-curred in our patients. The bleedings from the mainand short hepatic veins and right adrenal veins wereproperly managed during the operation, with a meanintraoperative blood loss of 1400 ml. Of the 68tumors resected, 65 were hepatocellular carcinomas(HCC). Their 1-, 2-, 3- and 4-year suvival rateswere 64.11%, 52. 82%, 44.90% and 36.98%, re-spectively, and the patients with HCC with capsulessurvived significantly longer than those with HCCwithout ***: The liver tumors involving the hepato-caval confluence could be safely resected simply un-der PTC, without routine use of total hepatic vascu-lar exclusion. As for HCCs in this area, the tumorwith capsule is a better indicator for surgical resec-tion than that without capsule.
