HLA class Ⅱ alleles and risk for peripheral neuropathy in type 2 diabetes patients

the potential impact of human leukocyte antigen （HLA） genotype variations on development of diabetic peripheral neuropathy （DPN） is not well determined. this study aimed to identify the association of HLA class II alleles with DPN in type 2 diabetes （t2D） patients. totally 106 t2D patients, 49 with DPN and 57 without DPN, and 100 ethnic-matched healthy controls were analyzed. Both groups of the patients were matched based on sex, age, body mass index （BMI） and duration of t2D. Polyneuropathy was diagnosed using electrodiagnostic methods. HLA-DRB1 and DQB1 genotyping was performed in all subjects by the polymerase chain reaction with sequence-specific primers （PCR-SSP） method. t2D patients with DPN showed higher frequencies of HLA-DRB1＊10 and DRB1＊12 alleles compared to control group （P = 0.04）. HLA-DQB1＊02 allele and HLA-DRB1＊07-DQB1＊02 haplotype were associated with a decreased risk for developing DPN in t2D patients （P = 0.02 and P = 0.05 respectively）. Also, patients with severe neuropathy showed higher frequencies of DRB1＊07 （P = 0.003） and DQB1＊02 （P = 0.02） alleles than those with mild-to-moderate form of neuropathy. the distribution of DRB 1 and DQB 1 alleles and haplotypes were not statistically different between all patients and healthy controls. Our findings implicate a possible protective role of HLA-DQB1＊02 allele and HLA-DRB1＊07-DQB1＊02 haplotype against development of peripheral neuropathy in t2D patients. therefore, variations in HLA genotypes might be used as genetic markers for prediction and potentially management of neuropathy in t2D patients.

the Varzaghan district at the northwestern margin of the Urumieh–Dokhtar magmatic arc, is considered a promising area for the exploration of porphyry Cu deposits in Iran. In this study we identified mono-and multi-element geochemical anomalies associated with Cu–Au–Mo–Bi mineralization in the central parts of the Varzaghan district by applying the concentration–area fractal method. After mono-element geochemical investigations, principal component analysis was applied to ten selected elements in order to acquire a multi-element geochemical signature based on the mineralization-related component. Quantitative comparisons of the obtained fractal-based populations were carried out in accordance with known Cu occurrences using Student's t-values. then,significant mono-and multi-element geochemical layers were separately combined with related geologic and structural layers to generate prospectivity models, using the fuzzy GAMMA approach. For quantitative evaluation of the effectiveness of different geochemical signatures in final prospectivity models, a prediction-area plot was adapted. the results show that the multi-element geochemical signature of principal component one(PC1) is more effective than mono-element layers in delimiting exploration targets related to porphyry Cu deposits.

AIM:to compare outcomes of applying preservative free artificial tears(PFAt)with and without hyaluronic acid(HA)in early postoperative course following photorefractive keratectomy(PRK).MEtHODS:In this triple-blinded randomized clinical trial,PRK procedure was performed on both eyes of 230 *** PRK,patients were divided into three groups:the HA^(+)group,44 patients PFAt containing HA;the HA-group,71 patients PFAt without HA were administered 5 times per day(every 4 h);the third group,115 patients received no PFAt before lens *** the 1st and 4th postoperative day,Visual Analogue Score(VAS)was utilized to evaluate patient’s level of *** were asked to complete a questionnaire about the severity of eye discomfort ranked from 0 to 10(0=no complaint;10=most severe complaint experienced).RESULtS:In eyes receiving PFAt with or without HA(Drop group),mean scores for epiphora,foreign body sensation,and blurred vision on the 1st postoperative day were statistically lower(Ptous keratitis(FK)was detected in 11(4.7%)eyes,and recurrent corneal erosion(RCE)was observed in 5(2.1%)*** the control group,FK was noted in 16(6.9%)eyes while 13(5.6%)eyes had RCE and 5(2.1%)eyes had corneal *** rate of complications was statistically lower in Drop group(P=0.009).However,the aforementioned scores were not statically different between HA^(+)and HA-group one and two(P=0.29).CONCLUSION:Following PRK,applying PFAt with and without HA yields faster visual recovery,decreases postoperative ocular discomfor t and haze formation;however there is no additive effect for HA.

Large Eddy Simulations are carried out to analyze flow past flat plate in different configurations and inclinations.A thin flat plate is considered at three inclination angles(α=30°,60°and 90°)and three aspect ratios(AR=0.5,2 and 5).the Reynolds number based on the free stream velocity and chord length of the plate at different inclination angles varies between 75,000 to 150,*** increase in the inclination angle while the aspect ratio(span to chord)is constant results in higher drag and lower lift on the *** the aspect ratio at a constant inclination angle increases the mean aerodynamic loading except for theα=30°and AR=0.5 case where the mean forces are larger than the other aspect ratios for this specific inclination *** small aspect ratio suppresses and blocks the separation of the flow from the top and bottom edges causing larger aerodynamic forces relative to AR=2,*** of the flow structures shows the tip vortices have a significant role in controlling the shedding vortices from the top and bottom ***α=30°and AR=0.5,the two tip vortcies control and suppress the flow separation from the top and bottom edges.A stable wake was found for this case with no *** the aspect ratio increases,the influence of the tip vortices on flow separation from the top and bottom edges *** a result,larger fluctuations were found for cases with higher aspect ratios.

Objective： the choice between total gastrectomy （tG） and subtotal gastrectomy （StG） for early adenocarcinoma of the lower third of the stomach is still a matter of debate and controversy amongst surgeons. the aim of this study was to compare the surgical outcomes, quality of life （QoL）, and local recurrence rates as well as other results of curative resection of early distal adenocarcinoma of the stomach using tG or StG performed in Iran. Methods： Hospital records of 66 patients with early distal adenocarcinoma of stomach that underwent even total or subtotal gastrectomy with perigastric （D2） lymphadenectomy between October 2001 and February 2006 in taleghani Hospital, Iran were reviewed retrospectively. Demographic data as well as clinicopathological factors including number of involved lymph nodes, tumor grading, depth of invation, tumor size, and tumor type were recorded. Post-operative outcomes including mortality and morbidity, tumor recurrence and quality of life were assessed. Univariate analyses using Fisher＇s exact test, the Student t-test, and the Pearson χ^2 test were used. Results： Local recurrence of tumor was found in 8 （23%） tG group patients and in 19 （61%） patients of StG group, the Pearson χ^2 test showed a significant higher recurrence rate in StG （P value=0.002, Relative Risk=2.68, Confidence Interval： 1.37-5.24）.the mean time interval between gastrectomy and tumor recurrence was not different between tG and StG. （19.75±5.1 vs. 18.0±7.8 respectively; P value=0.507） tumor size 〉5 cm （P value=0.004）, diffuse type （P value=0.034）, poor differentiation （P value=0.000） and serosal invasion （P value=0.012） were found to be significantly related to tumor recurrence in patients undergone gastrectomy. In none of the QoL measures a significant difference was found between tG and StG Conclusion： Our results show that subtotal and total gastrectomy methods with perigastric lymphadenectomy have a similar postoperative complication rate and surgical outcomes a
