minimally invasive spleen-preserving distal pancreatectomy: Does splenic vessel preservation have better postoperative outcomes? A systematic review and meta-analysis

BACKGROUND： minimally invasive spleen-preserving distal pancreatectomy （SPDP） can be performed with either splenic vessel preservation （SVP） or resection [Warshaw procedure （WP）]. The aim of this study was to evaluate the postoperative clinical outcomes of patients undergoing both methods. DATA SOURCES： Database search of Pubmed, Embase, Scopus, Cochrane, and Google Scholar was performed （2000-2014）; key bibliographies were reviewed. Qualified studies compar- ing patients undergoing SPDP with either SVP or WP, and as- sessing postoperative complications were included. Calculated pooled risk ratio （RR） with the corresponding 95% confidence interval （CI） by random effects methods were used in the meta-analyses. RESULTS： The search yielded 215 studies, of which only 14 observational studies met our selection criteria. The studies included 943 patients in total; 652 （69%） underwent SVP and 291 （31%） underwent WP. Overall, there was a lower incidence of splenic infarction （RR=0.17; 95% Ch 0.09-0.33; P〈0.001）, gastric varices （RR=0.16; 95% Ch 0.05-0.51; P=0.002）, and intra/postoperative splenectomy （RR=0.20; 95% Ch 0.08-0.49; P〈0.001） in the SVP group. There was no difference in in- cidence of pancreatic fistula （WP vs SVP, 23.6% vs 22.9%;P=0.37）, length of hospital stay, operative time or blood loss. There was moderate cross-study heterogeneity. CONCLUSIONS： SVP is a safe, efficient and feasible technique that may be used to preserve the spleen. WP may be more suit- able for large tumors dose to the splenic hilum or those associ- ated with splenomegaly. Randomized clinical trials are justified to examine the long-term benefits of SVP-SPDP.

Astyanax mexicanus has repeatedly colonized cave environments,displaying evolutionary parallelisms in many troglobitic traits.Despite being a model system for the study of adaptation to life in perpetual darkness,the parasites that infect cavefish are practically unknown.In this study,we investigated the macroparasite communities in 18 cavefish populations from independent lineages and compared them with the parasite diversity found in their sister surface fish populations,with the aim of better understanding the role that parasites play in the colonization of new environments.Within the cavefish populations,we identified 13 parasite taxa,including a subset of 10 of the 27 parasite taxa known for the surface populations.Parasites infecting the cavefish belong to five taxonomic groups,including trematodes,monogeneans,nematodes,copepods,and acari.monogeneans are the most dominant group,found in 14 caves.The macroparasites include species with direct life cycles and trophic transmission,including invasive species.Surprisingly,paired comparisons indicate higher parasite richness in the cavefish than in the surface fish.Spatial variation in parasite composition across the caves suggests historical and geographical contingencies in the host-parasite colonization process and potential evolution of local adaptations.This base-line data on parasite diversity in cavefish populations of A.mexicanus provides a foundation to explore the role of divergent parasite infections under contrasting ecological pressures(cave vs.surface environments)in the evolution of cave adaptive traits.

BACKGROUND Bowel preparation in children can be challenging.AIm To describe the efficacy, safety, and tolerability of sodium picosulfate, magnesium oxide, and citric acid(SPmC) bowel preparation in children.mETHODS Phase 3, randomized, assessor-blinded, multicenter study of low-volume, divided dose SPmC enrolled children 9-16 years undergoing elective colonoscopy. Participants 9-12 years were randomized 1:1:1 to SPmC ? dose × 2, SPmC 1 dose × 2, or polyethylene glycol(PEG). Participants 13-16 years were randomized 1:1 to SPmC 1 dose × 2 or PEG. PEG-based bowel preparations were administered per local protocol. Primary efficacy endpoint for quality of bowel preparation was responders(rating of ‘excellent' or ‘good') by modified Aronchick Scale. Secondary efficacy endpoint was participant's tolerability and satisfaction from a 7-item questionnaire. Safety assessments included adverse events(AEs) and laboratory evaluations.RESULTS 78 participants were randomized, 48 were 9-12 years, 30 were 13-16 years. For the primary efficacy endpoint in 9-12 years, 50.0%, 87.5%, and 81.3% were responders for SPmC ? dose × 2, SPmC 1 dose × 2, and PEG groups, respectively. Responder rates for 13-16 years were 81.3% for SPmC 1 dose × 2 and 85.7% for PEG. Overall, 43.8% of participants receiving SPmC 1 dose × 2 reported it was ‘very easy' or ‘easy' to drink, compared with 20.0% receiving PEG. Treatment-emergent AEs were reported by 45.5% of participants receiving SPmC 1 dose × 2 and 63.0% receiving PEG.CONCLUSION SPmC was an efficacious and safe for bowel preparation in children 9-16 years, with comparable efficacy to PEG. Tolerability for SPmC was higher compared to PEG.

Fatty liver in hepatitis C patients post-sustained virological response with direct-acting antivirals

AIm To determine steatosis and fibrosis prevalence in hepatitis C patients after a sustained virological response achieved with direct-acting antivirals.mETHODS Transient elastography with controlled attenuation parameter(CAP) was used to assess hepatic steatosis post-sustained virological response(SVR);the CAP technology was not available in the United States at study initiation.Liver stiffness/fibrosis was measured before and 47 wk after treatment completion.Patients with genotype 3 and patients with cirrhosis were excluded.RESULTS One hundred and one patients were included in the study.Post-SVR there were decreases from baseline in alanine aminotransferase(ALT)(63.1 to 17.8 U/L),aspartate aminotransferase(51.8 to 21.5 U/L) and fibrosis score(7.4 to 6.1 k Pa)(P mass index(29.0 vs 26.1 kg/m2),glucose(107.8 vs 96.6 mg/d L),ALT(20.4 vs 15.3 mg/d L),CAP score(296.3 vs 212.4 d B/m) and fibrosis score(7.0 vs 5.3 k Pa);P mpared to baseline,both patients with and without steatosis had change in fibrosis score post-SVR(7.7 k Pa vs 7.0 k Pa and 7.0 k Pa vs 5.3 k Pa);alternatively,(P mmon in hepatitis C virus(HCV) patients post-SVR.These patients continue to have elevated mean fibrosis score(≥ 7 k Pa) compared to those without fatty liver;some have advanced fibrosis.Long term follow up is needed to assess steatosis and fibrosis in HCV patients post-SVR.

A new physical simulation tool to predict the interface of dissimilar aluminum to steel welds performed by friction melt bonding

Optimization of the intermetallic layer thickness and the suppression of interfacial defects are key elements to improve the load bearing capacity of dissimilar joints. However, till date we do not have a systematic tool to investigate the dissimilar joints and the intermetallic properties produced by a welding condition. Friction melt Bonding (FmB) is a recently developed technique for joining dissimilar metals that also does not exempt to these challenges. The FmB of DP980 and A16061-T6 was investigated using a new physical simulation tool, based on Gleeble thermo-mechanical simulator, to understand the effect of individual parameter on the intermetallic formation. The proposed method demonstrates its capability in reproducing the intermetallic characteristics, including the thickness of intermetallic bonding layer, the morphology and texture of its constituents (Fe2AU and Fe4Ali3), as well as their nanohardness and reduced modulus. The advantages of physical simulation tool can enable novel developing routes for the development of dissimilar metal joining processes and facilitate to reach the requiring load bearing capacity of the joints.

Role of Fungal Biomass in N-Hexane Biofiltration

The biofiltration of n-hexane is studied to optimize determinants factors of hydrophobic VOC filtration efficiency. Four trickle-bed air biofilters (TBABs) were employed;two of which were supplied with nutrients buffered at a neutral pH, while another two at an acidic pH of 4 to induce and enhance fungal growth. The loading rate of n-hexane was kept constant in all TBABs at 13 g/m3/h. At each pH levels studied, the biomass of the TBABs was pre-acclimated using different ratios of n-hexane and methanol. The fungal biomass responsible for the degradation of n-hexane was then examined and quantified. Dichloran Rose Bengal Chloramphenicol agar was used for fungi quantification, and optical microscopy for classification. Effluent biomass was validated by measuring volatile suspended solids. Fungal counts resulting from n-hexane biodegradation were related to nitrate and carbon consumption. It was found that n-hexane elimination capacity closely followed biomass growth, and reached a steady-state at an optimum biomass density of roughly 3000 cfu/ml. major shifts in fungal species were observed in all TBABs. Dominant fungal species grew slowly to become the most numerous, and were found to provide maximum elimination capacity, although TBABs pre-acclimated to higher methanol concentrations took less time to reach this steady-state. It was concluded, therefore, that steady and monitored growth of TBAB biomass is an essential factor in maximizing fungi’s ability to metabolize VOCs and that a new ecological biofiltration model may be the most effective at VOC purification.

Development and assessment of a multifunctional chitosan-based coating applied on AZ31 magnesium alloy: Corrosion resistance and antibacterial performance against Klebsiella Pneumoniae

This work presents a simple method to functionalise the surface of AZ31 magnesium alloy by applying a duplex mgF_(2)/chitosan coating,which improves its corrosion resistance and provides it with some antibacterial performance.First,the effect of three chitosan solutions with different concentrations on the growth of the bacteria Klebsiella pneumoniae in nutritive medium(TSB)was evaluated by absorbance kinetics experiments,where the chitosan solution at 2%(m/V)was selected for the coating preparation.Before coating application,the AZ31 substrate was pretreated with hydrofluoric acid for 48 hours in order to form a mgF_(2)conversion layer.Subsequently,the coating was applied to the pretreated substrate through the dry-casting method.Samples of the alloy in each surface condition(bare,pretreated,and pretreated+coated with chitosan)were exposed to simulated body fluid(SBF)for 21 days at 37°C,with the solution renewed every 24 hours and the wastes stored.The surfaces were characterised by SEm-EDS,and XPS after the immersion tests,whereas the stored solutions were employed to measure the change in the mg-ions concentration.Electrochemical impedance spectroscopy and potentiodynamic polarisation were performed in each surface condition to compare their corrosion resistance in SBF.The antibacterial activity of the functionalised surfaces was evaluated by the plate counting method and compared with bare samples.All results were correlated and demonstrate that the modified surface of AZ31 achieved a higher corrosion resistance when it was exposed to SBF,as well as a reduction of the bacterial growth during in vitro tests.

Perceptions of Climate Change in Puerto Rico before and after Hurricane maria

This article analyzes the perceptions of Puerto Rican citizens of global climate change (GCC) before and after an extreme weather event, specifically Hurricane maria (Hm). The purpose of the current article is to evaluate Puerto Ricans’ perceptions of the impact of extreme meteorological phenomena and of GCC before and after Hm. This tropical cyclone entered the island as a category IV hurricane on September 17, 2017, causing enormous destruction, loss of life, and economic damage. In this study, two data samples were collected before and after Hm struck Puerto Rico (PR) (the second sample was collected approximately six months after the hurricane). Surveyed citizens with general knowledge of GCC increased from 43% to 62%, which the researchers consider a small increase, considering the severe destruction caused by Hm. This study also found that Puerto Ricans trust non-profit institutions and the scientific community more than state authorities. Furthermore, 85% of citizens believe that public policies on GCC should be directed by the state (federal, state, and municipal governments);this did not change after Hm. In addition, this study found that the poor response of the federal and state governments to the destruction caused by Hm increased citizens’ trust in the scientific community.

Effect of Lactose on Acid Tolerance of Yogurt Culture Bacteria

Lactose, the sugar naturally present in milk, provides energy to lactic acid bacteria used in fermented dairy foods. Increasing concentrations of lactose may improve survivability of lactic acid bacteria in the dairy foods and in human gut enhancing their probiotic benefits. Acid tolerance is an important probiotic characteristic. The objective was to determine the influence of lactose on acid tolerance of yogurt starter culture Streptococcus thermophiles ST-M5 and Lactobacillus bulgaricus LB-12. The M 17 broth was used for Streptococcus thermophiles ST-M5 and MRS broth was used for Lactobacillus bulgaricus LB-12. Lactose was added to both broths at 0% (control), 1%, 3%, and 5% (wt/vol). Both broths were acidified to pH 2.0. Upon sterilizing and tempering, both broths were inoculated. Acid tolerance was mily:Verdana;">determined as viable counts in acidified broths after 120 minutes of incubations. In an incubation period of 2 hours, dilutions were plated every 30 minutes.
