Evaluation of febrile neutropenic patients hospitalized in a hematology clinic

Objective:To evaluate the febrile neutropenic patients with hematological malignancies hospitalized in hematology clinic with poor hygiene ***:A total of 124 patients with hematological malignancies(69 male,55 female)hospitalized in hematology clinic with poor hygiene conditions depending on hospital conditions,between January 2007 and December 2010,were evaluated,***:In this study,250 febrile neutropenia episodes developing in 124 hospitalized patients were *** the patients,69 were men(56%)and 55 women(44%).A total of 40 patients(32%)had acute myeloid leukemia,25(20%)acute lymphoblastic leukemia,19(15%)non-Hodgkin's lymphoma,10(8%)multiple myeloma,and 8(8%)chronic myeloid *** our study,56 patients(22%)were diagnosed as pneumonia,38(15%)invasive aspergillosis,38(15%)sepsis,16(6%)typhlitis,9(4%)mucormycosis,and 4(2%)urinary tract ***-positive cocci were isolated from 52%(n=20),while Gram-negative bacilli 42%(n=16)and yeasts from 6%(n=2)of the sepsis patients,*** most frequently isolated Gram-positive bacteria were methicillin-resistant coagulase-negative staphylococci(n=18),while the most frequently isolated Gram-negative bacteria was Escherichia coli(n=10).Conclusions:Febrile neutropenia is still a problem in patients with hematological *** documentation of the flora and detection of causative agents of infections in each unit would help to decide appropriate empirical *** control procedures should be applied for preventing infections and transmissions.

Molecular epidemiology of HIV-1 strains in the south-east and east of Turkey

Objective:To detect the subtype characterization and drug-resistant mutations in HIV-1 strains after the refugee movement from Syria to Turkey between 2011 and 2014 in south east border lines. Methods: A total of 65 patients were included in this study, of which 57(88%) patients were antiretroviral therapy-naive patients. HIV-1 RNA was detected and quantii ed by realtime PCR assay. HIV-1 subtypes and circulating recombinant forms(CRFs) were identii ed by phylogenetic analysis(neighbor-joining method), and drug-resistant mutations were ***: Three major HIV groups were indicated. Two of these groups were located in subtype B. The other group showed heterogeneity. Subtype B(48/65, 73.8%), followed by CRFs(12/65, 18.5%) was the most common strain. Subtype of CRFs consisted of CRF01_AE(9/65, 13.8%) and CRF02_AG(3/65, 4.6%). Subtype C(1/65, 1.5%), sub-subtypes A1(2/65, 3.1%) and F1(2/65, 3.1%) were also detected with low prevalence. The rate of overall primary antiretroviral resistance was 4.9%(3/61). Drug-resistant rate for non-nucleoside reverse transcriptase inhibitors was 4.9%. The thymidine analogue mutation rate was 13.1%(8/61).Conclusions: HIV molecular epidemiology studies are necessary to determine transmission patterns and spread. Subtype B and CRF01_AE, CRF02_AG are the most prevalent strains in the south-east of Turkey. However, subtype C, sub-subtypes A1 and F1 are of low prevalence but persist in the south-east of Turkey. In the near future, changing of HIV epidemiology will be possible in Turkey due to migration movement in border lines and resistance testing will play an important role in HIV management.

AIM:To evaluate the effects of tenofovir disoproxil fumarate(TDF)use during late pregnancy to reduce hepatitis B virus(HBV)transmission in highly viremic ***:This retrospective study included 45 pregnant patients with hepatitis B e antigen(+)chronic hepatitis B and HBV DNA levels>107copies/mL who received TDF 300 mg/d from week 18 to 27 of gestation(n=21).Untreated pregnant patients served as controls(n =24).All infants received 200 IU of hepatitis B immune globulin(HBIG)within 24 h postpartum and 20μg of recombinant HBV vaccine at 4,8,and 24 *** transmission rate was determined by hepatitis B surface antigen and HBV DNA results in infants at week ***:At week 28,none of the infants of TDFtreated mothers had immunoprophylaxis failure,whereas2(8.3%)of the infants of control mothers had immunoprophylaxis failure(P=0.022).There were no differences between the groups in terms of adverse events in mothers or congenital deformities,gestational age,height,or weight in *** postpartum week 28,significantly more TDF-treated mothers had levels of HBV DNA<250 copies/mL and normalized alanine aminotransferase compared with controls(62%vs none,P<0.001;82%vs 61%,P=0.012,respectively).CONCLUSION:TDF therapy during the second or third trimester reduced perinatal transmission rates of HBV and no adverse events were observed in mothers or infants.

Assessment of the implementation status of best available techniques for cleaner production in the textile industry

The draft Integrated Pollution Prevention and Control(IPPC)regulation mandates compliance with best available techniques(BATs)for textile manufacturers.A study in Turkish textile facilities,covering 56 units across four sub-sectors,assessed the status of 488 BATs through on-site visits and *** aim was to gauge the sector’s adherence to *** findings revealed that 37% of surveyed BATs were fully implemented,rising to 88% when considering potential future *** suggests a strong industry inclination toward adopting BATs for cleaner production and *** study highlighted significant BAT-related investments in the textile sector,driven by environmental concerns,regulations,customer demands,resource efficiency,competition,and cost-benefit ***,the study results also indicated that there is still much work to do for the implementation of some *** was found that 60 BATs had lower implementation ratios(IR:0%-43%).Lower IR values for these BATs are mainly due to factors like specificity,high costs,long payback periods,operational difficulties,limited expertise,space constraints,customer requirements,quality concerns,operational issues,and sector-specific *** study recommends similar assessments in other European industrial sectors to evaluate compliance with mandatory BATs outlined in the Industrial Emissions *** insights from this study on the Turkish textile sector can serve as a valuable guide for future evaluations.

A geochemical perspective on the genesis of Cenozoic basic volcanism in northeastern Turkey:an overview of metasomatism and heterogeneity of the sub-continental lithospheric mantle in a post-collisional setting

The post-collisional Cenozoic basic volcanic rocks in NE Turkey show temporal variations in whole-rock lithophile element and highly siderophile element(HSE)systematics that are mainly associated with the nature of sub-continental lithospheric mantle(SCLM)sources and parental melt *** far,the traditional whole-rock lithophile geochemical data of these basic volcanic rocks have provided important constraints on the nature of SCLM *** lithophile element and HSE geochemical data of these basic volcanic rocks also reveal the heterogeneity of the SCLM source,which is principally related to variable metasomatism resulting from previous subduction(s)and post-collisional mantle-crust interactions in an extensional *** element geochemical features suggest that the parental magmas have derived from metasomatized spinel-to garnet-bearing SCLM sources for Eocene and Miocene basic volcanic rocks with subduction signatures whereas originated from spinel-to garnet-bearing SCLM sources for Mio-Pliocene and Plio-Quaternary basaltic volcanic rocks without the subduction *** element and HSE geo-chemistry also reveal that Eocene and Miocene basic vol-canic rocks were affected by more pronounced crustal contamination than the basaltic volcanic rocks of Mio-Pliocene and ***,the integrated lithophile element and HSE compositions of these basic volcanic rocks,together with the regional asymmetric lithospheric delamination model,reveal that the compositional variation(especially due to metasomatism)was significant temporally in the heterogeneity of the SCLM sources from which parental magmas formed during the Cenozoic era.

Effect of acetyl L-carnitine on human retinal pigment epithelium-19 cells in hypoxic conditions

AIM:To investigate the effect of acetyl-L-carnitine(ALCAR)on cell viability,morphological integrity,and vascular endothelial growth factor(VEGF)expression in human retinal pigment epithelium(ARPE-19)cells using a hypoxic ***:In the first set of experiments,the optimal CoCl_(2) dose was determined by exposing ARPE-19 cell cultures to different *** evaluate the effect of ALCAR on cell viability,five groups of ARPE-19 cell culture were established that included a control group,a sham group(200μM CoCl_(2)),and groups that received 1,10 and 100 mM doses of ALCAR combined with 200μM CoCl_(2),*** cell viability was measured by MTT *** morphological characteristics of cells were observed by an inverted phase contrast *** levels of VEGF and HIF-1α secretion by ARPE-19 cells were detected by enzyme linked immunosorbent assay(ELISA)***:ARPE-19 cells were exposed to different doses of CoCl_(2) in order to create a hypoxia ***,when exposed to a concentration of 200μM CoCl_(2),a notable decrease in viability to 83% was *** was found to increase the cell viability at 1 mM and 10 mM concentrations,while the highest concentration(100 mM)did not have an added *** cell viability was found to be significantly higher in the groups treated with a concentration of 1 mM and 10 mM ALCAR compared to the Sham group(P=0.041,P=0.019,respectively).The cell viability and morphology remained unaffected by the greatest dose of ALCAR(100 mM).The administration of 10 mM ALCAR demonstrated a statistically significant reduction in the levels of VEGF and HIF-1α compared with the Sham group(P=0.013,P=0.033,respectively).CONCLUSION:The findings from the current study indicate that ALCAR could represent a viable therapeutic option with the potential to open up novel treatment pathways for retinal diseases,particular relevance for age-related macular degeneration(AMD).However,to fully elucidate ALCAR’s application

A robust optimization method for label noisy datasets based on adaptive threshold: Adaptive-k

The use of all samples in the optimization process does not produce robust results in datasets with label *** the gradients calculated according to the losses of the noisy samples cause the optimization process to go in the wrong *** this paper,we recommend using samples with loss less than a threshold determined during the optimization,instead of using all samples in the *** proposed method,Adaptive-k,aims to exclude label noise samples from the optimization process and make the process *** noisy datasets,we found that using a threshold-based approach,such as Adaptive-k,produces better results than using all samples or a fixed number of low-loss samples in the *** the basis of our theoretical analysis and experimental results,we show that the Adaptive-k method is closest to the performance of the Oracle,in which noisy samples are entirely removed from the ***-k is a simple but effective *** does not require prior knowledge of the noise ratio of the dataset,does not require additional model training,and does not increase training time *** the experiments,we also show that Adaptive-k is compatible with different optimizers such as SGD,SGDM,and *** code for Adaptive-k is available at GitHub.

In this paper,new four fifth-order fractional nonlinear equations are derived and *** frac-tional terms are defined in the conformable sense and these equations are then solved using two effective methods,namely,the sub-equation and the generalized Kudryashov *** methods were tested on the proposed models and succeeded in finding new solutions with different values of parameters.A graphical representation of some results is provided and proves the efficiency and applicability of the proposed methods for providing solutions with known physical *** methods are good candi-dates for solving other similar problems in the future.

Subsequent pregnancy outcomes and fertility rates in the case series that underwent bilateral hypogastric artery ligation(BHGAL)due to severe postpartum hemorrhage

Objective:To evaluate the effect of bilateral hypogastric artery ligation(BHGAL)on women's pregnancy outcomes and fertility rates in cases with severe postpartum ***:Patients hospitalized in a tertiary care center between 2007 and 2018 were included in the *** records of cases were obtained from the hospital database,*** demographic data,morbidities or complications associated with BHGAL surgery,any additional haemostatic interventional or surgical procedures,preoperative admission and lowest postoperative haematocrit-hemoglobin values,blood products given,type of anaesthesia,intensive care unit(ICU)and length of hospital stay were *** cases without surgical sterilization,future fertility and subsequent pregnancy outcomes were assessed for at least two years after *** was obtained through face-to-face or telephone interviews with patients whose long-term fertility and subsequent pregnancy outcomes were ***:Sixty-eight patients with BHGAL surgery history were included in the study.26 Of 37 cases(70%)that did not undergo surgical sterilization had a desire for pregnancy after the *** of these cases(23%)were diagnosed with secondary *** fertility rate of these cases was 77%,and subsequent pregnancy was detected in 20 women.A total of 25 pregnancies,including first and second pregnancies,developed,and 15 of these pregnancies resulted in term delivery,six preterm births,one stillbirth,and three ***:The postoperative pregnancy rate was found to be higher than the rate of women with infertility problems,and this surgery does not seem to adversely affect pregnancy *** is a tertiary center experience and cohort studies with large patient series are needed.
