Cutaneous leishmaniasis control in Alta Verapaz (northern Guatemala): evaluating current efforts through stakeholders' experiences

Background:Cutaneous leishmaniasis(Cl),endemic in Guatemala,mostly affects poor people living in the northern region.A national control program that ineludes surveillanee,diagnose,and treatment offered free of cost by the Ministry of Health(MoH)has been in place since 2003.However,the incidence is increasing and treatment rates are not optimal,suggesting that current efforts are not being effective.This study aimed to understand barriers and facilitators of Cl control in Guatemala as experienced and perceived by key stakeholders in order to comprehend what works well and does not and suggest evidence-informed interventions.Methods:The study was conducted in the Coban municipality,the most endemic of Guatemala,situated in the Department of Alta Verapaz.Data were collected during May and June 2019 via focus groups and semi-structured interviews with key stakeholders,including local and national health personnel and residents of four communities of the endemic region.Thematic and content analysis of the collected data was conducted using NVIVO.Results:Three overarching issues hamper the effectiveness of current Cl efforts:resource scarcity,treatment challenges,and knowledge-action gaps.Scarce economic resources from the MoH and community residents negatively impact incidence,detection of cases and treatment rates in that preventive action is insufficient and healthcare access is low.In addition,local health workers often lack specialized Cl training and access to the national Cl control guidelines.With regards to the population living in the study area,misunderstanding of disease causation,shame associated with Cl lesions,treatment pain fear,and long(often uncertain)waiting times for diagnose and treatment negatively affect people's willingness to seek help,treatment adherence,and their trust on the healthcare provided.Conclusions:Culturally sensitive Cl preventive action must be developed.Given the scarce economic resources available for Cl control in the country,the involvement of trained community health workers

Facile synthesis of composite polyferric magnesium-silicate-sulfate coagulant with enhanced performance in water and wastewater

The coagulation process is a widely applied technology in water and wastewater treatment.Novel composite polyferric mag-nesium-silicate-sulfate(PFMS)coagulants were synthesized using Na_(2)SiO_(3)·9H_(2)O,Fe_(2)(SO_(4))_(3),and MgSO_(4) as raw materials in this paper.The effects of aging time,Fe:Si:Mg,and OH:M molar ratios(M represents the metal ions)on the coagulation performance of the as-pre-pared PFMS were systematically investigated to obtain optimum coagulants.The results showed that PFMS coagulant exhibited good co-agulation properties in the treatment of simulated humic acid-kaolin surface water and reactive dye wastewater.When the molar ratio was controlled at Fe:Si:Mg=2:2:1 and OH:M=0.32,the obtained PFMS presented excellent stability and a high coagulation efficiency.The removal efficiency of ultraviolet UV254 was 99.81%,and the residual turbidity of the surface water reached 0.56 NTU at a dosage of 30 mg·l^(-1).After standing the coagulant for 120 d in the laboratory,the removal efficiency of UV254 and residual turbidity of the surface wa-ter were 88.12%and 0.68 NTU,respectively,which accord with the surface water treatment requirements.In addition,the coagulation performance in the treatment of reactive dye wastewater was greatly improved by combining the advantages of magnesium and iron salts.Compared with polyferric silicate-sulfate(PFS)and polymagnesium silicate-sulfate(PMS),the PFMS coagulant played a better decolor-ization role within the pH range of 7-13.

A Weakly-Supervised Crowd Density Estimation Method Based on Two-Stage linear Feature Calibration

In a crowd density estimation dataset,the annotation of crowd locations is an extremely laborious task,and they are not taken into the evaluation metrics.In this paper,we aim to reduce the annotation cost of crowd datasets,and propose a crowd density estimation method based on weakly-supervised learning,in the absence of crowd position supervision information,which directly reduces the number of crowds by using the number of pedestrians in the image as the supervised information.For this purpose,we design a new training method,which exploits the correlation between global and local image features by incremental learning to train the network.Specifically,we design a parent-child network(PC-Net)focusing on the global and local image respectively,and propose a linear feature calibration structure to train the PC-Net simultaneously,and the child network learns feature transfer factors and feature bias weights,and uses the transfer factors and bias weights to linearly feature calibrate the features extracted from the Parent network,to improve the convergence of the network by using local features hidden in the crowd images.In addition,we use the pyramid vision transformer as the backbone of the PC-Net to extract crowd features at different levels,and design a global-local feature loss function(l2).We combine it with a crowd counting loss(lC)to enhance the sensitivity of the network to crowd features during the training process,which effectively improves the accuracy of crowd density estimation.The experimental results show that the PC-Net significantly reduces the gap between fullysupervised and weakly-supervised crowd density estimation,and outperforms the comparison methods on five datasets of Shanghai Tech Part A,ShanghaiTech Part B,UCF_CC_50,UCF_QNRF and JHU-CROWD++.

Isorhamnetin Downregulates MMP2 and MMP9 to Inhibit Development of Rheumatoid Arthritis through SRC/ERK/CREB Pathway

Objective To investigate the effect of isorhamnetin on the pathology of rheumatoid arthritis(RA).Methods Tumor necrosis factor(TNF)-α-induced fibroblast-like synoviocytes(FlS)was exposed to additional isorhamnetin(10,20 and 40µmol/l).Overexpression vectors for matrix metalloproteinase-2(MMP2)or MMP9 or SRC were transfected to explore their roles in isorhamnetin-mediated RA-FlS function.RA-FlS viability,migration,and invasion were evaluated.Moreover,a collagen-induced arthritis(CIA)rat model was established.Rats were randomly divided to sham,CIA,low-,medium-,and high-dosage groups using a random number table(n=5 in each group)and administed with normal saline or additional isorhamnetin[2,10,and 20 mg/(kg·day)]for 4 weeks,respectively.Arthritis index was calculated and synovial tissue inflammation was determined in CIA rats.The levels of MMP2,MMP9,TNF-α,interleukin-6(Il-6),and Il-1β,as well as the phosphorylation levels of SRC,extracellular regulated kinase(ERK),and cyclic adenosine monophosphate response element-binding(CREB),were detected in RA-FlS and synovial tissue.Molecular docking was also used to analyze the binding of isorhamnetin to SRC.Results In in vitro studies,isorhamnetin inhibited RA-FlS viability,migration and invasion(Plated the levels of MMP2,MMP9,TNF-α,Il-6,and Il-1βin RA-FlS(PlS migration and invasion,as well as the levels of TNF-α,Il-6,and Il-1β(Plation of SRC,ERK,and CREB(PlS viability,migration and invasion,as well as the negative regulation of MMP2 and MMP9(Plleviated synovial inflammation.Isorhamnetin reduced the levels of MMP2,MMP9,TNF-α,Il-6,and Il-1β,as well as the phosphorylation of SRC,ERK,and CREB in synovial tissue(Ply,the inhibit

Ensuring a sufficient oxygen supply is pivotal for the success of bioprinting applications since it fosters tissue integration and natural regeneration.Variation in oxygen concentration among diverse tissues necessitates the precise recreation of tissue-specific oxygen levels in imprinted constructs to support the survival of targeted cells.Although oxygen-releasing biomaterials,such as oxygen-generating microparticles(OMPs),have shown promise for enhancing the oxygen supply of microenvironments in injured tissues,whether this approach is scalable for large tissues and whether tissue-specific bioinks with varying OMP concentrations remain printable remain unknown.This study addresses this critical gap by introducing an innovative class of engineered oxygenated bioinks that combine colloidal-based microgels with OMPs.We report that incorporating nanosized calcium peroxide(nCaO_(2))and manganese oxide nanosheets(nMnO_(2))into hydrophobic polymeric microparticles enables precise modulation of oxygen release while controlling hydrogen peroxide release.Moreover,the fabrication of oxygenating and cytocompatible colloidal gels is achieved using an aqueous two-phase system.This study thoroughly evaluates the fundamental characteristics of the resulting bioink,including its rheological behaviors,printability,shape fidelity,mechanical properties,and oxygen release properties.Moreover,this study demonstrates the macroscopic scalability and cytocompatibility of printed constructs produced via cell-laden oxygenating colloidal bioinks.By showcasing the effectiveness of extrusion-based bioprinting,this study underscores how it can be used to fabricate biomimetic tissues,indicating its potential for new applications.The findings presented here advance the bioprinting field by achieving scalability with both high cell viability and the possibility of mimicking specifically oxygenated tissues.This work thereby offers a promising avenue for the development of functional tissues with enhanced physiological relevance.

Role of adaptation measures in addressing heatwave exposure in China

Heatwave exposure has increased dramatically because of climate warming and population growth,along with their interactive effects.However,effective adaptation measures can reduce these impacts.Nonetheless,the dynamic changes,regional inequality in adaptive capacity and their potential contributions to reducing exposure in the future remain unclear.This study quantifies the impact of adaptive capacity and underscores regional variations in heatwave magnitudes,population exposure and adaptation levels in China.We projected the future adaptive capacity using air-conditioner penetration,factoring in climate cooling requirements and individuals'purchasing power.Utilising population and gross domestic product(GDP)data from four Shared Socioeconomic Pathways(SSP1,SSP2,SSP3 and SSP5)and daily temperature data from four SSP-based emission scenarios(SSP1-2.6,SSP2-4.5,SSP3-7.0 and SSP5-8.5),we estimated heatwave duration,population exposure and avoided impacts through adaptation across China and its sub-regions.Results show a substantial increase in heatwave duration in Southwest and Southern China,especially under the SSP5-8.5 scenario,with a projection of 163.2±36.7 d during 2081-2100.Under the SSP3|SSP3-7.0 scenario,total exposure reaches 156.4±76.8 billion person d per year,which is the highest among all scenarios and 23 times greater than that in 1986-2005 without adaptation.Upon considering adaptation measures,a noteworthy reduction in population exposure is observed,especially in the SSP3|SSP3-7.0 and SSP5JSSP5-8.5 scenarios,with reductions of(62.6±3.9)%and(65.8±5.1)%,respectively,compared with the scenario without adaptation during 2081-2100.Remarkable regional disparities in avoided impacts are also evident,with variations of up to 50%across different regions.The implementation of effective and environmentally friendly adaptation measures can notably address climate change,thereby alleviating the profound threats posed to human well-being.

Integrated separation-electrochemical detection device based on wood column for online identification of enantiomer

Chiral chemicals have attracted significant interest in the pharmaceutical industry,yet the separation methods to get pure enantiomers from racemic mixture are still challenging.To date,the separation of enantiomers still mainly depends on chromatography using high-cost chiral stationary phases.Herein,wood channels were used as the handheld integrated device,and enantiomer separation was simultaneously detected using an electrochemical detector.In this method,a chiral UIO-66(l-UIO-66)modified enantiomer separation zone and carbonized wood based online detection zone are integrated along a single wood column.Based on the in situ separation results from the chronoamperometry data,the wood device shows excellent separation ability for a wide range of electrochemically active enantiomers,including 3,4-dihydroxyphenylalanine,amino acids,ascorbic acid,carnitine,and penicillamine with high chirality purity.The unbiased molecular dynamic simulations indicate that the excellent chiral recognition and separation are attributed to the different barriers from the bound states to the dissociated state of the enantiomers in the homochiral microenvironment of the framework.This integrated enantiomer separation-electrochemical detection device provides a novel,easy,and low-cost platform for the separation of pure enantiomer from racemic mixture.

Interactive effect of thermal aging and proton irradiation on microstructural evolution and hardening ofδ-ferrite in 308l stainless steel weld metal

In the harsh service environment of high temperature and intense neutron irradiation in water-cooled nuclear reactors,the austenitic stainless steel weld overlay cladding on the inner surface of the reactor pressure vessel suffers from thermal aging and irradiation damage simultaneously,which can induce microstructural evolution and hardening of the material.Since it is quite difficult to achieve this simul-taneous process out of the pile,two kinds of combined experiments,i.e.,post-irradiation thermal aging and post-aging irradiation were performed on 308 l stainless steel weld metals in this work.The interactive effect of thermal aging and proton irradiation on microstructural evolution and hardening ofδ-ferrite in 308 l weld metal was investigated by combining atom probe tomography,transmission elec-tron microscopy and nanoindentation tests.The results revealed that thermal aging could eliminate the dislocation loops induced by irradiation and affect the phase transition process by accelerating spinodal decomposition and G-phase precipitation,thus enhancing hardening of irradiatedδ-ferrite.For the effect of irradiation on the microstructure and hardening of thermally agedδ-ferrite,however,intensive collision cascades can intensify G-phase precipitation and dislocation loop formation but decrease spinodal decomposition,leading to a limited effect on hardening of thermally agedδ-ferrite.Furthermore,the interaction of thermal aging and irradiation can promote G-phase precipitation.Meanwhile,the interaction can causeδ-ferrite hardening,which is mainly influenced by spinodal decomposition,followed by G-phase and dislocation loops,where spinodal decomposition and G-phase cause hardening by inducing strain fields.

lipid metabolism disorders commonly occur during menopause.Estrogen deficiency has been shown to lead to excessive energy intake and abnormal lipid metabolism in ovariectomized rats,resulting in obesity.Probiotics exhibit anti-obesity properties,and their underlying mechanism has been widely reported.In this study,we demonstrated the metabolic benefits of lactobacillus plantarum CCFM1180 in suppressing appetite,controlling body weight,correcting obesity-induced abnormalities,enhancing liver lipid metabolism,and protecting liver function in estrogen-deficient rats.The mechanisms associated with the anti-obesity and anti-dyslipidemia effects of CCFM1180 on estrogen-deficient rats were clarified.The results showed that CCFM1180 dramatically reduced food intake by activating the expression of estrogen receptor alpha(ERα)and increasing the level of leptin in abdominal adipose tissue.These changes,combined with the increased butyrate concentration and recovered bile acid structure,helped enhance lipid metabolism.Additionally,CCFM1180 treatment was found to be safer than exogenous estrogen supplementation.Thus,l.plantarum CCFM1180 could be considered a new therapeutic strategy for preventing and alleviating menopausal lipid abnormalities.

Huangqin Decoction Delays Progress of Colitis-Associated Carcinogenesis by Regulating Nrf2/HO-1 Antioxidant Signal Pathwayin Mice

Objective:To investigate the effect of Huangqin Decoction(HQD)on nuclear factor erythroid 2 related-factor 2(Nrf2)/heme oxygenase(HO-1)signaling pathway by inducing the colitis-associated carcinogenesis(CAC)model mice with azoxymethane(AOM)/dextran sodium sulfate(DSS).Methods:The chemical components of HQD were analyzed by liquid chromatography-quadrupole-time-of-flight mass spectrometry(lC-Q-TOF-MS/MS)to determine the molecular constituents of HQD.Totally 48 C57Bl/6J mice were randomly divided into 6 groups by a random number table,including control,model(AOM/DSS),mesalazine(MS),low-,medium-,and high-dose HQD(HQD-l,HQD-M,and HQD-H)groups,8 mice in each group.Except for the control group,the mice in the other groups were intraperitoneally injected with AOM(10 mg/kg)and administrated with 2.5%DSS orally for 1 week every two weeks(totally 3 rounds of DSS)to construct a colitis-associated carcinogenesis mouse model.The mice in the HQD-l,HQD-M and HQD-H groups were given HQD by gavage at doses of 2.925,5.85,and 11.7 g/kg,respectively;the mice in the MS group was given a suspension of MS at a dose of 0.043 g/kg(totally 11 weeks).The serum levels of malondialdehyde(MDA)and superoxide dismutase(SOD)were measured by enzyme-linked immunosorbent assay.The mRNA and protein expression levels of Nrf2,HO-1,and inhibitory KElCH like ECH-related protein 1(Keap1)in colon tissue were detected by quantitative real-time PCR,immunohistochemistry,and Westem blot,respectively.Results:lC-Q-TOF-MS/MS analysis revealed that the chemical constituents of HQD include baicalin,paeoniflorin,and glycyrrhizic acid.Compared to the control group,significantly higher MDA levels and lower SOD levels were observed in the model group(Ply decreased,and the expression of Keap1 increased(Pl group,serum MDA level was decreased and SOD level was increased in the HQD-M,HQD-H and MS groups(P<0.05).Higher expressions of Nrf2 and HO-1 were observed in the HQD groups.Co
